Authors reviewed 110 cases of retrograde pyelography which performed in the Dept. of Urology, Kyung Hee University Hospital during the period from January 1977 to December 1980. And the results were as follows. 1. Indication was 34 cases (31%) of nonvisualizing kidney, 31 cases (28%) of obstructive lesion, 21 cases (19%), of inadequate filling in pelviocalyceal system, 7 cases (6.5%) of impaired renal function. 6 cases (5.5%) of upper tract abnormality, l cases (0.9%) of allergy to contrast media and other 10 cases. 2. On the nonvisualizing kidney, hydronephrosis (23.5%) was most common and normal (20.5%), ureter stone (20.5%) in order. 3. For assessment of obstructive lesion, UPJ stricture (39%) was most common and ureter stone (32%), normal (16%), ureter tumor, UVJ stricture in order. 4. Inadequate filling on excretory urogram revealed normal (24%), renal tumor (19%), renal tuberculosis (9.5%) and chronic pyelonephritis scar (9.5%) etc. 5. The patients who had poor renal function or poor general condition and upper tract abnormality were well defined with retrograde pyelography and available for diagnosis and treatment. 6. Complication were seen in 2 cases (1.8%). One was acute pyelonephritis and the other was pyonephrosis.

Renal angiography is now one of invaluable adjuncts to urologic disease with computed tomography and ultrasonography. Authors observed in 50 cases of renal angiography which performed in the Department of Urology, Kyung Hee University Hospital during the period from September. 1977 to August 1980. In detecting and defining both the presence and extent of renal diseases such as tumor, cyst, injury and vascular disorder, renal angiography is highly reliable and accurate.

Recently, the new modality of computed tomography has been demonstrated to define with remarkable clarity not only the normal anatomy of the urologic organs but also disease processes in the retroperitoneum and pelvic cavity We have performed 36 cases of C-T scan in urologic field. The results are as following. 1. C-T scan is an easily accomplished diagnostic method which provides highly accurate and clinically useful information about retroperitoneal tumor 2. C-T scan is useful diagnostic method for change of bladder wall or intravesical status on bladder tumor. 3. C-T scan is less helpful in detecting lymph node metastases of bladder tumor 4. C-T scan is valuable in evaluating the response of therapy and recurrence of the tumor.

A Clinical observation was made on the patients with bladder tumor admitted to the Department of Urology, Kyung Hee University Hospital during the period from January, 1973 to December,1979 Followings were the results: 1. During the period, 57 cases of bladder tumor among the 1716 total number of in-patients were hospitalized, giving a incidence rate of 3.3%. 2. Age distribution was between 4 and 85 years, the highest incidence rate was in the age group of 60-69 years. 3. Hematuria was the most common symptom occurred in 87.7%, frequency in 61.1%. dysuria in 52.6% and the others. 4. Among the 53 cases performed I.V.P., 79.3% of cases revealed normal upper tract, filling defect in the bladder was observed in 69.8% of them. 5. Associated diseases with bladder tumor were B. P. H. or B. N. C. each in 12.3% and U. T. I. in 31. 6%. 6. Pathologic examination was possible on 49 cases, transitional cell carcinoma occupied 44 cases, in 89.8% and Grade I was found most frequently. 7. 14 cases were studied by the non-invasive, simple method as computerized tomography: the effective staging procedure in invasion of bladder wall itself could be diagnosed very helpfully but distant metastasis not diagnosed very exactly, comparison with the operation stage. 8. 47 cases were treated, T. U. R. or T. U. C. were tried initially in 21 cases.

The laryngeal mask airway (LMA) is new method for maintaining the airway and has many advantages such as easy insertion without muscle relaxant, decreasing cardiovascular change, decreasing damage of pharynx and larynx and useful in difficult intubation. It has being increasingly used in the management of difficult airway problems, but has not been widely used in tracheal surgery. A 59 year old woman with tracheal stenosis due to tracheal tumor was admitted for tracheal reconstruction. The stenotic lesion was 5 cm above the carina and the length of the stenotic segment was 2 cm. Anesthetic management should be focus on maintenance of the airway and adequate ventilation with the number 3 sized LMA during the tracheal resection. The tracheal segmental resection and primary end-to-end anastomosis were performed without serious hypoxia and hypercarbia. We discuss the advantages and limitations of the LMA in tracheal surgery.

Neuronal migration disorders (NMD's) are a rare group of developmental structural lesions characterized by disorganization of cortical architecture with aberrant columnar and laminar arrangement, often causing intractable seizures. During September 1994 to February 1995, we operated on six patients with NMD to treat intractable seizures, Male female ratio of these 6 patients was 2:4 and mean age at seizure onset was 14 years old(range 6-28 years), indicating early onset of epilepsy. Mean age at seizure surgery was 29 years old(range 23-41 years), and mean follow-up duration after operation was 4 months(range 3-6 months). In their past medical history, three patients had experienced febrile seizure at pediatric age, and one of them had a history of anoxic damage during delivery. Following preoperative localization, the lesion was removed completely in five patients. In the other one patient part of the lesion was located the speech and motor area, leading to partial removal. On histologic examination, two of them showed cortical dysplasia and the other four revealed microdysgenesis. During follow-up for six months, five patients were free of seizure and in the other one patient, whose lesion was removed incompletely, the frequency of seizure decreased by 95%.

The present study was undertaken to investigate the influence of hypothermia on endothelium-independent vascular smooth muscle contractility and to determine the mechanism underlying the relaxation. Denuded aortic rings from male rats were used and isometric contractions were recorded and combined with molecular experiments. Hypothermia significantly inhibited fluoride-, thromboxane A2-, phenylephrine-, and phorbol ester-induced vascular contractions regardless of endothelial nitric oxide synthesis, suggesting that another pathway had a direct effect on vascular smooth muscle. Hypothermia significantly inhibited the fluoride-induced increase in pMYPT1 level and phorbol ester-induced increase in pERK1/2 level, suggesting inhibition of Rho-kinase and MEK activity and subsequent phosphorylation of MYPT1 and ERK1/2. These results suggest that the relaxing effect of moderate hypothermia on agonist-induced vascular contraction regardless of endothelial function involves inhibition of Rho-kinase and MEK activities.

Purpose: Neuroblastoma (NB) is the most common extracranial solid tumor found in children. To identify significant genetic factors for the risk of NB, several genetic studies was conducted mainly for Caucasians and Europeans. However, considering racial differences, there is a possibility that genetic predispositions that contribute to the development of NB are different, and GWAS study has not yet been conducted on Korean NB patients. Materials and Methods: To identify the genetic variations associated with the risk of pediatric NB in Korean children, we performed a genome-wide association analysis with 296 NB patients and 1000 unaffected controls (total n = 1,296) after data cleaning and filtering as well as imputation of non-genotyped SNPs using IMPUTE v2.3.2. Results: After adjusting for multiple comparisons, we found 21 statistically significant SNPs associated with the risk of NB (Pcorr < 0.05) within 12 genes (RPTN, MRPS18B, LRRC45, KANSL1L, ARHGEF40, IL15RA, L1TD1, ANO7, LAMA5, OR7G2, SALL4, and NEUROG2). Interestingly, out of these, 12 markers were nonsynonymous SNPs. The SNP rs76015112 was most significantly associated with the risk of NB (p = 8.1E-23, Pcorr = 2.3E-17) and was located in the RPTN gene. In addition, significant nonsynonymous SNPs in ADGRE1 were found in patients with MYCN amplification (rs7256147, p = 2.6E-05). In high-risk group, rs7256147 was observed as a significant SNP (p = 5.9E-06). Conclusion Our findings might facilitate improved understanding of the mechanism of pediatric NB pathogenesis. However, functional evaluation and replication of these results in other populations are still needed.

PURPOSE: We conducted this study to determine the efficacy of conventional treatments for patients with Hodgkin's disease and identify the patients who have poor prognosis and need high-dose chemotherapy and autologous stem cell transplantation. MATERIALS AND METHODS: Between Jun. 1989 and Dec. 1997, 50 patients were enrolled and 39 patients were evaluable. Patients were treated with radiotherapy (5 patients) or combination chemotherapy (21 patients) or combined chemotherapy and radiotherapy (13 patients) according to their disease stage. Chemotherapy regimens were C-MOPP (cyclo- phosphamide, vincristine, procarbazine, and prednisone), MOPP (mechlorethamine, vin- cristine, procarbazine, and prednisone), ABVD (doxorubicin, bleomycin, vinblastine, and dacarbazine), alternating C-MOPP/ABVD, and MOPP/ABV hybrid. Radiation therapy was performed when there was residual tumor after chemotherapy or bulky disease. The response to treatments was analyzed by clinical stage I-II and stage III-IV patients group, respectively. RESULTS: The complete response rate was 76.9% for total patients, 83.3% for stage I-II patients, and 71.4% for stage III-IV patients. Of the 30 patients achieving complete response, four (13.3%) relapsed at 6, 12, 22, and 28 months after complete response, respectively. The median follow-up duration was 24 months. Nine patients died. Four patients died of Hodgkins disease. Three-year overall survival rate was 72.9% for total patients, 72.5% for stage I-II patients, and 70% for stage III-IV patients. Two-year disease- free survival rate was 77.6% for total patients, 79% for stage I-II stage patients, and 73.9% for stage III-IV patients. The prognostic factor analysis showed that performance status affected the disease-free survival rate. CONCLUSION: Conventional treatments in patients with Hodgkins disease showed results comparable to previous studies. But we were unable to identify the patients, who need high-dose chemotherapy and autologous stem cell transplantation, because of small number of study patients and short follow up duration.

BACKGROUND: We investigated the clinical features and treatment outcomes of patients with mantle cell lymphoma (MCL) in Korea. METHODS: We retrospectively analyzed the clinical characteristics and prognosis of 131 patients diagnosed with MCL between January 2004 and December 2009 at 15 medical centers in Korea; all patients received at least 1 chemotherapeutic regimen for MCL. RESULTS: The median age for the patients was 63 years (range, 26-78 years), and 77.9% were men. A total of 105 patients (80.1%) had stage III or IV MCL at diagnosis. Fifty-two patients (39.7%) were categorized with high- or high-intermediate risk MCL according to the International Prognostic Index (IPI). Eighteen patients (13.7%) were in the high-risk group according to the simplified MCL-IPI (MIPI). The overall incidence of extranodal involvement was 69.5%. The overall incidence of bone marrow and gastrointestinal involvements at diagnosis was 41.2% and 35.1%, respectively. Cyclophosphamide, doxorubicin, vincristine, prednisolone, and rituximab were used frequently as the first-line treatment (41.2%). With a median follow-up duration of 20.0 months (range, 0.2-77.0 months), the overall survival (OS) at 2 years was 64.7%, while the event-free survival (EFS) was 39.7%. Multivariate analysis showed that the simplified MIPI was significantly associated with OS. However, the use of a rituximab-containing regimen was not associated with OS and EFS. CONCLUSION: Similar to results from Western countries, the current study found that simplified MIPI was an important prognostic factor in Korean patients with MCL.
