1.Regulation of the Wnt Signaling Pathways during Cell Culture of Human Mesenchymal Stem Cells for Efficient Bone Regeneration

Tissue engineering and bone regeneration techniques using mesenchymal stem cells (MSCs) have started to be applied to the field of oral and maxillofacial surgery. Clinically, a shortened treatment time and improved efficiency are necessary because of the patients' needs and the running cost of cell culture. In the present study, the cultivation process for human MSCs (hMSCs) was examined by regulating the Wnt signaling pathway. We activated Wnt signaling with LiCl and inhibited Wnt signaling with sFRP-3 (secreted Frizzled-Related Protein-3). The proliferation of LiCl-treated hMSCs was examined by studying the cell growth rate and performing BrdU assays. Osteogenic differentiation of sFRP-3-treated hMSCs was examined by alizarin red staining, and osteogenic gene expression on days 7 and 14 after induction was examined by reverse-transcription polymerase chain reaction (RT-PCR) analysis and quantitative real-time RT-PCR analysis. LiCl-treated hMSCs showed increased cell numbers and BrdU-positive cells as compared to the untreated cells. Alizarin red staining showed early mineralization of hMSCs on day 7 of the sFRP-3 treatment. A high expression level of the alkaline phosphatase gene on days 7 and 14 of sFRP-3 treatment was also demonstrated. These results suggest that the regulation of the Wnt signaling pathway contributes to the increased cell numbers and the early osteogenic differentiation of hMSCs. This study supports the possibility that the regulation of the Wnt signaling pathway contributes to the development of effective and efficient bone regeneration techniques.
2.An Opinion Poll of Doctors Concerning Working Conditions in Kohseiren Hospitals and the Medical Service System in Japan.

A survey was conducted by sending questionnaires to 466 doctors working at nine hospitals affiliated with the Aichi Prefectural Federation of Agricultural Cooperatives for Health and Welfare (Kohseiren).
The survey found that the length of service was under 5 years in 60% of the respondents and that many of them identified themselves strongly with the department of the teaching hospital where they were trained.
They were found to be little intersted in the present situation of the agricultural cooperatives and Japanese agriculture. However, they said they were satisfied with the present working conditions, because they could show their skills. Most approved of organ transplants after brain death but disapproved of human cloning. They showed a positive attitude toward the allowing of each patient to give access to his or her clinical records and standardization of medical treatment. Some 67% of the doctors admitted they had made medical mistakes and keenly realized the necessity of risk management.
While feeling uneasy about the recent upsurge of lawsuits brought by patients about medical treatment, they said they are endeavoring to provide safe and high quality medical service. Their recent concerns were rearrangement of dispensaries based on the subspecialty, reinforcement of postgraduate courses in medical schools and the postgraduate training system which will be a legal obligation from 2005. They suported the reform plans for the national medical service for aged patients and recognized the necessity of joining hands with local agricultural cooperatives in providing nursing care services. They said they wish to continue their career as a physican at the Kohseiren hospital.
3.Migration of a Retained Epicardial Pacing Wire into the Pulmonary Artery

Pericardial pacing wire placement may occasionally result in intravascular or intratracheal wire migration, infective endocarditis, and sepsis; reportedly, the incidence of complications is approximately 0.09 to 0.4%. We report a case of a retained epicardial pacing wire that migrated into the pulmonary artery. A 66-year-old man underwent coronary artery bypass grafting for angina pectoris, with placement of an epicardial pacing wire on the right ventricular epicardium, 6 years prior to presentation. Some resistance was encountered during wire extraction; therefore, it was cut off at the cutaneous level on postoperative day 8. Computed tomography performed 6 years postoperatively revealed migration of the pacing wire into the pulmonary artery, and it was removed using catheter intervention. Surgeons should be aware of complications associated with retained pacing wires in patients in whom epicardial wires are retained after cardiac surgery.
