1.Effect of hypoxia-inducible factor-1? on vascular relaxation reactivity following hemorrhagic shock in rats

Objective To study the effect of hypoxia-inducible factor-1?(HIF-1?) on endotheliumdependent and-independent vascular relaxation reactivity following hemorrhagic shock in rats.Methods Two hundred and eight SD rats were randomly divided into normal control group,shock control group and oligomycin group.Shock control and oligomycin groups were further divided into 0,0.5,1,2,3,and 4 h groups after hemorrhagic shock.Superior mesenteric artery(SMA) was used to compare the changes in vascular response to sodium nitroprusside(SNP) and acetylcholine(Ach) with or without administration of oligomycin,a kind of specific HIF-1? blocking agent,and to determine the expression of HIF-1? mRNA by RT-PCR.Results The vascular reactivity of SMA to SNP in each shock group was significantly decreased at lower concentrations(10-9,10-8 and 10-7mol/L) of SNP and oligomycin further depressed the response of SMA to SNP.The response of SMA to Ach in shock groups was transiently decreased after shock,and then began to increase during the compensatory period of shock,reached its peak at 2 h after shock,and then decreased again.The response of SMA to Ach decreased significantly in early hemorrhagic shock(0 to 1 h,P
2.Assessment of lung volume using multi-slice spiral CT and comparison with pulmonary function test

Objective To explore the value of multi-slice spiral CT (MSCT) with three dimensional (3D) postprocessing for assessing the lung volume and compare it with pulmonary function test (PFT). Methods In 60 cases, there were 34 healthy controls and 26 patients with emphysema. CT scan and PFT were performed within 3 d in all case. The total lung was respectively scanned at the end of full inspiration and full expiration, using GE Lightspeed 16 slices spiral CT. The total lung volumes and voxel index (VI) at threshold range of -1023 Hu to -900 Hu were measured by pulmonary quantitative software of AW4.2 workstation. Results The full expiration volume(Vex) significantly correlated with residual volume(RV) (r=0.908) as well as the full inspiration volume (Vin) with total lung capacity (TLC) (r=0.897). The value of (Vin-Vex) well correlated with forced vital capacity(FVC), as well as Vex/Vin with RV/TLC, VI Ain with TLC, VI Aex with RV. Conclusion Significant correlation of lung volume accessed by MSCT with 3D technique with PFT is useful for assessing the pulmonary function.
3.Renal impairment in Sjogren's syndrome (SS)

To further study the renal damage of SS with biopsy and recent determination. Methods 31 patients of SS with renal impairment from 1988 to 1995 were analyzed by routine, immunoassay,tubular function and biopsy examination. Results 25/31 cases presented dRTA, 3/25 with diabetes insipitus and 2/25 with hypokalemic paralysis. Glomerulopathy was occured in 8 of 31 (NS 5, GN 3) and mild renal failure( RF) in 4. Hypergamma-globulinemia was observed in 23 of 31 (74%). About 65% and 45% patients revealed anti ss-A and anti ss-B positive respectively. In 13 renal biopsies, 9 specimens showed CIN with large number of lymphoplasma cells infiltration and tubular atrophy and other 2 specimens revealed LN (type Ⅲ and Ⅳ ) and 2 MPGN. By IF, no positive findings were seen in 7 cases and deposition of IgA, IgM or 63 in some other patients. IgG deposits in the interstitial lymphoplasma cells in 1 patient. 19 patients were treated with prednisone and 3 combined with CTX. 14 cases remitted during the follow-up. In 4 patients with RF, Scr level returned to normal after treatment. Conclusion The renal impairment of SS may be the presenting or predominant feature. Their major clinical manifestations are dRTA and GN. The treatment with prednisone may decrease the infiltration of lymphoplasma cells in the interstitium, improve the renal function and correct RTA.
4.Curve behavior after brace treatment and possible factors contributing to further deterioration in patients with adolescent idiopathic scoliosis

Objective To assess the post-brace curve progression and to identify what radiographic or clinical factors may be predictive of deterioration after brace treatment. Methods The medical records of 112 patients with AIS who completed brace treatment 2 years ago and at least 18 years old at final follow-up were reviewed. Cobb angles were measured from radiographs at initial visit, brace prescription, brace weaning(in brace), first out-brace visit and final follow-up. The curve type, location and chronologic age, Risser sign and menarcheal status at weaning were recorded. Outcome was based on changes in Cobb angle between the first out-brace radiograph versus that of final follow-up. Curve behavior was also identified as improvement, maintenance or mild deterioration, and marked deterioration. The percentage of each type of behavior was calculated and analyzed. 5.CT Guided Puncture Aspiration and Sclerosing Treatrment of hepatic and Renal Cysts

Purpose: To summarize the efficiency and the experience of the puncture aspiration and sclerosing therapy of hepatic and renal cysts. Materials and methods: 27 patients were treated with percutaneous aspiration and alcohol injection of the cystic cavity using 20G Greene needles. Among the 27 patients, 11 were with hepatic cyst, 15 with renal cyst, and 1 with polycystic kidneys. The size of cysts ranged from 3.5 to 12 cm, and the amount of aspirated fluid varied from 21 to 550ml. Pure alcohol (99. 7%) was injected into the cavities after apiration. Results: All the puncture procedures were successful, and no major complications occurred, The clinical symptoms were relieved or disappeared after the treatment. 18 cases were followed for 3 months to 5 years. The efficient indices: Grade I one case (polycystic kidneys), Grade Ⅱ 4 cases and Grade Ⅲ 13 cases. Conclusion: CT guided percutaneous puncture aspiration and sclerosing treatment of hepatic and renal cysts can be considered as an effective alternative to surgical treatment. The simple aspiration of polycystic kidneys is very helpful in relieving symptoms and improving renal functions.

Objective To investigate lipopolysaccharide (LPS) stimulates the expression of nuclear factor-KB (NF-κB) mRNA and tumor necrosis factor-a( TNF-α) mRNA in rat's pancreatic acinus AR42J cell line, and further address the pathogenesis of acute pancreatitis. Method The AR42J cell line was stimulated with different concentrations of LPS (0.001, 0.01, 0.1, 1.0, 10 and 100 mg/L) for 18 hours, or stimulated with 10 mg/L of LPS for different lengths of time (2, 6, 12,18 and 24 hrs) .Then, the expressions of NF-κB-P65 mRNA and TNF-α mRNA were determined by using RT-PCR, the levels of TNF-α protein in the culture supernatant were measured with radio-immuno assay (RIA), and the correlation between the expressions of TNF-α mRNA and NF-κB mRNA was analyzed. Results Both the expressions of NF-κB mRNA and TNF-α mRNA were up-regulated when AR42J cell line was stimulated with 10 mg/L of LPS for 2 hours or with 0.001 mg/L of LPS for 18 hours in both dose-dependent and time-dependent manners. Similarly, the levels of TNF-α protein were up-regulated when AR42J cell line was stimulated with 0.01 mg/L of LPS for 18 hours or with 10 mg/L of LPS for 6 hours in both dose-dependent and time-dependent manners. Statistical analysis revealed the positive correlation between the expressions of TNF-α mRNA and NF-κB-P65 mRNA(r = 0.962, P < 0.01). Conclusions LPS stimulates the expressions of TNF-α mRNA and NF-κB mRNA in both dose-dependent and time-dependent manners, and their expressions are closely correlated, suggesting the inhibition of their expressions as a potential therapeutic target for acute panceatitis.

Objective To investigate the effects of Tat-NBD(NEMO-binding domain,NBD)peptide on LPS-stimulated AR42J acinus cells inflammatory response.Method Lipopolysaccharide(LPS)was added to culture media at doses of 10 mg/kg for 24 hours to stimulate the AR42J cells.For pretreatment.cells were incubated with different peptides for 2 hours before LPS stimulation.The expression of TNF-α mRNA WaS conducted using a semi-quantitative RT-PCR method.TNF-α protein in culture medium were detected by enzyme linked immunosorbent assay(ELISA).The expression and translocation of the NF-kB-p65 protein of AR42J was determined by Strept Actividin-Biotin Complex(SABC)method.Results LPS(10 mg/L)resulted in an increase of TNF-αmRNA and TNF-αprotein,whereas significant inhibitory effects were observed when cells were incubated with Tat-NBD(WT)just on dose of 0.1 me/L(P<0.05).The Tat-NBD(WT)peptide decreased inflammatory cytokine expression by a dose-dependent manner and its peak role was on dose of 100 mg/L.Consisting with TNF-α expression decrease,NF-kB-p65 expression signitieantly decreased in Tat-NBD(WT)pretreatment group,especially on the largest dose.NO significant changes in the control peptide group.Conclusions Tat-NBD(WT)peptide can inhibit the inflammation of acinus simulated by LPS.
