1.Surgical therapy for anorectal malignant melanoma

Objective To evaluate clinical features, diagnosis and treatment of anorectal malignant melanoma (ARMM).Methods The clinical data of 15 patients of ARMM in our hospital and 18 patients in the First Affiliated Hospital of China Medical University from 1990 to 2010 were reviewed.Twenty-five patients underwent curative surgical resection, 14 patients underwent abdominoperineal excision of the rectum (APR), and 11 patients underwent local excision (LE).Survival analysis was carried out.Fisher's exact test and Log-rank test was used to compare the effects of these two different surgical procedures.Results ARMM had a female predominance, the mean age was 22 -77(54.5 ± 7.6) years.The major clinical signs included hematochezia, anus pain.The misdiagnosis rate was 67% (22/33).The average tumor size was (3.5 ±1.7) cm.Thirty-one petients(94% ,31/33) had ARMM within 5 cm from anus margin.Mean survival time was (14.0 ± 6.5)months.The overall 1-,3-,and 5-year survival rates were 48% ,22% , and 10% , respectively.Local recurrence after curative LE was higher than APR (LE,64% vs APR, 21% , P = 0.049) , The overall 3-year disease-specific survival rates after curative LE was not significantly different from that of APR (LE, 28% vs APR ,31%, x2 = 0.268, P = 0.582).Conclusions Anorectal malignant melanoma has a high rate of misdiagnosis.Radical resection could not prolong the survival time significantly in anorectal malignent melanoma patients.
2.Preliminary study on a new model of Community-based Hypertension group-interference management model

369 patients with hypertension living in Xietu community were randomized into 2 groups:intervention group receiving the group-interference-management and control group receiving routine hypertension management services.We found that patients in the intervention group had better self-management behaviors,self-efficacy and health status in comparison with the control group (P<0.05).The systolic and diastolic blood pressure in the intervention group decreased more remarkably than that in the control group (P<0.05).Our result suggested that the community-based hypertension group-interference-management model is more effective to control hypertension than the routine hypertension management services.
3.Clinical characterization and genotype analysis of idiopathic mental retardation in male patients with epilepsy

Objective To detect genetic causes of idiopathic mental retardation/developmental delay in 20 male patients with epilepsy and to analyze their clinical characteristics of positive mutation carriers.Methods The families,consisted of the patient and his parents were recruited.Genomic DNA was extracted from peripheral blood,and candidate gene mutation screening was carried out by next-generation sequencing technology.Mutations in positive gene were verified by polymerase chain reaction(PCR) and direct sequencing.Results Three missense mutations were identified among 3 patients out of 20 cases,with a detection rate of 15%.They were:OPHN1 gene c.1996 C > G,RAB39B gene c.542 C > T and AFF2 gene c.427 A > T,none of which had been reported before.All of these mutations were likely to be pathogenic based on gene function,evolutionary conservation,variant frequency in normal population (NHLBI Exome Sequencing Project and 1 000 Genomes),bioinformatics prediction and inheritance patterns.In addition,all 3 genes disrupted were residing on the X chromosome previously demonstrated to be associated with X-linked mental retardation(XLMR),indicating that they were probably pathogenic or might serve as one of the risk factors.Conclusions Abnormal function of genes on the X chromosomal is one of the most impotent causes of XLMR.X chromosomal gene mutation screening would be recommended for male children suffering from idiopathic mental retardation with epilepsy.
4.Effects of 50 Hz sinusoidal electromagnetic field with different intensities on rat peak bone mass.

Studying effects of 50 Hz sinusoidal electromagnetic fields (SEMFs) with different intensities on peak bone mass (PBM) of rats may provide a theoretical basis for application of electromagnetic clinical field. 30 female SD rats, 6 weeks of age, were randomly divided into three groups: the control group, 0.1 mT electromagnetic field group (EMFs) and 0.6 mT EMFs. The EMFs groups were treated for 3 h/day. After 8 weeks, we examined their bone mineral densities (BMD) , measured their bone biomechanical properties, and made serum levels of osteocalcin (OC), tartrate-resistant acid phosphatase 5b (TRACP 5b), and histomorphometry. It was found that the BMD (P < 0.01), maximum mechanical load (P < 0.01) in the 0.1 mT group were significantly higher than those in the control group, and Yield strength (P < 0.05), the analyses of serum bone turnover markers and histomorphometric parameters were better than those in the control group (P < 0.05). However, the 0.6 mT group did not have significantly difference comparing with that in the control group. This study proved that 50 Hz 0.1 mT SEMFs can increased BMD, bone strength, and bone tissue microstructure. Therefore, 50 Hz 0.1 mT SEMFs can improve peak bone mass of rats.

5.Effect of icariin and genistein on bone protection

Aim To compare the pharmacological ac-tivity of icariin( ICA) and genistien ( GEN) against os-teoporosis after oral administration with them to growing rats and ovariectomized rats. Methods 25 mg·kg-1 icariin and 10 mg · kg-1 genistein ( equal in molar concentration) were administered to one-month-old fe-male SD rats every day for three months. Treatments at the same dosage were administered to the 6-month-old ovariectomized SD rats every day for three months. Their effects were compared on bone mineral density and biomechanical properties of femurs and vertebrae, serum levels of osteocalcin and tartaric acid phospha-tase 5b ( TRACP 5b) and histomorphometry. Results The results showed that, in young rats, icariin treat-ment significantly increased bone mineral density, the maximum mechanical loads of femurs and vertebrae as well as the bone qualities ( serum markers and microar-chitecture ) , whereas genistein treatment had little effects compared with the non-treatment control. How-ever, genistein treatment was more efficacious than icariin in preventing bone loss and deterioration of bone microarchitecture in ovariectomized rats. Conclusion Our data suggest that, since icariin has a higher os-teogenic activity but lower estrogenic activity, it has been found to be more efficacious than genistein in peak bone mass accrual only in young rats. In the ovariectimized rats, however, as the main force to pre-vent bone loss is the estrogenic activity, genistein has been found to be more efficacious than icariin in reduc-ing bone loss.
6.Fibrin glue/bone morphogenetic protein complex plus slow-release gentamicin for repairing infected bone defects in rabbits

BACKGROUND: Chronic osteomyelitis is difficult to manage clinically, and two or more operations were commonly needed. No satisfactory method for one-stage repair has been currently available.OBJECTIVE: To examine the possibility of using fibrin glue(FG) as the common carrier for both bone morphogenetic protein(BMP) and gentamicin for one-stage repair of infected bone defects.DESIGN: A completely randomized controlled experiment.SETTING: Center of Orthopaedic Surgery, Lanzhou General Hospital of Lanzhou Area Command of of Chinese PLA.MATERIALS: The experiment was conducted using 48 healthy adult Chinchilla rabbits of either sex on normal diet with body mass of 1.9 to 2.4 kg,provided by the Institute of Biological Products, Lanzhou, Gansu Province.INTERVENTIONS: Animal models of chronic osteomyelitis were established in 48 Chinchilla rabbits, and after thorough debriding of the wounds,an elongated semicircular bone defect was induced 1.5 cm to the medial proximal metaphysis in the tibias. Three treatments were subsequently given with FG, MP and gentamicin complex(Group A), FG/BMP complex(Group B), and nothing(control group, Group C), respectively.MAIN OUTCOME MEASURES: The general conditions of the rabbits were observed postoperatively. Bacterial culture of the bone specimen, bacterial counting, X-ray and histological examination were performed.RESULTS: Rabbits in Group A exhibited obviously better infection control and greater regenerated bone mass than those in Group B, and no significant difference was noted in infection control between Groups B and C, the latter group showing poor bone defect repair.CONCLUSIONS: FG, BMP and gentamicin complex helps to control infection and promotes bone formation, and can be used to repair bone defects caused by infection and open injury with severe contamination.
7.Analysis of osteoporosis related factors in premenopausal patients with early rheumatoid arthritis

Objective To identify the clinical features and risk factors of early rheumatoid arthritis (RA)-associated osteoporosis in premenopausal women. Methods A total of 76 premenopausal women with early RA were randomly selected in the Department of Kidney and Rheumatology in the hospital. A total of 84 health cases were randomly selected in our hospital as controls. Bone mineral density (BMD) was determined using dual energy X-ray absorptiometry (DEX). Bone metabolism (CTX, PINP) and inflammatory cytokines (IL-17, IL-6, TNF-α) were examined with quantitative enzyme-linked immune-sorbent assay (ELISA). Quantitative data were expressed as x ±s deviation and the data were compared between groups using non- parametric test (Z value). Multi-group comparison was performed with variance analysis. Qualitative data were compared with Fisher's test. Logistic regression was used to investigate the risk factors. Results ①Compared with the control group, BMD in the premenopausal women with early RA group [neck: (0.802 ±0.193) g/cm2, GT zone: (0.923±0.033) g/cm2, L1: (0.862±0.011) g/cm2] was significantly decreased [(0.981±0.032) g/cm2, (0.771 ±0.023) g/cm2, (0.912 ±0.012) g/cm2, F=14.401, 19.860, 6.560, respectively, both P<0.05). The prevalence of osteoporosis in this group was 7%(5/76), which was higher than controls 1%(1/84). ② According to values of Bone meta-bolism [(CTX: (0.37±0.21) ng/ml] and inflammatory cytokines, TNF-α: (9.8±4.1) pg/ml, IL-6: (33.6±5.7) pg/ml and IL-17: (129±24) pg/ml were markedly increased in premenopausal women in early RA group [(0.24 ±0.09) ng/ml, (6.7 ±1.9) pg/ml, (1.5 ±0.4) pg/ml, (45 ±7) pg/ml, Z=2.722, 5.932, 7.501, 4.370, respectively, both P<0.05]. ③ The premenopausal women with early RA group with osteoporosis were signifi- cantly difference with controls in BMI [(9±3) kg/m2 vs (16±3) kg/m2], bone density of neck [(0.85±0.20) ng/ml vs (0.88±0.14) g/cm2], L2 [(0.75±0.23) g/cm2 vs (0.88±0.14) g/cm2], L3 [(0.87±0.07) g/cm2 vs (0.93±0.14) g/cm2], L4 [(0.92±0.12) g/cm2 vs (0.94±0.16) g/cm2], serum ESR [47.8(22.0, 76.0) mm/1 h vs 18.8(8.7, 35.2) mm/1 h] and DAS28-CRP (5.3 ±1.2 vs 3.8 ±1.2) F=0.68, 14.632, 26.114, 20.931, 36.582, Z=3.21, 6.58, respectively, both P<0.05. ④ Logistic regression showed that IL-6 (Wald χ2=5.78, P=0.021), PINP (Wald χ2=5.12, P=0.031), CTX (Wald χ2=9.17, P=0.003), ESR (Wald χ2=9.24, P=0.011), DAS28-CRP (Wald χ2=17.28, P=0.001) were significantly positively correlated with osteoporosis. Moreover, ordered unconditional Logistic regression analysis of the variables (IL-6, PINP, CTX, ESR, DSA28) described above showed that DAS 28-CRP score [OR=1.58, 95%CI: (1.10, 2.20)] was the most important risk factor for osteoporosis in premenopausal women with early RA. Conclusion The incidence of osteoporosis is high in premenopausal women with early RA than healthy cases. DAS 28-CRP score is the important risk factor for premenopausal women with early RA- associated osteoporosis. Measures relieve symptoms of RA can help to prevent and treatment osteoporosis.
8.Effects of compound medicine of icariin and puerarin on peak bone mass in growing rats

Objective To study the effects of the compound medicine of icariin and puerarin on peak bone mass in rats during growth period, and to explore its possible mechanism. Methods Forty female Sprague-Dawley rats aged 1 month were randomly divided into normal control group( C), icariin group( I), puerarin group( P), icariin and puerarin compound groupc(I+P), 10 in each group. The body weights were recorded once every two weeks, and the bone mineral density was measured by dual-energy X-ray absorptiometry every month. After the bone mineral density of the whole body was significantly different between the control group and drug groups the animals were sacrificed. The right femur and vertebrae were separated to measure the bone mineral density. The biomechanical properties of the femur and vertebra were detected by AG-IS series desktop electronic universal testing machine. The bone formation index osteocalcin, PINP and bone resorption index were determined by ELISA. Changes in the contents of tartrate-resistant acid phosphatase 5b(TRACP 5b) and CTX-1; and changes in trabecular bone related parameters were recorded after magenta-picric acid staining. Results There was no significant difference in body weight between the two groups (P>0.05). There was no significant difference in whole body bone density after 1 month of treatment (P>0.05). After 2 months of treatment, the body bone density of the drug-administered group was higher than that of the control group. Whole body bone density, femur and vertebral bone density, femur maximum load value, maximum vertebrae load value and trabecular bone number and area, serum OC and PINP levels increased, while TRACP 5b and CTX-1 levels decreased(P<0.01) in drug group. The difference from the control group was statistically significant (P<0. 05). There were significant differences in biochemical parameters and bone histomorphology between the compound drug group and the two-flavor monomer group ( P<0. 01). There was no significant difference in bone mineral density and biomechanics, but the average value was higher than that of the monomer group. Conclusion The combination of icariin and puerarin can effectively increase the peak bone mass in rats.
9.Multivariate analysis of the incidence of hepatic encephalopathy after transjugular intrahepatic portosystemic shunt

Objective To analyze the correlation between the clinical characteristics and the incidence of postoperative hepatic encephalopathy in patients with cirrhosis who received transjugular intrahepatic portosystemic shunt ( TIPS) . Methods The clinical data of 80 patients with liver cirrhosis hospitalized in the Fifth Medical Center of PLA General Hospital from March 2015 to March 2017 were retrospectively analyzed. The related factors of hepatic encephalopathy complications after TIPS treatment were analyzed. Results The cumulative incidence of hepatic encephalopathy was about 23. 75%(19/80) in one year after TIPS operation. Through single factor analysis and multi factor Logistic regression analysis,it was found that the two factors of preoperative platelet level and the location of tips blood shunt were closely related to the occurrence of hepatic encephalopathy after TIPS operation. The ROC curve suggested that the AUC of preoperative platelet level and the location of tips blood shunt were 0. 657 and 0. 668, respectively. The sensitivity and specificity of the former were 70. 0%,52. 6% and the cutoff value was 51 ×109/L, respectively, while those of the latter were 70. 5% and 63. 2%, respectively. Conclusion Preoperative platelets level>51×109/L or shunting of the left branches of the portal vein may indicate a lower incidence of hepatic encephalopathy.
10. Effects of sinusoidal electromagnetic fields at different exposure times on osteoblastic activity and the underlying signal transduction mechanisms

Objective:
To investigate the signal transduction mechanisms of time-dependent effects of 50 Hz 1.8 mT sinusoidal electromagnetic fields(SEMFs)on osteoblastic activity.
Methods:
Newborn rat calvarial osteoblasts(ROBs)were treated with 50 Hz 1.8mT SEMFs for 30, 60, 90, 120, and 150 min, respectively.Intracellular alkaline phosphatase(ALP)activity was assayed, and protein expression of Runt-related transcription factor 2(Runx2), Smad1/5/8 and mitogen-activated protein kinases(MAPKs)were examined by Western blot.The Dual-Luciferase Reporter Assay System was used to measure the activity of the Wnt pathway.Immunofluorescence staining and laser confocal microscopy were applied to examine the nuclear translocation of Smad1/5/8 and β-catenin.Changes in ALP activity were determined after inhibiting p38 MAPK using a specific inhibitor.
Results:
ALP activity of ROBs increased after 30, 60, 90, 120 and 150 min of SEMFs treatment, compared with the control group(51.41±5.21, 59.47±4.02, 67.56±4.68, 63.69±3.92, and 58.16±3.61
