1.The Application of Chest Coronal CT Scan in Children

Objective To explore the way and the clinical value of chest coronal CT scan in children.Methods There were 16 patients with airway or pulmonary diseases proved by clinic underwent chest coronal scan,the ages ranged from 3 months to 5 years.Picker Ultra-Z spiral CT unit was applied,the aperture of the gantry was 70 cm,gantry's maximum tilted angle was 30?.The scanning parameter were :120 kV,200 mA,0.9 s,spiral pitch 1.5,thickness 2 mm,space 1.5 mm.Results The chest coronal images acquired in all cases were good.Tracheobronchial tree up to lobar bronchi were all displayed.Bronchial foreign body in 12 cases,bronchia tumor in 1 cases,lung hypogenesis in 1 case,lung inflammation in 8 cases,atelectasis in 5 cases and the local emphysema in 10 cases were demonstrated on CT images.Conclusion Children's chest coronal CT scan can show the whole tracheobronchial structure.It has high clinical applied values.
3.The value of multislice computed tomography in diagnosis of vascular rings associated with airway abnormali-ties in children

Objective To explore the value of multislice computed tomography (MSCT) in diagnosis of vascular rings associated airway abnormalities in children. Methods CT image data were retrospective analysis in 159 cases of vascular rings, including multiplanar reconstruction (MPR), maximum and minimum density project reconstructions. The relationship between the vascular rings and airway had been observed. Results Of 159 cases of vascular rings as-sociated with airway stenosis in 101 cases, the main airway stenosis in 79 cases, left main bronchial stenosis in 14 cas-es, right main bronchus in 8 cases, tracheal bridge in 14 cases, tracheal bronchial in 11 cases, symmetry bronchial in 2 cases. Conclusions Vascular ring often causes compression of airway narrow and dysplasia. MSCT can clearly display vascular rings and its relationship with airway, providing help for surgical and reasonable treatment.
4.CT and MRI features of malignant peripheral nerve sheath tumor of children

Objective To analyze CT and MR features of malignant peripheral nerve sheath tumor (MPNST)of children. Methods Sixteen patients with histologically proven MPNST were retrospectively reviewed.There were 8 male and 8 female, ages from 0.3 to 11.0 years, and median age was 2.5 years. Sixteen cases were performed with CT plain scan, and eight cases with CT enhancement scans, and three with MR examination. The imaging data were analyzed by two highly experienced doctors and obtained agreements after mutual consultation. Results Among 16 cases, 3 cases were located at neck, 5 cases at waist and back,3 cases at abdominal and pelvic, 2 cases at foot, 1 case at left clavicle, 1 case at right mediastinum, 1 case at right orbit. Fifteen cases appeared as solid masses and 1 case showed a diffuse growth. CT plain scan showed 8 cases were solid-appearing masses and 7 cases were cystic-solid mass. Enhanced CT showed enhancement of solid component was moderate to marked and gradually delayed enhanced while cystic component had no any enhancement. One was located on the left side of the neck and appeared as diffuse growth. Two cases of solid mass type appeared as hypo-intensity on T1WI and hyper-intensity on T2WI, and obviously high signal intensity of cystic component on T2WI, and with significantly heterogeneous enhanced.One case with diffuse growth appeared as hypo-intensity on T1WI and slightly hyper-intensity on T2WI, and with significantly enhanced. Sixteen cases appeared as invasive growth, 11 caseswith multiple organic metastases and recurrences, and 4 cases with neurofibromatosis type I and scoliosis.Conclusion CT and MR appearances of MPNST have certain characteristic features, and can demonstrateaggressive performance and multiple organic metastases, which is helpful for definite diagnosis and treatment plan.
5.Nutritional risk assessment in patients with chronic liver disease.

OBJECTIVETo use the European Nutritional Risk Screening (NRS)-2002 survey tool to investigate nutritional risk associated to different degrees of liver disease and to assess its ability to identify the nutritional risk of hospitalized patients with chronic liver disease.
METHODSA total of 366 hospitalized patients were assessed with the NRS-2002 on the day of admission. Patients who meet the criteria for malnourishment (NRS-2002 score of more than 3 points (severely impaired nutritional status with body mass index (BMI) less than 18.5 kg/m2) were selected for further study to determine liver function. Patients were classified according to liver dysfunction-related features, including cirrhosis status, Child-Pugh classification, and underlying disease causes (e.g.alcohol, hepatitis virus infection). Chi square test was used in statistical analysis of inter-group difference.
RESULTSThe incidence of patients surveyed who were at nutritional risk was 41.0%, and the incidence of malnutrition was 7.6%. The patients with liver failure showed the highest rate of nutritional risk (72.8%). Moreover, among the 97 patients with liver cirrhosis, significantly more had Child-Pugh grade B than grade A (88.6% vs.33.1%; x2=24.019, P=0.000). The cause of liver failure with the highest incidence of nutritional risk was alcohol-related liver disease (66.7%). The overall malnutrition rate among the total 156 patients classified by the NRS-2002 as being at nutritional risk was 76.2%.
CONCLUSIONThe NRS-2002 is a suitable screening tool for use in Chinese patients with mild early liver disease, but it must be interpreted carefully as its findings alone may promote a false positive rate. The NRS-2002 is less accurate in patients with end-stage liver disease.

6.Comparison and evaluation of three thyroid imaging reporting and data systems for medullary thyroid carcinoma

Objective:To investigate the diagnostic performance of different thyroid imaging reporting and data systems (TI-RADS) in the diagnosis of medullary thyroid carcinoma (MTC) .Methods:A total of 160 thyroid nodules diagnosed as MTC by postoperative pathology from Aug. 2011 to Aug. 2022 at the First Affiliated Hospital of Zhengzhou University were included. Additionally, 160 papillary thyroid carcinomas (PTC) and 160 benign nodules were randomly selected as controls during the same period. Differences in gender, age, nodule diameter and various ultrasound features were observed. The nodules were classified according to American College of Radiology (ACR) TI-RADS, artificial intelligence (AI) TI-RADS and Chinese (C TI-RADS). Then, receiver operating characteristic curve (ROC) were plotted to calculate the diagnostic value. The Kendall concordance coefficient was used to evaluate the interobserver consistency of each TI-RADS system.Results:There was no statistically significant difference in gender among the three groups ( χ2=1.17, P=0.558). However, significant differences were observed in age and nodule diameter ( F=12.08,40.12, P<0.001 for both). The area under ROC (AUC) for diagnosing MTC and benign nodules using ACR, AI, and C-TIRADS were 0.762, 0.773, and 0.761, respectively, with no statistically significant differences ( Z=1.33, 0.01, 0.87, P=0.183, 0.994, 0.386). However, the sensitivity of C TI-RADS (87.5%) was lower than that of ACR and AI TI-RADS (both 95.0%) ( P=0.018). After combining the biopsy threshold, the false negative rate of C-TIRADS was lower than that of ACR (30.6% vs. 41.3%) ( P=0.048) and AI TIRADS (30.6% vs. 43.1%) ( P=0.020). The inter-observer diagnostic consistency of C-TIRADS was superior to ACR (0.884 vs. 0.819, P<0.001) and AI TIRADS (0.884 vs. 0.839) ( P<0.001) . Conclusions:AI and ACR TI-RADS have higher sensitivity in diagnosing MTC, while C TI-RADS has a lower puncture missed diagnosis rate. AI has similar diagnostic performance to ACR TI-RADS and can replace ACR TI-RADS.
7.Design and application of an electronic report on monitoring environmental hygiene and disinfection and sterilization quality

Objective To explore the effect of an electronic report on monitoring environmental hygiene and disinfection and sterilization quality on improving the work efficiency and accuracy of monitoring report . Methods The electronic report on monitoring environmental hygiene and disinfection and sterilization quality was self-designed .5 200 results in key department of hospital infection were chosen and divided into the control group and the experimental group , each with 2 600 cases.The control group received the traditional manual monitoring report , and the experiment group received the electronic monitoring report .The differences of sorting the monitoring data in the department , the occurrence of the wrong and missing report in the clinical laboratory were compared between two groups .Results The cases of wrong and missing report were respectively 56 and 192 cases (9.5%) in the control group, and were higher than 18 and 41 cases (2.3%) in the experimental group, and the differences were statistically significant (χ2 =19.795, 102.449, respectively;P<0.01).The average time of sorting the monitoring data was (8.0 ±1.2)h in the control group, and was longer than (3.0 ± 0.9) h in the experimental group, and the difference was statistically significant (t =33.73,P <0.01). Conclusions The content of electronic report on monitoring environmental hygiene and disinfection and sterilization quality is intuitive , comprehensive and surveyability , and which is convenient to use for the monitoring and inspection personnel .
8.Roles of growth differentiation factor 10 in nervous system

Recent studies have demonstrated that superfamily of transforming growth factor-β (TGF-β) plays vital roles in the development of nervous system and neurological diseases.Growth differentiation factor l 0 (GDF 10) is one of the most prominent members of this superfamily.GDF 10 has been shown to participate in tissue formation during cerebellar development and be closely related to the outcomes of neuralgia,ischemic stroke and other neurological diseases.These findings provide novel ideas for research into pathogenesis of nervous system diseases and targets for development of new therapeutic approaches.This article reviews biochemical structure of GDFI0,its downstream signaling pathways,and its roles in cerebellar development,neuralgia and ischemic stroke.Also the possible neurological diseases that may be treated by GDF10 are listed.
9.Effects of combined nursing second line duty in the rehabilitation hospital at night

Objective To explore the effects of nursing second line night duty implementation in rehabilitation hospital combined with multiple departments.Methods From September 2015, the combined nursing second line duty has been implemented in the rehabilitation district of Taihe Hospital of Shiyan. The Chinese version of Nursing Job Stress Inventory (NJSI), a self-designed night nursing quality assessment standards and a nursing satisfaction questionnaire were applied to evaluate and compare the job stress, night nursing quality and nursing satisfaction before and after the intervention respectively.Results After the implementation of the nursing second line duty on night shift, the nurses' job stress was relieved obviously, and the quality of care at night and patients' satisfaction were improved as well. Compared with before the intervention, the difference was statistically significant (P<0.05).Conclusions The establishment of nursing second line duty in rehabilitation district relieve the nurses' work pressure, especially on working environment and their own quality. At the same time, it ensures the nursing quality and safety on night shift,and improves the patients' satisfaction to nursing work.
