Currently,human health due to corona virus disease 2019(COVID-19)pandemic has been seriously threatened.The coronavirus severe acute respiratory syndrome coronavirus 2(SARS-CoV-2)spike(S)protein plays a crucial role in virus transmission and several S-based therapeutic approaches have been approved for the treatment of COVID-19.However,the efficacy is compromised by the SARS-CoV-2 evolvement and mutation.Here we report the SARS-CoV-2 S protein receptor-binding domain(RBD)inhibitor licorice-saponin A3(A3)could widely inhibit RBD of SARS-CoV-2 variants,including Beta,Delta,and Omicron BA.1,XBB and BQ1.1.Furthermore,A3 could potently inhibit SARS-CoV-2 Omicron virus in Vero E6 cells,with EC50 of 1.016 pM.The mechanism was related to binding with Y453 of RBD deter-mined by hydrogen-deuterium exchange mass spectrometry(HDX-MS)analysis combined with quan-tum mechanics/molecular mechanics(QM/MM)simulations.Interestingly,phosphoproteomics analysis and multi fluorescent immunohistochemistry(mIHC)respectively indicated that A3 also inhibits host inflammation by directly modulating the JNK and p38 mitogen-activated protein kinase(MAPK)path-ways and rebalancing the corresponding immune dysregulation.This work supports A3 as a promising broad-spectrum small molecule drug candidate for COVID-19.

Transcatheter tricuspid valve intervention is the new frontier of interventional cardiology. The LuX-Valve is a radial force-independent orthotopic tricuspid valve replacement device developed in China. The LuX-Valve Plus transcatheter tricuspid valve replacement (TTVR) system is changed from the trans-atrial to the transjugular approach, which further reduces trauma and pulmonary complications compared with the first generation LuX-Valve. The first-in-human study has been completed at Zhongshan Hospital, Fudan University and an exploratory multicentre clinical study is underway. Echocardiography plays an important role in pre-TTVR screening, intraoperative guidance and postoperative evaluation and follow-up, especially two-dimensional transoesophageal echocardiography (2D-TEE) and three-dimensional transoesophageal echocardiography (3D-TEE). However, there is a lack of appropriate intraoperative guidance and assessment protocols. In this study, we briefly described the protocols and imaging considerations for intraoperative 2D-TEE and 3D-TEE to ensure the successful implantation of TTVR.

Tricuspid regurgitation (TR) interventions are under rapid development. The K-Clip? system is the first domestic transcatheter tricuspid annuloplasty system with unique clamping procedure to achieve annular reduction.Intraoperative echocardiographic monitoring procedures for transcatheter tricuspid annuloplasty have not been reported yet in China. Thus, this review aimed to propose the standard two-dimensional and three-dimensional transesophageal echocardiographic workplanes and procedures to guide and monitor the implantation of K-Clip system based on our experience in Zhongshan Hospital, Fudan University to provide a reference point for the intraoperative echocardiographic monitoring of future transcatheter tricuspid annuloplasty devices in China.
4.The status of nursing information ability and its relationships with information consciousness and information anxiety in intern nursing students

Objective:To explore the status of nursing information ability and its relationships with information consciousness, information anxiety in intern nursing students, provide basis for the cultivation of nursing information ability of intern nursing students.Methods:From December 2019 to January 2020,185 nursing students working as interns in Jinan Central Hospital Affiliated to Shandong University were surveyed by general information questionnaire, information anxiety questionnaire, information consciousness questionnaire and nursing information ability assessment scale.Results:The total score of nursing information ability of nursing students in practice was 111.6±19.7; nursing information ability was positively correlated with information consciousness ( r=0.358, P<0.001), and not related to information anxiety ( r=0.136, P=0.065); information consciousness had a predictive effect on nursing information ability( β=0.287, P<0.001), and information anxiety could moderate the relationship between information consciousness and nursing information ability ( β=0.175, P=0.011). Conclusions:The score of nursing information ability is above the medium level, while improving the nursing information ability through the development of information consciousness, and the information anxiety level of nursing students should also be paid attention to.
5.Premature complex-induced chronic cough :case report and literature review

The clinical presentation,chest image and electrocardiogram of 4 cases of premature complex-induced chronic cough admitted in the First Affiliated Hospital of Guangzhou Medical University from March 2008 to February 2009,were retrospectively analyzed.And other 6 cases of premature complex-induced chronic cough were retrieved for literature review.In total 10 patients,there were 3 males and 7 females aged 58(36-74) years,with a disease duration of 10.5 (3.0,264.0) months.The main clinical presentations were chronic dry cough with post-sternal thump and premature beats as shown by auscultation and 24 h-electrocardiogram examination.Cough was relieved after the premature beats were controlled with the treatment of arrhythmia drugs or radiofrequency ablation.Premature complex-induced cough is rare but it should be considered when the chief complaint of patients with premature beat is chronic cough.
6.Clinical role of transthoracic and transesophageal echocardiography in transapical mitral valve repair

Objective To evaluate the role of transthoracic echocardiography ( T T E ) and transesophageal echocardiography( T EE) in the process of transapical mitral valve repair using a novel edge‐to‐edge device( ValveClamp) and this device′s efficacy and safety in a preliminary clinical trial . Methods Six patients with moderate to severe or severe degenerative mitral regurgitation ( DM R) confirmed by T T E and T EE were enrolled . T T E was performed pre and post procedure as well as 30 days post procedures . Related cardiac structure and hemodynamic parameters ,including mitral regurgitation area ( MRA‐max ) , vena contracta width ( VCW ) ,mitral valve effective orifice area ( M VEOA ) ,left ventricular end diastolic diameter ( LVEDD ) , left ventricular end systolic diameter ( LVESD ) , left ventricular ejection fraction ( LVEF) ,max and mean mitral valve pressure gradient ( M VPG‐max and M VPG‐mean) were recorded and evaluated in a central core laboratory . Results All the procedures were successfully performed .M RA‐max , VCW and M VEOA decreased significantly post procedures ( all P < 0 .000 ) , and they remained no significant changes within 30 days post procedures ( all P > 0 .05 ) . M eanwhile ,M VPG‐max and M VPG‐mean slightly increased ( all P <0 .01 ) and left atrial anterior‐posterior dimension attenuated 30 days post procedures( P <0 .05) ,but all M VPG‐mean were lower than 5 mm Hg ( 1 mm Hg=0 .133 kPa) . T here were no significant changes in other hemodynamic parameters ( all P > 0 .05) . Conclusions T ransapical mitral valve repair using ValveClamp can be performed safely and a significant reduction in mitral regurgitation can be achieved in patients with DM R . T EE and T T E facilitate the patient selection for ValveClamp procedures as well as perioperative navigation and assessment .
7.Age and Sex Distribution of Chinese Chronic Cough Patients and Their Relationship With Capsaicin Cough Sensitivity

PURPOSE: An older female predominance has been reported among chronic cough patients in Western countries, which is considered to be associated with a higher cough sensitivity in females. However, the characteristics of Chinese chronic cough patients remain unclear. This study aimed to explore the age and sex distribution as well as their relationship with cough reflex sensitivity to capsaicin in Chinese chronic cough patients. METHODS: We analyzed the demographic features of 1,882 consecutive chronic cough patients who attended our cough clinic in Guangzhou, China. Cough sensitivity to capsaicin, which was defined as the lowest concentration of capsaicin causing 5 coughs or more (C5), was measured in 539 of the 1,882 patients and 68 healthy volunteers. RESULTS: The mean age of the patients was 43.0 ± 13.7 years and patients aged <50 years accounted for more than two-thirds of the study population. Around 87% of the patients were never-smokers. The proportion of females (51.5%) was almost equal to that of males (48.5%). The pattern of the age and sex distribution was consistently reflected within most common causes of chronic cough, while a female predominance was shown in patients with cough-variant asthma and patients aged ≥50 years. Female patients had higher cough sensitivity to capsaicin than male patients (log C5: 1.58 ± 0.84 vs. 2.04 ± 0.84 μmol/L, P = 0.001), and patients aged ≥50 years had higher cough sensitivity to capsaicin than patients aged <50 years. CONCLUSIONS: In China, patients with chronic cough have a roughly equal sex distribution and a middle-aged predominance, irrespective of a higher cough sensitivity to capsaicin in females and older patients. 8.Distribution and antibiotic resistance of Klebsiella pneumoniae strains in terms of K1 and K2 serotypes

Objective To investigate the distribution and antibiotic resistance of the K lebsiella pneumoniae strains isolated from clinical infections in terms of serotypes K1 ,K2 and virulence factor rmpA gene .Methods The hypermucoviscous phenotype of K .pneumoniae isolate was determined by string test .K1 and K2 serotypes and rmpA gene were detected using multiplex polymerase chain reaction .Results Of the 144 strains of K .pneumoniae ,the prevalence of hypermucoviscous phenotype ,K1 , K2 serotypes and rmpA gene was 62 .5% (90/144) ,52 .1% (75/144) and 65 .3% (94/144) ,respectively .The prevalence of K1 ,K2 and rmpA K .pneumoniae strains was 90 .7% (68/75) in K1 ,K2 serotypes .The prevalence of K1 ,K2 isolates and rmpA in hypermucoviscous or non‐hypermucoviscous phenoype was 63 .3% (57/90) ,85 .6% (77/90) and 33 .3% (18/54) , 31 .5% (17/54) ,respectively .The prevalence of serotype K1 ,K2 with or without rmpA gene was 72 .3% (68/94) and 14 .0% (7/50 ) respectively . Of the 42 K . pneumoniae strains isolated from liver abscess ,85 .7% (36/42) were hypermucoviscous phenotype and 88 .1% (37/42 ) were serotypes K1 , K2 . For the strains from other abscess , bacteremia ,community acquried pneumonia (CAP) ,urinary tract infection (UTI) and biliary tract infection ,the prevalence of hypermucoviscous phenotype was 81 .3% (13/16) ,40 .5%(15/37) ,85 .7% (12/14) ,52 .4% (11/21) and 21 .4% (3/14) ,respectively ,and the prevalence of serotypes K1 ,K2 was 56 .3% (9/16) ,29 .7% (11/37) ,64 .3% (9/14) ,38 .1% (8/21) and 7 .1% (1/14) ,respectively .K1 serotype isolate accounted for 61 .9% of the strains from liver abscess .The ratio between serotype K1 and K2 was similar in the isolates from other abscess ,CAP ,UTI or bacteremia .Non‐K1 ,K2 serotype isolates were common in biliary tract infection .The prevalence of extended‐spectrum beta‐lactamases (ESBLs ) was 5 .5% in hypermucoviscous phenotypes and 33 .3% in the non‐hypermucoviscous phenotypes .Conclusions rmpA gene is associated with the hypermucoviscous phenotype of K .pneumoniae strains and commonly identified in K1 ,K2 serotype isolates .Serotypes K1 ,K2 isolates are important pathogens in liver abscess and CAP ,and also common in other abscess ,UTI and bacteremia .K1 serotype isolate was most common in liver abscess .The prevalence of K1 or K2 serotype was similar in other infections . The prevalence of ESBLs is lower in hypermucoviscous strains than in non‐hypermucoviscous strains and is associated with lower resistance rate to most of the antibiotics tested .
9.Toileting behavior and its relationship to lower urinary tract symptoms in female nurses

Objective To explore the status of toileting behavior and its relationship to lower urinary tract symptoms (LUTS) in female nurses. Methods A total of 636 nurses were selected from three top three hospitals in Jinan by multi-stage sampling. The nurses′toileting behavior and LUTS were assessed by Women′s Toileting Behavior Scale and The International Consultation on Incontinence Questionnaire-Female Lower Urinary Tract Symptoms. Univariate analysis and hierarchical regression analysis were conducted to examine the factors associated with LUTS. Results The nurse groups were widespread adverse toileting behavior. Delayed voiding was the most severe problem in nurses. Among LUTS storage symptoms were the most severe,voiding symptoms followed and incontinence symptoms were mild. Hierarchical regression analysis exhibited that factors associated significantly with LUTS included age, body mass index, menstrual status, working experience, history of urinary tract infection and poor toileting behavior (mainly hard urination, delayed voiding, and anuria urination),which explained 9.1%,12.9% and 12.6% of the variance of storage symptoms, voiding symptoms and incontinence symptoms, respectively. Conclusions Poor toileting behaviors are highly prevalent in nurses and they are closely related to LUTS, leading to concerns about possible effects of working environment and poor bladder habits on LUTS. Cognitive-behavioral intervention for this group is essential for delivering information about correct toileting behavior and its association with LUTS. Hospital administrators are suggested to pay more attention to nurses′working environment and its impact on nurses′health in order to improve their quality of life and job satisfaction.
10.The Effects of Cangfu Daotan Decoction on the Expression of oatp4a1 and Level of Sex Hormone in the Endometrium and Ovary of Rats with Obesity-type Polycystic Ovarian Syndrome

OBJECTIVE:To explore the effects of Cangfu daotan decoction on expression of organic anion transporting poly-peptide(oatp4a1)and level of sex hormone in the endometrium and ovary tissues of rats with obesity-type polycystic ovarian syn-drome(PCOS). METHODS:Obesity-type PCOS rat models were established. 50 rats were randomized into a normal control(iso-metric normal saline)group,a model(isometric normal saline)group,a metformin(0.043 g/kg)group and Cangfu daotan decoc-tion high-dose and low-dose [5.68 and 1.42 g(medicinal materials)/kg] groups. The rats were given drugs ig once a day for 14 con-secutive days. The expression of oatp4a1 protein in the endometrium and ovaries of the rats was determined by ECL and Western blot. The levels of luteinizing hormone(LH),follicle stimulating hormone(FSH),testosterone(T)and estradiol(E2)in rat’s se-rum were detected by ELISA. RESULTS:Compared to the normal control group,those in the model group had weaker expression of oatp4a1 protein in the endometrium and ovaries,higher levels of serum LH,LH/FSH and T,lower levels of serum FSH and E2. There were statistical significances(P<0.01). Compared to the model group,those in metformin group,Cangfu daotan decoction high-dose and low-dose group had higher expression of oatp4a1 protein in the endometrium and ovaries,lower levels of LH/FSH and T,higher levels of serum E2;those in metformin group and Cangfu daotan decoction high-dose group had lower levels of serum LH,higher levels of serum FSH. There were statistical significances(P<0.01 or P<0.05). CONCLUSIONS:Cangfu daotan decoc-tion can increase the expression of oatp4a1 in the endometrium and ovaries of obesity-type PCOS model rat,alleviate phlegmy dampness and regulate the level of sex hormone.
