1.The Cprrelation between LVH, LV Function and 24-hour Ambulatory Blood Pressure Monitoring in Patients with Newly Diagnosed Hypertension.

BACKGROUND: Left ventricular hypertrophy is one of the major cardiovascular risk factors. So it is generally thought to be a predictor of complication and prognosis of hypertension. The 24-hour noninvasive ambulatory blood pressure monitoring (ABP) has been shown to be superior to office BP inpredicting target organ involvement in patients with hypertension and assessing antihypertensivve therapy. To determine the correlation between blood pressure and left ventricular hypertrophy in patients with newly diaggnosed systemic hypertension, we evaluate blood pressure by 24-hour ABP, office BP and echocardiiographic parameters of left ventricular hypertrophy. METHODS: From january 1995 to September 1995, in 22 patients with untreated essential hypertension who were diagnosed recently (within 1 month). They were studied by 24-hour noninvasive ambulatory blood pressure monitoring and cross sectional, M-mode and pulsed Doppler echocardiography for examining the relation between ABP and echocardiographic parameters. In the present study, we divided the oatuebts by two groups; white-coat hypertensive group and sustained hypertensive group. RESULTS: 1) Among the 22 patients who were diagnosed by office blood pressure, the white-coat hypertension was in 7 cases (31.8%) and sustained hypertension was 15 cases (68.2%). 2) In sustained hypertensive group, LV mass, LV mass index and relative posterior septal wall thickness were significantly increased compared with white-cost hypertensive group. 3) 24-hour ABP and systolic BP and loading % were significantly correlated with relative posterior septal wall thickness (p<0.05). CONCLUSION: In patients with newly diagnosed hypertension (especially with sustained hypertension), there was left ventricular hypertrophy expressed by increasing of LV mass, LV mass index, and relative posterior septal wall thickness. And, there were close correlation between 24-hour ABP monitoring-especially systolic BP and loading % of systolic BP and LVH.

5.The Significance of Fast Spin Echo MRI in Patients with Sudden Sensorineural Hearing Loss.

BACKGROUND: Patients with acoustic neuroma(AN) may have sudden sensorineural hearing loss(S-SNHL). The prevalence of AN in patients with S-SNHL is 0.8% to 30%. Thus, a lot of diagnostic tools have been used to screen of AN from S-SNHL. Usually, Gadolinium(Gd) enhanced MRI have been recommended. Recently, Fast spin echo MRI(FSE MRI) have been shown to be equally effective in detection of acoustic neuroma as Gd enhanced MRI. OBJECTIVE: To assess the value of FSE MRI as a screening test in patients with S-SNHL. MATERIALS AND METHODS: From January 1995 to August 1996, We evaluated FSE MRI and Gd enhanced MRI for 45 patients with S-SNHL. RESULT: We could screen 4 cases of AN patient from them. CONCLUSION: FSE MRI is effective in the detection of AN in S-SNHL patients.

6.Two Cases of Misdiagnosed Congenital Stapes Fixation.

Congenital stapes fixation showing normal finding of external auditory canal and tympanic membrane causes non-progressive conductive hearing loss in the range of 40 to 60 dB since childhood. It is a rare disease, but most commonly found among the isolated ossicular anomalies. Patients with this disease are very suitable candidates for stapes surgery, and successful hearing improvement can be expected by an appropriate surgical procedure. We report 2 cases of congenital stapes fixation which had been operated improperly. One case showed interposed homologous ossicle between the stapes head and the incus, and the other case showed transposed incus between the malleus handle and the stapes head along with the anterior crus fracture. They showed hearing improvements of more than 30 dB after stapedotomy with placement of the Fisch type stapes prosthesis. We are reporting two cases of misdiagnosed congenital stapes fixation with a discussion.

7.Study on the Development of the Mouse Ossicles Using Micro-CT.

BACKGROUND AND OBJECTIVES: Detailed information on how ossicles change in the developing temporal bone would be helpful in understanding biomechanical knowledge of ossicles. In this study, three dimensional structural images of mouse were investigated through Micro-CT to understand developmental anatomy of its ossicles. MATERIALS AND METHOD: Thirty five temporal bones of BALB/c mice, aged from gestational day 11 to postnatal day 21, and adults were investigated. Three-dimensional highresolution reconstructions of the ossicles of the left sided temporal bones were obtained using a Micro-CT system. The right sided temporal bones were used for the histological study. RESULTS: Internal biomechanical characters (volume fraction, structure model index, degree of anisotropy, etc) of murine ossicles and temporal bones were attained to the adult level at postnatal day 21. CONCLUSION: This study will be useful in understanding the more accurate microstructure of murine temporal bone, and will provide the anatomical basis for the future animal studies.

8.Congenital Stapes Anomalies with Normal Eardrum.

OBJECTIVES: A non-progressive and conductive hearing loss with normal eardrum, but no history of trauma and infection, is highly suggestive of a congenital ossicular malformation. Among ossicular anomalies, stapes anomaly is the most common. The purpose of this study is to describe patterns of stapes anomaly and to analyze its surgical outcome with special reference to its patterns. METHODS: We conducted a retrospective case review. The subjects comprised 66 patients (76 ears) who were decisively confirmed by the exploratory tympanotomy as congenital stapes anomalies without any anomalies of the tympanic membrane and external auditory canal. The preoperative and postoperative audiological findings, temporal bone computed tomography scan, and operative findings were analyzed. RESULTS: There were 16 anomalous patterns of stapes among which footplate fixation was the most common anomaly. These 16 patterns were classified into 4 types according to the status of stapes footplate. Successful hearing gain was achieved in 51 out of 76 ears (67.1%) after surgical treatment. CONCLUSION: Footplate fixation was usually bilateral, whereas stapes anomalies associated with other ossicular anomaly were usually unilateral. The success of the surgical treatment of stapes anomaly might depend on its developmental status of the footplate. Stapes anomalies were detected without any fixed patterns, therefore, it is quite possible to detect a large variety of patterns in future.

9.Role of IgG, IgA, and IgE Antibodies in Nasal Polyp Tissue: Their Relationships with Eosinophilic Infiltration and Degranulation.

To confirm local production of IgE, and evaluate role of immunoglobulins on eosinophil activation in nasal polyp (NP) tissue, we measured IgG, IgA, secretory IgA(sIgA), total (tIgE) and specific IgE (sIgE) to Dermatophagoides pteronyssinus(DP) by ELISA in NP tissue homogenates from 51 subjects. They were classified according to skin reactivity to DP: group I, 15 highly atopic subjects; group II, 18 weakly atopic subjects; and group III, 18 non-atopic subjects. Eosinophil cationic protein (ECP) level was measured by CAP system. Highest level of DP-sIgE was noted in group I, followed by group II and III (p<0.05). Nine (60%) of group I and 4 (22%) of group II subjects had detectable level of DP-sIgE with no significant differences in IgA, sIgA, and IgG. All of NP tissue had eosinophilic infiltration with no significant difference in activated eosinophil count or ECP level among 3 groups. A significant correlation was noted between EG2+ cell count and tIgE (r=0.55, p<0.05), and DP-sIgE level (r=0.60, p<0.05). A significant correlation was also noted between ECP and IgG (r=0.51, p<0.05) and DP-sIgE level (r=0.47, p<0.05) with no significant correlation with IgA or sIgA. These results suggest that DP-sIgE was detectable in NP tissue from weakly atopic subjects as well as highly atopic subjects. IgG and sIgE may have potential roles in eosinophil degranulation in NP tissue.
