1.Successful hybrid operation of an acute mobile thrombus in the abdominal aorta induced by chemotherapy.

Acute mobile thrombus of the abdominal aorta after chemotherapy is a very unusual finding, which can be a potential source of arterial embolism. We report here on a case of an acute mobile aortic thrombus with renal infarction. We successfully treated the patient with hybrid operation-open surgical and endovascular approach. Our case shows that hybrid treatment using wire-directed balloon catheter thrombectomy is a feasible, minimally-invasive treatment for a mobile aortic thrombus.

2.Triple Procedure for Phacoemulsification, Foldable IOL Implant and Trabeculectomy with Mitomycin-C.

The purpose of our study is to evaluate the visual, refractive and IOP results following combined small incision phacoemulsification, foldable lens implantation, and fornix based trabeculectomy with Mitomycin-C. Seventy-six eyes of 64 patients with cataract and glaucoma had undergone the combined procedure. Minimal follow-up period was 6 months(mean 12.8+/-4.8 months). All the eyes that were free of preexisting macular disease and endstage glaucomatous optic nerve damage demonstated significant improvement in visual acuity. Postoperative astigmatisn was negligible(-0.41diopters). The average IOP reduction was 7.7mmHg. Functioning filtrating bleb persisted in 76.3% of the eyes. Complications with this method were not significant compared with other studies reporting the combined procedure. The small incison phacotrabeculectomy with mitomycin-C technique that we describe appears to be effective in early restoration of visual acuity and reduction of IOP without significant complications.

4.Metronidazole in amoebiasis 1. The use of metronidazole on the treatment of hepatic and pulmonary amoebiasis.

Total 9 cases of extraintestinal amoebiasis, amebic liver abscess and amebic lung abscess, were treated with metronidazole. The diagnosis and progress check were done by (1) stool examination, (2) size and pain of liver, (3) hematological examination, (4) urinanalysis, (5) X-ray or liver scanning (6) examination of aspirated pus from liver and (7) immobilization test of E. histolytica. Results are summarized as follows: Amebic hepatitis: Metronidazole was given to 3 cases with daily dose of 0.9-1.2 gm for 3-7 days. Clinical symptoms were completely restored by the treatment. Amebic lung abscess: One case was treated with metronidazole for 3 days with the daily dose 1.2 gm. X-ray findings showed remarkable improvement by the treatment. An additional administration of the drug for 6 days resulted complete resolution of the pneumonic changes. Amebic liver abscess: Four cases were treated with metronidazole. The daily doses were 0.4-2.4 gm and continued for 2-10 days. Aspiration of the abscess for 1-10 times or continuous drainage of pus were performed during the course supplementarily. All cases were recovered to normal by the combining treatment. No remarkable side effect was noticed by the drug administration, and no relapse was experienced up to date.

5.Granular Cell Tumor of the Esophagus: Analysis of five cases with review of four additional cases in Korean literature.

Granular cell tumor(GCT) has been identified in all organ systems, most often in the oral cavity and subcutaneous tissue. GCT of the esophagus has been considered rare. However, this neoplasm is being recognized with increased frequency since the use of fiberoptic endoscopy has become widespread. We report 5 cases of granular cell tumors involving the esophagus, all of which were incidentally found. Four additional cases of GCT which appeared in the Korean literature were reviewed.

6.Two cases of allergic contact dermatitis to sodium fusidate.

Sodium fusidate, obtained by fermentation of the fungus, Fusidium coccineum, has a steroid structure and shows a very high antitaphycoccal activity. The allergic potentirl of sodium fusidate is low and few cases of contact allergy to sodium fusidate have been reported. We present two cases of allergic contact dermatitis to Fucidin cintentione developed postoperative dressing of a skin biopsy and the other after laser treatment of a vascular nevi. The patch test results showed positive reaction to sodium fusidate in both cases.

7.Pseudolipomatosis of the Gastrointestinal Mucosa: Report of 6 cases with analysis of possible factors involved during endoscopic procedure.

Pseudolipomatosis of the colonic mucosa has been recognized as a lesion featured with aggregations of gaseous spaces in the lamina propria, but its pathogenesis remains still unclear. This paper describes 6 cases of pseudolipomatosis occurring in the mucosa of stomach and large intestine, and the possible factors involved in gastrointestinal endoscopic procedure to produce gaseous entrapment in the lamina propria were analysed. All cases received either gastroscopy or colonoscpy before endoscopic biopsy. Mucosal tissues from both stomach and recto-sigmoid colon revealed multiple aggregations of small air-spaces resembling fatty infiltration in the lamina propria. Rarely were similar infiltrations within the muscularis mucosae and adjacent lymphoid follicles. Evidence for pneumatosis cystoides intestinalis or ulcerative colonic lesion was not associated, although one showed a small gastric ulcer nearby. Repeated inflations and deflations of the stomach or colon during the endoscopic procedure with miner mechanical trauma by instrument to the mucosa seem to contribute to its pathogenesis.
8.Analysis of 33 Knees with Lateral Instability

In acute tears of the lateral complex of the knee, it is extremely important to make a complete diagnosis not to overlook the other injured structures in the knee since tears are rarely limited to just the lateral collateral ligament. Among the primary individual structures of the lateral ligament complex that may be involved are the lateral collateral ligament, lateral capsule, popliteus muscle, arcuate ligament complex, iliotibial band, biceps femories, intermuscular septum, and of course, the lateral meniscus as well. Further, it is important to repair lateral tears as soon as possible after injury even though the lateral instability of the knee is less frequent but because it is more easily overlooked and more disabling than the other instability of the knee. Therefore, to evaluate the result of surgically treated patients having the lateral instability, we analyzed the 33 cases with minimum 1 year follow-up period who were treated at the Orthopaedic Department, Kangnam St. Mary's Hospital, from February, 1982 to June, 1985. Among 145 knee ligament injury cases, 39(26.9%) had lateral instability but six were lost to follow-up. Among these remaining 33 cases, 5(15.2%) had isolated lateral collateral ligament injury, 18(54.5%) had associated anterior cruciate ligament injury, 4(11.8%) had associated posterior cruciate ligament injury and 26(78.9%) had injury of other lateral structures including lateral collateral ligament. In all cases having associated anterior cruciate ligament injury there was severe anterolateral rotatory instability under the general anesthesia. The severity of the anterolateral rotatory instability was very much correlated with the severity of the lateral structural injuries. Eighteen out of 19 cases having associated injuries of anterior or posterior cruciate ligament, had 5 mm or more joint opening at the 0 varus stress radiogram. Thirteen(92.8%) out of fourteen isolated ligament complex injuries, and 8(53.3%) out of 15 cases having associated anterior cruciate ligament injury had good-excellent or fair(+) result. None of the patients who had associated anterior and posterior cruciate ligament injuries had good-excellet result. Therefore, the patients who had the lateral ligament complex injury associating with anterior cruciate ligament tend to have residual anterolateral rotatory instability even though the repair or reconstruction was carefully done in comparision with the patients of isolated lateral ligament complex or anterior cruciate ligament injury. Because anterior cruciate ligament injury associated with lateral ligament complex increases the anterolateral rotatory instability significantly.

10.Endovascular Aneurysm Repair.

In recent years, the interest in minimally invasive surgery has grown, and the same trend is observed in vascular surgery and interventional radiology, leading to what is called endovascular surgery. Since the first use of a stent-graft for the endovascular exclusion of an abdominal aortic aneurysm (AAA), endovascular aneurysm repair (EVAR) has greatly expanded, and more than 50,000 devices have been implanted until now. The endovascular graft can be implanted from a remote access site in the groin with a less anesthetic requirement. The endovascular graft is advanced over guidewires up the femoral and iliac arteries. Once in position, the graft is deployed immediately distal from the renal arteries. The aorta is not clamped and the blood loss is less than with open surgery. EVAR for AAA offers an important new alternative to open surgical procedure. The mortality rates after EVAR are reported between 0~5%. Long-term follow-up reports are not available, but mid-term follow-up of EVAR reveals an incidence of re-intervention between 10~20% and a rate of late rupture of between 0.5~1.5% per year. The problems of endoleaks and graft failure continue to be the challenges that require technological innovations. Based on the currently available evidence, EVAR is an appropriate treatment for selected patients, especially those at high risk for open surgical repair.
