4.Clinical and Histopathological Observations on the Neurilemmomas ( 1974 ~ 1984 ).

Fifty-two cases of neurilemmoma, which occurred in the skin and soft tissue and filed at the Department of Clinical Pathology, Catholic University Medical College from 1974 to 1984, were included for the clinical study. Forty four cases whose microscopic slides were available were reviewed for the histopathologic observation. The results were as follows : Clinical observations 1) The average age of the patients was 42.8% years old, and the average duration of tlie tumor was 3.9 years. 2) Eight cases were acompanied with tenderness and cases with tingling sensation. Clinica] signs described prior to biopsy were palpable mass(15), slow growing mass (8), palpable cystic mass(6) and hard mass(4). 3) The mean diameter was 2.6m (0.45cm-8cm), and the frequent sites were head and neck (32.7%), upper extremities(28.8%), lower extremities (23.11%) and trunk (15.4%) in decreasing orders. Histopathologic observations 1) 33 cases were composed of both Antoni A and type B with variable composition, but only Antoni type A was observed in 6 cases and Antoni type B in 5 Cases. 2) On the areas of Antoni type B, there were several degenerative changes including cystic change (52.3%), hemorrhage(50%), hyalinization(27.3%), necrosis(25%), and calcification(4.5%). 3) In 3 cases out of 44, the tumor was observed in the dermis and the tumors composed of Antoni A only occurred on the tongue, neck, and upper arm.

5.Morphometrical and Histological Changes of Glomeruli and Proximal tubules of Contralateral Kidney in Uninephrectomized Rat.

This study was to investigate the histomorphomeric and histological changes of glomeruli and proximal tubules of contralateral kidney in uninephrectomized rat. Rats were Sprague Dawley strain and female 1-year-old. Uninephrectomized rats were sacrificed after 20 days, 30 days, and 40 days. The results were as follows: 1. The mean areas and diameters of the glomeruli were markedly increased at the 30 days. 2. In the glomeruli, the mean nuclear areas of podocytes were markedly increased, but those of cells of the parietal layer were decreased. At the 20 days, those of the endothelial cells were slightly incerased. 3. The lumens of the endothelial cells were irregularly expanded and folded at the 20 days. The basal lamina of the 30 days was markedly thickened in the electron microscopic finding. 4. At the 30 days, the mean areas and mean thicknesses of proximal tubules were markedly increased, but those were not normal. At the 40 days, the mean thicknesses of proximal tubules were restored to the almost normal. 5. Light microscopic changes of the proximal tubules had no difference between the control and the experimental group. In electron microscopical findings, the cytoplasm of the proximal tubules were densely filled with the enlarged mitochondria at the 30 days. At the 40 days, some large vacuoles appeared in the cytoplasm.

6.A Case of Dysplastic Nevus.

We report herein a case of dysplastic nevus, which developed on the snterior chest and abdomen in a 48-year-old American Catholic priest. Clinical features present as brownish to pinkish patches with irreguar border fading into surrounding skin, ranging from 0.5 to 1.5cm in diameter. Histopathological findings show disordered proliferation of slightly atypical melanocytes in the epidermis and fusing of epidermal rete ridges, concentric eosinophilic fibrosis along elongated rete ridges, lymphohistiocytic infiltration in the dermis, and extension of intraepidermal melanocytic hyperplasia beyond the shoulders of dermal nevus component. One year follow-up revealed no evidence of recurrence.

7.A Case of Transient Acantholytic Dermatosis.

We report herein a case of transient acantholytic dermatosis. The patient was a 24-year-old pregnant woman in the last trimester, who had pruritic, discrete, erythematous or brownish papules on the lateral aspect of the neck. Histopathologic findings of the skin lesion showed changes resembling Darier's disease showing well circumseribed, small foci of acantholysis with acantholytic and dyskeratotic cells over the suprahasal clefts. The patient showed good response to isotretinoin(Roaccutane) therapy, 20mg/day, for 30 days.

8.A Case of Xeroderma Pigmentosum with Various Skin Tumors.

A 24-year-old female was seen with multiple, pinkish to skin colored and brownish to black pigmented macules, papules, patch and plaque on the face, neck, upper chest, extensor surface of both forearms and dorsal hands. Histopathological diagnoses from the 75 skin biopsy specimens taken during the follow-up period of 22 months include 29 skin tumors : one malignant melanoma, one precancerous melanosis, 16 basal cell carcinamas, four squamous cell carcinomas, four actinic keratoses, one melanoacanthoma, one keratoacanthoma and one angiofibroma. It was discussed that melanoacanthoma would be considered as the one of the associated beniign skin tumors in xeroderma pigmentosum.

9.A Case of Generalized Pustular Psoriasis in an Infant Trated with Etretinate.

We present herein a case of generalized pustular psoriasis in a 17-month-old female infant, who showed good response to etretinate therapy. The baby showed generalized pustular eruptions with high fever and marked leukocytosis preceded by annular scaly erythematous and annular pustular lesions. Etretinate was given in a dose of 10mg(0.75mg/kg/day) for 10 days, and then tapered to 7mg, 5mg, and 4mg in every 10 to 13 days. Clinical improvement was noted after 7 days' treatment and almost all of the skin lesions including erythroderma were cleared after 6 weeks' treatment. She is now maintained with 3mg of etretinate without recurrence.
