1.A Case of Neurofibromatosis with Unusual Giant Pigmentation.

We have experienced a case of neurofibromatosis with unusual giant pigmentation, which presents itself as a 28 cm x 40 cm sized, serrated bordered light brownish patch speckled with small dark hyperpigmented macules on his back. Histopathologic examinations of light brown patch and speckles showed only an increase in basal melanin pigmentation.

2.HLA Antigens, T and B Lymphocytes and other Associated Laboratory Findings in Patients with Vitiligo.

Although vitiligo is a common disorder with well-known clinical features, its etiology is still vague. It has been suggested that immunologic factors are important in the pathophysiology of vitiligo. And the association of HLA antigens with vitiligo has been reported. Authors examined HLA phenotypes in thirty-nine patients, T lymphocytes in thirty- three patients, B lymphocytes in thirty-one patients, and quantitation of IgG, IgA, IgM and IEP(immunoelectrophoresis) in twenty-five patients with vitiligo in this study. T3 and T4 were investigated in twelve patients, CBC in thirty-nine and urinalysis in thirty-five patients with vitiligo for screening of associated diseases ; thyroid disease, anemia and diabetes mellitus. The results are as follows; The frequency of HLA A9, Bw15 and Bw22 was significantly decreased in vitiligo patients compared to control group(p=0. 003, 0. 02, 0. OR) 2. There were no significant differences in the results of total and active lymphocytes and B lymphocytes in vitiligo patients compared to the control group. The results of quantitation of IgG, IgA, IgM and IEP showed within normal lirnits except one, an increased IgG, 2,200rng/100ml (normal: 500 1,500) and polyclonal gammopathy. 4. Among twelve vitiligo patients, one showed increased TR, 355ng/100ml (normal: 80 220) and T4, 17. 3pg/100ml (normal: 5 13). 5. CBC and urinalysis were within normal limits in all patients.

3.Huge Peripelvic Lymphatic Cyst: A Case Report.

Peripelvic lymphatic cyst or the kidney is a rare case in Korean Urological literature. We have experienced a case of it in 30 years old woman who had a palpable fluctuant mass in the upper right quadrant of the abdomen. The operated cyst was dark-red colored appearance and filled with amber colored fluid about 800cc. We have removed it together with kidney. The cyst measured 200gm. in weight, 10 x 5 x 4.5 cm in size and 0.5cm thickness of the cystic wall. Histological finding of the cystic wall showed a consisting of endometrial cell underlying tissue was collagen fiber and fibrotic change. The fluid of cyst contained 98 % of lymphatic cell.

4.Paraganglioma of Cauda Equina.

This case report describes a paraganglioma of the cauda equina in a 37-year-old man, as documented by light microscopy and immunohistochemistry. The patient experienced low back pain of 3 years duration, with the recent onset of sciatic pain and altered sensation in the right leg. Magnetic resonance imaging of L4 vertebral level revealed an ovoid, solid mass in the cauda equina. The mass was measured 1.5 cm in the greatest diameter. The histologic appearance was characterized by organoid pattern with clusters of chief cells (zellballen). Immunohistochemically, tumor cells are positive for keratin, epithelial membrane antigen, vimentin, neuron specific enolase and chromogranin.

For making a diagnosis of bladder Ca. the comparative study was performed on 12 cases with bladder washings and urine in cytology by the routine papanicolaou stins, and all of those were con firmed by cystoscopic examination. Results are as follows: 1. Bladder washing Cytology showed better results than urine cytology in 8 of 12 patients. 2. Of those cases, the significant cases were 83% on bladder washing cytology and 25% on urine of bladder cancer. 3. Neoplastic cells were found in 10 cases of the bladder washings, instead of 4 cases of bladder urine.

7.The Experimental Study on Corneocytes Chronically Irritated Skin (II).

An experimental study concerning the effect of chronic irritation of corneocytes was made in relation to their number, size and shape. The desquamating portion of the stratum corneum was sampled with the detergent scrub technique using Triton X-100. The experimental subjects were scrub nurses who had worked in the operating room for more than 3 years and ward nurses were used as a control group. The corneocytes of skin irritated by daily scrubbing differed, from those of the non-irritated skin of the ward nurses. About twice as many cells were collected per cm2 skin surface from the scrub nurses on the first experimental day. Two and four days later the number was markedly decreased and became similar to that of the control group. The surface of the corneocytes was 15% smaller in the experimental group than that of the control group, through out the experiment. There was no significant difference between the two groups as regards corneocyte morphology.

8.Epidemiologic Study on Urolithiasis in Korea.

A clinical and epidemiologic study was performed on urolithiasis in Korea. The subjected cases are all of the patients whom visited to Department of Urology, College of Medicine, Chung Ang University Hospital for 3 years. The emphasis of this study laid on regional distribution of urolithiasis or climate and attitude of working activity, so on. The following results were obtained: 1. The urolithiasis were found to be 216 cases (4.0%) in the all of the out-patients(5382 cases) for 3 years(1976-1979) 2. Age distribution was between 6 and 82 years old, shows the highest incidence in 2l to 50 years old(81.9%). 3. In 216 cases of out-patients, 159 cases were male and 57 cases in female with ratio of 2.8:1. 4. Specific location of the stones showed 75% in ureter, 18.5% in kidney, 5.5% in bladder, and other are 2 cases of urethra and prostatic stone. 5. The ureteral stone was located most frequently in the lower 1/3 of the ureter with 69.5% all ureteral stones. 6. The seasonal distribution of urolithiasis in Korea was found 33.8% in the summer season which is around July. 7. Urinary calculi in this study were much more likely to found individuals who have sedentary occupation of 30.5% of all cases 8. The highest incidence of urolithiasis in Korea indicated the province of Jeon Buk (14.3%), next place is the province of Kyung Buk and Kyung Hee, like this order.

9.Necrotizing Vasculitis of the Gallbladder: A case report.

We report a case of necrotizing arteritis involving the gallbladder. This case was clinically diagnosed as cholelithiasis with cholecystitis, and necrotizing arteritis was found in the surgically resected specimen. Vascular changes were similar to those seen in classic polyarteritis nodosa, involving medium-sized muscular arteries and characterized by fibrinoid necrosis and panarterial and periarterial inflammation varying from active to resolving stages. Acute cholecystitis is a rare initial clinical manifestation of the systemic vasculitis. If acute cholecystitis is found in the absence of obvious cause, careful examination is essential. Since steroid therapy improves the prognosis in the systemic vasculitis, clinicians and pathologists should be aware of this unusual lesion.

10.Eletronmicroscopic Study of Generalized Syringoma.

Syringoma is a benign adenoma which usually appear during puberty or adolescence. The lesions are limited to the lower eyelid but may occur on the chest, axillae, abdomen, neck, anterior chest and vulva or be generalized. We recently had an opportunity to observe a 42 year-old woman with generalized syringoma. In histopathologic study of the skin lesion, the numerous cystic and tubular ducts were showed on upper dermis and mid-dermis. Large dilated cysts were usually located in the upper derrnis. A comma-like excrescence similar to tadpoles and solid epithelial cord or ductal structure were showed. In electromicroscopic study, the luminal border of the inner cells was lined with numerous short microvilli and the cytoplasrn showed a periluminal filamment, keratohyaline granules, multivesiculated dense bodies. The outer cells had elongated nucleic and glycogen rich cytoplasm. The cells were connected by well developed desmoaome. The lumina. contained numerous small vesicles and a few multivesiculated bodies. According to these results, this case showed the similar findings of imrnature intraepidermal eccrine sweat duct in electronmicroscopic study.
