1.Growth Hormone Therapy in Short Stature Children.

Since 1985 recombinant DNA-derived human growth hormone (hGH) became available for treatment of various growth disorders in children. GH stimulates linear growth in children with growth hormone deficiency (GHD) and also demonstrates efficacy in the treatment of Turner syndrome, chronic renal failure, Prader-Willi syndrome, and growth failure secondary to small for gestational age (SGA). Although recently FDA approved GH therapy for idiopathic short stature, the effect of GH treatment remains unclear. GH therapy is generally safe but GH has potential side effects. The decision of GH therapy in idiopathic short stature should be made individualized under consideration of the efficacy, cost-effectiveness, and psychosocial aspects, and GH therapy must be carried out by pediatric endocrinologists or experienced physicians.

2.HLA Antigens, T and B Lymphocytes and other Associated Laboratory Findings in Patients with Vitiligo.

Although vitiligo is a common disorder with well-known clinical features, its etiology is still vague. It has been suggested that immunologic factors are important in the pathophysiology of vitiligo. And the association of HLA antigens with vitiligo has been reported. Authors examined HLA phenotypes in thirty-nine patients, T lymphocytes in thirty- three patients, B lymphocytes in thirty-one patients, and quantitation of IgG, IgA, IgM and IEP(immunoelectrophoresis) in twenty-five patients with vitiligo in this study. T3 and T4 were investigated in twelve patients, CBC in thirty-nine and urinalysis in thirty-five patients with vitiligo for screening of associated diseases ; thyroid disease, anemia and diabetes mellitus. The results are as follows; The frequency of HLA A9, Bw15 and Bw22 was significantly decreased in vitiligo patients compared to control group(p=0. 003, 0. 02, 0. OR) 2. There were no significant differences in the results of total and active lymphocytes and B lymphocytes in vitiligo patients compared to the control group. The results of quantitation of IgG, IgA, IgM and IEP showed within normal lirnits except one, an increased IgG, 2,200rng/100ml (normal: 500 1,500) and polyclonal gammopathy. 4. Among twelve vitiligo patients, one showed increased TR, 355ng/100ml (normal: 80 220) and T4, 17. 3pg/100ml (normal: 5 13). 5. CBC and urinalysis were within normal limits in all patients.

3.A Case of Dissolution of Barium-impaction by Gastrografin.

Ileus caused by impaction of hardened residual barium stagnated in the colon, a rare complication of barium-enema examination, constitutes a particularly serious problem when it occurs in congenital megacolon patients. The administration of gastrografin in such case has proven efficacy in dissolution of impacted barium refratory to routine means of evacuation. We experienced a case of barium-impaction occurred in 6-months old male who had congenital megacolon. Gastrografin enema was a safe and simple method used in the management of this case.

4.Two Cases of Diabetic Hyperglycemic Hyperosmolar Coma Treated with Maintaining CAPD in Chronic Renal Failure Patients with CAPD.

We reported two cases of diabetic hyperglycemic hyperosmolar coma treated with maintaining continuous ambulatory peritoneal dialysis(CAPD) in chronic renal failure(CRF) patients with CAPD. Hyperglycemia is common complication in CAPD, but there were only few reports of severe hyperglycemia accompanying with hyperosmolar coma in CRF patients on CAPD therapy. Furthermore, to date, no specific management for CAPD patients with nonketotic hyperosmolar coma has been established. The best strategy for management of hyperosmolar coma with CAPD patients is changing CAPD therapy to HD. Recently, we experienced two cases of hyperglycemic hyperosmolar coma in diabetic CAPD patients successfully treated with intravenous hydration and insulin therapy without discontinuing CAPD therapy. Because no such case has been reported, we report two cases of diabetic hyperosmolar coma with CAPD treated without conversion to HD. These cases suggest that adequate fluid replacement accompanying electrolytes and osmotic balance as well as insulin therapy might be a major role for safe treatment of hyperglycemic hyperosmolar coma in patient with CAPD.

6.Insufficiency Stress Fractures of the Femoral Neck in Elderly Patients.

PURPOSE: We wanted to compare the insufficiency stress fractures of the femoral neck (group I) with the traumatic femoral neck fractures (group II) in patients who were 70 years of age and older. MATERIALS AND METHODS: Between January 2000 and October 2006, we evaluated 10 insufficiency stress fractures among 191 femoral neck fractures in patients who were 70 years of age and older. We compared these fractures with the traumatic femoral neck fractures by using the bone mineral density (BMD), neck-shaft angle, the hip axis length (HAL) and the ratio of the HAL to the femoral neck width. RESULTS: The incidence of insufficiency stress fracture was 5.2%. There were 6 cases of displaced fractures and 4 cases of non-displaced fractures. All of non-displaced fractures revealed the tension (transverse) type. The mean neck-shaft angle was 130.45 degrees in group I and this was 131.94 degrees in group II. The mean HAL was 117.6 mm in group I and 115.3 mm in group II, and the ratio of the HAL to the femoral neck width was 0.30 in each group. The BMD (T-score) was -3.73 in group I and -3.4 in group II. CONCLUSION: The BMD of the insufficiency fracture group was significantly lower than that of the traumatic femoral neck fracture group. However, there were no significant differences in the neck-shaft angle, the HAL and the ratio of the HAL to femoral the neck width between the 2 groups.

Since 2013, the Hospital-based Influenza Morbidity and Mortality (HIMM) surveillance system began a H7N9 influenza surveillance scheme for returning travelers in addition to pre-existing emergency room (ER)-based influenza-like illness (ILI) surveillance and severe acute respiratory infection (SARI) surveillance. Although limited to eastern China, avian A/H7N9 influenza virus is considered to have the highest pandemic potential among currently circulating influenza viruses. During the study period between October 1st, 2013 and April 30th, 2016, 11 cases presented with ILI within seven days of travel return. These patients visited China, Hong Kong, or neighboring Southeast Asian countries, but none of them visited a livestock market. Seasonal influenza virus (54.5%, 6 among 11) was the most common cause of ILI among returning travelers, and avian A/H7N9 influenza virus was not detected during the study period.
