Fracture of capitellum humeri, so called Kocher fracture, was first described by Haln in 1853. It is rare and has been occured 0.5% to 1% of incidence of all elbow fracture. The mechanism of this fracture mostly results when one tries to break a fall and lands on the hand with the elbow in some degree of flexing or falls directly on the elbow in a position of full flexion. 2.A Case of Malignant Atrophic Papulosis.

A 15-year-old female patient had malignant atrophic papulosis(Degos' disease) that affected the skin and probably intestinal tract. She had multiple erythematous papules with porcelain-white atrophic centers on the trunk and extremities. Also she suffered from intermittent nausea, vomiting and abdominal pain. She received appendectomy for abdominal pain 2 months ago. Histopathologic studies showed atrophic epidermis and necrobiotic dermis. There were endothelial thickening, fibrinoid necrosis of bload vessels, perivascular lymphocytic infiltation in the lower dermis. She was treatd with acetyl salicylic acid and dipyridamole, but there was no improvement.

3.Experimental proliferative vitreoretinopathy.

The authors performed experimental study to produce proliferative vitreoretinopathy by intravitreal injection of homologous fibroblasts in pigmented rabbits. Twenty four eyes of 12 rabbits were divided into control and experimental groups. In experimental group, the eyes were pretreated with expanding gas, perfluoropropane for the vitrectomy. In control group, no such procedure was preceded. In both groups, 5.0×10⁴, 1.0×10⁵ and 1.5×10⁵ cells of homologous fibroblasts were injected into the vitreous cavity. All the eyes were observed regularly with indirect ophthalmoscope followed by fundus photography. Those eyes were enucleated in 1, 2, 4 weeks for the light and electron microscopic studies. The development and severity of the proliferative vitreoretinopathy were correlated with the numbers of the injected fibroblast. This was more pronounced in the experimental group.

4.Intraocular Pressure Changes in the Uneventful Extra-capsular Cataract Extraction and Extra-capsular Cataract Extraction with Vitreous Loss followed by Anterior Chamber Lens Implantation.

These days the posterior chamber lens implantation following the extracapsular cataract extraction has become the standard procedure except those patients who cannot take it in one reason or another. In past, however, the anterior chamber lens implantation was popular for sometimes. Even now in those patient complicated by vitreous loss, the anterior chamber lens implantation following anterior vitrectomy is indicated in some cases. We have become interested in the difference of the intraocular pressure change in the patients who underwent the uneventful extra-capsular cataract extraction followed by anterior chamber lens implantation in routine manner and in those whose extra-capsular cataract extraction were complicated by vitreous loss and eventually needed the anterior vitrectomy before the anterior chamber lens implantation. We reviewed the medical records of 15 cases of the former group and 11 cases of the latter one in 1986 through 1988. There was some difference in the postoperative pressure in the two groups, but it was not statistically significant.

5.Two Cases of Pigmentary Demarcation Lines Associated with Pregnancy.

Pigmentary demarcation lines are borders of abrupt transition between more deeply pigmented skin and that of lighter pigmentation. We describe two patients with pigmentary demarcation lines associated with pregnancy symrnetrially involving the buttocks and flexor as jects of legs. Hormonal changes in pregnancy and compression by the enlarged uterus of Sl and S2 peripheral nerves may be responsible for these changes.

6.Clinical observation of the bilateral branch vein occlusion.

The clinical evaluation was done in eight eyes of four patients with bilateral branch retinal vein occlusion. The onset of age was from 36 to 65 years, and three pa dents were women. Hypertension was the most common associated systemic disease. Retinal neovascularization was complicated in six eyes, and five eyes had vitreous hemorrhage. The vitrectomy was performed in three eyes. We believe that branch retinal vein occlusion associated with hypertension should be followed carefully in the fellow eyes.

7.A Study on the Correlation between Pain and Psychological Stress in Endometriosis patients.

OBJECTIVE: The patients of endometriosis may complain of dysmenorrhea, pelvic pain, and dyspareunia, but may be asymptomatic. We studied on the correlation between the pain and the psychologic stress, Also, we studied on the correlation between serum CA-l25 levels and the psychologic stress. METHODS: The data was obtained from il7 endometriosis patients. Psychological stress was studied with STAI(Stait-Trait Anxiety Inventory) and BDI(Beck Depression Inventory). We compared the degree of anxiety and depression according to existence of pain and the duration of pain. Also we examed the scores of STAI and BDI according to the level of serum CA-125. RESULTS: The pain group had more depression and anxiety, in comparison to the pain-free group. And the depression and anxiety significantly increased in long-term pain group. There was no correlation between the level of serum CA-125 and the depression,or axiety. CONCLUSION: The pain of endometriosis may play a role of pychological stress to the patients, so it seems to increase depression and anxiety.

8.A Case of Buschke-Lowenstein Tumor in Renal Transplant Recipient.

Buschke-Lowenstein tumor is a rare disease in the category of designated as verrucous carcinoma characterized by its invasive downward penetration of underlying tissues in the perineum and perianal regions. Viruses, unclean sanitation and cytotoxic immune reaction have been proposed as the etiology of the tumor. However, among all the causes, recent studies have emphasized on the associaton of the tumor and human papilloma virus (HPV). Expecially, HPV also has been discovered in several cutaneous and anogenital lesions of solid organ transplant recipients. We herein report a case of Buschke-Lowenstein tumor in a renal transplant recipient with HPV 6 and 16 coinfection proved by HPV genotyping of DNA extracted from the biopsy specimen of the tumor.

9.Whitening Effect of Cosmetics Containing Magnesium L-Ascorbyl-2-Phosphate(VC-PMG, Vitamin C Derivatives) Assessed by Colorimeter.

BACKGROUNDS: An inhibitory effect of magnesium L-ascorbyl-2-phosphate (VC-PMG, a stable derivative of ascorbic acid) on melanogenesis has been described. Furthermore, glabridin in licorice is known to have inhibitory effects on melanogenesis and widely used for raw materials for depigmenting agents. OBJECTIVE: The purposes of this study are to provide objective data by measuring the visual clinical effects of VC-PMG with the colorimeter and to promote the development of de-pigmenting agents. METHODS: 20 volunteers joined the study. With an artificial UVB irradiation, eight tanned areas were made on the inner side of the forearm. During two months, each tanned area was treated with five whitening cosmetics with 3% VC-PMG and increasing concentration of licorice from 0% to 3%. Darkness degree of each area was measured weekly by the colorimeter and the visual assessment. RESULTS: For all cosmetics, whitening effect was measured by colorimeter and visual as-sessment. The cosmetic containing VC-PMG 3% and licorice 1% had more whitening effect than any other cosmetics of different concentrations. Moreover, VC-PMG 3% alone also had whitening effect in some volunteers. CONCLUSION: VC-PMG was clinically found to have whitening effect.
