1.A Study on the Correlation between Pain and Psychological Stress in Endometriosis patients.

OBJECTIVE: The patients of endometriosis may complain of dysmenorrhea, pelvic pain, and dyspareunia, but may be asymptomatic. We studied on the correlation between the pain and the psychologic stress, Also, we studied on the correlation between serum CA-l25 levels and the psychologic stress. METHODS: The data was obtained from il7 endometriosis patients. Psychological stress was studied with STAI(Stait-Trait Anxiety Inventory) and BDI(Beck Depression Inventory). We compared the degree of anxiety and depression according to existence of pain and the duration of pain. Also we examed the scores of STAI and BDI according to the level of serum CA-125. RESULTS: The pain group had more depression and anxiety, in comparison to the pain-free group. And the depression and anxiety significantly increased in long-term pain group. There was no correlation between the level of serum CA-125 and the depression,or axiety. CONCLUSION: The pain of endometriosis may play a role of pychological stress to the patients, so it seems to increase depression and anxiety.

2.Vitiliginous Hypopigmented Patches Developed during Photochemotherapy ( PUVA ) in a patient with Psoriasis.

We observed a 51-year-old male, who developed depigmented patches on the lower abdomen after photochemotherapy(PUVA) of psoriasis without evidence of phototoxic reaction. The depigmented patches did not orginate from the sites of previous psoriatic lesions. Fontana-Masson stain of the biopsy specimen revealed nearly absent melanin pigments in the epidrmis of the hypopigmented patch, while lots of pigments were obsereved on the surrounding hyperpigmented area. Electron micrograph showed no melanocytes in the hypopigmented patch. Thus we report this case as having vitiliginous patches induced by photochemotherapy of psoriasis.

3.A Case of Malignant Atrophic Papulosis.

A 15-year-old female patient had malignant atrophic papulosis(Degos' disease) that affected the skin and probably intestinal tract. She had multiple erythematous papules with porcelain-white atrophic centers on the trunk and extremities. Also she suffered from intermittent nausea, vomiting and abdominal pain. She received appendectomy for abdominal pain 2 months ago. Histopathologic studies showed atrophic epidermis and necrobiotic dermis. There were endothelial thickening, fibrinoid necrosis of bload vessels, perivascular lymphocytic infiltation in the lower dermis. She was treatd with acetyl salicylic acid and dipyridamole, but there was no improvement.

4.Bone Marrow Pressure of the Femoral Heads of Korean Adults

Bone marrow pressure is neither arterial pressure nor venous pressure of extremity, that is thought the precapillary pressure of bone. It is different between long bone and flat bone, and is also different in various site of the bone. Author measured bone marrow pressure of 84 normal femoral heads of Korean adults for basic data of early diagnosis of avascular necrosis of femoral head. The results are followings : 1. The baseline bone marrow pressure of Korean healthy adults is average 22.5 ±7.2mmHg. 2. The most significant change of bone marrow pressure in stress test is observed at 5 min. after injection of 5cc normal saline. 3. The increase of bone marrow pressure at 5 min. after stress test was average 7.4 ±4.4 mmHg. 4. Sex and age factors do not influence bone marrow pressure.

7.MRI Study about the Early Changes of Lumbar Disk Degeneration using Magnetization Transfer Contrast (MTC).

PURPOSE: To obtain magnetization transfer ratio(MTR) of the annulus fibrosus and nucleus pulposus and to assess the feasibility of utilizing the changes of these MTRs as an early indicator of disk degeneration. MATERIALS AND METHODS: MR images of lumbar spine with magnetization transfer(MT) technique in 42 patients were obtained. spin echo techniques (600/14) with same TR/TE with 1KHz off-resonance saturation were employed in 1.0T MR system. MTRs were calculated in two regions, anterior annulus fibrosus and nucleus pulposus, and the results were compared between the normal and degenerative disks, from grade I to IV, on T2-weighted images. RESULTS: MTRs of the nucleus pulposus were 17.6% in the normal disks, and 26.7%, 28.4%, 29.1%, 29.7% in degenerative disks, from grade I to IV, respectively, with a significant difference(P<0.05). On the other hand, MTRs in the annulus fibrosus were 30.2% in the normal disks and 31.5%, 33.2%, 32.1% and 35.6% in degenerative disks, from grade I to IV, respectively, without significant difference. CONCLUSION: Since MTRs are significantly higher in degenerative nucleus pulposus than those of the normal disks, increased MTRs in the nucleus pulposus can be used as an early sign of the degeneration of the nucleus pulposus.

9.Two Cases of Proliferating Trichilemmal Tumors Occurring in Unusual Sites.

Proliferating tumor is one of rare benign tumors of skin appendage. It is usually solitary, more common in women after the fourth decade of life, and is almost exclusively confined to the scalp and back of the neck. We report herein two cases of proliferating trichilemmal tumor which occurred not on the scalp but on unusual sites. In the first case a 28 year-old woman had a pedunculated reddish mass with verrucous surface measuring 1.5cm in diameter in the opening site of the left external auditory canal. Shave biopsy and electrodesiccation was done for the purpose of histopathologic study and treatment. In the second case a 13 year-old girl had a dome-shaped pink colored papule measuring 0.3cm in diameter on the left elbow joint area. Excision biopsy was done. No recurrence was observed during he follow up period of 28and 12 months respectively in both cases.

10.Artifacts in MR imaging caused by air and fat: an experimental study.

To investigate the nature of artifacts caused by air and chemical shift by fat, a test tube phantom study was performed in 2.0T and 0.5T superconducting magnetic resonance imaging systems in various imaging sequences. Spatial mismapping and signal change made image distortion in spin echo images of the air-column and severe signal voids in gradient echo images. Chemical shift artifacts appeared in the planes of frequency-encoding axis as crescent-shaped signal voids and summated bright signals, Both artifacts were more prominent in high field strength, Images at 0.5T showed more severe artifacts in the T2-weighted images than those in the other sequences. As the acquisition matrix increased, truncation artifacts decreased and susceptibility artifacts in gradient echo images diminished. Familiarity with causes, appearances and mechanisms of various artifacts will help radiologists for interpretation of MRI.
