Hematogenous contact dermatitis may be produced by the systemic administrationof a drug to an individeal who has been previously sensitized by the topical application of the drug or related chemicals. 37 year-old male patient had frequent rhus dermatitis. He took rhus with chicken for treatment of peptic ulcer, on next day generalized skin rashes with itching sensation were developed. the aptient was diagnosed as hematogenous contact dermatitis or systemic exzematous contact dermatitis according to the history, symptoms and the result of patch test.

4.Thumb Reconstruction with a Free Neurovascular Wrap

On the length, strength, free lateral motion, and perfect mobility of the thumb, depends the power of the human hand. The thumb is called pollex because of its strength and that strength is necessary to the power of the hand being equal to that of all the fingers."So stated Sir Chales Bell in the fourth Bridgewater Treatise, first published in 1833. Morrison and O'Brien advocated reconstruction of the thumb with a free wrap around flap from the big toe to recreate a stable, sensate and functional digit including the nail in 1980. From March, 1982 to July, 1988, Twelve thumb reconstructions were performed using the wrap around procedure at Hanyang University Hospital;12 successful thumb reconstructions were reviewed at an average of 16 months after surgery. There were several complications;these included skin necrosis(4), malunion(1), resorption of the bone graft(7). We considered that cosmesis and function were good;pinch grip averaged 68% of normal, sensibility returned in all patients as assessed by two-point discrimination(average 9.5mm). Over all the results are as follows. 1. The wrap around technic provides a good method of reconstruction. 2. The wrap around technic is one stage operation and can be used in the level of metacarpal amputation. 3. The surgeon should be familiar with microsurgical technic.

5.A Case of Mauriac Syndrome.

In 1930s, Mauriac described a syndrome in diabetic children consisting of stunted growth, hepatomegaly, and delayed puberty. This syndrome was related to poorly controlled diabetes of long duration. A 14-year-old girl, who had been diagnosed as insulin dependent diabetes mellitus three years ago but had not been well controlled for diabetes, visited to hospital because of short stature, delayed puberty, and visual disturbance. On physical exam her height was 146cm(<3 percentile) and her weight was 34kg(<3 percentile) and sexual maturation was delayed as Tanner stage I. The liver was 3FB palpable below the right subcostal margin. She had hyperglycemia and elevated plasma cortisol. She was diagnosed as Mauriac syndrome. On ophthalmologic examination, the cataracts were observed on both eyes and she had also peripheral neuropathy on lower extremities. Her blood glucose was controlled strictly by regular insulin during admission, and her weight was increased and hepatomegaly was improved at discharge, two months later. We report a case of Mauriac syndrome with a brief review and related literatures.

6.A Case of Pemphigus Vulgaris Localized to the Oral Mucosa and Esophagus.

We recently encountered a case of pemphigus vulgaris, which was localized within the oral mucosa and esophagus. The patient was a 62-year-old women, who had had a few recurrent erosions on the oral mucosa for 1 year. Newly developed erythematous erosions on the esophagus appeared 7 days pre previously. Histopathological findings showed suprabasal blisters in the epithelium of the buccal and esophageal mucosa. On direct immunofluorescence, intercellular IgG and C3 bindings were found in the epithelium of the buccal mucosa. Treatment with oral steroids resulted in a remission of the lesions and the other skin lesions were not found for a period of 6 months.

7.A Predictive Index of Left Ventricular Performance after Mitral Valve Replacement in Pure Mitral Regurgitation.

The index on myocardial contractility of pure mitral regurgitation(MR) after mitral valve replacement is believed to be useful in determining proper operation time before irreversible myocardial damage by volume overload. Thus the authors examined pre and post-operative echocardiographic results of 20 cases of pure MR patients who had been admitted to Pusan National University hospital and compared the usefulness of each index. Pre and post-operative echocardiographic results were as follows respectively ; 1) Left ventricular internal dimension in diastole(LVIDd) were 6.49+/-0.19cm and 5.51+/-0.17cm. 2) Left ventricular internal dimension in systole(LVIDs) were 4.26+/-0.12 and 3.79+/-0.18cm. 3) Ejection fraction were 71.28+/-1.57% and 59.24+/-3.05%. 4) Fractional shortening(FS) were 34.09+/-1.18% and 27.21+/-1.84%. 5) Mean Vcf were 1.53+/-0.08cm/sec and 1.18+/-0.09cm/sec. 6) Left ventricular end-diastolic volume index(EDVI) were 196.35+/-18.33cc/m2 and 98.46+/-9.96cc/m2. 7) Left ventricular end-systolic volume index(ESVI) were 55.28+/-5.12cc/m2 and 41.88+/-6.07cc/m2. 8) Left ventricular end-systolic wall stress/ESVI(ESS/ESVI) were 2.3+/-0.21 and 3.91+/-0.83. ESS/ESVI showed significantly(p<0.05) elevated postoperative change, LVIDd, LVIDs, EF, FS, EDVI, ESVI revealed significantly(p<0.05) reduced postoperative change whereas Mean Vcf had no postoperative change. After examining correlation between preoperative and postoperative results of each index, it is believed that LVIDd, LVIDs, and ESS/ESVI were useful prognostic indices but were unrelated to postoperative ejection phase indices such as EF, FS, and MEan Vcf. Especially in case of LVIDd>6cm, LVIDs>4cm, and ESS/ESVI<2, the prognosis was poor due to myocardial damage by mitral regurgitation.

10.A Histopathologic Studies for Endometrium of Early Pregnancy.

Endometrium of early pregnancy were compared with nonpregnant endometnum by inimunohistochemical and ultrastructural techniques with respect to glandular and stromal elements of endometrium. The results obtained were as follows: 1. The AS cell in decidua has all the features of actively secreting glandular epithelium and shows the pronounced arrays of glandular endoplasmic reticulum and moderate numbers of ribosomes ultrastructually. Therefore the AS cell indicate considerable protein production, presumably contributing to both cell gowth and the production of secretions. 2. The process of decidualization can be characterized morphologically and immunohistochemically by the accumulation of basement membrane-like materials, such as laminin and type IV collagen which may be related to the hormonal stimulation occuring during pregnancy and trophoblastic attachment. 3. The decidual cells show strong positive for vimentin and some large mature decidual cells show weakly positive for lysozyme and cti- antitrypsin, which might represent more the sequential differentiation of stromal cells into decidual cells than origin of histiocytes. 4. Immunoreactivity with S-100 protein was found in glandular and stromal cells of decidua but negative in endometrium of nonpregnant women. So some humoral factors related to pregnancy stimulate expression of S-100 protein in glandular and stromal cells of decidua.
