1.Alterations of Insulin-like Growth Factor-I mRNA and IGF Binding Protein-5 mRNA Expression Following Hypoxic-Ischemic Brain Injury in Immature Rat.

PURPOSE:Insulin-like growth factor-I(IGF-I) is an essential anabolic factor for postnatal rat brain development and IGF-I expression is highly abundant during the first 21 days, critical growth period. Hypoxic-ischemic brain insults occurring during the perinatal period result in neuronal necrosis and permanent brain damage. To investigate the regulation of the action of IGF-I in response to such a hypoxic insult, we examined the gene expression of IGF-I and IGFBP-5 during the first 72 hr after hypoxic-ischemic injury in immature rat brain. METHODS:Ligation of the right carotid artery of 7-day-old rats was followed by 2 hour exposure to 8% oxygen to produce severe hypoxic brain damage. Using reverse transcriptase-polymerase chain reaction(RT-PCR), the expression of IGF-I mRNA and IGFBP-5 mRNA was determined in both hypoxic and control brains at post 1, 4, 12, 24, 48 hr and 72 hr after hypoxic-ischemic insult. RESULTS:The IGF-I mRNA and IGFBP-5 mRNA expression of hypoxic brain were not different from those of controls at 1 hr of recovery but IGF-I mRNA expression was decreased rapidly at post 4 hr, this decrease more pronounced at 12 hr of recovery. IGF-I mRNA and IGFBP-5 mRNA expression were increased at 48 hr and 24 hr of recovery, respectively and both IGF-I mRNA and IGFBP-5 mRNA expression showed similar level of controls at 72 hr of recovery. CONCLUSION: Out findings suggest that IGF-I play a important role in both neuronal loss and repair process following hypoxic-ischemic brain injury and IGFBP-5 is also strongly involved in the repair of damaged brain tissue by mediating IGF-I action. 6.Perinatal Factors Affecting Thyroid Stimulating Hormone(TSH) and Thyroid Hormone Levels in Cord Blood.

PURPOSE: The aim of the this study was to evaluate the effect of various perinatal conditions on TSH and thyroid hormone levels in cord blood. METHODS: Cord blood samples were collected from 130 neonates immediately after birth. TSH, T3, and free T4 levels were measured by the radioimmunoassay(RIA) method. The effects of gestational age, sex, birth weight, delivery method, perinatal asphyxia, maternal diabetes mellitus(DM), and preeclampsia on TSH and thyroid hormone levels were assessed by ANOVA test, Student t-test, and multiple regression analysis. RESULTS: Birth weight and sex did not affect TSH and thyroid hormone levels. TSH level increased according to gestational age(P<0.05). TSH level was 4.42+/-0.66 microIU/mL in infants born vaginally, which was higher than that of cesarian section delivery(3.31+/-0.33 microIU/mL)(P<0.05). TSH level was 5.18+/-0.93 microIU/mL in asphyxiated newborns and 2.97+/-0.84 microIU/mL in non-asphyxiated newborns(P< 0.05). TSH level in infants with maternal DM(8.911+/-1.25 microIU/mL) was higher than that of infants without maternal DM(4.32+/-0.42 microIU/mL)(P<0.05). TSH level was 5.28+/-0.42 microIU/mL in infants with maternal preeclampsia and 3.65+/-0.46 microIU/mL in infants without maternal preeclampsia(P<0.05). Thyroid hormones were lower in infants with perinatal asphyxia(P<0.05). In asphyxiated infants, T3 level was 75.33+/-55.65 ng/mL and free T4 was 0.54+/-0.21 ng/mL. T3 and free T4 level was 109.85+/-41.77 ng/mL and 0.76+/-0.22 ng/mL each in infants without perinatal asphyxia. Among the perinatal factors, gestational age, 1 min Apgar score and maternal DM influenced TSH level independently. CONCLUSION: In our study, cord blood TSH and thyroid hormone levels were affected by perinatal stress events.

7.Two Cases of Plexiform Neurilemmoma.

Plexiform neurilemmoma is a relatively rare, benign peripheral nerve sheath tumor, whieh usually arises in either the dermis or subcutaneous tissue. These tumors may occur singly or as multiple lesions (plexiform neurilemmomatosis), We report an unusual case of plexiform neurilemmomatosis associated with cafe au lait spots reminiscent of neurofibromatosis clinically and another case of plexiform neurilemmoma on the finger. A Biopsy revealed the characteristic palisaded arrangement of spindle cells within well circumscribed elongated nodules, The skin lesions were completely excised without recurrence thereafter.

8.Evaluation of the Ez step FOBTM for Fecal Occult Blood Test.

BACKGROUND: Fecal occult blood tests (FOBT) have been recommended for gastro- intestinal bleeding and colon cancer screening. This study compared the effectiveness of two fecal blood screening kit, OC-Hemodia II and Ez step FOB for fecal occult blood. METHODS: The detection limit was evaluated by using OC-control and serially diluted samples. The comparison study between OC-hemodia II and Ez step FOB were evaluated in 143 cases. RESULTS: The concordance rate between OC-Hemodia II and Ez step FOB was 85.6% and 50% of non-concordance cases have history related to gatrointestinal bleeding. Ez step FOB was possible to detect 35 ng/mL in serially diluted OC-control. CONCLUSIONS: The result of Ez step FOB satisfactory to clinical application and showed good concordance rate compared to OC-Hemodia II.
