1.A Case of Acute Central Stromal Melting after LASIK.

LASIK is an effective and exact procedure to correct myopic refractive error but fatal complication may be happened during making corneal flap using microkeratome. We report a patient who developed acute central stomal melting following LASIK on postoperative day 4. This complication was treated by lifting the flap and scraping the interface of corneal flap & central stroma with a metal blade. After 4 months, overcorrected hyperopic refractive error was retreated with excimer laser ablation and patient recovered successful visual acuity. The pathogenesis of this case is not completely understood. Corneal endothelium and central cornea stroma may be mechanically damaged due to excessive suction ring application during LASIK procedure. Interface epithelial ingrowth may be associated with early postoperative inflammation and infection of corneal flap may produce proteinase which melts corneal flap. If corneal flap melting happens after LASIK procedure, early surgical debridement of melting stroma and proper reattachment of the flap may be needed.

2.Effect of H2 - Receptor Antagonists on UVB - Suppressed Contact Hypersensitivity.

The effects of histamine type 2(H2) receptor antagonists, cimetidine and ranitidine, on the induction of contact hypersensitivity and UVB induced suppression of contact hypersensitivity were studied. Our experiment showed that treatment with cimetidine or ranitidine slightly enhanced contact hypersensitivity to DNCB, but the suppression of contact hypersensitivity elicited by UVB were moderatly reversed by ranitidine. These facts suggest that H2-receptor antagonist may be responsible for th blocking histamine induced suppressor factor by acting upon UV-induced suppressor T cell.

3.Three Cases of Fibrous Papule of the Face.

Fibrous papule of the face occurs nearly always as a solitary lesion on the face, especially on the nose. The lesion which appears as a dome shaped papule, is skin colored, pigmented or red, We present herein three cases of fibrous papule of the face, which developed on the ala nasi of 45-year-old man, and 49-year-old woman, and on the nostrile of 25-year-old woman. Histopathologic findings showed dermal angiofibrotic changes in two cases. In one of the two cases, there were atypical cells with large irregular nucleus and a few multinucleated giant cells interspersed in the edematous dermis, and there was distinct perifollicular fibrosis in the other case.

4.Two Cases of Backer' s Nevus.

Becker's nevus has been known as a sudden onset of hyperpigmentation on shoulder or arm in the second decade of life with slow peripheral extension and later development of hypertrichosis. We observed two cases of typical Becker's nevus which was confirmed by characteristicclinical and histopathological findings. One was 45-year-old male who has a patch of hyperpigmentation and hypertrichosis on left upper arm at birth, the other was 2()-year-old male who developed on right shoulder after sunburn which is a salient feature by Becker's initial description.

5.Clinical Results of Nd:Yag Laser Posterior Capsulotomy.

From Feb. 1987 to Feb. 1988, 93 cases of the Q-switched Nd : Yag laser posterior capsulotomy by using of choherent Model 7900 were carried out and 80 cases(76 patients)had been followed up for more than 2 months. The results were as follows: 1. 72 aphakic eyes and 8 pseudophakic eyes(12 traumatic cataract and 11 congenital cataract and 57 adult and senile cataract) were followed. 2. Postoperative visual acuity of 0.7 or better was achieved in 54 eyes(67.5%) and 1.0 or better in 32 eyes(40%). 3. Complications were transient increased IOP 17 eyes(21.2%) and Vitreous pro lapses with anterior hyaloid rupture 15 eyes (18.7%) and anterior uveitis 2 eyes(2.5%) and cystoid macular edema 1 eyes(1.2%) and corneal edema 1 eyes(1.2%) but most cases were without permanent damages.

7.A Case of Acute Graft-versus-Host Disease.

We report a case of acute graft-versus-host disease, which developed after bone marrow transplantation because of acute myelocytic leukemia in a 39-year old male, The pruritic, erythematous maculopapular eruptions began to developed on the perioral regions, and spreaded the face, the oral mucosa, both hands, and buttocks at the twenty fourth day after bone marrow transplanta.tion. The eruptions were confluent to form erythematous patches. Iistopathological findings show parakeratosis, lymphoid cell exocytosis, and papillary edema, lymphohistiocytic infiltration, and melanophage in the upper dermis, and basal vacuolation. He was treated systemically by methylprednisolone, and antilymphocytic globulin, and tapically by emollients and steroids.

8.A case of onychogryphosis associated with onychomycosis.

We present a case of onychogryphosis of both great toe nails associated with onychomycosis of all toe nails occurring in a 90-year-old female patient. She also had senile dementia. Affected nail plates were thickened, increased in length, and curved like the horn of a ram. The nail plates were removed surgically form the nail bed. Direct microscopic examination revealed many hyphae with potassium hydroxide. Trichophyton rnbrum was isolated from the culture in Sabouraud glucose agar media. On histopathologic examination, we found many hyphae in the nail plate. We suggest an onychomycosis as one of the causes of onychogryphosis.

9.A Case of Subcutaneous Granuloma Pyogenicum.

We report herein a case of subcutaneous granuloma pyogenicum developed on the fourth interdigital space of right foot of 36-year-old housewife. Histopathological findings showed characteristic lobular capillary hemangioma. The subcutaneous nodule had a thick fibrous capsule and were composed of multiple, confluent cellular, angiomatous lobules separated by loose fibrous tissue. Most lobules contained round, oval or angular capillaries, lined by proliferated prominant endothelial cells projecting into the lumina of the vessels. Clinically, the lesion was a tender subcutaneous nodule with macerated surface aind soft corn or traumatic epidermal cyst were considered in the differential diagnoses.

10.A Study of Demodex from Eyelashes.

Demodex is easily found from eyelashes and suggested as the one of the causes of some eye diseases. This study was to investigate the occurence rate, and classification of Demodex from eyelashes, and its relationship with blepharitis. The data of this study were compiled from 27 blepharitis patients and 173 patients without blepharitis (control group) who had visited St. Paul's Hospital from October 1981 to December 1981. Three eyelashes from each eyelid-total 12 lashes from a patient-were extracted under the slit lamp, located on a slide glass and fixed with polyvinyl lactophenol mounting medium. (countinued..)
