2.Nodular Pigmented Villonodular Synovitis of the Right Shoulder Joint: One Case Report

In 1941, Jaffe and coworkers studied a lesion with histologic appearance of fibrous stroma, pigmented deposition and histiocytic infiltration as well as giant cell, for which they named pigmented villonodular synovitis, bursitis and tenosynovitis. Thereafter many authors have discussed etiology, clinical and radiological features, pathology and treatment regarding the disease. Almost all, this disease is monoarticular and knee joint is most common site. We experienced a case of localized pigmented villonodular synovitis involving an shoulder joint which was treated by local excision and obtained good result.

Fracture of capitellum humeri, so called Kocher fracture, was first described by Haln in 1853. It is rare and has been occured 0.5% to 1% of incidence of all elbow fracture. 4.Trichorrhexis Nodosa: report of a case and Scanning Electron Microscopic Findings.

Trichorrhexis nodosa is a distinctive response of the hair shaft to physical or chemical trauma, characterized by the development of node-like swelling, through which the shaft readily fractures. We have described here a case of trichorrhexis nodosa with scanning electron microscopic findings. Nodular swelling of the affected hair showed longitudinal fissures and fractures in the cortex with loss of cuticular scales. At the site of fracture, a paintbrush-like effect was seen.

6.A Clinical Study of the Children's Ankle Fractures with Growth Plate Injury

The fractures of the ankle in children are differ from those of adults in terms of the fracture pattern and the possibilities of influence to growth. The injuries of the ankle in children occur mostly at the bony structure rather than ligamentous structure, and some injuries of the growth plate may result in the arrest of entire growth plate or parts of growth plate and these may lead to leg length discrepancy or deformity. Many authors agree that the prognosis of injuries of the growth plate is dependent on the fracture type, the age of the patient at the onset of injuries, the degree of displacement of the fracture fragments, whether the injuries open or closed, and the efficacy of reduction. But the authors can not postulate one uniformed method of classification and treatment of the children's ankle fracture because of the variability of injuries. We have reviewed 35 cases of children's ankle fracture treated at our clinics from Dec. 1979 to Feb. 1984. The results obtained are as follows: l. Over half of the patients were between the ages of 13 and 16 years (54.3%). 2. The incidence of each type as Salter-Harris classification was that of type II(48.6%), type I(20.0%),type III(14.3%),type IV (8.6%). 3. The traffic accidents were the main causes of injury (68.5%). 4. The type I fractures occured mostly under 10 years of age and the type II fractures in adolescents. 5. All the type I fractures were treated conservatively without any complication, and the type II fractures were treated conservatively in 13 cases (76.5%) and operatively in 4 cases (23.5%) with variable results. 6. Overall incidence of complication was 20.0%, consisted with two cases of conservatively treated but not accurately reduced type II fractures, a conservatively treated type III fracture, a triplane fracture, and the others.

7.Clinical Study for External Fixators of Open Tibial Fractures

From August, 1981 to November, 19S5, the Hoffmann and Monofixateur apparatus were employed in the treatment of thirty five open tibial fractures that we considered to have a poor prognosis with more conventional forms of treatment. The results were obtained as follows; l. Application of Monofixateur was minimum iatrogenic soft tissue injuries and easier, simpler than Hoffmann and two external fixators were favorable device with rigidity in wound management. 2. Two external fixators could obtained sufficient stability when 3 or more pins were applied at each fragment of fracture in Hoffmann and 2 or more pins in Monofixateur. Monofixateur could obtained high stability with small number of fixation components and more stronger against anterior-posterior bending and in Hoffmann more stronger against lateral bending. 3. External components of Monofixteur was located in anterior surface, therefore dynamization converted tensile force of anterior surface of tibia to compression force and then bone union was activated. 4. After operation, in Hoffmann correction of compression, distraction and angular, rotational deformity were possible and in Monofixateur correction of compression distraction, and anterior or posterior angulation was possible. 5. Because of rigid stability of two external Bxators, these allowed early movement, avoiding joint stiffness and duration ambulation unilateral assembly of device in position biomechanically favorable. 6. The lateral view of fracture site could not be confirmed accurately due to overlapping of Hoffmann's adjustable connecting rod and bony shadow. In Monofixateur A-P and lateral view of Fx. site could be confirmed. 7. Accurate reduction and compression played a important role in bone union. 8. The time to fixation for all of the fractures averaged 20 weeks in Hoffmann and 16 weeks in Monofixateur. The time to union averaged 24 weeks in Hoffmann and 20 weeks in Monofixateur.

8.Surgical Treatment of Cervical Spondylotic Myelopathy.

Myelopathy or dysfunction of the spinal cord, can be caused by degenerative processes of the cervical vertebrae. Cervical spondylotic myelopathy can be divided into five distinct syndromes on the basis of clinical presentation by Ferguson. Absolute indication for surgery is the progression of neurologic deficit. Decompression may be achieved using an anterior, posterior, or a combined approach, but each patient has unique clinical conditions that require individualized treatment. The purpose of the study was to evaluate the operative results by the clinical manifestation. In evaluating the results, the evaluation system established by the Japanese Orthopedic Association was employed. The average preoperative score in the 14 patient was 8.7 points and the average postoperative score was 12.7 points. The better results have been obtained for those who were managed with decompression within 1 year after onset of symptoms and those who had lateral type. In conclusion, the prognosis for the recovery of the spinal cord function is related with the onset of clinical symptoms and degree of neurological deterioration, so early detection and operative decompression for cervical spondylotic myelopathy may be the best method for the prevention of those unwanted and potentially devastating neurological deteriorations.

9.Treatment of the Unstable Thoracolumbar Fractures Using Cotrel

Surgical stabilization using instrumentation for thoracolumbar injuries offers several advantages such as nearly anatomic reduction of fractures, protection of neurologic structures and most importantly early ambulation of the patient. The treatment of fracture-dislocation of the thoracolumbar spine has been progressively improved over the past decades and recently a lot of new device have been introduced to improve fixability of the involved vertebrae three dimensionally and short segmental fixation as possible. The authors have treated 17 cases of unstable thoracolumbar fracture using Cotrel-Dubousset instrumentation in the Department of Orthopedic Surgery, Kangdong Sacred Heart Hospital from Jan. 1988 to Jan. 1989 with the following results: 1. Bursting fractures are the most common type(52.9%) of mechanism of injury. 2. Excellent anatomic reductions were obtained and compression, wedging and local kyphotic deformities were nicely corrected. 3. Short segmental fixation can preserve the maximal spinal mobility. 4. No significant loss of reduction or loosening of implant was found. 5. Remarkable neurologic recoveries were observed in cases of incomplete cord lesion. 6. Significant neurologic recovery was found in patients treated with laminectomy and Cotrel-Dubousset instrumentation.

10.Arthroscopic Management of the Tibial Condylar Fractures

Fractures of the tibial condyles, involving as they do weight-bearing articular surfaces and frequently accompanied by soft tissue injuries such as collateral ligaments, cruciate ligaments, and menisci present a variety of problems in treatment and prognosis. Slee, Apley, and others maintained the opinion that most fractures of the tibial condyles could be managed conservatively. On the other hand, Rombold, Schatzker, and others seemed to consider closed treatment to be virtually s form of therapeutic nihilism and adviced open reduction. But recently most authors agree that the method of treatment has to be selected in each individual case, and recommand anatomical reduction of the fracture as possible and early knee motion. The authors have treated 17 cases of tibial condylar fractures with arthroscopic management and extraarticular distal approach in the Department of Orthopedic surpery, Kang Dong Sacred Heart Hospital from Oct. 1986 to Jul. 1989. Of the above cases, 11 cases could be followed for a period of anywhere from 1 year to 31/2 years and analysed according to the cause, classification, treatment, and result. The following results were obtained from the analysis of 11 tibial condylar fractures. l. Of the 11 cases, 7 (63.6%) were male and 4 (36.4%) were female. 2. Of the 11 cases, 7 (63.6%) were due to traffic accident (5 pedestrians, 2 occupants) and 3 (27.3%) were due to fall from height. 3. Among 11 cases, 7 (63.6%) were associated with other injuries. 4. Bone graft was needed in 5 cases. 5. All were treated by arthroscopic management with anatomically and functionally good results. 6. By arthroscopic mangement, it was easy to find and treat the accompanying intraarticular lesion. 7. With early active and C.P.M. exercise, almost full range of motion was obtained in all cases.
