1.Percentage of primary open angle glaucoma in retinal vein occlusion patients

Background Researches determined that the pathogenesis of retinal vein occlusion (RVO) is associated with primary open angle glaucoma (POAG) in Caucasian population.However,the relationship between RVO and POAG in Asian population is unclear.Objective This study was to survey the percentage of POAG in RVO patients in China.Methods A prospective cross-sectional observational study based on hospital was performed.Three hundred and seventy five patients with newly diagnosed RVO by fluorescein fundus angiography (FFA) were consecutively enrolled in Peking University Third Hospital from October 2011 to May 2013,and gonioscopy was carried out in all the patients.Glaucoma was diagnosed according to the criteria of the International Society of Geographical and Epidemiological Ophthalmology (ISGEO).RVO was classified into central retinal vein occlusion (CRVO),hemicentral retinal vein occlusion (HRVO) and branch retinal vein occlusion (BRVO) based on fundus photograph and FFA,or classified into arteriovenous crossing RVO (AV-RVO),optic cup RVO (OC-RVO),optic nerve RVO without optical nerve head swelling (NONHS-RVO) and RVO with optical nerve head swelling (ONHS-RVO) based on the sites of venous occlusion.The percentages of POAG in different types of RVO were calculated.This study was approved by Ethic Committee of Peking University Third Hospital,and written informed consent was obtained from each patient before entering study cohort.Results Three hundred and seventeen RVO patients finished screening of glaucoma,with the response rate 84.5%.The percentage of POAG is 8.2% in RVO patients,including 7.1% in the CRVO patients,23.5% in HRVO patients and 6.2% in BRVO patients.The percentage of POAG was significantly higher in OC-RVO patients (27.9%) and NONHS-RVO patients (13.2%) than that in AV-RVO patients (4.7%) and ONHS-RVO patients (1.3%) (all at P<0.05).Conclusions The overall percentage of POAG in RVO patients in China is 3-4 times higher than general population.Higher percentage of POAG is found in OC-RVO patients and NONHS-RVO patients,suggesting that pathogenesis of RVO is probably associated with the glaucomatous anatomic changes of optical nerve head.
2.Risk factors for lymph node metastasis in gastric adenocarcinoma

Objective To study the relationship of different histological types and degrees of infiltration of gastric adenocarcinoma on lymph node metastasis.Methods The clinical data of 1 615 cases of gastric adenocarcinoma were analyzed retrospectively,and the rate and number of lymph node metastasis in different histological types and with different depths of invasion were statistically analyzed.Results There were 46 227 lymph nodes collected from 1615 cases,with an average of 28.62 per case. Lymph node metastasis was found in 1046(64.77%) cases of gastric cancer and 10 173(22.01%) lymph nodes. Different TNM stages showed different rates of metastasis: T1(M)2.78%,T1(SM)13.61%,T2 55.59%,and T3 84.84% respectively,while the metastatic rate for histolgically well-differentiated carcinoma was 50.25%,and for low-differentiated earcinoma was 78.95%,and both had statistically significant difference(P

Objective To observe the influences of stemona alkaloids on esterase isozymes activities and glycogen content of Oncomelania hupensis in order to explore the molluscicidal mechanism of stemona alkaloids.Methods O.hupensis snails were immersed in the liquid of stemona alkaloids at the concentration of 6.5 mg/L(72 h LC50),the surviving ones were picked out and sampled after being immersed for 12,24,48,72,96 h,then PAGE and anthrone colorimetry methods were used to observe the changes of liver esterase isozymes activities and tissue glycogen content of O.hupensis during the immersion period.A group of snails immersed in de-chlorinous water served as control.Results Esterase isozymes activities firstly increased,and then decreased until almost lost completely during the 96 h immersion period.Meanwhile,glycogen content gradually decreased as the immersion time extended.After being immersed for 96 h,glycogen content decreased by 72.00% compared with the control group.Conclusion Stemona alkaloids could inhibit the viability of O.hupensis by causing decrease of esterase isozymes activities and glycogen content.
4.Curative Effect and Adverse Reaction of Oxcarbazepine on Treating Epilepsy in Children

Objective To study the curative effect and adverse reaction of Oxcarbazepine(OXC)on treating epilepsy in children.Me-thods One hundred and twenty-nine children with different types of epilepsy were orally given OXC,and the drug dose was added gradually.According to the seizure frequency,the cases were divided into 2 groups.Group A:more than 3 seizures occurred in 3 months prior to to take OXC;group B:more than 3 seizures occurred in 1 year prior to taking OXC.After 5 months and 1 year from beginning to take OXC,the original curative effect of the 2 groups was evaluated,respectively;on the other hand,the adverse reaction and the retention were studied.Results 1.Original effect:the rate of seizure-free was 45.8% and the total curative efficiency was 66.7% in group A(n=47);the rate of seizure-free was 92.3% in group B(n=13);in 60 partial epilepsy children,the rate of seizure-free was 56.7% and the total curative efficiency was 73.3%;Both rates were 62.2% and 75.6% of patients with OXC monotherapy and 40%,66.7% of patients were given OXC in combination with another antiepileptic drugs.2.Drug adverse reaction:24% of patients were found to have adverse reaction and most of the symptoms were light and most transient.3.Tolerability:patients' retention of OXC in 1 year was 72.2%,and in 2 years was 80.0%.Conclusions The antiepileptic effect of OXC is satisfactory and adverse reaction is light and mostly transient,OXC as a new antiepileptic drug is well tolerated as monotherapy and adjunctive therapy and should be used extensively.J Appl Clin Pediatr,2009,24(1):53-55
5.Efficacy and safety of Minilase-s on patients with dyspepsia: a randomized placebo-controlled double blind multicenter clinical trial

Objective To investigate the efficacy and safety of Minilase-S on patients with dyspepsia.Methods A randomized,placebo-controlled,double blind and multicenter study was conducted.Two hundred and forty patients with dyspepsia symptoms(anorexia,fullness,abdominal discomfort and distension)were collected according to total symptom scores over 20 with visual analog scales.Each patient was randomly received either Minilase-S(2 capsules t.i.d)or placebo(2 capsules t.i.d)for 2 weeks.The symptoms scores were evaluated at treatment week 1,week 2,and 1 week after discontinued therapy.Results Two hundred and sixteen patients(105 patients in Minilase group and 111 patients in placebo group)finished the study.There was no difference in demographic data,anorexia,fullness,discomfort and distension score and the total symptom score between two groups.However,at treatment week 1,week 2 and 1 week after discontinued therapy,symptoms and total symptom score were significantly decreased in Minilase-S group compared to placebo group(all P value<0.05).The total effective rates in treatment week 1,week 2 and 1 week after discontinued therapy were 64.76%,77.05%and 66.99%,respectinely,which were higer that those in placebo group(27.93%,37.84% and 29.36%,respectively)(P<0.05).There was no severe side effects in both Minilase-S and placebo groups.Conclusions Minilase-S can significantly improve symptoms in patients with dyspepsia,which may be as one choice in the management of dyspepsia or in combined therapy.
6.Optimization of sperm alkaline single-cell gel electrophoresis.

OBJECTIVETo investigate the main factors that influence the results of sperm alkaline single-cell gel electrophoresis (SCGE), optimize the conditions, and standardize its procedures.
METHODSUsing alkaline SCGE, we detected the DNA fragments of sperm treated with different concentrations of H2O2 and determined the influences of the number of agarose gel layers, pH during DNA unwinding and electrophoresis, the time of DNA unwinding and electrophoresis, and cumulative sperm number on the results of sperm alkaline SCGE. Then we optimized the procedures, analyzed the repeatability of the optimized method, and examined 40 semen samples using the method.
RESULTSThree agarose gel layers could reduce the background. The optimal pH during DNA unwinding and electrophoresis was 10, and the best times for DNA unwinding and electrophoresis were 40 min and 30 min, respectively. Fifty sperm were adequate to ensure the reliability of the results. Based on the percentage of tail DNA, the intra- and inter-assay repeatabilities of the optimized sperm alkaline SCGE were 3.12% and 7.13%, and by the DNA damage score, they were 2.38% and 6.09%, respectively. Sperm DNA fragments were significantly increased in the infertile patients with oligoasthenoteratozoospermia as compared with healthy fertile males (P <0.05).
CONCLUSIONThe optimized sperm alkaline SCGE, highly repeatable and easy to be standardized, can be applied to the clinical detection of sperm DNA fragmentation in infertile men.

7.A retrospective study of clinical and immunologic features of 67 cases of patients with scrub typhus

Objective To study the clinical and immunologic features of 67 cases of patients with scrub typhus.Methods Epidemiological data,clinical manifestations,laboratory and image examination results and treatment of 67 cases of patients with scrub typhus hospitalized from January 2010 to December 2013 at the First Affiliated Hospital of Nanjing Medical University were retrospectively analyzed.Results Autumn-winter type scrub typhus was predominant in the 67 patents.The main clinical manifestations included hyperpyrexia (100%,67/ 67),eschar or ulcer (91.0%,61/67),rash (52.2%,35/67),lymphadenopathy (46.3%,31/67) and other non-specific symptoms.Laboratory test results:①80.6% (54/67) of the patients had normal or decreased white blood cell count,62.7% (42/67) of them with elevated lymphocytes (> 40%),thrombocytopenia was detected in 34.3% (23/67) of the patients,the lowest platelets count was 14 × 109/L.②14 of 17 cases (82.4%) had normal percentage of CD3+ T lymphocytes,CD8+ T lymphocytes increased in 16 of 17 cases (94.1%),while the percentage of CD4+ T lymphocytes (14 of 17,82.4%) and B lymphocytes (15 of 17,88.2%) decreased in most of the patients.③C reactive protein (CRP),ferritin and erythrocyte sedimentation rate (ESR) were elevated in 95.2% (59/62),93.8% (45/48) and 89.1% (49/55) of the patients,respectively.④Elevated alanine aminotransferase (77.6%,52/67) and aspartate aminotransferase (80.6%,54/67) were frequent findings.⑤94.6% (35/37) of the patients had increased adenosine deaminase.⑥ Antinuclear antibody was positive in 31.0% (13/42) patients.⑦Epstein-barr virus DNA was detected in 15.6% (7/ 45) cases,77.6% (38/49) was accompanied with chest radiographic abnormalities.Two cases had tuberculosis.All the patients recovered after therapy with doxycycline,levofloxacin or moxifloxacin.Conclusion Autumn-winter scrub typhus has typical clinical manifestations in the 67 patients,and it can cause patients' immune disorders.
8.The coronary introvascular ultrasound features of patients with benign prostatic hyperplasia

Objective To study the characteristics and clinical significance of the coronary plaques in patients with benign prostatic hyperplasia (BPH).Methods A total of 91 patients undergoing coronary artery angiography (CAG) were selected and divided into two groups according to with or without BPH,56 cases in BPH group,46 cases in control group.The qualitative and quantitative characteristics of 102 lesions area were analyzed by intravascular ultrosound (IVUS),including external elastic membrane-cross-sectional area (EEM-CSA),minimal lumen area (MLA),plaques area (PA),plaques burden (PB),reference external elastic membrane-cross-sectional area (REEM-CSA),reference minimal lumen area (RMAL),reference plaques area (RPA) and reference plaques burden (RPB).Results IVUS showed a higher rate of area stenosis than did the CAG [(58.2± 7.1)% vs.(55.9 ± 5.2)%,P<0.01].BPH group had more soft plaques,eccentric plaques,positive remodeling and less calcified than did the control group (P<0.05).The MLA and RMAL of BPH group were smaller than those of control group:MAL [(5.61±0.96) mm2 vs.(6.06±0.75) mm2,P<0.05],RMAL[(9.26±1.05) mm2 vs.(10.02±1.10) mm2,P<0.05]; while the EEM-CSA,PA,PB,RPA,RPB were larger than those of control group:EEM-CSA[(14.51±1.10)mm2 vs.(13.37±1.02)mm2],PA[(8.90±1.24) mm2 vs.(7.31±1.04) mm2],PB[(61.26±6.53)% vs.(54.53±5.69)%],RPA[(4.26±1.15) mm2 vs.(3.73±1.33) mm2],RPB [(31.30±8.37) % vs.(26.81±8.75) %,P<0.05].Conclusions IVUS has a higher value on evaluation of mild to moderate coronary artery stenosis.Evidence of myocardial ischemia in patients with BPH needs further excludeing coronary heart disease.
9.Comparison the applications of tracheal intubation with GlideScope video laryngoscope and Macintosh direct laryngoscope in snoring patients.

OBJECTIVE:
To compare the efficacy and safety of applications of GlideScope video laryngoscope (GSVL) and Macintosh direct laryngoscope (MDLS) during endotracheal intubation under general anesthesia of abdominal snoring surgeries.
METHOD:
Forty patients ranged from American Society of Anesthesiologists I to II scheduled for snoring surgery under general anesthesia with endotracheal intubation were randomly divided into GSVL group and MDLS group (20 cases in each group), After traditional induction, endotracheal intubation (ETT) performed by GSVL or MDLS. Glottic exposure, the time of endotracheal intubation(from mask ventilation ending to successful intubation), the times of tracheal intubation operation were recorded in both groups. The mean artery pressure(MAP), heart rate(HR) were monitored before induction (basic value T0), before tracheal intubation (T1), after tracheal intubation (T2), 1 min after tracheal intubation (T3), 3 mins after tracheal intubation (T4).
RESULT:
The glottic exposure of GSVL group is superior to that of MDLS group. There was no significant difference in the time of tracheal intubation between MDLS group (27 +/- 11)s and GSVL group (26 +/- 11)s (P < 0.05). Compared with T0, MAP of T1 was lower in two groups (P < 0.05), while HR were similar both in two groups (P > 0.05). MAP of T2 and T3 increased and HR became faster (P < 0.05). MAP of T4 declined, but HR still increased (P < 0.05). Between the two groups, there was no significant difference of MAP or HR (P > 0.05).
CONCLUSION
Compared with MDLS,the GSVL is of benefit to improve the glottic exposure in endotracheal intubation to the snoring patients,so as to improve the success rate of tracheal intubation. But there is no obvious advantages via GSVL in preventing hemodynamic reaction during tracheal intubation.

10.Protective effect of total flavonoids of epimedium on the kidney in experimental diabetic rats.

OBJECTIVETo investigate the influence of total flavonoids of epimedium (TFE) on the streptozocin (STZ)-induced kidney injury in diabetic rats and discuss the possible mechanism.
METHODSDiabetes was produced by a single injection of streptozocin (40 mg/kg, iv) in male SD rats. The rats were randomly divided into three groups (n = 10): control group, model group and TFE group (100 mg/kg, ig). Animals were sacrificed 12 weeks later. The level of blood glucose, blood urea nitrogen (BUN) and creatinine (Cr) as well as the renal index were determined. Detect the specific biochemical of renal tissue: superoxide dismutase (SOD), malondialdehyde (MDA). Use masson staining to observe the morphology of the renal tissue. Immunohistochemistry was employed to determine the protein levels of transforming growth factor-beta1 (TGF-beta1).
RESULTSCompared to control group, the enhancement of blood glucose, renal index, BUN and Cr was found in model group, which was significantly attenuated by treatment with TFE. Meanwhile, elevated MDA level in renal tissue as well as decreased SOD activities in renal tissue were significantly remitted by TFE. Furthermore, TFE decreased the expression of TGF-beta1.
CONCLUSIONTFE can evidently relieve renal damage in rats with diabetic nephropathy induced by STZ, which might be related to antioxidation and modulating the expression of TGF-beta1 protein.
