1.Late effects of modern therapy for Hodgkin lymphoma

Extended-field and subtotal nodal radiation therapy (RT),developed in the 1960 s,was first reliably curative treatment for early-stage Hodgkin lymphoma (HL). However, the large volume of normal tissue irradiated resulted in significant delayed toxicity, including cardiac disease and second cancers (SC).The 30-year cumulative incidence of heart disease among adult survivors receiving 40-45 Gy of extended-field or mantle RT is approximately 30 %; the incidence of SC is similar. How to improve disease control while reducing the toxicity of treatment has been a major objective of HL trials. Contemporary involved-field RT (IFRT) and involved-node RT (INRT) reduces irradiated volumes and produces significant reductions in normal tissue dose, and response-adapted therapy may be a means of identifying those patients most likely to benefit from treatment reduction or intensification, and enhanced screening will facilitate early intervention to reduce the clinical burden of late effects. There is a increasing interest in elucidating the genetic correlation of treatment toxicity.
2.Retrospective analysis of 91 patients with T cell non-Hodgkin's lymphoma

ObjectiveTo analyse treatments and prognostic factors of T cell non-Hodgkin lymphoma (T-NHL). MethodsNinety-one patients with T-NHL were retrospectively analyzed, and clinical features,histopathology, laboratory data were included in Kaplan-Meier and prognostic analysis. Results Median age was 38 years,58 (63.7 %) had high-intermediate and high risk by IPI,72 (79.1%) presented with advanced stage disease,extranodal disease was present in 64.8 % of patients.The overall response rate (ORR) for the whole group was 63.8 %,and the estimated 3,5-year ORR were 55.5 %,41.3 % respectively.Compared with CHOP-like regimen, the addition of etoposide could improve the survival of patients, meanwhile radiation therapy could improve the outcome of patients with NK/T cell lymphoma and mediastinal bulky disease, and consolidation chemotherapy with HSCT could improve the survival and reduce the recurrence of patients.Clinical stage,B symptoms,ECOG score,the level of LDH,extranodal involvment,anemia,initial treatment outcome, IPI score, the level of serum albumin and fibrinogen were predictive to overall survival. Cox multivariate analysis showed initial treatment outcome and B symptoms were independent prognostic factors.IPI and m-PIT were useful for stratified patients into different prognostic risk groups. Conclusion T-NHL is a heterogeneous group of malignancies with an inferior long term outcome. New treatment modality needs to be explored for these patients,and new drugs and HSCT may be good choices.
3.Effect of bortezomib combined with doxorubicin on lymphoblastic lymphoma Molt-4 cells

Objective To investigate the effect of bortezomib combined with doxorubicin on lymphoblastic lymphoma cell line Molt-4.Methods Molt-4 cells were cultured in the presence of bortezomib and doxorubicin,cell viability was monitored by CCK-8 and trypan-blue exclusion.Apoptosis was detected by flow cytometry and mitochondrial membrane potential,expression of Fas was also measured with flow cytometry.Results Molt-4 cell proliferation was substantially inhibited in concentration-dependent manners when treated with either bortezomib or doxorubicin.The combination of both drugs synergistically inhibited Molt-4 cell proliferation at 48 hours [(57.24±0.10) %].Combination therapy further enhanced bortezomib and doxorubicin induced apoptosis [48 h (23.08±1.25) %] (P < 0.05).Detection of mitochondrial membrane potential showed that combination therapy could promote apoptosis (15.84 %,5.38 %,5.52 %) but did not significantly change the level of Fas expression (P > 0.05).Conclusion Combination therapy of bortezomib with doxorubicin efficiently inhibits proliferation and induces apoptosis of Molt-4 cells.Activation of mitochondrial and intrinsic apoptotic pathway may play important roles.
4.Effect of combination with magnetic nanoparticle of Fe3O4 and adriamycin on Raji cell line

Objective To investigate the effect of combination with magnetic nanoparticle of Fe3O4and adriamycin (ADM) on Raji cell line.Methods Raji cells were cultured with Fe3O4-magnetic nanoparticle and ADM after using mechanical absortion polymerization,and the cell viability was detected by MTT and trypan-blue exclusion.The apoptosis was detected by flow cytometry,and the expression of p53 and NF-κB were measured by Western blot. Results Raji cell proliferation ratio was significantly inhibited in concentration-and time-dependent manners in both ADM and Fe3O4-MNP-ADM groups (r =0.412,P =0.027;r =0.523,P =0.014).The percentages of apoptotic cells induced by ADM and Fe3O4-MNP-ADM were 8.76 % vs 14.85 %,35.08 % vs 44.50 %,44.00 % vs 69.40 % in 12,24 and 48 hours,respectively (t =-9.137,-4.808,-6.337; P =0.012,0.041,0.024).The grey straps of NF-κB measured by Western blot were 4.22±0.32,3.31±0.28 in ADM and Fe3O4-MNP-ADM group (t =-54.416,P =0.035),whereas the grey straps of p53 were 1.042±0.114,1.270±0.091,respectively (t =33.963,P =0.047).Conclusion Combination therapy of Fe3O4-MNP and ADM could inhibit proliferation and induce apoptosis of Raji cells line,which may due to upregulation of p53 and down-regulation of NF-κB.
5.Nonsense Mutation of XPAC Gene in a Pedigree with Xeroderma Pigmentosum Group A

Objective To determine XPAC gene mutation in a Chinese pedigree with xeroderma pigmentosum group A.Methods All exons of XPAC gene were analyzed by PCR-DNA sequencing in the pedigree.The mutations were confirmed by restriction fragment length polymorphism(RFLP).Results By PCR-DNA sequencing,a nonsense mutation of C631T was identified which caused R211X substitution in exon5of XPAC gene.The mutated gene encoded a defective XPA protein truncated by63amino acids in C-terminus.The parents were all heterozygotes.The results were confirmed by RFLP.Conclusions A nonsense mutation is found in XPAC gene of a pedigree of xeroderma pigmentosum group A.This mutation may impair XPA protein function of DNA repair,and as a consequence,cause skin aging and carcinogenesis.
6.Experimental study on the interaction between hepatoma cells and hepatic stellate cells

Objective To investigate the effects of the interaction between human hepatoma cells and hepatic stellate cells on their growth state,and study its role of interaction on the progression of hepatocellular carcinoma.Methods Human hepatoma cell line HepG2 and hepatic stellate cell line hepatic stallate cells (HSC)-T6 were used and the methods including methyl thiazolyl tetrazolium (MTT) assay,flow cytometry (FCM) analysis,immunohistochemistry,and electron microscopy were employed in this experiment.The effects of conditioned medium (CM) of HepG2 on the activation and proliferation of HSC were explored.The effects of activated HSC CM on HepG2 proliferation were investigated.The uhrastructural changes of the two co-cultured cells were observed.Results MTT assay result showed that HepG2/HSC CM could promote HSC/HepG2 proliferation.FCM result demonstrated that HepG2/HSC CM could influence the cell cycle distribution in HSC/HepG2.Immunohistochemistry exhibited that after the treatment of HepG2/HSC CM,the expression ofα-smooth muscle actin (α-SMA) in HSC and proliferating cell nuclear antigen (PCNA) in HepG2 were increased.When HepG2 and HSC were co-cultured,the ultrastructure of HSC displayed an activated feature.Conclusions HepG2 cells can induce the activation and proliferation of HSC,and the activated HSC can also stimulate the proliferation of HepG2.Interaction between hepatoma cells and hepatic stellate cells may play an important role in the progression of hepatocellular carcinoma.
7.A Nonsense Mutation in Transglutaminase1Gene and Loss of Enzyme Activity in a Family with Lamellar Ichthyosis

Objective To detect the activity of transglutaminase1(TGM1)and gene mutation in a family with lamellar ichthyosis.Methods Immunohistochemistry technique was used to detect the activity of transglutaminase1.Complete encoding sequences of TGM1gene were analyzed in this family by using PCR-DNA sequencing.Results No activity of transglutaminase1was detected in the proband's skin.A nonsense mutation of C604T located in exon4of TGM1gene was identified by PCR-DNA sequencing,which caused a premature termination of Q202X and a defective polypeptide truncated by615amino acids in C-terminus.A heterozygous C604T mutation was carried by both of the proband' s parents.Conclusions The proband of lamellar ichthyosis in this family shows loss of transglutaminase1activity,which is resulted from a truncated transglutaminase1coded by the homozygous mutant TGM1gene.
8.Clinical effect of oxaliplatin-containing and cisplatin -containing regimen in TACE of advanced hepatocellular carcinoma

Objective To explore the clinical effect and side-effect of oxaliplatin(L-OHP)-containing regimen and cisplatin (DDP)-containing regimen in TACE of advanced hepatocellular carcinoma (HCC). Methods 108 patients with advanced HCC were randomly divided into experimental group(n=55) and control group (n =53). The experimental group were treated with TACE using L-OHP. After dilute with glucose solution, L-OHP(130 mg/m2) and FT207 (500-750 mg/m2) were injected into blood vessel respectively. ADM (40 mg/m2) and LP (10~30 ml) were emulsified and then used for vessel embolism according to the size of focus. The control group received TACE with DDP, DDP (40 mg/m2) and F]'207(500~750 mg,/m2) were diluted with glucose solution, and also according to the size of tumor' s focus, ADM(40 mg/m2) and LP(10~30 ml) were emulsified for vessel embolism, and then diuretic. Results The total effective rate of experimental group was 67.3 % (37/55), and that of control group was 47.2 % (25/53), and the difference was with statistical significance (P <0.05). The descent rate of AFP of experimental group was 73.1%(31/43), and that of control group was 44.7 % (17/38), and the difference was with statistical significance (P <0.05). The main side effects were gastrointestinal reactions, the incidence rate of nausea and vomiting in experimental group were lower than control group, and the difference was with statistical significance. The incidence rate of leucopenia, damage of hepatic function and peripheral neuritis were not significant. Damage of heart and kidney were not found in the two sets. Conclusion L-OHP -containing regimen in TACE of advanced HCC is an efficient method, with good security and good tolerance to patients.

10.Clinical study on post-operative metastasis prevention of progressive stage of gastric cancer by weichang'an.

OBJECTIVETo observe the effect of Weichang'an (WCA, a Chinese preparation) in preventing post-operative metastasis of progressive stage of gastric cancer.
METHODSA prospective randomized, controlled study was conducted by dividing the 148 patients of progressive staged gastric cancer after radical operation into the WCA group, the chemotherapy (CT) group and the WCA + CT group, to observe the survival rate, metastasis rate, quality of life (QOF) and tumor-bearing survival time after relapse (TST) in patients.
RESULTSThe 1-, 2- and 3-year survival rate after operation in the WCA + CT group was 89.51%, 69.77% and 55.76% respectively, which was significantly higher than that in the CT group (83.86%, 59.33% and 49.43%) respectively (P < 0.05), but showed insignificant difference as compared with that in the WCA group (93.23%, 79.34% and 71.78%). Only 1-year metastasis in the WCA group was 15.25%, and in the WCA + CT group was 15.52%, the two were significantly lower than that in the CT group (35.48%, P < 0.05). But the comparison of 2-year metastasis rate among the 3 groups (28.81%, 41.38% and 45.16%) and 3-year metastasis rate among them (33.90%, 46.55% and 51.61%) were insignificantly different. The QOF and TST were markedly better in the WCA group than those in the CT group.
CONCLUSIONWCA has preventive effect on relapse and metastasis in post-operational gastric cancer patients.
