Diabetic macular edema(DME)and age-related macular degeneration(ARMD)are the leading causes of visual impairment and blindness worldwide, and their common pathological features are increased vascular permeability and abnormal neovascularization, in which cytokines such as vascular endothelial growth factor(VEGF)and angiopoietin-2(Ang-2)play an important role. Intravitreal injection of anti-VEGF agents significantly changed the clinical management of DME and ARMD, but limitations such as the non-responsive cases, the treatment burden and risks caused by frequent injections need to be overcome. Faricimab, a novel bispecific monoclonal antibody that simultaneously targets VEGF-A and Ang-2, can effectively reduce vascular permeability, decrease the number of neovascularization and alleviate retinal edema. Registered clinical studies have shown that Faricimab is effective in improving vision and reducing retinal edema, which is non-inferior to Aflibercept and Ranibizumab, maintains a long dosing interval, and has a high safety profile. This article reviews the latest advances in the treatment of DME and ARMD with Faricimab.

Objective To examine the interfractional dosimetric variations among inverse three-dimensional (3D) plan, forward 3D plan, and two-dimensional (2D) plan of intracavitary brachytherapy for cervical cancer, and to discuss the risk of implementing the interval plan on different implantation applicators at short time intervals. Methods Twenty-five groups of CT-guided intracavitary brachytherapy (two consecutive radiations at ≤4 d apart) plans from 11 cervical cancer patients who received radical radiation therapy in our hospital were reviewed and compared. The dwelling location and time of the first intracavitary brachytherapy plan (Plan-1) were simulated on the CT image of the second intracavitary brachytherapy to form Plan-1-S.The target coverage indices and D 2 cc of organs at risk (OARs) of Plan-1-S and Plan 2(actual plan of the second intracavitary brachytherapy) under the three planning modes were recorded and compared using the paired t-test, Wilcoxon signed rank test, and ANOVA.Results The D90, D100, and V100 of high-risk CTV were significantly lower in Plan-1-S created under the inverse mode in the actual plan (-9.11±13.46%,-13.16±18.79%, and-7.80±13.34%, P=0.002, 0.002, and 0.005, respectively).D90, D100, and V100 of the interval plan had the greatest reduction under the inverse mode (76%, 80%, and 76%, respectively). The maximum reductions in D90, D100, and V100 were 332.14 cGy (2D), 244.12 cGy (forward), and 41.76%(inverse).OAR overdose occurred most frequently under the forward mode;the rates of D90, D100, and V100 reductions accompanied by one OAR overdose were 29.41%, 37.50%, and 25.00%, and the rates of D90, D100, and V100 reductions by two OAR overdoses were 5.88%, 12.50%, and 6.25%,respectively.Overdose occurred most frequently in the small intestine (36%).Comparison of the three planning modes showed that the inverse plan had a greater reduction in each target coverage index than the 2D plan. Conclusions The simulated interval plan can significantly reduce target coverage and increase the risk of OAR overdose regardless of the planning mode and the short time intervals, and is therefore not recommended for clinical application.

6.Ultrasound contrast agents in liver imaging.
Acta Academiae Medicinae Sinicae 2002;24(5):519-522
Ultrasound contrast agents may modify any of the three basic physical interactions between ultrasound and body tissues: absorption, reflection and refraction, and then the amount of backscatter can be increased. New imaging techniques including second harmonic imaging, interval-delay harmonic imaging, pulse-inversion harmonic imaging and sub-harmonic imaging proved to increase the sensitivity of altrasound contrast agents. There are several types of agents that have been used for liver imaging: free gas bubbles, saccharide microparticle suspensions, encapsulated microbubbles, perfluorocarbons, organ or tumor-specific agents. Ultrasound contrast agents can show characteristic vascular features of liver, specific types of hepatic tumors, or characteristic dynamic phases of perfusion imaging; therefore they can provide meaningful information for clinical diagnosis.

7.A study of risk factors and predictive factors in intraoperative floppy iris syndrome during phacoemulsification
International Eye Science 2017;17(11):2126-2129
AIM:To determine the incidence of intraoperative floppy iris syndrome (IFIS) in patients undergoing phacoemulsification in a Chinese hospital, and to assess new risk factors and predictive factors for IFIS.METHODS:A prospective, observational case series. In the consecutive cataract surgeries performed in one year, the medicine administration, pupil size (PS)before and after mydriasis, and signs of IFIS were recorded.RESULTS:Totally 807 patients (1068 eyes) underwent cataract surgeries.Among the 1068 eyes, signs of IFIS were noted in 34 eyes.Strong positive correlations were showed between finasteride (6.4%,OR =5.885),tamsulosin (25%,OR=21.578),reserpine (16.7%,OR=12.947),clozapine (66.7%,OR=139.467),post-panretinal photocoagulation (14.3%,OR =10.789) and IFIS.Pupil size was inversely related to IFIS incidence (P<0.001).A dilated pupil smaller than 7.25mm could predict iFiS (P<0.001).CONCLUSION:The incidence rate of IFIS was 3.18%.Reserpine,clozapine and post-panretinal photocoagulation emerged as new risk factors for IFIS.A small dilated pupil may imply IFIS occurrence.

OBJECTIVE:
To describe our experience in the clinical manifestation and treatment of malignant tumors of the external and middle ear.
METHOD:
The study reviewed 39 patients between 1994-2011 in our hospital, including 15 pinna tumors, 18 external canal tumors and 6 middle ear tumors. 23 males and 16 females were enrolled in this study. The mean age of patients at the time of surgery was 59. Radiotherapy or radiotherapy and chemotherapy were the only possible treatment in 6 cases. Thirty-three patients were treated surgically, and 9 patients also received radiotherapy after surgery.
RESULT:
All of the patients had been followed up over 3 years, except for 1 case of external canal and 1 case of middle ear tumor. The 3-year survival of pinna, external canal and middle ear tumors were 86.7%, 82.4% and 60.0% respectively. At the last follow up, the pinna tumors showed that the survival rate was 100% in T1, T2 and Tx stage, and 0% in T4 stage; the external canal tumors showed that the survival rate was 90% in T1 stage, and 66.7% in T2, T3 stage; the middle ear tumors showed that the survival rate was 100% in T1 and T2 stage, 0% in T3 stage.
CONCLUSION
The T staging system is for an important prognostic factor, and it is important for an early diagnosis and radical surgery to achieve a better therapeutical result.

Chinese Journal of Cancer 2001;20(2):160-163
Objective: The current study was designed to investigate the role of foods such as fish juice, pickles, bacon, salted fish, smoked food, fruit and vegetables in the etiology of esophageal cancer in Chaoshan region of China. Method: A large case-control study was conducted to investigate 1248 patients with squamous cell carcinoma of the esophagus and 1248 controls matched by sex and age. Results: After adjusting for the effects of occupation, areas of residence, income, alcohol, cigarette and tea, a strong association with a clear dose-response was observed between the frequencies of fish juice eating and esophageal cancer (P<0.01), and the relative risk for women was 23.5 and for men was 3.1, respectively. The results for pickles were similar to fish juice. A protective effect of fruits on esophageal cancer was observed both in women (OR=0.13) and in men (OR=0.46), and the risk was decreased (P< 0.01) as frequencies of fruits eating was increased. The risks were significantly higher in bacon and salted fish consumers than that in non-consumer, but the dose-response relationship was not significant. No significant effects, however, were detected in vegetables and smoked food consumption. Conclusions:To the authors' knowledge, this is the first report on relationship between the fish juice and high risk of esophageal cancer in population. Further epidemiological and experiment study are required to find a biological causal relationship between them.
