1.Preparation and evaluation of oridonin derivatives

Objective In order to study the structure-activity relationship of oridonin,Some derivatives of oridonin were synthesized and their cytotoxicity activity was evaluated.Methods Oridonin (Ⅰ) was isolated and purified by silica gel column chromatography.The derivatives were synthesized from oridonin through oxidation and acylation.All compounds were confirmed by spectrum analyses and their cytotoxicity was evaluated by MTT method.Results Six compounds were obtained and identified as: 14-acetyl-oridonin(Ⅱ),1,14-diacetyl-oridonin(Ⅲ),14-p-tolylsulfonyl-oridonin(Ⅳ),1-oxo-oridonin(Ⅴ),14-acetyl-1-oxo-oridonin(Ⅵ),14-p-tolylsulfonyl-1-oxo-oridonin(Ⅶ).Among them compounds Ⅳ,Ⅵ,and Ⅶ were new compounds.Five compounds showed better cytotoxicity than oridonin.Conclusion The cytotoxicity of 1oxo oridonin derivatives is first evaluated and shows the stronger cytotoxicity than oridonin.
2.Predictive value of preoperative 18F-FDG PET/CT imaging combined with coagulation parameters for recurrence in patients with early cervical cancer

Objective:To explore the value of preoperative 18F-fluorodeoxyglucose(FDG) PET/CT parameters combined with coagulation parameters in predicting the recurrence after surgery in patients with early cervical cancer. Methods:A total of 120 patients(age range: 25-70 (47.9±8.5) years) with cervical cancer who underwent preoperative 18F-FDG PET/CT examination and blood coagulation index test in Shengjing Hospital of China Medical University between January 2012 and December 2014 were retrospectively analyzed. Patients were divided into recurrent group and non-recurrent group according to the follow-up results. Independent-sample t test or Mann-Whitney U test was used to compare the differences of related parameters between the recurrent and non-recurrent patients. Then the receiver operating characteristic (ROC) curve analysis was performed, and univariate and multivariate Cox analyses were used to evaluate the predictive value of parameters. Results:The follow-up time was 2-60 months. There were 36 recurrent cases, accounting for 30.0% (36/120) of all patients. The mean standardized uptake value (SUV mean), peak of standardized uptake value (SUV peak) and total lesion glycosis (TLG) were significantly different between recurrent ( n=27) and non-recurrent ( n=62) subgroups of moderately differentiated patiens (8.90±3.00 vs 7.50±2.90, 12.00±3.70 vs 10.20±4.50, 144.48(43.79, 366.46) vs 60.23(28.46, 113.15) g; t values: 1.968, 2.063, U=547.000, all P<0.05); the maximum standardized uptake value (SUV max), SUV mean, SUV peak, TLG were significantly different between recurrent ( n=7) and non-recurrent ( n=12) subgroups of poorly differentiated patiens (16.10±4.70 vs 7.60±2.33, 8.70±2.10 vs 4.40±1.50, 13.30±4.40 vs 5.60±1.80, 140.37(131.44, 143.94) vs 31.64(15.84, 92.14) g; t values: 5.363, 4.829, 5.429, U=3.000, all P<0.05); D-dimer and fibrinogen (FIB) of recurrent and non-recurrent patients were also different (175.00(100.00, 256.00) vs 86.00(51.25, 115.25) mg/L, (3.10±0.50) vs (2.80±0.50) mg/L; U=619.500, t=2.962, both P<0.05). The ROC curve indicated that SUV max (area under curve (AUC)=0.651), SUV mean (AUC=0.650), SUV peak (AUC=0.675), TLG (AUC=0.703), D-dimer (AUC=0.795) and FIB (AUC=0.672) could predict the recurrence of the disease(all P<0.01). Univariate Cox analysis revealed that the International Federation of Gynecology and Obstetrics (FIGO) staging (hazard ratio ( HR)=2.363, 95% CI: 1.217-4.590), SUV max ( HR=4.855, 95% CI: 1.488-15.841), SUV mean ( HR=4.451, 95% CI: 1.573-12.597), SUV peak ( HR=7.190, 95% CI: 2.203-23.469), TLG ( HR=4.396, 95% CI: 2.238-8.633), D-dimer ( HR=4.761, 95% CI: 2.470-9.253) and FIB ( HR=3.196, 95% CI: 1.596-6.400) were predictive factors of recurrence (all P<0.05). Multivariate Cox analysis showed that D-dimer ( HR=2.974, 95% CI: 1.476-5.990) and SUV peak ( HR=3.826, 95% CI: 1.063-13.778) were the main predictors (both P<0.05). ROC curve of SUV peak combined with D-dimer showed the AUC of 0.841( P<0.01). Conclusions:SUV peak and D-dimer are main indexes to evaluate the recurrence after surgery in patients with cervical cancer. SUV peak combined with D-dimer have good predictive value for the recurrence of cervical cancer.
3.Effect of fluoride concentration on the corrosion behavior of cobalt-chromium alloy fabricated by two different technology processes.

OBJECTIVEThis study aims to determine the effect of fluoride concentration on the corrosion behavior of cobalt-chromium alloy fabricated by two different technology processes in a simulated oral environment.
METHODSA total of 15 specimens were employed with selective laser melting (SLM) and another 15 for traditional casting (Cast) in cobalt-chromium alloy powders and blocks with the same material composition. The corrosion behavior of the specimens was studied by potentiodynamic polarization test under different oral environments with varying solubilities of fluorine (0, 0.05%, and 0.20% for each) in acid artificial saliva (pH = 5.0). The specimens were soaked in fluorine for 24 h, and the surface microstructure was observed under a field emission scanning electron microscope after immersing the specimens in the test solution at constant temperature.
RESULTSThe corrosion potential (Ecorr) value of the cobalt-chromium alloy cast decreased with increasing fluoride concentration in acidic artificial saliva. The Ecorr, Icorr, and Rp values of the cobalt-chromium alloy fabricated by two different technology processes changed significantly when the fluoride concentration was 0.20% (P < 0.05). The Ecorr, Icorr, and Rp values of the cobalt-chromium alloy fabricated by two different technology processes exhibited a statistically significant difference. The Icorr value of the cobalt-chromium alloy cast was higher than that in the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20% (P < 0.05). The Ecorr, tRp alues of the cobalt-chromium alloy cast were lower htan those of the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20% (P< 0 .05).
CONCLUSIONFluoride ions adversely affected the corrosion resistance of the cobalt-chromium alloy fabricated by two different technology processes. The corrosion resistance of the cobalt-chromium alloy cast was worse than that of the SLM group cobalt-chromium alloy when the fluoride concentration was 0.20%.

4.Percentage of primary open angle glaucoma in retinal vein occlusion patients

Background Researches determined that the pathogenesis of retinal vein occlusion (RVO) is associated with primary open angle glaucoma (POAG) in Caucasian population.However,the relationship between RVO and POAG in Asian population is unclear.Objective This study was to survey the percentage of POAG in RVO patients in China.Methods A prospective cross-sectional observational study based on hospital was performed.Three hundred and seventy five patients with newly diagnosed RVO by fluorescein fundus angiography (FFA) were consecutively enrolled in Peking University Third Hospital from October 2011 to May 2013,and gonioscopy was carried out in all the patients.Glaucoma was diagnosed according to the criteria of the International Society of Geographical and Epidemiological Ophthalmology (ISGEO).RVO was classified into central retinal vein occlusion (CRVO),hemicentral retinal vein occlusion (HRVO) and branch retinal vein occlusion (BRVO) based on fundus photograph and FFA,or classified into arteriovenous crossing RVO (AV-RVO),optic cup RVO (OC-RVO),optic nerve RVO without optical nerve head swelling (NONHS-RVO) and RVO with optical nerve head swelling (ONHS-RVO) based on the sites of venous occlusion.The percentages of POAG in different types of RVO were calculated.This study was approved by Ethic Committee of Peking University Third Hospital,and written informed consent was obtained from each patient before entering study cohort.Results Three hundred and seventeen RVO patients finished screening of glaucoma,with the response rate 84.5%.The percentage of POAG is 8.2% in RVO patients,including 7.1% in the CRVO patients,23.5% in HRVO patients and 6.2% in BRVO patients.The percentage of POAG was significantly higher in OC-RVO patients (27.9%) and NONHS-RVO patients (13.2%) than that in AV-RVO patients (4.7%) and ONHS-RVO patients (1.3%) (all at P<0.05).Conclusions The overall percentage of POAG in RVO patients in China is 3-4 times higher than general population.Higher percentage of POAG is found in OC-RVO patients and NONHS-RVO patients,suggesting that pathogenesis of RVO is probably associated with the glaucomatous anatomic changes of optical nerve head.

SIRT1 as the most famous of the sirtuins family members has been verified involved in a variety of age-related diseases. While the SIRT1 formation is paid more and more attention in age-related cataract. Now, we briefly overviewed the research progress on the role of SIRT1 in age-related cataract.
6.The clinical observation of Qianlietai tablets on curing the chronic-abacterial-prostatitis

Objective To evaluate the clinical curative effect of Qianlietai tablets on curing the chronic-abac- terial-prostatitis.Methods A 6 to 24 months observation was performed of 45 cases of sufferers of chronic-abacterial prostatitis who were supposed to take Qianlietai tablets 3 times per day with 5 pieces each time.The before and after treatment change of the chronic abacterial prostatitis symptom grade and the numbers of leucocytes examinated in the normal inspection of prostate succus were compared,the side-effects of the treatment were observed as well.Re- suits After the treatment,there were 16 cured cases,15 effective cases,8 markedly effective cases and 6 non-effec- tive cases in the total 45 cases of sufferers.Conclusion Qianlietai tablets has a clear curative effect on curing chronic abacterial prostatitis with a good tolerance.
7.Characteristics of HIV/AIDS cases with non-marital or non-commercial heterosexual transmission in Hangzhou

Objective :
To analyze the characteristics of HIV/AIDS cases with non-marital or non-commercial heterosexual transmission in Hangzhou, and the influencing factors for new infection and local infection, so as to provide the evidence for AIDS prevention and control.
Methods :
From 2017 to 2019, the newly reported HIV/AIDS cases with non-marital or non-commercial heterosexual transmission in Hangzhou were recruited, and their demographic information, previous sexual behaviors and history of HIV testing were collected in the questionnaire survey. The multivariate logistic regression model was used to analyze the influencing factors for new infections and local infections.
Results :
A total of 522 participants from 668 newly reported HIV/AIDS cases with non-marital or non-commercial heterosexual transmission in Hangzhou during this period were surveyed.Among 522 cases, 263 ( 50.38% ) were aged 40 years or above, 218 ( 41.76% ) were married, 326 ( 62.45% ) had an educational level of junior high school or below, and 340 ( 65.13% ) were not local. Among 504 cases whose infection time could be determined, 72 ( 14.29% ) were newly infected within one year; age of 40 years below ( OR=4.148, 95%CI: 1.956-8.795 ), history of HIV testing ( OR=2.049, 95%CI: 1.163-3.609 ) and history of sexually transmitted diseases ( OR=2.169, 95%CI: 1.076-4.374 ) were risk factors for new infection. Among 454 cases whose infection location could be determined, 267 ( 58.81% ) were infected in Hangzhou; educational level of high school or below ( OR=2.538, 95%CI: 1.252-5.145 ) , Hangzhou residence ( OR=7.835, 95%CI: 4.227-14.353 ), living in Hangzhou for a year or over ( OR=18.960, 95%CI: 8.755-41.060 ) and monthly income of 3 000 yuan or over ( OR=2.630, 95%CI: 1.546-4.474 ) were risk factors for local infection.
Conclusions
The HIV/AIDS cases with non-marital or non-commercial heterosexual transmission in Hangzhou are mainly floating population and less educated. The newly infected cases are more likely to be young and middle-aged people and patients with sexually transmitted diseases, the locally infected cases are more likely to be people with permanent residence, less educated and high income.
8.Quality of life and its influencing factors among people living with HIV/AIDS in Hangzhou City

Objective :
To investigate the quality of life among people living with HIV/AIDS in Hangzhou City and analyze the influencing factors, so as to provide insights into the control of AIDS.
Methods :
From 1 January 2014 to 30 June 2018, the demographic characteristics, medical expenditures and disease status were collected from HIV/AIDS patients living in Hangzhou City, and the quality of life was assessed using the simplified Chinese version of Medical Outcomes Study-HIV Health Survey ( MOS-HIV ). Factors affecting the quality of life were identified among HIV/AIDS patients using multivariable linear regression analysis.
Results :
A total of 2 808 HIV/AIDS patients were surveyed, including 1 684 cases with HIV infections and 1 124 cases with AIDS. The participants included 2 510 men ( 89.39% ) and 298 women ( 10.61% ), and were predominantly at ages of 25 to 39 years ( 1 531 cases, 54.52% ). The physical and mental health scores were 53.87±6.96 and 46.03±9.09, respectively. Multivariable linear regression analysis identified age, average monthly income, self-paid medical expenses during the past year, and the latest CD4+T cell count as factors affecting physical and mental health ( P<0.05 ).
Conclusions
The quality of life is low among people living with HIV/AIDS in Hangzhou City, and is associated with age, income, medical expenditures and CD4+T cell count.
9.Factors affecting immune reconstitution in HIV/AIDS patients after antiretroviral therapy

Objective:
To investigate the immune reconstitution of HIV/AIDS patients and its influencing factors after receiving antiviral therapy (ART) in Hangzhou City, so as to provide insights into improving the treatment effects and quality of life in HIV/AIDS patients.
Methods:
A retrospective cohort of HIV/AIDS patients who began antiviral treatment between January 1, 2016 and August 31, 2021 and had a baseline CD4+T lymphocyte (CD4) counts of less than 500 cells/μL or a baseline CD4/CD8+T lymphocyte (CD8) ratio of less than 0.8 in Hangzhou City was followed up until August 31, 2023. Demographic information, antiviral therapy in formation, CD4 counts, and CD4/CD8 were collected from the Chinese Disease Prevention and Control Information System. A good immune reconstitution was defined as having CD4≥500 cells/μL and CD4/CD8≥0.8. The immune reconstitution status of HIV/AIDS patients were analyzed, and factors affecting immune reconstitution were identified using a multivariable Cox proportional risk regression model.
Results:
A total of 3 349 HIV/AIDS patients were enrolled, with a median age at ART of 31 (interquartile range, 20) years. There were 3 075 males (91.82%), 1 600 cases with college education and above (47.78%) and 2 455 cases at WHO clinical stage Ⅰ-Ⅱ(73.31%). There were 1 368 cases with good immune reconstitution, accounting for 40.85%, and the proportion of HIV/AIDS patients with good immune reconstitution that began ART in 2016 was the highest, reaching 51.90%. Multivariable Cox proportional risk regression model identified WHO clinical stage (Ⅰ-Ⅱ, HR=2.529, 95%CI: 2.023-3.162), timely ART (HR=1.196, 95%CI: 1.027-1.394), initial treatment regimen (TDF+3TC+NVP/EFV, HR=2.185, 95%CI: 1.891-2.524; integrase inhibitors, HR=8.509, 95%CI: 6.706-10.795), baseline CD4/CD8 (≥0.1, HR: 1.600-4.515, 95%CI: 1.061-6.661), baseline hemoglobin (<90 mg/dL, HR=0.327, 95%CI: 0.121-0.880), hepatitis B infection (HR=0.619, 95%CI: 0.457-0.840) and hepatitis C infection (HR=0.308, 95%CI: 0.099-0.956) as factors affecting immune reconstitution in HIV/AIDS patients.
Conclusion
The immune reconstitution in HIV/AIDS patients after ART is associated with WHO clinical stage, timely ART, initial treatment regimen, baseline CD4/CD8, baseline hemoglobin and hepatitis B or C infection.
