1.Values of the integrated visual and auditory continuous performance test in diagnosing children with attention deficit hyperactivity disorder in clinic and school in China

Objective To explore the psychometric properties of the Chinese version of integrated visual and auditory continuous performance test (IVA-CPT) ,and to assess the diagnosis value of the Chinese version of IVA-CPT in two samples of children with attention deficit hyperactivity disorder (ADHD) in clinic and school.Methods 112 participants were divided into 4 groups:clinic group, clinic control group, school group and control group from school.The participants were measured by IVA-CPT and the results were compared.Results ①The integrated quotients of IVA-CPT had concordance with the dimensions of the DSM-ⅣV and ASRS.Each integrated quotients of IVA-CPT and each dimension of the DSM-ⅣV and ASRS were negatively correlated(school: r=-0.277, P<0.05;clinic: r=-0.423, P<0.05).The score of HI dimension was negatively correlated with integrated attention quotient and integrated inhibition quotient of IVA-CPT (r=-0.480, P<0.01;r=-0.452, P<0.01).② The results of the IVA-CPT and the comprehensive assessment had consistency for diagnosis of children with ADHD from the total subjects,the clinic ,and the school.And there were no difference between the sensitivity, the specificity, the coincidence rate, the missed diagnosis rate and the misdiagnosis rate of IVA-CPT when used to diagnose children with ADHD in clinic and school(x2=3.396, P>0.05).③The IVA-CPT evaluation system had good stability according to the retest of a random sample of 25 subjects(P<0.01).The pretest and posttest results of each full quotients of IVA-CPT were significantly correlated (P<0.01).Conclusion The IVA-CPT not only can be used as auxiliary diagnostic tools to diagnose children with ADHD in clinic,it can be used as auxiliary diagnostic tools to diagnose children with ADHD in the community.
2.Construction of efficient conjugal plasmids between Escherichia coli and Streptomycetes.

Conjugal plasmid pGH112 has been developed based on the replicons of Streptomyces coelicolor plasmid SCP2 and E. coli ColE. The plasmid contains ampicilin resistance gene(amp) for selection in E. coli and thiostrepton resistance gene (tsr) for selection in Streptomycetes, and a 0.76 kb oriT fragment of (IncP) RK2. Conjugal transfer of pGH112 was performed from E. coli to S. coelicolor A3(2), S. avermitilis, S. lividans TK54, S. toxytricini NNRL15443, S. venezuelae ISP5230 and Sacc. erythraea by conjugation, results show that the plasmid was able to transfer efficenctly from E. coli to Streptomycetes, was stably inherited in the recipients. pGH113 was constructed from pGH112 by combining the constitutive ermE promoter with green fluorescent protein gene(gfp).

3.Management and outcome of 47 patients with acute myocardial infarction complicating cardiogenic shock

Objective To evaluate the clinical outcomes of patients with acute myocardial infarction(AMI)complicating cardiogenic shock underwent various treatments.Methods From January,2002 to May,2007,47 AMI patients with cardiogenic shock were treated in our department by optimal medication (dopamine,epinephrine,norepinephrine,etc.),intrar-aortic balloon pump(IABP),mechanical ventilation when indicated,percutaneous coronary intervention(PCI)or coronary artery bypass graft (CABG).Outcome and factors related to mortality for these patients were analyzed in this retrospective study.Results Besides optimal medication and IABP in all patients,31 patients underwent PCI(66.0%),6 patients received emergency CABG(12.8%).The overall in-hospital mortality rate was 36.2%(17/47),6 patients(14.9%)died before coronary revascularization and 11 patients(21.3%)died after revascularization.Nine patients died of pump failure and 8 patients died of renal and(or)respiratory failure.Regression analysis showed that acute renal failure(r=0.734,P=0.000),acute respiratory failure (r=0.606,P=0.000)and diabetes(r=0.372,P=0.012)were positively related to in-hospital mortality.Conclusion Despite improvements in treatment options for AMI patients complicating cardiogenic shock,in-hospital mortality remained high,especially for patients complicating further with acute renal failure and acute respiratory failure.
4.Analysis of clinical features and genetic mutations in a Chinese family affected with Menkes disease.

OBJECTIVETo delineate the clinical features and potential mutation of the ATP7A gene in a family affected with Menkes disease.
METHODSClinical data of a patient and his family members were analyzed. Sanger sequencing and multiplex ligation-dependent probe amplification (MLPA) assays were performed to detect the mutation of the ATP7A gene.
RESULTSThe patient was admitted at the age of 5 months due to severe epilepsy and marked delayed psychomotor development. Significantly light complexion, pudgy cheeks and sparse fuzzy wooly hair were noted. Cranial magnetic resonance imaging and angiography revealed cortical atrophy, leukoencephalopathy and circuitous of intracranial vessels. The plasma ceruloplasmin was decreased. MLPA has identified a deletion spanning exons 8 to 12 of the ATP7A gene. His mother was found to be a heterozygous carrier of the same mutation.
CONCLUSIONThe clinical features and a novel mutation of the ATP7A gene of the family have been delineated.

5.The impact of massive intraoperative RBC transfusion on laboratory results and prognosis in patients underwent surgery for malignancies

【Objective】 To explore the effects of massive intraoperative RBC transfusion on multiple clinical test indicators and prognosis of patients, underwent tumor surgery in order to provide evidence for rational blood transfusion and effective intervention of complications caused by massive blood transfusion in tumor patients. 【Methods】 A total of 208 patients who underwent tumor resection in our hospital from January 2019 to December 2020 and received intraoperative RBC transfusion(>10 U) were selected as the study subjects. According to the amount of blood transfusion, they were divided into group A: 10~15 U, 144 patients; Group B: >15~25 U, 48 people; Group C: >25 U, 16 people. Data of liver function, coagulation, electrolyte, platelet count and short-term prognosis were collected and compared among 3 groups before and after surgery. 【Results】 No significant difference was noticed in patient pre-operation variables including ALT (U/L), AST (U/L) and TBIL (μmol/L) among three groups recieved massive blood transfusion (P>0.05), while AST was significantly lower than that after operation (P<0.05) : 105.33±238.18 vs 113.50±185.04 vs 291.25±457.33 (P<0.05). After operation, PT (s) (14.12±2.10, 14.79±2.67 and 16.10±4.06), INR(1.25±0.20, 1.31±0.26 and 1.44±0.38) and APTT (s) (30.52±5.63, 34.57±12.80 and 34.80±10.49) extended significantly than those before operation (P<0.05), while Plt (×109/L) decreased significantly (142.32±70.07, 100.04±57.50 and 85.40±41.10)(P<0.05). After operation, serum K+ and Ca2+ decreased significantly, Na+ and Cl- increased significantly, and pH value decreased (P < 0.05). Hospital stay of group C (d) was 33.73±34.62 vs 17.74±14.83 vs 20.92±17.69 (P<0.05). The mortality rate was 2.8%(4/44) vs 6.3%(3/48) vs 18.8%(3/16)(P<0.05), and mortality rate of group C was higher than the other two groups. 【Conclusion】 Postoperative dysfunction of liver and coagulation in tumor patients may be related to intraoperative RBC transfusions and consequent acid-base imbalance and electrolyte disturbance. The more the units of RBC transfused, the more abnormal the patients' clinical indicators, also the longer the hospital stay and the worse the short-term prognosis.
6.Effect of electroacupuncture at Neiguan (PC 6) on pulmonary function during one-lung ventilation in patients with lobectomy.

OBJECTIVE:
To observe the protective effect of electroacupuncture (EA) at Neiguan (PC 6) on pulmonary function during one-lung ventilation (OLV) in patients with lobectomy, and explore its action mechanism.
METHODS:
Sixty patients with lobectomy were randomly divided into an observation group and a control group, 30 cases in each one. The patients in the control group were treated with general anesthesia, and OLV was given when surgery began; when the surgery finished, air was removed from the thoracic cavity and two-lung ventilation was performed. On the basis of the treatment in the control group, the patients in the observation group were treated with EA (disperse-dense wave, 2 Hz/100 Hz of frequency) at Neiguan (PC 6) 30 min before anesthesia induction until the end of the surgery. 7.Detection Rate of Central Nervous System Leukemia Can Be Improved by Cell Preservation Solution.

OBJECTIVE:
To investigate whether cell preservation solution can prolong the survival time of leukemia cells and increase the survival rate, so as to improve the detection rate of central nervous system leukemia.
METHODS:
Kasumi cells were added into cerebrospinal fluid (CSF) supernatant with or without cell preservation solution to compare cell viability and biological characteristics at different time point. Wright Giemsa staining was used to compare cell morphology; cell counting, CCK-8 method, and trypan blue staining were used to compare the cell number, and flow cytometry was used to compare the cell viability. The expression of AML-ETO tumor fusion gene was detected by fluorescence quantitative RT-PCR.
RESULTS:
At different time points (8 h and 24 h), the survival, molecular biological characteristics and RT-PCR result of the cells in CSF with cell preservation solution were significantly better than those in normal cerebrospinal fluid.
CONCLUSION
Cell preservation solution can effectively improve the survival time and survival rate of leukemic cells, thereby increase the detection rate of CNS leukemia.

8.Advantages of Chinese Medicine in Treating Dominant Disease: Benign Prostatic Hyperplasia

China Association of Chinese Medicine organized specialists in andrology of Chinese and western medicine to explore the population and treatment stage of benign prostatic hyperplasia (BPH) with Chinese medicine as the leading therapy. Chinese medicine has great advantages in the treatment of benign prostatic hyperplasia. However, it is necessary to make clear the stage when Chinese medicine or modern medical treatment can be used as the leading therapy, and the conditions under which Chinese and western medicine can be combined to achieve the best treatment efficacy. The specialists agreed Chinese medicine as the leading therapy for the treatment of BPH in the following populations or conditions: the elderly and weak patients with basic diseases, BPH symptoms, and cannot tolerate anesthesia and surgery, the patients with BPH symptoms and cannot tolerate the adverse reactions or the possible adverse reactions of western medicine; the patients with mild [international prostatic symptom score (IPSS) ≤ 7] or moderate lower urinary tract symptoms (IPSS ≥ 8) and the quality of life not significantly affected, the patients with bladder detrusor hypofunction, bladder dysfunction and cannot be treated surgically, or with incomplete bladder emptying after surgical treatment; the BPH patients with prostatitis as the main clinical manifestation, the patients with non-acute complications after operation. BPH is one of the dominant diseases in urology and andrology of Chinese medicine, and the symptoms, complications, and prognosis of BPH patients need to be fully considered during the clinical treatment. When Chinese medicine is taken as the leading therapy, it is essential to regularly review the serum level of prostate-specific antigen to exclude the possibility of prostate cancer, and apply Chinese medicine for full treatment course and cycle. At the same time, Chinese and western medicine can be combined to achieve the most effective, convenient, economical, and satisfactory treatment, which can carry forward the advantages of Chinese medicine in treating this disease.
