[Objective]To explore the clinical effect of the new stem-less total hip arthroplasty.[Method]Exposed the hip joint through posterior-exterior incision,dislocate the femoral head,clear away sick tissues inside joint,throw into the centric pin by guide of the centric localizer,repaired bone by machine of modeling bone,set the cup and fixed by centric screw and the screw of substantia spongiosa bone on trochanter major and trochanter minor of the femoral.The acetabulum prosthesis according to the traditional total hip arthroplasty were fixed.[Result]Nine cases were followed up for average 20 months,evaluated by Harris Score standard,excellent 5 cases,good 2 cases,fair 1 cases,poor 0 case,the rates of excellent and good is 88.89%.[Conclusion]Because of keeping entire neck and part head of the femur,biomechanics conduction and distribute of the stem-less hip of prosthesis achieves natural biomechanics state of the human body,the treatment effect of short-term is satisfying,the treatment effect of long-term will go on get observation in future.

Objective To investigate the effectiveness of interventional therapy for the atherosclerotic renal artery occlusion (ARAO). Methods During the period of June 2001-Dec. 2007, 16 patients with ARAO (total of 16 occluded arteries) underwent interventional managements, including percutaneous endovascular renal artery revascularization, balloon dilatation angioplasty and stent placement. Follow-up survey was made at regular intervals. The patent condition of the renal artery was evaluated with ultrasonography and digital subtraction angiography. The blood pressure and the renal function were determined and the data were statistically analyzed in order to assess the intermediate and long-term effect of the interventional therapy. Results Of 16 patients, technical success was achieved in 15 (93.8%) and failure occurred in one. During a follow-up period of 9 - 24 months, 3 patients died. According to the data obtained at each patient’s last follow-up survey, the hypertension fell to normal in 3 (25.0%), was improved in 7 (58.3%) and showed no marked change in 2 patients (16.7%), with a clinical efficacy of 83.3% (10 / 12). The renal function was improved in 2 (16.7%), stabilized in 6 (50%) and deteriorated in 4 patients (33.3%), with an effective rate of 66.7% (8 / 12). Conclusion For the treatment of atherosclerotic renal artery occlusion, the interventional therapy carries high successful rate and can effectively lower the blood pressure level, in addition, it can also protect the renal function in a certain degree.

Objective To investigate whether the extact of ginko biloba (EGb761) can protect the retinal ganglion cells (RGC) after acute high intraocular pressure induced retinal ischemia-reperfusion injury. Methods Thirty Sprague-Dawley rats were randomly divided into three groups. The negative control group, positive control group, treatment group were given normal saline 5 mL/kg, 1% erigeron breviscapus (vant) hand-mazz (EBHM) 5 mL/kg, 1% EGb761 5 mL/kg, respectively 2 hour after injury and every day after operation. Retinal ganglion cells were retrograde labeled by injection of 3% fluorogold (FG) into both side of superior colliculus 5 days before the death of the rats. Results The average survival rate of retinal ganglion cells were 61.46%, 72.73% and 76.20% in negative control group, positive control group and treatment group respectively. Conclusion EGb761 has neuroprotective effect on retinal ganglion cells in acute high intraocular pressure induced retinal ischemia-reperfusion injury.

Objective To perform an acceptance test for the IMRT system with independent collimator. Methods An ion chamber dosimeter were used to measure the startup characteristics of the accelerator and the absolute dose at isocenter and given characteristic points for three clinical cases ( a lower nasopharyngeal carcinoma, a lung cancer and a cervical cancer). The characteristic points represented the organs at risk or the target. A Mapeheck2 was used to measure dose maps of basic test fields and the treatment fields for the clinical cases. The basic test fields were as follows: 1 ). Symmetric fields in size of 2 cm ×2 cm, 5 cm ×5 cm, 10 cm× 10 cm, 20 cm ×20 cm, 2 cm × 10 cm, 10 cm ×2 cm, 5 cm ×20 cm and 20 cm ×5 cm;2). Asymmetric fields in size of 2 cm ×2 cm (x1 =4 cm, y1 = 10 cm;x2 = -2 cm, y2 = -8cm) and 5 cm ×5 cm (x1 = -2 cm, y1 = -5 cm;x2 =7 cm, y2 = 10 cm) ;3) A 20 cm ×20 cm composite field composed of five 20 cm× 4 cm narrow bar fields side by side. Gamma Index was used to compare calculated and corresponding measured dose distributions. When the criterion was 3% dose difference or 3 mm distance-to-agreement, the pass rate was required to be more than 90%. Results The accuracy of machine output was better than 2% when machine monitor units increased to 4. Among all basic test fields and all the treatment fields of three clinical cases, the maximal absolute dose error was -3.67%, and only the composite test field and two treatment fields of the lower nasopharyngeal carcinoma case had a pass rate slightly less than 90%, which were 83.6%, 88. 3% and 89. 7% ,respectively. For the three clinical cases the treatment delivery times were 15, 14, and 27 minutes, respectively. Conclusions The overall commissioning results are acceptable, and the system can be used in clinic.

Background and purpose: Primary intracranial epithelioid haemangioendothelioma (EH) was rare. This study was to discuss the clinical pathological features, radiographic findings, treatment and prognosis of primary intracranial EH combined with literature review. Methods: We reviewed a case of EH reported from our hospital, and comprehensively analyzed the related literature. Results: Based on our report and review of the literature, EH is rare and with borderline or uncertain behavior. The original tumors demonstrated specific image features. The tumor usually appeared as a small nest or cords composed of eosinophilic epithlioid or spindled cells.Immunohistochemical assay were positive for endothelial markers CD31, CD34 and FⅧ. EH may be treated with complete surgical resection whenever possible and is sensitive to radiation. The EH has a favorable prognosis. Conclusion: EH has variable malignant potential, and should be differentiated from meningioma. Total resection and close follow-up is recommended. Additional radiotherapy is advised for residual tumors.

Objective To study the molecular characters of porA and porB genes which encode outer membrane proteins (OMP), and predominated clonal complex of Neisseria meningitidis isolates from Guangdong province. Methods Eighteen Neisseria meningitidis isolates from Guangdong province during year 1967 to 2007 were recovered and reconfirmed by API NH biochemical system, and serogrouped by antiserum. The characters of porA and porB gene were analyzed by DNA sequencing. The allele profiles and the sequence types (ST) were determined by multilocus sequence typing (MLST). Based on their allelic profiles, the evolution relationship was analyzed by PHYLIP software. The predominant clonal complex was determined through comparing with the information of reference strains from the PubMLST database. Results For porA gene, type 20 was more frequently in the variable region (VR) 1 and type 9 in VR2 before year 2004. However, for porB gene, type 4 was more frequently in VR Ⅰ, type7 in VR Ⅳ, type 11 in VR Ⅴ,and type 10 in VR Ⅵ, respectively. The multi-types character was presented in VR Ⅴ and VR Ⅵ after2004. VR Ⅶ and VR Ⅷ can not be found among all the isolates except for one W135 isolate in 2007. Among the seven housekeeping genes, the polymorphism of abcZ was the lowest one with 4 allele numbers, while pgm was the highest one with 13 allele numbers. The predominant clonal complex was ST-5 before 2004. The ST-4821 clone complex appeared in 2004 and caused cases every year since then. More important, highly invasive ST-11 clonal complex firstly appeared in Guangdong in 2007. Conclusion The molecular characteristic of OMP genes presents polymorphism for the Neisseria meningitidis isolates from patients in Guangdong province during 1967 to 2007. ST-5 is the predominant clonal complex before 2004 and the highly invasive clonal complex is circulating in recent 3 years. It suggests that the surveillance based on laboratory should be further enhanced.

Objective To explore the relation between the infection of Helicobacter pylori (H .pylori)and colorectal tumor.Methods From January 2012 to January 2013,the medical data of 152 patients with colorectal tumor were collected.According to the findings of colonoscopy examination, the patients were divided into the colorectal adenoma group (84 cases)and the colorectal carcinoma group (68 cases).A total of 88 healthy individuals were also enrolled as control.The differences of H .pylori infection were compared among the colorectal adenoma group,the colorectal carcinoma group and the healthy control group.The distribution of H .pylori infection in different age,gender and nation was analyzed in colorectal tumor group.According to the location,maximum diameter,number,type of pedicle and pathological type of adenoma,the colorectal adenoma group was divided into subgroups and the condition of H .pylori infection was compared among the subgroups.Chi-square test was used in the statistical analysis.Results The positive rates of H .pylori infection of colorectal adenoma group, colorectal carcinoma group and healthy control group were 70.2% (59/84),72.1 % (49/68)and 53.4%(47/88),respectively,where the former two groups were both higher than that of the healthy control group,and the differences were statistically significant (χ2 =5 .147 and 5 .637,both P <0.05).Regarding the H .pylori infection,there was no difference in age,gender and ethnicity among 152 patients with colorectal tumor (all P > 0.05 ).Among 84 patients with colorectal adenoma,there was no statistical difference in the positive rate of H .pylori infection in different location,maximum diameter,number, type of pedicle and pathological type of adenoma (all P > 0.05).Conclusion H .pylori may have promoting effects on the genesis and development of colorectal tumor.

AIM: Study results are various in the mechanism and influential factors of backleg pain in patients with lumber intervertebral disc protrusion. This study aimed to explore the correlation of sagittal index and changes in nucleus pulposus inflammatory factor to low back and leg pain. METHODS: The patients with single unilateral prolapse of lumbar intervertebral disc who were non-central type lumbar disc herniation (L3-4, L4-5 or L5-S1) and non-lumbar spondylolisthesis accompanied with spinal canal stenosis and intraspinal canal disease surgically treated with limited laminectomy from October 1999 to February 2005. The total number of subjects was 41 including 30 men and 11 women and the patients ranged from 28 to 72 years old in age. Of them, 12 were the type of bulge, 15 were the type of protrusion and 14 were the type of free disc herniation of the lumbar spine. Another 17 involved intervertebral discs obtained in the decompression of lumbar bursting fractures were taken as a control group, in which were 10 males and 7 females aged 19-73 years. A questionnaire survey of painful symptoms to evaluate degree of low back and leg pain with visual analogue scale (VAS) was performed by the same non-experimental physician on admission. The ratio of the maximum sagittal diameter of protrusion and the sagittal diameter of the same level vertebral canal was sagittal index to describe degree of protrusion. The matrix metalloproteinase (MMP)-3 and interleukin (IL)-1 contents were determined. Backleg pain degree of the protrusion was analyzed. RESULTS: All patients were included in the final results. Objective To detection analysis of blood infectious disease markers in patients before blood transfusion or before surgery .Methods HBsAg ,anti-HBs ,HBeAg ,anti-HBe ,anti-HBc ,anti-HCV ,anti-HIV ,TP-Ab were detected for 3 542 cases of se-rum using chemiluminescence immunoassay(CLIA) and ELISA .Results For HBsAg ,there were 704 cases of positive among 3 542 cases ,the proportion was 19 .8% ;For anti-HCV ,there were 52 cases of positive among 3 032 cases ,the proportion was 1 .72% ;For anti-HIV ,there were 3 cases of positive among 3 049 cases ,the proportion was 0 .09% ;For TP-Ab ,there were 200 cases of positive among 3 153 cases ,the proportion was 6 .34% .The infection rate was different in different gender of HBV ,anti HCV ,anti HIV and TP-Ab .But for HBV ,anti-HCV and TP-Ab ,the detection rate of male was higher than female(P<0 .05);The infection rate was different in different age of HBV ,anti HCV ,anti HIV and TP-Ab different age .The detection rate of HBV and Anti-HCV in 21-40 years old was the highest ,They were 31 .9% and 2 .33% ;The detection rate of TP-Ab increases with age increased ,more than 60 years old was the highest ,up to 7 .55% .Detection of the above two methods ,for HBsAg ,there were 51 cases did not match ,the proportion was 1 .44% ;for anti-HCV ,there were 1 case did not match ,the proportion was 0 .03% ;for Tp-Ab ,there were 3 cases did not match ,the proportion was 0 .1% ;for anti-HIV ,CLIA agrees well with ELISA ,there were 3 cases of positive .The samples that did not match were detected by the third party verification of the reagent .The result displayed that CLIA was more accurate . Conclusion CLIA is a high-sensitivity detection method which could effectively avoid positive specimens missed ,detection of infec-tious disease markers in patients before transfusion or before surgery by CLIA had important significance .
