Septic arthritis of the sternoclavicular joint(SCJ) is a rare condition and accounts for 0.5%-1.0% of septic arthritis.SCJ infections often require surgical intervention.Diabetes mellitus, rheumatoid arthritis, intravenous drug use, intraarticular injection and immunosuppressive disorders are predisposing factors.Staphylococcus aureus and Pseudomonas aeruginosa are the most common bacteriologies.Early diagnosis of SCJ infection requires a high index of suspicion and a confirmatory CT or MRI scan. The characteristic imaging features include intramedullary and soft tissue gas, sequestra, soft tissue swelling and destruction or widening of joint space.Management strategies have ranged from conservative antibiotic therapy to en-bloc resection of the sternoclavicular joint with or without ipsilateral pectoralis major muscle transposition. The shoulder function in most patients were well preserved.

Literature review of the world on hospital brands, data collection in summary and comparison, and specialist consultancy were used to set the appraisal indicators, while the stratum analysis was used to calculate the weight coefficients of individual indicators, for building the appraisal indicators system for hospital brand effect. In addition, the indicators system and appraisal model were called into play to explore the use of hospital brands, offering scientific references for hospital management.

Objective: This study aimed to investigate the clinical characteristics, treatment options, and prognosis of multiple myeloma (MM) patients complicated with extramedullary plasmacytoma (EMP). Methods:The case of MM concurrent with EMP are rare. During the period between June 2005 and June 2013, clinical experience, diagnosis, and treatment of MM with EMP cases were retrospectively analyzed. Results:Fourteen cases were diagnosed with MM complicated with EMP in the Third Hospital of Peking Uni-versity, Beijing, China. Among these 14 cases, seven were males and seven were females. The mean age of patients was 56.9 years (ranging from 40 to 80 years old). Nine of the cases were diagnosed with EMP during the preliminary diagnosis, and the remaining five cases developed EMP during the course of treatment. Among the 14 cases, six were solitary EMPs and eight were multiple EMPs. Among the multiple EMP cases, five died, one showed disease progression, and two demonstrated partial regression. Conclusion:The prognosis of MM complicated with EMP is poor. Thus, new treatment strategies should be further explored.
5.Application of enteral nutrition tube placement and feeding equipment.
Chinese Journal of Gastrointestinal Surgery 2012;15(5):442-444
Although considerable clinical problems were solved by parenteral nutrition since 1960s, because of the risk of infectious and metabolic complications and advance in feeding tube placement, feeding methods, and artificial ingredient nutrients, enteral nutrition from 400 years ago has been brought to attention again. This review article is aimed to illustrate the issues related to enteral tube feeding in esophageal surgery.

This study seeks to explore the early signs of cognitive impairment in patients with obstructive sleep apnea hypopnea syndrome (OSAHS). According to polysomnography, twenty patients diagnosed with OSAHS and twenty normal controls underwent event-related potential (ERP) examination including mismatch negativity (MMN) and P300. Compared with normal controls, OSAHS patients showed significantly prolonged latency of MMN and P300 at Cz. After controlling age and body mass index (BMI), MMN latency positively correlated with apnea hypopnea index (AHI), oxygen reduction index, stage N1 sleep and arousal index, while MMN latency negatively correlated with stage N3 sleep and mean blood oxygen saturation; and P300 latency positively related to AHI and oxygen reduction index; no relationships were found among MMN latency, MMN amplitude, P300 latency and P300 amplitude. These results suggest that the brain function of automatic processing and controlled processing aere impaired in OSAHS patients, and these dysfunction are correlated with nocturnal repeatedly hypoxemia and sleep structure disturbance.

Objective To explore effect of endogeneous gangliosides(Gls) and integrin ?2?1 on protein phosphotyrosine expression of pp125 focal adhesion kinase (pp125FAK) after adhesion of SK-N-SH neuroblastoma cells to collagen(Col). Methods SK-N-SH cell line with high expression of integrin ?2?1 was cultured in presence of D-threo-1-phenyl-2-decanolamino-3-morphinoline-1-propanol(D-PDMP).Effect of endogeneous Gls,anti-?2 and anti-?1 monoclonal antibody on protein phosphotyrosine expression of pp125FAK during adhesion of SK-N-SH cells to Col were determined by immunoprecipitate and Western blotting. Results After 6 days, endogenous Gls in cells were almost depleted.Gls-depletion,anti?2 and anti-?1 monoclonal antibody were able to decrease pp125FAK expression of SK-N-SH cells adherent to Col respectively.GD2,the major component of neuroblastoma cell Gls could reco-ver pp125FAK expression to a certain degree. Conclusions Endogenous tumor Gls regulate protein phosphotyrosine expression of pp125FAK during adhesion of neuroblastoma cells to Col.It is suggested that tumor Gls may increase signal transduction of tumor cell integrin ?2?1 by increasing tyrosine phosphorylation of pp125FAK.

The purpose of this study is to investigate the activity and mechanism of a new anti-tumor agent T03. MTT and colony formation assay were performed to determine anti-proliferation activity of T03 in vitro. Antitumor activity was observed by Renca xenograft model in vivo. The effect of T03 on cell cycle and apoptosis were measured by FCM analysis. Western blotting was performed to investigate the expression level of proteins in HepG2 cell lines treated with T03. T03 had anti-tumor activity by inhibiting tumor cell growth and colony formation in vitro, especially on hepatocellular carcinoma cells (HCC). At the concentration of 10 micromol x L(-1), T03 induced cell apoptosis and cell cycle arrest in HCC. Moreover, it proved that T03 reduced the tumor weight with the rate of 42.30% without any obviously side effect in Renca xenograft model. At the concentration of 2.0 micromol x L(-1), T03 was able to reduce the level of p-c-Raf (Ser259), and thus blocked Raf/MEK/ERK and AKT signaling in HepG2 cell lines. The result suggested that T03 has the potential to inhibit cell proliferation and induce cell apoptosis both in vitro and in vivo, particularly active against HCC, indicating T03 and its analogues may serve as a new anti-cancer drug against hepatocellular carcinoma.

Objective To explore the clinical effect of high intensity focused ultrasound(HIFU)combined with transcatheter arterial chemoembolization(TACE)in the treatment of primary liver cancer. Methods A total of 106 patients with primary liver cancer were divided into two groups:50 cases were treated with TACE,and the other 56 were treated with combination of HIFU and TACE.The changes of AFP levels and the size of tumors after three months treatment were analyzed and compared with each other. The survival rates for one,two and three years were calculated with Kaplan-Meier method and compared between the two groups. Results In the two groups,AFP decreased significantly after treatment, but the combined group was superior to the other in AFP decrease. In the combined group, the 1-,2-and 3-year survival rates were higher than those in the TACE group with 82.3%,60.8%and 39.2% vs 68.0%,42.6%and 21.0%respectively(P<0.01).No serious complications were seen, such as burn of skin, bleeding, gastrointestinal perforation. Conclusion The use of HIFU combined with TACE in the treatment of patients with primary liver cancers is feasible and safe. The combined group is superior to simple TACE for the management of primary liver cancers, and the former is more effective in decreasing AFP level and improving survival rates.
