AIM To discuss the effects of losartan on AT 1R mRNA expression and renin angio tensin system (RAS) level in brain of spontaneously hypertensive rat (SHR). METHODS Sixteen 6 week old male SHR and 16 sex and age matched WKY were divided into losartan ( n =8) and control groups ( n =8) respectively. Losartan (30 mg?kg -1 ?d -1 ) or equal volume of 0.9% saline solution was administered for 18 weeks by gavage. AT 1R mRNA expression was examined by reverse transcription polymerase chain reaction (RT PCR). Renin activity (RA) and angiotensinⅡ(AngⅡ) levels were measured by radio immunoassay. Angiotensin converting enzyme activity (ACEA) was measured by ultraviolet spectrophotometar. RESULTS Enhanced brain AT 1R mRNA expression was found in SHR compared with WKY ( P 0.05, respectively). CONCLUSION The increase of AT 1R mRNA expression and RAS levels in brain may be involved in the pathogenesis of hypertension in SHR. Losartan protects brain by decreasing expression of brain AT 1R mRNA.

BACKGROUND: Serum creatinine (SCr) is the most commonly used parameter to estimate renal function impairement, but there are some shortcomings. Many factors including age, gender, drug, diet, muscle mass and metabolic rate can influence SCr, leading to an inaccurate estimation of kidney impairment. Studies have shown that cystatin C (CysC) is not affected by factors such as muscle mass, age, gender, diet, inflammation or tumor. The present study was undertaken to compare the sensitivity of CysC and SCr in evaluating renal function impairment at early stage of shock. METHODS: Seventy-one patients aged 38.3±21.4 years, who had been treated at the Emergency Medicine Department of the First Affiliated Hospital, Sun Yat-sen University between February 2006 and June 2007, were studied. They were divided into groups A, B, C, and D according to the shock time. Serum sample was drawn from each patient at 1, 2, 3, 4 hours after shock to determine SCr and CysC. CysC and SCr were determined again at 72 hours and 7 days after shock. RESULTS: CysC increased earlier than SCr in the 71 patients, and CysC decreased slower than SCr when shock was corrected. CysC increased at 1 hour after shock. There was a negative correlationship between CysC, SCr and glomerular filtration rate (GFR), especially at early stage of shock. CONCLUSIONS: There is renal injury at early stage of shock. CysC is more sensitive than SCr in assessing renal function at the early stage of shock.

Objective To study the relationship between overlapping symptoms (OS) and psycho- mental factors in patients with functional dyspepsia (FD) in order to understand the possible mechanisms of OS.Methods One hundred and three FD patients diagnosed by Rome Ⅲ criteria were divided into with or without OS groups by Rome Modular Questionnaire. The emotional status, sleep status, anxiety and depression status were evaluated by Zung Anxiety and Depression Scale and quality of life was eveluated by SF-36 questionnaire. The results were compared between the two groups. Results The proportion of the patients with and without OS accounted for 62.5% and 37.5%,respectively.The patients with OS had a worse emotional status ("bad%" and "very bad%" being 39.1% and 20.2% vs.15.4% and 12.8%,P=0.018) and a worse sleep status (45.3% and 11.0% vs.19.1% and 7.7%,p=0.018)than those without OS.The proportion of the patients with anxiety (49 % ) and depression (68 % ) status in OS group was significantly higher than that in without OS group (19% and 44%,respectively,P<0.05). The quality of life of the patients with OS was worse in the dimensions of psychological health (47.9), vitality (47.1) and mental component score (32.8)than those in without OS group (57.8,57.0,37.0, respectively,P<0.05).Conclusions The psycho-mental factors are associated with overlapping symptoms in patients with FD,which suggests that abnormal psycho-mental factors may be involved in the mechanisms underlying overlapping gastrointestinal symptoms in FD.

Objective To explore whether the pathophysiological mechanisms are same or not in different subtypes of functional dyspepsia (FD) patients. Methods Severity of symptoms and (44.2± 14. 6) years. Multi-gastrointestinal functions were also tested. Results No statistical difference was found among epigastric pain syndrome (EPS), postprandial distress syndrome (PDS) and mixed type in the data of gastric emptying function, gastric myoelectrical activity, autonomic nerve function, simultaneous introgastric pressure and water loading test, and psychological status assessment as well (P>0.05). There was a positive linear correlation between postprandial fullness and half gastric emptying time (correlation coefficient=11.5, P= 0. 043). Early satiation was negatively related to maximal water intaking volume (correlation coefficient=-6. 8, P=0. 033). Conclusions In this study, it suggested that the FD patients with different pathophysiologic mechanisms cannot be distinguished efficiently according to Rome Ⅲ criteria, and then be treated in a well-targeted manner.

Objective To explore the application of the exercise test (ET) in paroxysmal kinesigenic dyskinesia (PKD).Methods We conducted the ET in 33 controls and 45 PKD patients following standardized protocols, and the decrement in the amplitude and in the area of compound muscle action potentials (CMAP) after long ET and other seven parameters were calculated and compared in both groups, and the normal range of parameters was defined as the mean ± two standard deviation of the control values. Results According to the control group, the normal range of the change in the amplitude of CMAP immediately after short ET was-13%-27% and the normal range of decrement in the amplitude of CMAP after long ET was less than 33%.And the ET was abnormal in 16 of the 45 PKD patients (36%).Comparing the nine parameters between two groups,we found that both the decreases of the amplitude (25.5% ±13.4% vs16.2% ±8.6%,t=-3.72,P=0.00) and the area(31.8% ±16.3% vs19.0% ± 16.8%,t =-2.39,P =0.02) of CMAP after long ET in PKD patients were larger than those in controls. No statistically significant difference was found between 19 non-ion channel disease patients and 14 normal controls. No statistically significant difference was found between the nine parameters in 27 PKD patients before and after medication, though all the patients had obvious clinical improvement. Conclusion We found that abnormal ET may appear in PKD patients, and this finding hinted that abnormal muscle membrane excitability might be an underlying mechanism responsible for PKD.

In-vitro assay methods were established to evaluate transactivation and binding activity of compounds on peroxisome proliferator-activated receptor y (PPARγ). Firstly, plasmids were constructed for transactivation assay of PPARγ response element (PPRE) triggered reporter gene expression, and for cell-based binding activity assay of the chimeric receptor, which was fused with PPARγ ligand binding domain (LBD) and yeast transcriptional activator Gal4. Secondly, by using PPARy competitive binding assay based on time resolved-fluorescence resonance energy transfer (TR-FRET), affinities of compounds and drugs to PPARγ were evaluated. In application of these above methods, the PPARγ activating potency and characteristics of different compounds were evaluated, and a novel benzeneselfonamide derivative, ZLJ01, was found to have comparable binding activity and affinity with the well-known PPARy agonist, but lack of PPRE mediated transactivation activity. In preliminary study on in-vitro hypoglycemic activity, ZLJ1 was found to promote insulin-stimulated glucose uptake by liver cells. Therefore, we believe that combining transactivation and binding activity as well as affinity evaluation, the system could be used to screen non-agonist PPARγ ligand as anovel PPARγ modulator
