3.MRI diagnosis of pituitary abscess and its clinical significance

Objective To investigate the MR features of pituitary abscess.Methods The MR features of 14 cases of pituitary abscess proved by surgical pathology and clinical treatments were analyzed retrospectively.Results Pre-contrast MR showed hypointense heterogeneous intensity on T_1 WI in 12 cases and iso-hyperintense on T_1 WI in 2 cases,hyperintense on T_2 WI in all cases.Post-gadolinium MR showed the ring-like enhancement around the uneven edge of abscess and the surrounding enhanced meninges connecting to the focus.The normal pituitary could not be identified in all 14 cases.The MR specific findings include the fluid-fluid level,nodule on the edge and the enhanced patchy shadow.Conclusions The pituitary abscess has specific findings on MR examination,which can be used to combine with clinical symptoms to achieve the diagnosis before operation,so that the cases could be treated with antibiotic without operation.
4.Advance on nitric oxide combined photothermal therapy for tumor treatment

Photothermal therapy (PTT) has attracted significant attention due to minimal side effects and high treatment specificity. However, it often requires very high temperature to achieve complete tumor ablation under a single PTT. Such high temperature brings obvious thermal damage and inflammatory response to the body, affecting the therapeutic effect. In recent years, nitric oxide (NO) has been used to significantly inhibit tumor growth and enhance the sensitivity of tumor cells of temperature and drugs, thus enhancing the therapeutic effect. However, compounds as NO donors often have some disadvantages such as poor biocompatibility and untargeted delivery, etc., therefore, this medical application based on NO therapy is limited. In conclusion, the organic combination of NO donors and photothermal agents (PTAs) is expected to overcome the shortcomings of single therapy and achieve the antitumor effect of "1 + 1 > 2". In view of the rapid development of NO combining with PTT in tumor therapy, this review firstly introduces the antitumor mechanisms of different types of NO donors. Then the treatment strategy based on NO combined with PTT is discussed. Finally, the prospects and challenges of this combination therapy strategy in the clinical treatment of cancer are discussed.
5.Therapeutic effect of regular urethral dilatation on patients with postoperative urethral stricture

Objective: To determine the therapeutic effect of regular urethral dilatation on patients with postoperative urethral stricture. Methods: A total of 142 patients underwent urethral stricture. The unimproved patients after the surgery were divided into a reoperation group and a regular urethral dilatation group. All the patients were followed up for 1-3 months, and the curative effect was compared. Results: Of the 142 patients, 42 had no improvement and 27 of them had reoperation, and symptoms in 21 were improved. Another 15 patients received regular urethral dilatation, and 5 improved. hTere was signiifcant difference between the 2 groups (P<0.01). Conclusion: Regular urethral dilatation has some effect on postoperative patients, but no obvious effect on patients with dissatisifed operation for urethral stricture or restenosis.
6.Retrospective studies on 713 cases chronic obstructive pulmonary disease.

OBJECTIVETo study the diagnostic criteria of severity of chronic obstructive pulmonary diseases (COPD), early intervention, and the effect of health education on the relationship between smoking and health.
METHODSData from 713 remissive COPD patients who had results of pulmonary function test in the past 10 years were reviewed. All patients were classified by correlative diagnostic criteria, and the results were compared with the diagnostic criteria of WHO Global Initiative for chronic obstructive lung disease (GOLD). Other indexes such as history of smoking, quantity of smoking and the rate of giving up smoking were also analyzed.
RESULTSAccording to the diagnostic criteria set by the Chinese Respiratory College in 1997 and traditional pulmonary function criteria, 339 patients were identified as having severe COPD, taking up 56.0% of all COPD patients; 200 patients were moderate (28.0%); 114 patients were mild (16.0%). FEV(1)/FVC in mild COPD patients was 70.5% +/- 1.1%, higher than the criteria of 70% in GOLD. Three hundred and thirty-five patients were smokers, taking up 46.9% of total patients. In these patients, 312 patients were men (93%) and 23 patients were women (7%). The average smoking history was 32.9 years, and the average quantity of daily smoking was 22. Two hundred sixteen patients had given up smoking which took up 64.5% of all the smokers.
CONCLUSIONSIt was debatable that FEV(1)/FVC was decisive criteria for COPD diagnoses by GOLD and the revised diagnostic criteria of Chinese Respiratory College in 2002. It might miss diagnosing a number of COPD patients who having combined ventilatory disturbance or were in earlier stages. Only 15.9% of mild COPD patients had seen a doctor, that indicated a very low rate of early intervention which was disadvantageous for the prevention and cure of COPD. In contrast to the rate of 28.0% when giving up smoking in ordinary people who had received general intervention, the rate of giving up smoking in COPD patients remained 64.5% much higher and was dissatisfactory. Health education should be improved to include measures to give up smoking.

Objective To investigate whether elevated pre-procedural C-reactive protein (CRP) and Interleukin-6 (IL-6) concentrations may be relevant to early outcome in patients undergoing PCI.Method 100 consecutive patients undergoing pereutaneuous coronary intervention (PCI) were included in our study.Peripheral blood samples for CRP and IL-6 testing were withdrawn before PCI.Acute vascular complications resulted from PCI were determined by subsequently coronary angiography.The early coronary events during hospitalization were clinically followed.Results Thirty patients developed acute vessel occlusion,and another one developed subacute coronary thrombosis at 2 days after PCI.Increased levels of CRP correlated well with the occurrence of vascular complications as regards the significant difference existing amongⅠvsⅢandⅠvsⅣquartile groups,P
8.Multiple acute cerebral infarcts as initial manifestation of occult systemic malignancy with 12 case report

Objective To characterize the clinical manifestations, laboratory findings of patients with occult sys?temic malignant neoplasms, whose initial manifestation presented as multiple acute cerebral infarcts including coagula?tion function,radiological imaging and microembolic signals (MES) detection by transcranial Doppler sonography (TCD) and to explore the possible underlying mechanisms. Methods All clinical records, laboratory hematological tests includ?ing hypercoagulable states measured by D-dimer levels, brain MRI including DWI, and TCD monitoring MES, the treat?ment and prognosis were retrospectively reviewed in 12 patients with multiple acute cerebral infarcts as the first manifes?tation of occult systemic malignancy. Results The clinical manifestations presented as localized neurological dysfunction, e.g. hemiparesis, aphasia, hemiparesthesia, dysarthria, vertigo and seizures, etc. DWI revealed multiple disseminated acute cerebral infarcts in multiple arterial territories such as the bilateral anterior or anterior plus posterior cerebral circu?lation in all patients. Eleven of 12 patients tested had elevated D-dimer. TCD detected MES in 5 of 7 patients. There were 12 patients diagnosed with occult systemic malignancy including 5 lung cancer, 3 pancreatic cancer, 1 gastric can?cer, 1 colon cancer, 1 endometrial adenocarcinoma and 1 metastatic poorly differentiated mucinous adenocarcinoma with unknown primary. Ten patients already had remote metastasis at diagnosis. The prognosis was usually poor and there were 7 cases with ischemic stroke recurrence, 4 cases with acute myocardial infarction, 3 cases died during hospitaliza?tion. Conclusions When patients present with multiple disseminated acute cerebral infarcts involving multiple arterial territories as initial manifestation, the underlying occult systemic malignancy should be considered. Hypercoagulopathy and MES might provide the clues to the diagnosis.
9.Screening of the target genes transactivated by PS1TP1 protein with suppression subtractive hybridization technique.

OBJECTIVETo construct a subtractive cDNA library of genes transactivated by PS1TP1 protein with suppression subtractive hybridization (SSH) technique.
METHODSSuppression subtractive hybridization technique and bioinformatics technique were used, the mRNA from HepG2 cells transfected with pcDNA3.1 (-) -PS1TP1 and pcDNA3. 1 (-) empty vector was isolated, respectively; cDNA underwent two times of nested PCR, amplified cDNA fragments were subcloned into pGEM-Teasy vectors to set up the subtractive library.
RESULTSThe subtractive library of genes transactivated by PS1TP1 was constructed successfully. Sequence analysis was performed in 43 clones randomly, and the full length sequences were obtained with bioinformatics method and searched for homologous DNA sequence from GenBank, altogether 12 coding sequences were gotten, which consisted of 10 known and 2 unknown ones.
CONCLUSIONThe obtained sequences may be target genes transactivated by PS1TP1 protein among which some genes coding proteins involved in cell cycle regulation, metabolism, immunity and cell apoptosis. This finding brought some clues for studying the biological functions of PS1T1.

10.Construction and immunogenicity evaluation of chimerical DNA vaccine of human papillomavirus type 11.

OBJECTIVETo construct chimerical DNA vaccine plasmid of human papillomavirus type 11 (HPV11) L1-E7, and to evaluate its immunogenicity.
METHODSMolecular cloning techniques were used to construct recombinant plasmid pcDNA3 L1-E7 as a DNA vaccine. BALB/c mice were vaccinated with DNA recombinants through muscle injection.IL-2 and gamma-INF secreted by immunized spleens lymphocyte and HPV 11 L1 or E7 specific antibodies were assayed by ELISA method. Spleens lymphocyte proliferation was measured by MTT assay.
RESULTSThe chimerical DNA plasmid of pcDNA3 L1-E7 was constructed correctly. Specific anti-HPV11 E7 and L1 antibodies, specific lymphocyte proliferation and secretions of IL-2 and gamma-INF were detected in vaccinated mice.
CONCLUSIONSpecific immune response, including cellular immunity and humoral immunity, could been detected in mice vaccinated with chimerical DNA vaccine of pcDNA3 L1-E7.
