1.The trend of intraocular lens design: aspheric or spherical?

Spherical aberration is the major cause which affects the functional vision of the patients implanted with spherical intraocular lenses (IOLs). Aspheric IOLs are designed to eliminate the aberration like the nature lens, thus they can produce better visual quality. If it is feasible to customize IOL design to the individual cataract patient, aspheric IOLs may substitute the traditional spherical IOLs and become the trend of IOLs.
2.Attention to the research about congenital cataract candidate gene

Congenital cataracts is the leading cause of visual disability in children worldwide.With the evolution of molecular biological technique,the researches of inherited congenital cataract mainly focus on localization and fuction characterization of the disease-associated gene,including crystallin,connexin,major intrinsic protein gene,etc.And great progress has been made for such genes in eye development,age-related cataract,epigenetics study.Thus,great attention must be paid to the functional research about congenital cataract candidate gene.
3.Decalcified bone matrix and bone cement compound at various proportions in repairing rabbit femoral defect

BACKGROUND:BACKGROUND:Present studies have shown that bone cement have less osteoinduction and slow degradation in the body,so the effect of alone application is not satisfactory. Therefore,people have modified them,hoping to research a new materials that can overcome all shortcomings above-mentioned. OBJECTIVE:To investigate the ability of decalcified bone matrix (DBM) and acrylic bone cement (ABC) compound in filling and repairing bone defect,and to determine the best component proportion of the composition. DESIGN,TIME AND SETTING:Randomized controlled animal experiments were performed at the Experimental Animal Center,Chongqing Medical University from May to September 2008. MATERIALS:ABC and DBM were prepared into compound material at various proportions (2:8,3:7,4:6,5:5,6:4). METHODS:The animal models of bilateral femur bone defects and fillers were established in the New Zealand White rabbits. The complex material was implanted on the bone defect; the ABC was used as blank control group. MAIN OUTCOME MEASURES:The complex and alone materials were examined by biomechanics and scan electron microscopes. Gross observation,histopathological and X-ray test were used at 4,8,12 weeks after operation respectively to observe the repaired and filled status of bone defect. RESULTS:DBM composite quality of the scope of 3:7 -6:4,the composite materials that existed in more cracks more than 100 ?m,when the DBM was less than 3:7,the material inside the aperture and the majority of material was similar to ABC,mass ratio greater than 6:4 when the DBM and the ABC could not effectively together solidification. 5.Clinical significance of APACHE Ⅱ score in patients with acute myocardial infarction

Objective To explore the clinical significance of APACHE Ⅱ score on risk stratification in patients with acute myocardial infarction.Methods One hundred and forty-two patients with acute myocardial infarction were included in the study who were admitted to hospital consecutively from Jan.2011 to Dec.2012.High risk group and low risk group were divided by the APACHE Ⅱ score.Some clinical variables at the first 24 h after admitted to hospital and occurring during the following 1 year were recorded Results The age in high risk group was 78.9 ±8.1,significantly higher than that in low risk group (65.8 ± 10.2;t =6.835,P < 0.001).The cases with Killip Ⅲ and Ⅳ in high risk group were 10(29.4%) and 2(1.8%),lower than in low risk group (6 (1.8%) and 0 (0%) ; x2 =21.950,15.777 ; P < 0.005).The level of creatine kinase-MB (CK-MB) and cardiac troponin Ⅰ in high risk group were (147.7 ±21.5) U/L and (105.5 ± 17.6) U/L,higher than in low risk group((105.5 ± 17.6) U/L,(42.9 ± 6.3) U/L; t =11.541,5.785 ; P < 0.001).The incidence of severe cardiac events(malignant arrhythmia,heart failure,non fatal myocardial infarction and death from any cause in high risk group were 58.8% (20/34),47.0% (16/34),17.6% (6/34) and 17.6% (6/34),higher than in low risk group (29.6% (32/108),3.7% (4/108),6.4% (7/108) and 6.4% (7/108) ; x2 =8.274,36.665,3.876 and 3.876 ; P < 0.05).Conclusion APACHE Ⅱ score is likely to be a simple and practical tool for risk stratification in patients with acute myocardial infarction.

9.ERCP with double-channel endoscope for patients with Billroth-Ⅱ gastroenterostomy

Objective To evaluate the therapeutic effect and safety of forward-viewing,doublechannel endoscope in the process of ERCP in patients with bile duct diseases after Billroth-Ⅱ gastroenterostomy.Methods ERCP was applied to 46 patients with bile duct diseases using forward-viewing,doublechannel endoscope.All patients had the history of Billroth-Ⅱ gastroenterostomy.The success rate of selective cannulation,complication rate and the outcome of all the patients were recorded.Results The procedure succeeded in 38 patients with a success rate of 82.6%,and failed in 8.Of all the failing cases,the afferent loop couldn't be accessed in 6,failure in biliary cannulation in 2.Of 38 successful cases,bile duct stones were successfully removed in 31,metal stents placed for malignant bile duct obstruction in 4,and endoscopic balloon dilation for benign common bile duct stricture in 3.All patients recovered without complications of perforation,delayed bleeding or pancreatitis.Conclusion For patients with history of Billroth-Ⅱgastroenterostomy,ERCP with forward-viewing,double-channel endoscope yields to a higher success rate and lower complication rate.
10.Effect of mechanical ventilation guided by esophageal pressure on hemodynamics and oxygen metabolism of SAP porcine model with IAH

Objective To study the effect of mechanical ventilation guided by esophageal pressure on hemodynamics and oxygen metabolism of severe acute pancreatitis (SAP) swine model with intra-abdominal hypertension (IAH) Methods By self-controlled study,SAP model was made by infusing sodium taurocholate (5%) into the pancreatic duct in 6 domestic swine.Mechanical ventilator mode was volumeassist control with tidal volume 10 ml/kg ; FiO2 40% and PEEP 5 cm H2O (routine ventilation) given to SAP model swine.After 3 h ventilation,pneumo-peritoneum was made with N2 gas to increase the intraabdominal pressure (IAP) to 25 mm Hg in SAP swine.Three more hours later,PEEP was adjusted as the measurement of esophageal pressure (Pes guided ventilation) to such a level that trans-pulmonary pressure stayed above 0 cm H2O during end-expiratory occlusion.During the investigation period,heart rate (HR),cardiac output index (CI),central venous pressure (CVP),mean arterial pressure (MAP) and pulmonary arterial wedge pressure (PAWP) were continuously recorded with the aid of Swan-Ganz catheter and ECG monitor,and oxygen partial pressure of artery (PaO2) and carbon dioxide partial pressure of artery (PaCO2) were measured by blood-gas analysis.In addition,systemic oxygen delivery (DO2) and systemic oxygen consumption (VO2) were calculated by using the data of blood-gas analysis of arterial and central venous blood.Results No swine model was subjected to barotrauma.After routine mechanical ventilation,there were significant differences in HR,CI,MAP,CVP,PAWP,Ppeak,Pplat,Pes,pulmonary compliance (Cstat),PaO2,and DO2 between SAP and IAH in swine (all P < 0.05).Compared with routine ventilation,however,PaO2 and Cstat improved significantly with lower CI and increased Pplat after PEEP adjusted according to measurements of esophageal pressure (all P < 0.05).The lactate decreased significantly after esophageal pressure guided ventilation (all P < 0.05).There were no significant changes in PaCO2,HR,MAP,CVP and PAWP in IAH swine after mechanical ventilation with routine parameters (all P > 0.05).Conclusions There were remarkable effects on oxygen metabolism in response to mechanical ventilation guided by esophageal pressure.In case of clinical application of mechanical ventilation,the results of this study are in favor of setting transpulmonary pressure according to measurements of esophageal pressure in SAP patients with IAH in an early stage.
