1.Application of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.

OBJECTIVETo study the applied value of double joystick technique in reduction-internal fixation for femoral shaft fracture in adults.
METHODSThirty-four patients (24 males and 10 females) with femoral shaft fractures were treated with reduction assisted by double joystick technique and internal fixation with interlocking intramedullary nail from September 2010 to June 2013. The average age of the patients was 41 years old, ranged from 17 to 65 years old. The duration of the disease course ranged from 3 to 7 days, with a mean of 5 days. The fractures belonged to AO types 32A (5 cases), 32B (20 cases) and 32C (9 cases) and located in left femur for 18 patients and right femur for 16 patients. The patients were followed up, and fracture healing and complications were observed. The curative effect were evaluated according to Thorsen femur fracture evaluation standard.
RESULTSThe operative time ranged from 40 to 110 min (mean 75 min) and intraoperative blood loss ranged from 200 to 300 ml (mean 250 ml). All the patients obtained a good fracture reduction and were followed up for 12 to 24 months (mean 18 months) after the surgery. All the fractures united between 4 and 8 months with a mean of 5 months. No complications such as breakage of nail, infection, osteofascial compartment syndrome, refracture and fracture malunion were found. According to Thorsen femur fracture evaluation standard, 30 patients obtained an excellent result, 3 good and 1 fair.
CONCLUSIONIn surgery of reduction-internal fixation for femoral shaft fracture in adults, the use of double joystick technique obtains good reduction result, short operative time, less injury, high healing rate of bone fractures, less complications and good limbs function, so it is worthy of popularizing in clinic.

6.Effect of Tangnaoluotong Combined with Intravenous Low Intensity Laser Irradiation on Blood Rheology and Endothelin of Diabetes Mellitus and Cerebral Infarction Patients

Objective To observe the effect on the patients of diabetes mellitus and cerebral infarction in terms of level of blood rheology and endothelin after using both intravenous low intensity laser irradiation (ILILI) and traditional Chinese medicine Tangnaoluotong in the treatment. Method Ninety patients of diabetes mellitus and cerebral infarction, having the symptom of deficiency in both vital energy and yin with blood stasis, were divided into three treatment groups at random:ILILI group (A), TCM treatment group (B), and a group with compound treatment of ILILI and TCM (C). The level of blood rheology and endothelin were observed after 1 month’s treatment. Result Compared with group A and group B, group C could significantly reduce the level of fibrinogen and endothelin, improve the deformed ability of RBC. Conclusion ILILI with Tangnaoluotong can significantly alter the blood rheology and endothelin on the patients of diabetes mellitus and cerebral infarction, which result in a better curative effect.
7.Theory and Practice on Construction of TCM Jaundice Thematic Database

The traditional Chinese medicine (TCM) literature database of jaundice is an important means of the realization of the standardization and digitalization of TCM liver disease syndrome differentiation and treatment.
This article analyzed the background of jaundice literature database and its establishment and significance . The building of TCM jaundice database was described in this paper from literature data source, TCM jaundice the-saurus, literature collection and analysis. A brief introduction was made on contents of the database and the function. Deficiencies of the literature database such as the standardization, quality, daily maintenance, talent and team training were also explained .
8.Allogenic acellular extracellular matrix repairs high anal fistula

BACKGROUND:Treatment methods for high anal fistula include fistula removal, incision and thread drawing, selective mucosal flap displacement, fibrin glue closure, fistula liagtion between sphincter, most of which show many advantages, including long-term healing, a low success rate, high recurrence rate, and high postoperative complication rate.
OBJECTIVE:To observe the clinical effect of acelular extracelular matrix in the treatment of high anal fistula, and to explore a minimaly invasive treatment for high anal fistula.
METHODS: Totaly 100 cases of high anal fistula were randomly divided into treatment group and control group, 50 cases in each group. Treatment group were treated with alogenic acelular extracelular matrix, and control group were treated with traditional low incision with high thread-drawing. Then, we observed and compared the operation time, bleeding volume, postoperative pain score (visual analog scale score), postoperative pain
duration, anal incontinence severity score (Wexner score), wound healing time, one-stage success rate, cure rate, recurrence rate.
RESULTS AND CONCLUSION: Compared with the control group, the treatment group showed lower scores in the operation time, bleeding volume, wound healing time, visual analog scale score, postoperative pain duration, and anal incontinence severity score (P < 0.05), but higher scores in one-stage success rate and cure rate (P < 0.05). There was no difference in the recurrence rate between the two groups. These findings indicate that the alogenic acelular extracelular matrix for treatment of high anal fistula exhibits smal trauma, quick recovery,
short course of treatment, high cure rate and has no damage to the anal function and appearance.
9.Cystatin C and serum creatinine in estimating acute kidney injury of shock patients

BACKGROUND: Serum creatinine (SCr) is the most commonly used parameter to estimate renal function impairement, but there are some shortcomings. Many factors including age, gender, drug, diet, muscle mass and metabolic rate can influence SCr, leading to an inaccurate estimation of kidney impairment. Studies have shown that cystatin C (CysC) is not affected by factors such as muscle mass, age, gender, diet, inflammation or tumor. The present study was undertaken to compare the sensitivity of CysC and SCr in evaluating renal function impairment at early stage of shock. METHODS: Seventy-one patients aged 38.3±21.4 years, who had been treated at the Emergency Medicine Department of the First Affiliated Hospital, Sun Yat-sen University between February 2006 and June 2007, were studied. They were divided into groups A, B, C, and D according to the shock time. Serum sample was drawn from each patient at 1, 2, 3, 4 hours after shock to determine SCr and CysC. CysC and SCr were determined again at 72 hours and 7 days after shock. RESULTS: CysC increased earlier than SCr in the 71 patients, and CysC decreased slower than SCr when shock was corrected. CysC increased at 1 hour after shock. There was a negative correlationship between CysC, SCr and glomerular filtration rate (GFR), especially at early stage of shock. CONCLUSIONS: There is renal injury at early stage of shock. CysC is more sensitive than SCr in assessing renal function at the early stage of shock.
10.Effects of Schistosoma japonicum infection on the expression of Arg-1 and Fizz-1 proteins in liver tis-sues of mice with high-fat diet induced obesity

Objective To investigate the effects of Schistosoma japonicum infection on selective activation of macrophages and insulin resistance in liver tissues of mice with high-fat diet induced obesity and the possible mechanism. Methods Thirty-six male C57BL/ 6J mice were randomly divided into 3 groups in-cluding normal control group(NC group,n=12),high-fat diet feeding group(HF group,n=12)and high-fat diet feeding with Schistosoma japonicum infection group(HSj group,n=12). Specimens were collected 6 and 12 weeks after high-fat diet feeding. The levels of fasting blood glucose(FBG),fasting plasma insulin (FINS)and homeostasis model assessment for insulin resistance index(HOMA-IR)were detected. The ex-pression of interleukin-6(IL-6),arginase-1(Arg-1)and found in inflammatory zone-1(Fizz-1)at mRNA and protein levels were detected by semi-quantitative reverse transcription polymerase chain reaction(RT-PCR)and immunohistochemistry,respectively. Results The mice from HF group showed higher levels of HOMA-IR than those form NC group and HSj group by the end of 6 and 12 weeks after infection(P﹤0. 05). The levels of HOMA-IR in mice from HF group were increased by the end of week 12 after infection as com-pared with those at week 6(P>0. 05). The levels of IL-6 in mice from both HF group and HSj group were higher than those from NC group by the end of week 6 after infection(P﹤0. 05). Higher levels of IL-6 were detected in mice from HF group as compared with those from HSj group and NC group by the end of week 12 after infection(P﹤0. 05). The expression of Arg-1 and Fizz-1 in mice form HF group were lower than those from NC group by the end of 6 and 12 weeks after infection(P﹤0. 05). Arg-1 was highly expressed in mice form HSj group,followed by those from NC and HF groups 12 weeks after infection. The expression of Fizz-1 in mice from HSj group was the highest among the three groups by the end of week 6 and 12 after infection (P﹤0. 05). Conclusion The proinflammatory effects on mice with diet induced obesity were induced dur-ing acute infection of Schistosoma japonicum cercariae(6 weeks). The chronic infection of Schistosoma ja-ponicum cercariae(12 weeks)might be helpful in reversing hepatic insulin resistance in mice with diet in-duced obesity by changing the polarity of macrophages in liver tissues.
