1.Experimental Study on Acute Gastric Mucosal Lesion.

Acute gastric mucosal lesions (AGM lesions) is a general term applied to conditions characterized by the acute development of mucosal lesions in the form of erythema, mucosal hemorrhage, erosions and ulcerations in the mucosa of the stomach and duodenum. Although the pathopphysiological events leading to the formation of these lesions remain unknown, we do know that they are after preceded by one of many situations. It has become traditional to use the term stress ulcer to describe AGM lesions proceded by a major stress such as that of an operation or of severe thermal burns or hemorrhagic shock. The silent clinical manifestation of acute gastric mucosal lesions, regardless of their cause, is bleeding. AGM lesions were experimentally produced by brain injury and administration of steroid. This experimental study was conducted in order to study the so called AGM lesions, especially on production and pathology of them. This experimental animals, normal adult rate, were divided into 4 groups the first group of brain injury, the second group of brain injury and administration of steroid, the third group of administration of steroid only, and the fourth group of normal control with administration of normal saline and normal rats. The frequency of AGM lesions was studied in relation to each experimental group, experimental period and grade of lesions. The AGM lesions were divided into 3 grades depending on the macroscopic and microscopic findings. 1. AGM lesions were observed in 34 out of 63 all experimental animals except for control group of animal. Majority of the lesions were found in the glandular portion of the stomach. 2. In the brain injured group, the lesions that was erythematous and superficial mucosal erosion were found in 3 out of 21(14.3%), which were observed only in experimental period of 3-5 days. 3. In the group with brain injury and steroid administration, the lesions were found in 16 out of 21 animals(76.2%) among them grade 1 was in 2 out of 16, grade 2 in 10 and grade 3 in 4, which observed in the period of 2-7 days. 4. In the group with administration of steroid, the lesions were found in 15 out of 21 animals(71.4%), among them grade 1 was 2 out of 15, grade 2 in 11 and grade 3 in 2, which started to be observed form 2nd day through out the experimental period. 5. No lesions were investigated in the control group. No correlation between the variety of brain injury and production of AGM lesion was studied.

2.Emphysematous prostatitis combined with a liver abscess caused by Klebsiella pneumoniae.

Emphysematous prostatitis is a rare condition that is characterized by gas and abscess accumulation in the prostate. This uncommon but serious disease requires special attention because of its nonspecific presentation such as dysuria, frequency, urgency, fever, acute urinary retension and/or perineal pain. We report here on a case of emphysematous prostatitis that was combined with a liver abscess caused by Klebsiella pneumoniae. A 55-year-old man was admitted due to a 3-day history of urinary retention with a febrile sensation. He had histories of liver cirrhosis and uncontrolled diabetes mellitus. After blood and urine cultures, levofloxacin 500 mg was started intravenously. On the third hospital day, he displayed septic shock. The kidneys-ureters bladder (KUB) X-ray showed pockets of air-accumulation in the lower pelvic cavity, and abdominopelvic computed tomography (CT) with contrast enhancement taken on the third day showed a 5-cm liver abscess with an emphysematous prostate. Intravenous antibiotics was changed to metronidazole 500 mg q.i.d. and cefotaxime 2 g t.i.d. On the sixth day, transurethral resection and drainage were performed under spinal anesthesia. However, the postoperative bleeding continued and hypovolemic shock persisted, which then provoked multiorgan failure. The patient died on the 11th hospital day. K. pneumoniae was cultured from the liver abscess aspirate, blood and prostatic tissue, and the bacteria were sensitive to both levofloxacin and cefotaxime. Emphysematous prostatitis is a difficult to diagnose because of its rarity and nonspecific symptoms. A delayed diagnosis can cause high mortality and morbidity, so making a prompt clinical diagnosis of this condition is essential. Appropriate antibiotics with early adequate incision and drainage are also needed.

3.A Fatal Case of Naegleria fowleri Meningoencephalitis in Taiwan.

After bathing at a hot spring resort, a 75-year-old man presented to the emergency department because of seizure-like attack with loss of conscious. This is the first case of primary amebic meningoencephalitis (PAM) caused by Naegleria fowleri in Taiwan. PAM was diagnosed based on detection of actively motile trophozoites in cerebrospinal fluid using a wet-mount smear and the Liu's stain. The amoebae were further confirmed by PCR and gene sequencing. In spite of administering amphotericin B treatment, the patient died 25 days later.

4.Biocontrol capability of bacteriophages against soft rot disease caused by Dickeya chrysanthemi

Aims:
This study aims to evaluate the effectiveness of bacteriophages isolated from Klang and Penang, Malaysia against Dickeya chrysanthemi that causes soft rot disease.
Methodology and results:
Basic characterization such as dextrose test, citrate test, lactose fermentation test and ornithine test were carried out on D. chrysanthemi. Activity of bacteriophages against D. chrysanthemi was evaluated using spot test. Double agar overlay assay was performed to purify and enumerate the quantify of bacteriophages.Bacteriophages were also checked for its effectiveness in controlling soft rot on post-harvested vegetables: potato (Solanum tuberosum), cucumber (Cucumis sativus) and apple (Malus domestica). Results showed that D. chrysanthemiable to utilize citrate and dextrose as the source of energy, which indicated that D. chrysanthemi inclined to choose fruits and vegetables containing citrate and dextrose as the target of attack. Clear zone observed on the bacterial lawn (spot test) indicated the ability of the bacteriophages to infect and lyse D. chrysanthemi. All the bacteriophages studied herein reached the highest concentration on day 3 and were monovalent.
Conclusion, significance and impact of study
All the isolated bacteriophages were able to restrain the spreading of soft rot caused by D. chrysanthemi either work alone or as cocktail. This study provides information for the formulation development of bacteriophage against soft rot disease cause by D. chrysanthemi. Furthermore, this study reveals the potential of locally isolated bacteriophages against the D. chrysanthemi and paving the application of phage treatment on agriculture products that are not limited to potatoes, cucumber and apple.
