1.Effect of subtotal gastrectomy on glucose metabolism in patients with gastric disease combined with diabetes mellitus

ObjectiveTo investigate the effect of subtotal gastrectomy on glucose metabolism in patients with gastric disease combined with diabetes mellitus.MethodsThe clinical data of 126 cases with gastric disease combined with diabetes mellitus receiving subtotal gastrectomy were analyzed retrospectively,including subtotal gastrectomy Billroth Ⅱ type 69 patients (Billroth Ⅱ type group),subtotal gastrectomy Billroth Ⅰ type 57 patients(Billroth Ⅰ type group).The fasting blood glucose,postprandial blood glucose before surgery and 1,3,6 months after surgery and the improvement of glucose metabolism were compared.ResultsThere was no significant difference in the fasting blood glucose,postprandial blood glucose before surgery and 1,3,6 months after surgery in Billroth Ⅰ type group (P >0.05).The fasting blood glucose,postprandial blood glucose 3,6 months after surgery [ (10.7 ±2.9 ),(10.4 ± 2.3 ) mmol/L and (12.4 ± 2.6),(12.0 ±3.2) mmol/L] were higher than those before surgery and 1 month after surgery [(14.1 ±3.2),(13.4 ±3.8) mmol/L and(19.0 ±3.9),(18.1 ±4.3) mmol/L] in Billroth Ⅱ type group with significant differences (P < 0.05 or < 0.01 ).The improving rate of glucose metabolism in Billroth Ⅰ type group [ 7.02%(4/57) ] was lower than that in Billroth Ⅱ type group [ 92.75%(64/69)] with significant difference(P < 0.01 ).ConclusionsSubtotal gastrectomy has obvious effect on improving glucose metabolism in patients with gastric disease combined with diabetes mellitus.The use of subtotal gastrectomy Billroth Ⅱ type is prior to subtotal gastrectomy Billroth Ⅰ type.

3.Preliminary studies of prevention of interleukin-6 antisense oligonucleotide liposomes on after cataract in rabbit

Objective:To investigate the safety and efficacy of interleukin-6 antisense oligonucleotide liposomes on the experimental animals for prevention of after cataract. Methods:Thirty healthy New-Zealand rabbits(30 left eyes) received extracapsular lens extraction,and were randomly divided into 3 groups: anterior chamber injection of 0.1ml of interleukin-6 antisense oligonucleotide liposomes(Group A),blank liposomes(Group B) after operation respectively;and control group(Group C).All left eyes were examined by slit-lamp microscope and ophthalmoscope regularly.The aqueous humor IL-6 concentrations were measured 1 day before operation,and 1 day,3 days,7 days,14 days and 30 days after operation through enzyme-linked immunoabsorbent assay.Three months later,photographs of the posterior lens capsules were taken through digital slit-lamp microscope and the histopathological changes of posterior lens capsules were observed by light microscope. 4.Study analgesic effect site of eupatorium odoratum L and its mechanism

Objective To study the analgesic effect of Eupatorium odoratum L and its mechanism .Methods Mice were random‐ly divided into 6 groups .Swinging tail method in spinal cord destroyed mouse ,hot‐plate and twisted body method in naloxone blocked mouse ,and formalin‐induce inflammatory pain method were done to investigate the analgesic effect of Eupatorium odoratum L .Each pain index under each experiment condition was determined after intra gastric administration of each group of mice .The concentration of NO and PGE2 in serum and brain were measured to study its mechanisms .Results High‐dose of Eupatorium odor‐atum L significantly enhanced pain threshold in either spinal cord destroyed or naloxone blocked mouse with pain induced by hot and acetic acid(P<0 .05) .High and middle dosage groups of Eupatorium odoratum L markedly reduced the first‐and second‐phase re‐sponses of formalin‐induced inflammatory pain mouse(P< 0 .05) ,and decreased the writhing times and licking rear feet times caused by twisted body method(P<0 .05) .The level of NO and PGE2 in serum and brain of mouse induced by formalin were both decreased(P<0 .05) .Conclusion Eupatorium odoratum L has notable analgesic effect ,which may be related to reduce the level of NO and PGE2 in central and peripheral site .
5.Comparative study of peripheral blood IL-2 determination by ELISA with PBL IL-2 mRNA expression by real time PCR in recipients of renal transplantation

Objective To compare the consistency and accuracy of determination of peripheral blood lymphocyte(PBL)interleukin-2(IL-2)with ELISA and evaluation of the expression of PBL IL-2 mRNA with real time PCR(RT-PCR).Methods In 20 kidney recipients,the PBL IL-2 level was determined with ELISA and the mRNA expression in PBL IL-2 was evaluated by real time fluorescence quantitative PCR(RT-FQ-PCR)before operation and C0/C2 time on the 7th day after operation.The same detection and evaluation were also done in 15 chronic allograft nephropathy(CAN)patients and 5 non CAN patients.Results IL-2 and IL-2 mRNA expression showed positive correlation before operation and 1 year later after operation,R2=0.58 and 0.4 respectively,both P0.05.On the 7th day after operation,IL-2 and the inhibition rate of IL-2 mRNA expression were(16.31?9.59)% and(69.92?7.10)%,respectively.A positive correlation existed between C2 IL-2 concentration and the inhibition rate of IL-2 mRNA expression,R2=0.56,P0.05.Also,IL-2 inhibition and PBL IL-2 mRNA inhibition showed no linear correlation.Conclusions The technique of determining PBL IL-2 by ELISA can be used for evaluating immunity status and immunosuppressive tolerance in patients with renal transplantation before the operation and at stable stage after operation.However,it should not be used for assessing perioperative immunity status and evaluating effect of cyclosporine.
6.Application of Modified Pie-crusting Technique in Releasing Medial Tightness During Primary Total Knee Arthroplasty

Objective To analyze and evaluate the safety and efficacy of modified Pie-crusting ( PC) technique for releasing medial tightness during primary total knee arthroplasty ( TKA) . Methods We completed primary TKA by the same performer with modified PC technique in 30 patients (34 knees) with genu varus from March 2014 to June 2016.By using a special curved scalpel with width limit of 3 mm and depth limit of 5 mm to poke the tension parts , we released the anterion bondle of superficial medial collateral ligament ( sMCL) and posteromedial corner structures ( PMCS) during tension happened in extension , and we released the posterior bondle of sMCL during tension happened in flexion .According to the gap value measured intraoperatively , we divided these cases into three groups: extension with flexion tension group ( 10 knees ) , extension tension group ( 13 knees ) , and flexion tension group (11 knees).The difference between medial and lateral gap value no more than 1 mm was defined as gap balance.We calculated the gap balance rate of each group .Series of weighted frontal X-ray were conducted at fixed period to evaluate the varus angle of the knee postoperatively.The range of motion (ROM), HSS scores and WOMAC scores were also recorded at the same time . Results Among the 34 knees , 31 knees reached the medial and lateral gap balance at both extension and flexion .There was a difference of 2 mm in medial and lateral gap value at extension in 1 knee and the same difference at flexion in the other 2 knees.The total postoperative gap balance rate was 91.2%( 31/34 ).The constrained inserts were implanted in 3 cases.No technical-related complications happened after the surgery.After the releasing procedure, the flexion gap value had an increase of 1 mm (range, 1-3mm) in the extension tension group , and the extension gap value had an increase of 1 mm ( range, 1-2 mm) in the flexion tension group, without significant difference (Z=-1.118, P=0.264).The ROM was 83.3°±14.7°preoperatively and 100.7°±14.2° postoperatively (t=-7.714, P=0.000).The median alignment of the knee was 11.5°(range, 7°-32°) preoperatively and 1° (range, 0°-4°) postoperatively (Z=-5.092,P=0.000).The HSS scores were (42.7 ±16.3) points preoperatively and (88.1 ± 9.9) points postoperatively (t=-21.868, P=0.000).The WOMAC scores were (76.2 ±8.2) points preoperatively and (11.4 ± 9.7) points postoperatively (t=31.726, P=0.000).All of them were significantly improved in comparison with those before the surgery. Conclusions Using modified PC technique is safe and effective in medial releasing during primary TKA .Both extension and flexion gap value will be affected by releasing tensed fiber at extension or flexion position .

Objective To study the chemical constituents from Vernonia chunii of Hainan Province.Methods The constituents were isolated and purified by repeated column chromatography.Their structures were identified on the basis of physiochemical properties and spectral data.Results Four compounds were isolated and identified as:lupenyl propionate(Ⅰ),lupenyl(Ⅱ),oleanolic acid(Ⅲ),and daucosterol(Ⅳ).Conclusion Compound Ⅰ is a new compound,the others are isolated from this plant for the first time.
8.EFFECTS OF TRANSPOSITION OF FLAPS TO THE EXPRESSION OF bFGF IN CONCUSSION AREAS OF GUNSHOT INJURY

Flap pedicled with the arteria epigastrica superficialis was transposed to primarily repair firearm wound after the rabbit's posterior limb was shotted. Recovery of the wound was observed and the content of bFGF mRNA in myoideum of the concussion area was measured by reverse transcroption polymerase chain reaction(RT PCR). All wounds repaired with transposition of fascia flaps attained primary healing and the content of bFGF mRNA in myoideum of the concussion area was higher than that in the control group. The result suggests that transposition of arterialized fascia flaps could primarily repair firearm wounds successfully and the high expression of bFGF is one of the causes.
9.The effect of controlled hypotension induced by sodium nitroprusside alone or combined with propofol on human platelet aggregation

Objective Hypotension can be induced by sodium nitroprusside(SNP) combined with propofol more easily and with less amount of each drug. But both SNP and propofol were reported to inhibit platelet aggregation. The purpose of present study was to investigate the effect of controlled hypotension induced by SNP alone or in combination with propofol on human platelet aggregation. Methods Fifty-six ASA Ⅰ -Ⅱ patients (30 male, 26 female), aged 20-54 (36.0 ? 11.2) years and weighing 42-79 (67.6?14.3)kg, undergoing elective neurosurgery were randomly assigned to one of four groups: A control group ( n = 14) ; B propofol group ( n = 14) ; C SNP group( n = 14) and D SNP + propofol group ( n = 14) . The patients were premedicated with luminal 0.lg and atropine 0.5 mg. Anesthesia was induced with midazolam 0.2 mg ?kg-1 , fentanyl 5 ?g?kg-1 and vecuronium 0.1 mg?kg-1 and maintained with isoflurane inhalation and intermittent boluses of fentanyl and vecuronium. In group A and B no hypotension was induced. In group C and D hypotension was induced by 0.01 % SNP infusion (0.5-5 ?g?kg-1?min-1 ) alone or combined with propofol infusion(2-3 mg?kg-1?h-1 ) . As soon as the dura was cut, hypotension was induced. MAP was reduced by 30 % and maintained at ( 67.80 ? 9.64 ) mm Hg on average. Radial artery was cannulated for continuous BP monitoring. In order to avoid the effect of colloid and homologous transfusion on platelet function, only Ringer' s lactate was infused during operation in the four groups. Blood samples were taken from peripheral vein before skin incision, 30min and 1h after hypotension was induced and 2h after surgery for determination of platelet aggregation, prothrombin time and plasma level of NO2 -/NO3- .Results Platelet aggregation was significantly inhibited in group C and D at 30 min and Ih after induction of hypotension as compared with the baseline (before operation) . There was significant difference in the inhibition of platelet aggregation among the four groups. The inhibition was greatest in group D. There was no significant change in prothrombin time after induction of hypotension in the four groups. Plasma level of NO2-/NO3 - increased significantly at 30 min and 1h after induction of hypotension in group C and D. Conclusions SNP combined with propofol has inhibitory effect on human platelet aggregation during controlled hypotension and increased in plasma NO2 -/NO3-level may be the mechanism.
10.The effects of propofol on lung nitric oxide synthase activity in rats

Objective To investigate the effects of propofol on lung nitric oxide synthase (NOS) in rats and the mechanism of its pulmonary artery and bronchus dilating effects. Methods Forty SD rats weighing (210+40)g were randomly divided into two groups: propofol group received propofol 100mg-kg-1 ip and when its righting reflex was lost propofol was continuously infused at 10mg'kg-1 h-1 through the vein in the tail for 10min, then the animal was killed; control group received normal saline 10 ml'kg-1 ip and the animals were killed 20 min later. After sacrificing the animals the chest was opened and the lungs were removed. The NO level in the broncho-alveolar lavage fluid (BALF) was measured in 10 animals. NOS activity and NO content of the lung tissue homogenate were examined in 10 animals. Cellular distribution and expression of endothelial NOS (eNOS) and neural NOS (nNOS) in the lung were examined by immunohistochemistry in 5 animals.
