1.Changes in segmental functions of support leg during sprint running.

Sprinting speed is the final result of sprinting movement. Though sprint runners always try to sprint as rapidly as possible, their speed changes with time. Such changes in speed are caused mainly by physical movement in the support phase of sprinting, because sprint runners encounter deceleration force and generate acceleration force in only this phase. Especially, the support leg has an immediate effect on the sprinting speed.
The purpose of this study was twofold : (a) to determine whether functions of the support leg segments change with changes in sprinting speed and the sprinting situation, and (b) to investigate the characteristics of changes in the functions of the support leg segments.
Ten male sprint runners (age 20.7±1.2 yr, height 1.72±0.06m, body mass 63.3±4.7 kg) participated in the study. Subjects performed three sprints from starting blocks, and were instructed to execute exhaustive sprint runs. Their movement patterns were analyzed for five support phases : (S1) at 77.4±3.1% of maximum sprinting speed (MSS) during the acceleration period, (S2) at 95.4±1.9% of MSS during the acceleration period, (S3) at MSS, (S4) at 94.1±1.4% of MSS during the deceleration period, and (S5) at 90.8±2.2% of MSS during the deceleration period. These phases were designed to allow comparison of movement patterns in different situations with equal sprinting speeds. The linear dynamics approach was adopted to determine the direct effects of segmental movement on the sprinting speed. This is based on the theory that the velocity of a segmental center of mass (SCM) is the vector sum of the velocity of the adjacent lower joint and the relative velocity of the SCM with respect to the adjacent lower joint.
The force acting on each segment of the support leg created a stereotypical pattern in spite of differences in sprinting speed and the sprinting situation. However, the support time and the magnitude of the force acting on the shank and the foot changed with the sprinting speed. The sprinting speed was closely related to impulses acting on the shank and the foot. In contrast, the magnitude of the force acting on the thigh remained unchanged. The impulse acting on the thigh was not related to the sprinting speed, but there was a close relationship between impulses acting on the thigh and the shank, and the foot.
In conclusion, (1) The performance of the function of the thigh remains unchanged in spite of differences in sprinting speed and the sprinting situation. The thigh always works to attain its task, which is to sprint as rapidly as possible ; (2) The functions of the shank and the foot have a direct influence on changes in sprinting speed. These changes are caused by changes in the magnitude of the forces acting on the shank and the foot, and the support time ; (3) While the function of the thigh does not affect the sprinting speed directly, it affects the functions of the shank and the foot. The function of the thigh thus has an indirect influence on sprinting speed ; (4) The thigh and the foot work to compensate for decreases in the performance of the shank during the deceleration period.
2.Surgical Outcomes of Cervical Myelopathy in Patients with Athetoid Cerebral Palsy: A 5-Year Follow-Up.

STUDY DESIGN: Observational cohort study. PURPOSE: To assess the surgical outcomes of posterior decompression and fusion for cervical myelopathy in patients with athetoid cerebral palsy. OVERVIEW OF LITERATURE: Patients with athetoid cerebral palsy demonstrate involuntary movements and develop severe cervical spondylosis with kyphosis. In these patients, surgery is often performed at an early age because of myelopathy. A few studies have reported about the long-term outcomes of surgical treatment; however, they contain insufficient information. METHODS: From 2003 to 2008, 13 patients with cervical myelopathy due to athetoid cerebral palsy underwent posterior fusion surgery and were included in this study. The Japanese Orthopaedic Association (JOA) score, neck disability index (NDI), C2–7 angle on radiography, and need for additional surgical treatment were examined at 1 and 5 years postoperatively. RESULTS: The mean C2–7 angle was −10.5°±21.1° preoperatively and was corrected to −2.9°±13.5° immediately postoperatively. This improvement was maintained for 5 years. The JOA score was 9.5±2.5 preoperatively and 12.2±1.7 at the 5-year follow-up. NDI was 17±6.9 preoperatively and 16±7.5 at the 5-year follow-up. Patient satisfaction with surgery on a 100-point scale was 62.2±22.5 at the 5-year follow-up. Three patients needed additional surgery for loosening of screws. These results demonstrate good surgical outcomes for posterior fusion at 5 years. CONCLUSIONS: Posterior decompression and fusion should be considered a viable option for cervical myelopathy in patients with athetoid cerebral palsy.

BACKGROUND: Patients may receive negative results from a specific IgE (sIgE) test such as the ImmunoCAP (CAP) despite a documented history of systemic reaction to a Hymenoptera sting. Thus, further testing may be required using another serological method or venom skin prick tests to confirm allergy diagnosis and correct species. OBJECTIVE: To evaluate the sensitivity and the specificity of CAP and IMMULITE 3gAllergy (IMMULITE) for detecting sIgE to Paper wasp (WA) and Yellow Jacket (YJ) venoms using patient clinical history as the comparator. METHODS: Sera from 70 participants with a history of systemic reactions (SR) to WA and/or YJ stings were tested using CAP and IMMULITE. Fifty participants from this group had negative results on CAP. To assess specificity, sera from 71 participants who had never experienced either a WA or YJ sting were tested using CAP and IMMULITE. Fifty participants from this group tested positive using CAP. RESULTS: In participants with a history of systemic reaction to a Hymenoptera sting, yet who tested negative for WA and/or YJ sIgE according to CAP, the positivity rate according to IMMULITE was 20-42% using 0.10 IUA/mL as the limit of detection (LoD), per the manufacturer's specification. When the LoD for CAP (0.35 IUA/mL) was applied to the IMMULITE results, positivity according to IMMULITE was 14-26%. Overall, sensitivity, specificity, and agreement with SR were greater for IMMULITE than for CAP. For YJ: sensitivity (IMMULITE:CAP), 42.8%:28.5%; specificity, 53.5%:39.4%; agreement, 48.2%:34%. For WA, sensitivity (IMMULITE:CAP), 58.6%:28.5%; specificity, 49.3%:47.8%; agreement, 43.9%:38.3%. CONCLUSION: The IMMULITE performed well for detecting sIgE to Hymenoptera venom

OBJECTIVEExposure to polychlorinated biphenyls (PCBs) is considered to have culminated between 1950 and 1970 in Japan, and exposure through diet, the major exposure route, has decreased significantly over the last 10 years. The primary goal of the present study was to investigate the long-term trends and congener profiles of serum and dietary levels of PCBs using historical samples.
METHODSUsing banked samples collected in 1980, 1995, and 2003 surveys, we determined the daily intakes and serum concentrations of 13 PCB congeners (#74, #99, #118, #138, #146, #153, #156, #163, #164, #170, #180, #182, and #187) in women.
RESULTSThe total daily PCB intake [ng/day, geometric mean (geometric standard deviation)] decreased significantly from 523 (2.5) in 1980 to 63 (3.2) in 2003. The serum total PCB level (ng/g lipid) in women <40 years of age decreased significantly from 185 (1.8) in 1980 to 68 (1.8) in 2003. In contrast, the level in women >50 years of age increased significantly from 125 (1.7) in 1980 to 242 (1.7) in 2003. Specifically, the serum concentrations of hexa (#138, #146, #153, #156, #163, and #164) and hepta (#170, #180, #182, and #187) congeners increased significantly. A comparison of the serum PCB levels of women born from 1940 to 1953 revealed that their serum total PCB level was significantly higher in the 2003 survey [242 (1.7), n = 9] than in the 1995 [128 (2.0), n = 17] surveys. This increase in the total PCB level was attributable to increases in the hepta congener groups.
CONCLUSIONPresent results suggest a decreased rate of elimination of hepta congeners with aging in females, rather than a birth-generation phenomenon.
6.A Case Report on Effect of Asenapine Sublingual Tablets on Intractable Nausea in a Patient of Terminal Malignant Lymphoma with Diabetes Who Cannot Take Oral Medicine

Introduction: We report a case of intractable nausea of a terminal malignant lymphoma patient with diabetes, which improved by sublingual administration of asenapine. Case: A 78-year-old man suffering from diffuse large B-cell lymphoma with diabetes presented intractable nausea and vomiting. Those symptoms were thought to be due to masses and nodules in the right frontal lobe and the cerebellum, and/or due to edema in the peripheral brain parenchyma. Because it was difficult to take medicines orally, we selected injections to control those symptoms. However, the combination of metoclopramide, haloperidol, and hydroxyzine injections failed to relieve nausea. Olanzapine is effective against nausea but is contraindicated for diabetic patients, so asenapine, one of the multi-acting receptor-targeted antipsychotics the same as olanzapine, was expected alternatively. The patient was administrated asenapine sublingually 5mg once a day before bedtime. This administration of asenapine remarkably improved his nausea. Discussion: Sublingual asenapine dose may be an effective therapeutic option for intractable nausea.
