Kasabach-Merritt phenomenon does not occur with common hemangioma, rather it is associated with the more aggressive Kaposiform hemangioendothelioma and rarely with other vascular neoplasm. We report the case of an adult who was diagnosed as Kaposiform hemangioendothelioma complicated by Kasabach-Memtt phenomenon. This is the first report in Korea of an adult with Kasabach-Merritt phenomenon who has osteolytic changes of femur, pelvic bone, and lumbar spine.

9.The Treatment of Liver Hemangioma Associated with Kasabach-Merritt Syndrome in Neonates.

Hemangioma is the most frequent liver tumor in infancy. The treatment of hepatic hemangioma includes medical, surgical, and non-operative interventional therapy. There are no standard medical regimens currently considered consistently effective. MDMP (megadose methylprednisolone) and alpha-interferon can be used for medical treatment. Interventional occlusion of feeding arteries in symptomatic hepatic hemangioma is considered a safe and effective alternative to early open surgery. Untreated symptomatic patients with heart failure have a high mortality rate. For this reason symptomatic patients with heart failure may require non-operative treatment such as interventional embolization, because hepatic resection is burdened with high risk. We report our experiences of two patients with neonatal liver hemangiomas whose clinical courses were complicated by cardiac failure to whom medical treatment and/or interventional vascular occlusion were done. Both patients had Kasabach-Merritt syndrome complicated with cardiac failure. Initially, the masses were considered unresectable. In one case, we performed interventional therapy in addition to medical treatment. In another case, we tried medical therapy with megadose steroid and alpha-interferon. Unfortunately, in spite of the treatments, the patients died of severe hemorrhage.

10.A Case of Stewart-Treves Syndrome Associated with Disseminated Intravascular Coagulation.

Stewart-Treves syndrome is an angiosarcoma that develops in chronic lymphedematous skin. Kasabach-Merritt syndrome is characterized by the occurrence of disseminated intravascular coagulation (DIC) in combination with benign angiomatous tumors in most cases, but angiosarcoma has also been reported to be associated with DIC in rare cases. Here we report on a case of a 74-year-old woman who had angiosarcoma on a large area of the right trunk and arm around a previous mastectomy site. She developed DIC after laminoplasty, and she finally expired from multiorgan failure 3 months after the initial diagnosis of DIC. Her preoperative laboratory findings were normal, and she did not have any other underlying diseases that were possibly related with the development of DIC, except the large area of angiosarcoma. In conclusion, we report here on a rare case of Stewart-Treves syndrome that co-occurred with DIC and we believe that accurate evaluation of a skin lesion is important for Stewart-Treves syndrome patients who display systemic symptoms.
