2.Clinical Studies of infectious croup prevailing in winter of 1975.

Clinical studies were performed on 32 cases of infectious cuoup prevailing in winter of 1975, admitted to department of pediatrics. The symptoms were critical and clinical course was more repidly progressed, so incidence of emergent tracheostomy was increased than previous years. Incidence of complicated Pulmonary disease was high, but prognosis was good relatively. According to these findings, specific prevalent viral infection was suspected, but identification of virus was not performed.

3.The Effect of Halothane-induced Acidosis on Focal Cerebral Ischemia in Rat.

The current study was performed to investigate the influence of acidosis on focal cerebral ischemia in view of morphometric assay and neuropathological examination. The acidosis was induced by increment of halothane concentration and by decreasing respiratory rate. The mean pH were 7.423+/-0.012 in control group and 7.184+/-0.038 in acidosis group. Twenty-four hours after MCA occlusion(MCAO), neutral red staining and perfusion fixation was performed. The ischemic area was measured and morphometric analysis was undertaken. In acidosis group, the infarct area was 25.23+/-4.78% of the total cerebral area;in control group, the infarct area was 27.69+/-4.05%. The histopathological findings were examined under light microscopy, in which the field scanning was carried out from the midline by 0.5mm interval at cortical and basal ganglia levels. These results indicated that although there was no satistically significant difference in infarct area between acidosis and control group, increased acidosis aggravated the extent of histopathologic ischemic neuronal damage.

4.Frozen-Thawed RBCs transfusion in the Dog.

The cryopreservation of Red Blood Cells has many advantages of which the most important one is that it can be stored for a long period. However, in Korea, Research regarding frozen blood is still in its early stage. We evaluated the effects of transfusion of the frozen-thawed RBCs in dogs. The whole bloods were collected from 5 dogs, and the packed RBCs were obtained by centrifugation method. We made the frozen RBCs by using 40% glycerol method and stored it in -80 degrees C refrigerate for 1 month. The frozen RBCs were thawed in the 37 degrees C water bath and washed by Cell washer according to the standard protocol, and evaluated the status of them being compared with that of the unfrozen. The majorirty of the results were satisfactory to the allowable limit except high plasma hemoglobin and potassium. The frozen-thawed bloods were transfused to the two dogs and carefully observed the effects and its complications. The results were that the average value of the hemoglobin was elevated about 0.6g/dL more after transfusion than before, and there were no significant complication related to the transfusion. Thus, The frozen thawed blood transfusions in case of the experiment with dogs were proved to be safe and as effective as fresh blood, and The above method appeared to be feasible to human blood.

6.A Case of Large Cystic Cerebral Cysticercosis.

Author report a case of neurocysticercosis that involved the right cerebral hemisphere subcortically. The subject was 38-year-old Korean male. The headache, epileptiform seizures and left sided motor weakness were the symptoms. Simple skull X-rays showed no significant pathological findings. Right carotid angiogram disclosed the signs of a large mass in the centrosylvian region. CT scan also demonstrated large, multiple and round cystic lesions within the substance of the brain itself in the right frontoparietal region. At operation cyst wall and cysticercus in the fluid content of a mass were obtained. Postoperatively with the aids of occurrence of cysticercrus in the skin and positive serologic test, the diagnosis again confirmed.

7.Autologous Frozen-thawed Blood Transfusion in the Bone Marrow Donors.

BACKGROUND: The cryopreservation of Red Blood Cells have been proved to have many advantages in western countries. However, in Korea, clinical application of frozen-thawed blood is still in its early stage. We tried autologous frozen-thawed blood transfusion and evaluated the effects and complications. METHODS: Nine units of whole blood were collected from the autologous (n=1) and allogeneic bone marrow donors (n=3) and made packed RBCs by a centrifugation method. We made frozen RBCs using high glycerol method and stored them in -80degrees C freezer for 1 month. Eight units of frozen RBCs were thawed and washed by Cobe spectra (Cobe, USA). Autologous transfusions were done to the patient and donors while the bone marrow collection procedures were in process for bone marrow transplantation and we carefully observed its effects and complications. RESULTS: The mean RBC recovery rate were 89.8% and the supernatant plasma hemoglobin, K+, LD, osmolality, and simulation test were satisfactory to the allowable limit. After transfusion, the hemoglobin value was significantly elevated being comparable to that of the fresh blood. A case showed mild hematuria related to this transfusion but soon it disappeared in a day. CONCLUSION: The autologous frozen-thawed blood transfusions were safe and as much effective as the fresh blood. It would be essential to have skillful techniques in post-thaw washing process.

8.Prediction of Soft Tissue Profile Changes after Mandibular Set-back Surgery.

We usually accept 1 : 1 ratio soft tissue change in mandibular set-back surgery. But we cannot sure whether we can use this ratio as a long term predictor after surgery. We investigated the change of hard tissue and its effects to soft tissue and examined the predictability of cephalometric analysis in short term and long term follow-up periods in mandibular set-back surgery. Subjects were 15 patients (5 male, 10 female) performed mandibular set-back procedure only by BSSRO with rigid fixation. Cephalometric data were obtained before orthodontic treatment, immediate before surgery and immediate after surgery, 6 months, and 18 months after surgery consecutively. Differences in soft and hard tissue changes among the time intervals were examined using analysis of variance (ANOVA) ; the association between immediate surgical change in chin landmarks and subsequent short and long term soft and hard tissue changes were examined using linear regression analysis 1. Soft tissue mandibular structures were positioned posteriorly and superiorly after surgery. Average mandibular set-back were 8.17mm at Pog. The horizontal and vertical hard tissue mandibular changes were stable for 18 months after surgery. Although there was a small degree of change, soft tissue and hard tissue remain relatively stable after surgery and there was no clinically discernable changes between 6 months and 18 months post-operatively. 2. Some part of upper lip (Ls & stms)and most part of mandibular soft tissue moves postero-superiorly after mandibular set-back. The ratio of horizontal changes of hard tissue to soft tissue at Inf. labial sulcus(B'), Pogonion(Pog') were 84.7%, 74.7% after 18 months. respectively. 3. Predictability of the soft tissue change is less certain than hard tissue and this predictability is decreased over time. These result suggests that we cannot predict the surgical outcomes exactly. It is recommendable that the oral surgeon should be careful in using the computerized surgical prediction software program as a predictor of long term soft tissue change.

9.Does the Periprosthetic Fracture Pattern Depend on the Stem Fixation Method in Total Hip Arthroplasty?

Background:
Management of periprosthetic femoral fractures (PFFs) is reportedly challenging. Different patterns of PFFs would occur based on whether stem fixation was primarily cemented or cementless and whether these patterns would be associated with clinical outcomes, such as subsidence, fracture union, and complications, after stem revision.
Methods:
A retrospective comparative study was performed, involving 52 PFF patients treated with tapered fluted modular stems (TFMSs). In the 52 patients with Vancouver B2 or B3, including 21 cemented stems and 31 cementless stems, fracture patterns and bone stock were analyzed. Clinical outcomes after revision surgery using the TFMSs were compared between the two groups.
Results:
Transverse or short oblique type PFFs occurred around the cemented stem with loosening at the bone-cement interface. The Paprosky type III femoral deficiency and Vancouver type B3 fracture were observed more frequently in the cemented stem group. Otherwise, spiral fractures occurred more frequently in the cementless group (p < 0.001). Excessive subsidence of > 5 mm was observed more frequently in the cemented stem group (p < 0.001). The re-revision rates were higher in the cemented group than in the cementless group (p = 0.047).
Conclusions
In our study, it was found that the patterns of transverse or oblique PFFs were more frequently produced with cemented stems, while long spiral fractures were more frequent with cementless stems. Stem subsidence and reoperation related to complications were more common in patients with PFFs around cemented stems than those with PFFs around cementless stems.
10.Antifungal Effects of New Synthetic Materials, KAF-200522 and KAF-200522-HCl, on in vitro and in vivo Models.

KAF-200522 and its chloride form, KAF-200522-HCl, were invented in Chemon inc. as new triazole antifungal agents with excellent activities in vivo and in vitro against wide range of fungi. As a result of in vitro susceptibility measurements, 80% minimum inhibitory concentrations (MIC80) of both test articles against Candida albican sp. and Aspergillus fumigatus sp. were below 0.0156 microg/mL, which were over 4,100 times lower than those of fluconazole against fluconazole resistant C. albican sp. and A. fumigatus sp., and were over 16 times lower than those of amphotericin B against above same fungi. Additionally, against representative dermatophytes, Trichophyton sp., the MIC80s of both test articles were below 0.0156 microg/mL which were over 64 times lower than those of fluconazole and amphotericin B. As in vivo antifungal activities in A. fumigatus sp. infected mouse models, KAF-200522 treatment group at 600 mg/kg showed 80% survival rate which was 2 times higher than that of amphotericin B and showed 13.7 days in the mean survival time (MST) which was about 2.1 times higher than that of amphotericin B. But in KAF-200522-HCl treatment groups, all animals were found dead in contrast to 40% survival rate in amphotericin B treatment group, however dose dependent increases in MST was revealed. In conclusion, antifungal activities of KAF-200522 and its mimics, KAF-200522-HCl in vitro and in vivo were confirmed in this study, therefore the potentiality of the present compounds to be developed into new antifungal drug was expected.
