Hemoglobin(Hb), zinc protoporphyrin(ZPP) and blood lead(PbB) levels were determined for 1,851 blood samples collected from healthy urban population to establish reliable baselines for Hb, ZPP and PbB levels by age and sex. ZPP values were analyzed with a Hmatofluorometer and PbB determinations were concurrently carried out using flameless atomic absorption spectrophotometry. The blood sampling period was about 6 months from May, 1991 and the summarized results were as follows; 1. The mean value of Hb in male female were 14.55+/-1.81 g/dl and 12.61+/-1.81 g/dl respectively and there was statistically significant difference(p<0.05). 2. The mean value of ZPP in pre-schoolchildren was 37.49+/-13.31 microgram/dl for male, 35.77+/-11.85 microgram/dl for female and that of ZPP in after 7 years groups was 31.91+/-8.23 microgram/dl for male, 30.11+/-9.11 microgram/dl for female and there was statistically significant difference(p<0.05). 3. The mean value of PbB in pre-schoolchildren was 25.10+/-5.21 microgram/dl for male, 24.45+/-4.18 microgram/dl for female and that of PbB in after 7 years groups was 24.28+/-3.00 microgram/dl for male, 21.99+/-5.05 microgram/dl for female and there was statistically significant difference(p<0.05).

We studied the left ventricular function with the use of M-mode and 2-D Echocardiography to 32 patients with congestive heart failure before and 2 weeks after medication of prazosin 2 mg tid during July 1979 and Dec. 1980 in Heart Center, Paik Hospital, In-Je Medical College, Busan, Korea. The results were following; 1) LVDeD before and after vasodilaor therapy was each 5.64+/-0.76 and 5.04+/-0.57.cm, LVSeD before and after therapy each 4.52+/-0.51 and 3.70+/-0.57cm, and F.S. before and after therapy 19.87+/-3.66 and 26.59+/-6.75%. The difference of LVSeD, LVDeD and F.S. was each statistically significant. 2) LVDeV before and after therapy was each 182.2+/-58.07 and 132.80+/-47.01 ml LVSeV before and after therapy each 92.35+/-23.60 and 50.65+/-27.21ml, and E.F. before and after therapy each 49.31+/-7.07 and 61.86+/-11.74%. The difference of LVDeV, LVSeV and E.F. was each statisitically significant. 3) LVPWTh before and after therapy was each 0.89+/-0.11 and 0.77+/-0.08cm, and Vcf before and after therapy 0.83+/-0.11 and 1.03+/-0.27 cir./sec. The difference of LVPWTh and vcf before and after therapy was each statistically significant. 4) The difference of VSTh, RVD, LAD, and Aorta Dimension before and after therapy was each statisitically significant.

Spina Bifida Occulta*

A clinical observation was made on 97 patients of renal calculi who were admitted to the Department of Urology, Chonam University Medical School during the 5 years period from January, 1976 to December, 1980. The following results were obtained. 1) The incidence of renal calculi in 97 patients was 38.0% of urolithiasis in 255 patients and 6.8% of 1430 inpatients. The was an increasing tendency of incidence of renal calculi with annual increasing number of inpatients. 2) There were 51 male and 46 female, a ratio of 1.1:1. The age of two thirds patients of renal calculi ranged from 31 to 50 years, with peak age incidence in the fifth decade. 3) Definite past history of urinary calculi was found in 23 cases (23.7%). 4) The clinical symptoms on admission were flank pain in 92.8%, hematuria in 15.4%, frequency in 5.1% and suprapubic pain in 4.1%. 5) Urinalysis revealed hematuria in 77.3%, proteinuria in 54.1%, pyuria in 42.3%, bacteriuria in 28.9% crystalluria in 9.3% and normal in 8.2% 6) There were 42 renal units of right renal stone and 39 renal units of left renal stone with a ratio of 1.1:1. Unilateral renal calculi were found in 81 patients (83.5%). 7) Treatment consisted of pyelolithotomy in 51 patients (52.6%), pyelonephrolithotomy in 11 patients (11.3%), nephrectomy in 4 patients (4.1%) and nephrolithotomy in 3 patients (3.1%) in order. 8) The incidence of stone ranged from 1.1 to 4.0 gm in weight was 50.6%. The mean weight of removed renal calculi was 25.8 gm in nephrectomy, 19.0 gm in pyelonephrolithotomy, 5.7 gm in nephrolithotomy and 4.9 gm in pyelolithotomy. 9) A nephrostomy tube extended into the upper ureter was located in 30 cases (30.9%) and there was no difference in duration of nephrostomy tube inlaying between pyelolithotomy and pyelonephrolithotomy. 10) Postoperative complications were observed in 22 cases (26.2%) and the most common one was remnant stones in 6 cases (27.4%), of which 5 cases had multiple stones overweighed 10.0 gm. 11) The chemical analysis of 43 renal calculi showed calcium oxalate in 14 cases (32.6%), calcium phosphate in 78 cases (16.3%), the mixed type of calcium oxalate and calcium phosphate in 6 cases (14.0%) and the mixed type of calcium oxalate and magnesium phosphate in 5 cases (11.6%). There were 24 single chemical component of stones. As a result, oxalate stone (67.5%) and phosphate stone (55.9%) were prevalent.

Molluscum Contagiosum is a self-limited or contagious caused by a virus of the Pox virus group. It has ocular importance to examine the eyelids for Molliscum in case of conjunctivitis unresponsive to routine therapy. We had an experience of a case of Molluscum Contagiosum on the left lower eyelid. On the corresponding site of this Molluscum at the bulbar conjunctiva, the limbus, and the cornea, no follicles, epithelial keratitis nor erosions were seen. This patient was managed by simple excision of the lesion under local infiltration anesthesia. Now, we report our experience with this patient along with the review of other papers in this report.

Paget's disease of bone (osteitis deformans), a chronic osseous disease of unknown etiology, is extremely rarein Asia although well known in western countries. This disease is characterized by the simultaneous absorption ofbone and the new bone formation of abnormal character affecting middle and later age groups. The authorsexperienced a case of monostotic Paget's disease involving left pelvic bone in 56-years old female. To ourknowledge, this is the first cas report of monostotic Paget's in Korea.

OBJECTIVES: This study was carried out to evaluate manganese exposure marker and the clinical significance of high signal intensity observed on brain magnetic resonance imaging(MRI) in mild steel/arc welders. METHODS: Brain MRI, blood and urinary manganese concentration measurements, physical and neurologic examinations, questionnaire and biochemistry were performed on 53 mild steel/arc welders(exposed group) and 112 non-manganese handling productive workers(control group). We calculated the pallidal index, the ratio of signal intensity of globus pallidus to subcortical frontal white-matter on sagittal 71-weighted MRI. And we studied the relationships between the pallidal index and other measurements. RESULTS: In 35(66.3%) exposed workers, high signal intensity were observed on the globus pallidus. Blood and urinary manganese levels were higher in exported group than control group. Pallidal index were significantly correlated with working duration, blood manganese level, urinary manganese level and positive rates of clinical symptoms and signs. CONCLUSIONS: The results of this study suggests that high signal intensity finding on brain MRI may be considered as useful tool for the evaluation of workers who have manganese exposure.

BACKGROUND: The drug abuse has become one of social problems especially in the adolescents. It has been well known that the drug abuse is concerned with interaction of biopsychosocial factors and farnily relation factors. So I have studied the general characteristics and rearing attitude and family state of the adolesents who were in a jail because of drug abuse and have intended to give some help for establishing effective strategies for family education and therapy. METHODS: The study was done for adolescents who were in a jail because of violation the illegal drug control law. The group was total 55 men and the age distribution was frorn 15 to 20 years. The Questionnaire was used in Mar 1996 for study and I analyzed 53 cases excluding 2 cases because they were nearly illiterate. The FACES III(Family Adaptability and Cohesion Evaluation Scales), family APGAR score and PBI(Parental Bonding Instrument) were used for evaluating the characteristics of family function and the parental rearing attitude. Statistical management with SAS was executed for data analysis. RESULTS: The average age of the group was 6.9+/-1.4. 48.1% of them lived in the metropolitan area, 40.4% lived in the middle-sized cities and 11.5% lived in the small country towns. The education level of 91.8% was under middle-school graduate. They have use drugs since the age of 14.6+/-1.8, they began to use glue sniffing at the first time in 78.8% and butane gas inhalation in 17.3%. The state of family showed the death of father in 28.9% and the death of mother in 13.5 % and 76.9% of deaths occured before they have become 13 years. Their parents divorced in 25 %. The mean APGAR score was 4.56+/-2.6, which means moderately dysfunctional state of family. In FACES III adaptability score was 20.3+/-5.7 and cohesion score was 26.2+/-6.9. According to the family type by FACES III, there were 5 cases(9.4%) in balanced, 19 cases(35.9%) in midrange, and 29 cases(54.7%) in extreme. In PBI mother overprotection score was 16.24+/-4.59, mother care score was 22.5+6.8, father overprotection score was 16.71+/-5.1 father care was 21.94+/-6.24. CONCLUSIONS: The drug abused adolescents in a jail were in hadicapped family because of deaths and divorce of parents. Their family function score was low and more than half of them had the extreme type of family. Therefore it is wise for us to have a concern for the family problem when understanding and managing drug abused adolescents.
