1.Subperiostedal Ganglion of the Tibia: A Case Report

Subperiosteal Ganglion is a rare cystic lession, although ganglions are frequently found in close proximities to joints and tendon sheaths. It is characterized by a thin-walled cyst containing tenacious mucoid fluid and by the pedicle attached to periosteum. This report describes a middle-aged man with a subperiosteal ganglion which occured beneath the periosteam of the tibia. Presenting symptoms were pain and circumscribed swelling over the anteromedial aspect of the right proximal tibia.

2.Clinical Observation on Urinary Fistula.

Twenty-three cases of urinary fistulas which were admitted to the Department of Urology, Chosun University Hospital during the period from Jan. 1973 to Dec. 1979 were analysed clinically. The following results were obtained. 1. Eight cases of all fistulas were in men and 15 in women. 2. The most prevalent age group was 51 to 60 years. 3. The most commonly involved site was urethra (37.5%) and the most common fistula type wag vesicovaginal fistula (30.4%). 4. The most common etiology of vesicovaginal fistula was obstetric cause. 5. Seventeen of 23 urinary fistulas were operated upon and all but 3 were repaired successfully on the first surgical attempt.

3.Clinical Observation on Bladder Tumor.

A clinical observation was made on 45 of 53 patients with bladder tumors seen at the Department of Urology, Chosun University Medical School during the period from January. 1973 to December, 1979. The following results were obtained: 1. During that period. 820 patients were hospitalized and there were 53 cases of bladder tumors giving a rate of 6.5% 2. There were 32 males and 13 females a ratio of 2.5: 1 and 71. 1% of the patients was over 50 years of age. 3. The most common symptom was grose hematuria (82. 2%) and the others were frequent urination (35. 6%), dysuria (31. 1%), painful urination (22. 2%) and lower abdominal pain (22. 2%). 4. 22 cases (48.9%) of the patients visited the hospital within three months after the appearance of the initial symptom and 75.6% within 1 year. 5. 24 cases (53. 3%) had multiple tumors and 21 cases (46.7%) single tumor, Regardless of origin, lateral wall was involved in 30 cases (66. 7%) and the trigone in 20 cases (44.4%). 6. IVP performed in 43 cases revealed normal upper urinary tract in 27 cases (62. 8%) and filling defect of the bladder in 40 cases (93. 0%). 7. The pathologic examination performed in 37 cases revealed transitional cell carcinoma in 29 cases (78. 4%). 8. The operation was performed in 25 cases: Partial cystectomy in 11 cases open resection with coagulation in 3 cases, TUR in 5 cases, radical or simple total cystectomy in 6 cases and ileal diversion only is 1 case. 9. Of the 14 cases possible in follow up study 10 (71.4%) survived over 1 year after treatment.

4.A Case of Neurilemmoma of the Penis and the Perineum.

Neurilemmoma is a neoplasing from Schwann cells. It is usually solitary and may be benign or malignant solid or cystic, encapsulated or diffuse. We report a rare case of neurilemmoma developed in the penis and the perineum in a 19 years old male and the relevant literature has been reviewed.

5.One Case of Duplicated Ureter Associated with Ectopic Ureteral Orifice.

One case of right duplicated ureter associated with single ectopic ureteral orifice to the vaginal vestibule in a 23 year old girl was presented. In a case, while normal self-voiding, urinary leakage has been complained since infant without other urinary symptoms and she was treated with right ureteroheminephrectomy.

6.Trend of Isolation and Genotypes of Vancomycin-esistant Enterococci Isolated from Tertiary Care Hospital in Wonju Area.

BACKGROUND: Vancomycin-esistant enterococci (VRE) were first recovered from clinical isolates in Korea in 1992, and the incidence has been steadily increasing. The goal of this study was to determine the isolation trend of VRE by year and genotypes of VRE isolated from clinical specimens in Wonju area. METHODS: We investigated the patients' medical records to determine the incidence of VRE among enterococci isolated from 1995 to 1999 in Wonju Christian Hospital, performed antimicrobial susceptibility tests to vancomycin and teicoplanin by agar dilution methods, and genotyped them with multiplex polymerase chain reaction for 117 cryopreserved VRE isolates. RESULTS: VRE were first isolated in December 1995. Overall incidence of VRE during the period of 1996 to 1999 was 6.1% (164/2,682). The annual incidence of VRE was 1.9% in 1996, 5.5% in 1997, 6.7% in 1998, and 9.7% in 1999. The species of VRE included 115 (69.7%) E. faecium, 11 (6.7%) E. faecalis, 17 (10.3%) E. casseliflavus, 10 (6.1%) E. gallinarum, and 12 (7.3%) Enterococcus species. Of 117 VRE, E. faecium and E. faecalis strains were phenotyped as VanA, and genotyped as vanA with the exception of five VRE which the genotypes were not identified. All E. gallinarum and E. casseliflavus strains were genotyped as vanC- and vanC-, respectively. CONCLUSIONS: VRE were first isolated in 1995. And the isolation rate of VRE were increasing trend from 1.9% in 1996 and 9.7% in 1998. Most of VRE were E. faecium with vanA genotype.

8.Evaluation of the Diagnostic Performance of the AdvanSure TB/NTM Real-Time PCR Kit for Detection of Mycobacteria.

BACKGROUND: The AdvanSure TB/NTM real-time PCR kit (AdvanSure) was newly developed in Korea to detect Mycobacterium tuberculosis and nontuberculous mycobacteria (NTM) utilizing a specific primer and TaqMan probe targeting the IS6110 and rpoB genes which are unique to these species. The purpose of the present study was to evaluate the clinical utility of AdvanSure by comparing the results of acid-fast staining, mycobacteria culture, COBAS Amplicor MTB PCR (Amplicor), and AdvanSure. METHODS: A total of 182 specimens (105 respiratory and 77 nonrespiratory specimens) were obtained from 165 patients, and acid fast bacilli (AFB) staining, mycobacteria culture, and Amplicor were performed on all specimens. AdvanSure was also performed on the above specimens using the SLAN real-time PCR detection system. The sensitivity and specificity of AdvanSure were analyzed using AFB staining and culture. RESULTS: Of the 182 specimens, M. tuberculosis was detected in 43 specimens and NTM was detected in 12 specimens according to PCR and/or culture. The sensitivity and specificity of the AdvanSure based on AFB culture were 97.3% (36/37) and 95.5% (127/133) in M. tuberculosis and 75.0% (9/12) and 100% (0/133) in NTM, respectively. CONCLUSION: AdvanSure could be useful for detecting M. tuberculosis and NTM in the clinical laboratory with high sensitivity and specificity.
