3.Experience of Concurrent Collection of Plosrno During a Ploteletpheresls.

BACKGROUND: There has been rapid increase in demand for fresh frozen plasma and its derivatives in Korea. It may be due to active therapy by the clinicians especially hematologic and coagulopathy areas. So, now, the plasma is lack of supply of full demand. So, We studied the concurrent collection procedure of plasma during a plateletpheresis for the purpose of plasma supply. METHODS: Over three month period from January 1996 to March 1996, we introduced a program of concurrent collection of plasma (CCP) during plateletpheresis at Korea University Guro Hospital, in which the additional plasma are collected for preparation of FFP. Donors were enrolled in our 62 volunteer plateletpheresis. The 31 volunteers (group I) were done plateletphersis only, another volunteers (group II) were done CCP. We compared the platelet concentrate volume, to the characteristics of donor's condition and the adverse reaction during plateletpheresis between two groups. RESULTS: Before the plateletpheresis, there were no differences in the age, weight, height, pre-platelet count between the two groups. Also there were no adverse reactions through the procedure, such as perioral paresthesia, hypotension, hyperventilataion, chills, nausea and vomiting. The mean platelet count per unit did not show significant differences between two groups. In group 1, the mean platelet concentrate volume was 300 mL, yield was 4.5 x 1011 and group II, the volume was 315 mL, the yield was 5.1 x 1011 and the fresh plasma volume was 223 mL. CONCLUSION: We have experienced the concurrent collection procedure of plasma during a plateletpheresis from 31 volunteers without main adverse reactions.

4.Nocardia asteroides complex Isolated from Cerebrospinal Fluid and Surgical Wound Site: Three Case Reports.

Nocardia is an aerobic gram-positive, weak acid-fast, branching, filamentous bacteria causing various clinical infections such as pulmonary nocardiosis, systemic nocardiosis, extrapulmonary nocardiosis, cutaneous nocardiosis and nocardial mycetoma. They are most commonly caused by N. asteroides complex. Humans become infected by inhaling contaminated air-borne dust particles or by traumatic implantation of the bacterium into the subcutaneous tissues. We isolated Nocardia asteroides complex from the hemo-vac of trauma patient and CSFs of two hydrocephalus patients. Nocardia asteroides complex has been isolated less commonly in Korea than in other countries. We think that it is important to extend an incubation time of culture plate when we find the gram positive, branched, beaded filaments and coccoid cells.

5.A Case of Molluscum Contagiosum on the Lower Eyelid.

Molluscum Contagiosum is a self-limited or contagious caused by a virus of the Pox virus group. It has ocular importance to examine the eyelids for Molliscum in case of conjunctivitis unresponsive to routine therapy. We had an experience of a case of Molluscum Contagiosum on the left lower eyelid. On the corresponding site of this Molluscum at the bulbar conjunctiva, the limbus, and the cornea, no follicles, epithelial keratitis nor erosions were seen. This patient was managed by simple excision of the lesion under local infiltration anesthesia. Now, we report our experience with this patient along with the review of other papers in this report.

6.2 Cases of Lumbar Hernia.

Lumbar hernia presents as a reducible protruding mass in the flank region between the 12th rib and the iliac crest. The superior lumbar hernia (Grynfeltt's) is covered by the thin latissimus dorsi muscle and bounded by the erector spinae and internal obliquus abdominis muscle. The lumbodorsal fascia forms the floor of the Grynfeltt's triangle. Spontaneous or acquired non-traumatic lumbar hernia occurs more frequently after middle age. The untreated lumbar hernia progresses in size gradually. As in most hernias, the larger the defect, the more complicated and difficult is the repair. Here we report 2 female patients with spontaneous lumbar hernia diagnosed at Presbyterian Medical Center with computerized tomogram. Their treatment consisted of tension-free surgical repair utilizing the overlap method for defect edge and the application of prolene mesh.

7.Radioisotope Penogram in Diagnosis of the Impotence.

Forty two patients including 1O cases of the normal control were studied with the radioisotope penogram. The causes of the impotence were arteriogenic in 1O cases, venogenic in 6, diabetes mellitus in 8, neurogenic in 2, endocrinogenic in 1 and psychogenic in 15, ranging from 19 to 70 years of age. Ten normal volunteers for the control group were ranged from 21 to 38 years of age. All were subjected to Snap-Gauge test and measurement of the penile blood pressure with estimation of the penile brachial index (P.B.I.). We calculated the Penogram Index in time-activity curve of the radioisotope penogram and studied the inter-relationship between the Penogram Index and the etiologic diseases, Snap-Gauge test, PBI. The results were obtained as follows; 1. Inter-relationship between the diseases and the penogram index, arterial index (Index A) and venous index (Index V1, V2); 1) Index A was 1.44 in normal control group, 0.55 in arteriogenic. 0.62 in venogenic, 0.64 in diabetes mellitus, 0.95 in neurogenic, l.29 in endocrinogenic and 1.03 in psychogenic impotence. This suggested that Index A was significantly correlative with state of the penile arterial inflow. 2) Index V1, V2 were 0.09, 90 in normal control group, 0.09, 51 in arteriogenic, 0.40, 29 in venogenic, O.8l, 38 in diabetes mellitus, 0.84, 90 in neurogenic, O.92, 34 in endocrinogenic, and 0.91, 64 in psychogenic impotence. This suggested that Index V1 and V2, particularly V1 was representative of maintenance of the penile blood flow and the penile venous outflow or leak. 2. Relation between Snap-Gauge test and Index A; Correlated in 77.8% of the patients and the normal controls, respectively. 3. Relation between P.B.I. and Index A; Correlative. 4. Relation between P.B.I and Index V1, V2; Not correlative Therefore, the radioisotope penogram was a very useful method in diagnosis of the organic impotence, particularly vasculogenic impotence to different late arteriogenic and venogenic.

8.Comparison between Wound Closure Methods in the Reversal of Diverting Ileostomy

Background/Aims:
The objective of this study was to determine the more appropriate wound-closure method by comparing the effectiveness of two methods in a group of patients who underwent ileostomy repair.
Methods:
The study conducted after obtaining the approval of the Institutional Review Board (IRB) included 58 patients ≥19 years of age who underwent ileostomy at the Department of Surgery at the Presbyterian Medical Center. This was a retrospective, single-center trial. Patients who underwent ileostomy closure between January 2011 and September 2017 were assigned to the primary wound-closure (PC, n=25) group and the purse-string wound-closure (PSC, n=33) group. Post-repair complications, such as wound infection, delayed healing, and patient satisfaction related to wound management, were investigated and compared according to the wound-closure method.
Results:
The PSC group had a significantly lower surgical site infection rate than the PC group (0% vs. 44%, p<0.001). The wound-healing period was also significantly different between the PC and PSC groups (mean 27.18 days vs. 20.96 days, p=0.023). However, the postoperative wound-healing delay of >30 days was not significantly different (39% vs. 20%, p=0.114). In addition, there were no significant differences in the response to questionnaires on patient satisfaction between the two groups.
Conclusions
PSC has a lower surgical site infection rate and the wound-healing delay was not very different from that of PC. Therefore, if patients are at risk of wound infection, such as in severe wound contamination, long operating time, and immunocompromised conditions, we should consider PSC as a wound closure method of choice.
9.A Changing Pattern of Isotope Scan of the Midline Ectopic Thyroid: A Case Report

A 17 month-old girl presented in the pediatric clinic on November 27th, 1990 with a neck mass. The mass was 2 cm in diameter, firm in consistency and movable on the upper pole of the thyroid cartilage in the midline. The technetium thyroid scan showed a hot reactivity at the compatible site of the mass, but no other radioactivity in either site of the normal thyroid positions. At her second visit on January 23th, 1996, the mass had enlarged up to 3.5cm in diameter in the same location of the neck. The follow up thyroid scan revealed a walnut sized, snowman-like radioactivity. One of the snowman-like double images seemed to be a lingual ectopic thyroid and the other a midline ectopic thyroid remnant in the infrahyoid level. This interpretation was supported by the computed tomography of the neck, which showed a ligual mass in the foramen cecum area and an another mass in the anterior comis-sure of the larynx in the mildline. Thyroid function test was normal except a slightly increased TSH. As a result of this changing pattern of thyroid radioactive images, a case of a lingual thyroid as well as another midline ectopic thyroid tissue at the infra hyoid level is reported.
