1.Correction of the plunging nasal tip: 7CASES.

The primary factors determining nasal tip outline are shape and position of the alar cartilages. The relation of the alar cartilages to the upper lateral cartilages, septum, and soft tissue envelope defines the conformation of the tip-lobule complex. Plunging tip is a condition as long nose, tip drooping and becomes more accentuated with aging. Hanging columella is a prominent downward bowing of the columella. Plunging tip and hanging columella are common causes of acute nasolabial angle.Correction of the plunging tip in the long nose is usually achieved satisfactorily through shortening of the lateral walls by excising an adequate amount of cartilage from the septal, lateral, and alar cartilage. Tip rotation in a cephalic direction can be achieved commonly by resection of the cephalic portion of the lateral crura, excision of a triangular segment of the caudal margin of the septum and a cartilaginous septal transfixion incision involving excision of a superiorly based triangle of septum with cephalic rotation of the entire nasal lobule.Modification of the caudal margin of the septum is done to achieve three goals: (1) cephalic rotation of the tip, (2) shortening of nasal length, and (3) alterations in the nasolabial angle. We have elevated and rotated in a cephalic direction nasal tip by means of resection of cephalic portion of lateral crura, resection of the caudal margin of the septum and mucous membrane, and /or invagination technique for correction of the plunging tip and hanging columella. From March 1996 to February 1998, we have performed the tip-plasty in 7 patients of the plunging tip and hanging columella. We have found good cosmetic results with improved nasolabial angle.

3.Changes of Nitric Oxide Metabolite and Tumor Necrosis Factor-alpha in the Urine of Patients with Superficial Bladder Tumors after BCG Immunotherapy.

Bacillus Calmette-Guerin(BCG) is widely used in the therapy of superficial bladder tumors. Clinical and animal studies suggest that immunological responsiveness to BCG antigens correlates with anti-tumor activity. But its mode of action is unclear. In this studies, we evaluated the changes of nitric oxide(NO) metabolite and tumor necrosis factor-alpha(TNF-alpha) in the urine of patients with superficial bladder tumors after BCG immunotherapy. Both NO and TNF-alpha in the urine specimens of patients treated with BCG were analyzed prior to BCG instillation and 4, 8 and 24 hours afterwards. After 6th BCG instillation. urinary NO increased significantly and showed a maximum of secretion at eight hours(p<0.05). After 6th BCG instillation, urinary TNF-alpha increased and showed a maximum of secretion at eight hours but had no statistical significance. These results suggest that intravesical BCG therapy increased the urinary NO and TNF-alpha secretion which may be related to the anti-tumor activity.

5.Experimental Study on Cerebral Cysticercosis.

Cerebral cysticercosis may produce many complications and various types of tissue reactions to the parenchyma of the brain. The purpose of the present investigation was to study the nature of the reactions of the brain tissue responding to a direct contract with the fluid contents of cysticercus cyst. The experiments were performed in 15 albino rats in which the brain lesions were made on the frontal region by injecting the fluid contents which was obtained from subcutaneous nodules of human patient with cysticercosis. The fluid contents, 0.5 ml for each animal, was injected with 26 gauge hypodermal needle into the left cerebral hemisphere beneath the cortex for experimental group, and same amount of normal saline into the right hemisphere indentical to the region of the left for the control group. Histopathological studies were carried out on the brain lesions at interval of 3 days, 7 days and 15 days following injection of the contents and normal saline. Findings of generalized and regional edema of the brain were observed in varying degrees, which were evident and severe in 3-day and 7-day experimental groups and decreasing in severity thereafter. In the group of normal saline injection, the microscopic findings of the lesions were mainly of a mild inflammatory reactions with scattered fibroblast or condensation of the nervous tissue. In 3-day experimental group, there were pleomorphic infiltration of neutrophils and histiocytes. In 7-day experimental group, there were prominent cellular reaction, gliosis and vascular proliferation more than those observed in 3-day group. In 15-day experimental group, inflammatory cells and gliosis were reduced in number and degree, but definitive granuloma with proliferation of vascular fibroblast in its outer layer was developed.

7.Raeder's Paratrigeminal Syndrome.

The authors report two cases of paratrigeminal syndrome. They were male and involved in the left side, and had no known hypertension. The syndrome was produced and encountered in patients who had undergone a percutaneous carotid angiography and surgery with ligation of common carotid artery for the treatment of carotidcavernous sinus fistula. The clinical manifestations were unilateral, intense, throbbing supraorbital and periorbital pain recurring regularly, accompanied by drooping of the eye lid and miosis with normal sweating on the same side as the pain. The cases were treated conservatively with diphenylhydantion and antimigraine preparations with apparent success. It is probable that the underlying pathology of the syndrome in these cases may be compression of the internal carotid sympathetic fibers due to swelling of the interal carotid arterial wall. The clinical features and courses were compatible with those of group 2 of Raeder's syndrome.

8.Rigid ureteroscopic removal of ureteral stone performed at outpatient department.

Between January 1990 and February 1991, ureteroscopic removal of ureteral stone was performed in 40 cases at outpatient department under local anesthesia(36 cases) and epidural anesthesia(4 cases). The results were as follows. The success rate of ureteral stone removal was 82.1% (32 among 39 cases) in lower ureter, 86.7 %(26 among 30 cases) in female and 70.0%(7 among 10 cases) in male. Anesthetic methods were local(36 cases), and epidural(4 cases), Mean duration of hospitalization was 1.4 days(0-7 days). The associated complication rate was low, with most patients being treated conservatively. Therefore, we believe that the ureteroscopic stone removal at outpatient department can be done safely if we select patients properly(especially women). have adequate equipment and execute properly the basic technique.

9.Surface characteristics and biocompatibility of bioinert nitrides ion plated titanium implant.

Even though titanium(Ti) and its alloys are the most used dental implant materials, there are some problems that Ti wears easily and interferes normal osteogenesis due to the metal ions. Ti coated with bioactive ceramics such as hydroxyapatite has also such problems as the exfoliation or resorption of the coated layer. Recent studies on implant materials have been proceeding to improve physical properties of the implant substrate and bio Compatibility of the implant surfaces. The purpose of the present study was to examine the physical property and bone tissue compatibility of bioinert nitrides ion plated Ti. Button type specimens(l4mm in diameter, 2.32mm in height) for the abrasion test and cytotoxicity test and thread type implants(3.75mm in diameter, 6mm in length) for the animal experiments were made from Ti(grade 2) and 316LVM stainless steel. Ti specimens were ion plated with TiN, ZrN by the low temperature arc vapor deposition, and the depth profile of the TiN/Ti, ZrN/Ti ion plated surface was examined by Auger Electron Spectroscopy. Three kind of button type specimens of TiN/Ti, ZrN/Ti and Ti were used for abrasion test, and HEPAlClC7 cells and CCD cells were cultivated for 4 days with the specimens for cytotoxicity test. Thread type implants of TiN, ZrN/Ti, Ti, 316LVM were implanted on the femur of 6 adult dogs weighing l0kg-l3kg. Two dogs were sacrified for histological examination after 45 days and 90 days, and four dogs were sacrificed for the removal torque test of the implants after 90 days. The removal torque force was measured by Autograph (Shimadzu Co., AGS-l000D series, Japan). Abrasion resistance of TiN/Ti was the highest, and that of ZrN/Ti and Ti were Followed. The bioinert nitride ion plated Ti had much better abrasion resistance, compared with Ti. In the cytotoxicity test, the number of both cells were increased in all specimens, and there were no significant difference in cytotoxic reaction among all groups (p>0.1). In histological examination, 316LVM showed the soft tissue engagement in interface between the implant and bone, but the other materials after 45 day5 noted immature new bone formation in the medullary portion along the implant surface, and those after 90 days showed implant support by new bone formation in both the cortical and the medullary portion. The removal torque force of TiN/Ti showed significantly higher than that of Ti(p0.05), and that of 316LVM was lowest among all groups(p<0.05). These results suggest that bioinert nitrides ion plated Ti can resolve the existing problems of Ti and bioactive ceramics, and it may be clinically applicable to human.

10.Changes in Toxicological Characteristics after Sales of Nonprescription Drugs in Convenience Stores.

PURPOSE: On November 15, 2012, sales of OTC (Over-The-Counter) drugs began at convenience stores, which changed the accessibility of some drugs. As a result, the exposure and access patterns of these drugs could have changed. In this study, we reviewed the changes in the characteristics of drug poisoning patients because of the reposition of nonprescription drugs according to the revised Pharmaceutical Affairs Act. METHODS: A retrospective study was conducted to evaluate changes in characteristics of drug poisoning patients between 2008 and 2016. A registry was developed by an emergency medical center in a local tertiary teaching hospital, and patients who visited the center were enrolled in this registry. We compared two periods, from 2008 to 2012 (Pre OTC) and from 2013 to 2016 (Post OTC), for type of intoxicant, time from poisoning to visiting the emergency center, intention, psychiatric history, previous suicidal attempt, alcohol status, and emergency room outcomes. The primary outcome was the number of patients who took acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). Secondary outcomes were ICU admission rate, mortality rate, and number of patients who visited the ER when the pharmacy was closed after taking acetaminophen and NSAIDs (nonsteroidal anti-inflammatory drugs). RESULTS: Among 1,564 patients, 945 and 619 patients visited the emergency room during pre and post OTC periods. The number of patients with acetaminophen and NSAIDs poisoning decreased from 9.2% to 6.1% (p=0.016). The ICU admission rate and mortality rate in the emergency room did not show significant results in the relevant patient groups, and so was the number of patients visiting ER when the pharmacy was closed taking acetaminophen and NSAIDs. CONCLUSION: Despite the sales of nonprescription drugs at convenience stores, the number of acetaminophen and NSAIDs poisoning patients decreased.
