In order to search for anti-Herpes simplex virus (HSV) type-1 agents from Korean medicinal herbs and Korean traditional prescriptions (herb complexes), we selected 80 medicinal herbs and 45 prescriptions, based on a review of the Korean traditional medicine books. Both methanol extracts and boiling-water extracts were tested by means of the MTT assay (tetrazolium based colorimetric assay). Ten of the 125 methanol extracts: CM-11, CM-18, CM-19, CM-21, CM-22, CM-39, MM-3, MM-18, MM-29, MM-73 (see explanation of nomenclature below), showed efficacy against HSV-1. Twelve of the water extracts: CW-2, CW-3-I, CW-3-II, CW-18, MW-3, MW-5 MW-6, MW-12, MW-47, MW-69, MW-73 and MW-75 were active. #3 (individual herb) and #73 (individual herb) were interesting because both water and methanol extracts were active. Especially, #3 is a part of composition of Hong-il-laksamd ngbang and Hojanghaedokt'ang which have anti-HSV-1 activitives. The SI value of MW-69 and CW-18 was relative high as 10.2+/-0.7 and 11.8+/-2.2. The cytotoxic effect on Vero cells of Panch' n-ch' ngbang, Taraxacum platycarpum H. Dahlst. and acycloguanosine was determined by MTT assay. Water extracts of Panch' ngch' ngbang (prescription) and Taraxacum platycarpum H. Dahlst. showed very weak cytotoxic effects on Vero cells at > 100 mug/ml but acycloguanosine showed strong cytotoxic effects on Vero cells at > 100 mu g/ml. As a result, #3, #73, MW-69 and CW-18 are considered as potentially useful for anti-HSV-1 agent and will be the focus of further research. Pheochromocytoma is one of the surgically curable hypertensive syndrome and may arise wherever chromaffin cells are located. A case of a left large adrenal pheochromocytoma developed in a 49 year old male is presented with brief review of literatures.

PURPOSE: There is no definitive operative indications for treatment of Gallbladder (GB) polypoid lesions. The objective of this study was to determine the indications for surgical treatment of polypoid lesions of the gallbladder. METHODS: Surgically resected polypoid lesions of the gallbladders of 111 patients who were treated at the Department of Surgery, EulJi Medical College, from Jan. 1991 to Jun. 2000 were evaluated based on patient age & sex, histologic result, and size & shape of the polyp. RESULTS: Out of the 111 cases, benign polyps were found in 88 cases (79%) and malignant polyps in 23 cases (21%). In sessile polyps, the average diameter of the benign polyps was 8.1+/-9 mm, and that of the malignant polyps was 16.6+/-9 mm, while in pedunculated polyps, the average diameter of the benign polyps was 9.5+/-8 mm, and that of the malignant polyps was 27.0+/-22 mm. CONCLUSION: In conclusion, surgical treatment is recommended if a sessile polyp is more than 5 mm and a pedunculated polyp is more then 10 mm because, in these cases, the chance of malignancy cannot be excluded. Also, if there are any symptoms related with the GB, regardless of how large it is and whether GB stone exists or not, surgical treatment is desirable.

A kidney containing more than 1,000ml fluid in its collecting system is generally defined as giant hydronephrosis. The clinical manifestations of giant hydronephrosis are indistinct. The patient seems to seek medical help late due to tolerable symptoms and particularly in the state of poor economic condition. We present a giant hydronephrosis of contents of 60 liters associated with ureteropelvic junction stricture on a 43-year-old woman. The final diagnosis was performed by exploration of abdomen and nephrectomy was done.

Virilization in infants and children is most commonly associated with congenitalbilateral cortical hyperplasia in the adrenogenital syndrome. Two cases of adrenogenital syndromes developed in sisters of a 15 and a 13 year-old girlsare presented with brief review of literatures.

Circulating immune complexes (ClC) were detected by platelet aggregation test (PAT) in 40.0% of 45 diabetics and by polyethylene glycol precipitation-complement consumption test (PEG-CC test) in 30.6% of 36 diabetics as compared to 5% and 10% of 20 normal control subjects for each test. The prevalence of CIC in diabetics was significantly higher than in the normal controls (P < 0.05%). There were no correlations between the presence of ClC detected by PAT and the duration of the disease, insulin treatment, or diabetic complications. Thus multiple factors must contribute to the increase of ClC in diabetics. The role of these various factors needs to be studied.

Cystic retrochiasmatic craniopharyngioma may reach enormous size by expanding into the posterior fossa along the retroclival area, which is very unusual finding. An 11-year-old girl presented with walking disturbance and irritability. On the neurological examination, she had positive Babinski sign, hyperactive deep tendon reflex, and left lateral gaze nystagmus. She also had right hearing disturbance. Initial magnetic resonance(MR) images revealed a huge, thin capsuled cystic mass which extended into the cerebellopontine angle down to the medulla oblongata. Secondary adjuvant chemotherapy, radiotherapy, and reoperation were necessary after first tumor removal by a suboccipital approach, due to the progression of disease. Finally, neurological condition was much improved without remnant tumor although some sequelae were remained. We report this interesting case because of its rarity, complex clinical presentation, and challenge in its management.
