1.A Case of Patient with Lung Cancer in the Terminal Stage whose Appetite and General Condition had been Maintained Following the Oral Administration of \it{Shoyogan}

Shoyogan is a Kampo prescription described in the Jinkuiyaolue text, but there have been no case reports regarding this medicine. We report a patient with terminal stage lung cancer, whose appetite and general well-being was maintained following oral administration of this medicine.A 62-year old woman had her main tumor in the left pulmonary lobe, which invaded to the bilateral peritoneum and the mediastinum. Metastasis was recognized in the left femoral bone which had been broken, the ilium, chest and lumbar vertebral bones, and the liver. Her general condition was very poor in spite of the administration of corticosteroids and narcotic medicines, when she began to take shoyogan 5 months after the onset of her symptoms. Cytological examination of her pleural effusion revealed adenocarcinoma. Following the administration of shoyogan, her appetite recovered, nutrition improved, and she was able to survive while maintaining her quality of life for another 2 months.In this paper we discuss the efficacy of shoyogan in improving the general well-being of patients having the above-mentioned conditions, or jueyinbing in Kampo parlance. Although preparing shoyogan is laborious, we believe this medicine should be used more frequently.

2.Keishikashakuyakuto Ameliorated Diverse Autonomic Symptoms such as Diarrhea, Abdominal Pain,Urinary Retention and Orthostatic Hypotension in a Case of Pure Autonomic Failure

Pure autonomic failure (PAF) is a degenerative disorder with diverse autonomic nervous symptoms, but without somatic nervous symptoms. We encountered a patient with PAF who was successfully treated with keishikashakuyakuto. A 61-year-old man complained of diarrhea, abdominal pain, dysuria and orthostatic hypotension. PAF was strongly suspected, based on a low level of plasma noradrenalin at rest and a finding of severe diffuse sympathetic nerve injury on 123I MIBG myocardial scintigraphy. Various Kampo formulas were not effective, or could not be administered continually. Urinary retention was treated with self-catheterization. After the extract of keishikashakuyakuto was administered, his complaints of diarrhea and abdominal pain gradually decreased, and he was able to eat various kinds of food. His daily living activities improved.Moreover, he could urinate by himself, so the self-catheterization was stopped. Five years later, the diagnosis of PAF was clinically confirmed, but his daily living activities did not deteriorate. This suggests that keishikashakuyakuto can be effective for diseases with diverse autonomic nervous symptoms, such as the present case.
3.A Case of Psoriasis Vulgaris with Exanthema Concentrates on the Head Successfully Treated with Jizusoippo

A 59 year-old woman with rashes of forehead and chest visited our hospital in hope of treatment with Kampo medicine. The rashes had occurred at the age of 50 years old and gradually spread. She had been diagnosed with psoriasis vulgaris by a nearby dermatologist. Though she had been treated with the steroid ointment, the symptom had repeated exacerbation and remission for nine years. At the first examination, her exanthems were damp, concentrated on frontlet and was seen in posterior cervical region, precordial region, upper and lower limb. Jizusoippo was prescribed, and the exanthem improved significantly in 4 months. In conclusion, jizusoippo can be effective in cases of psoriasis that the exanthems are damp and concentrate on head.
4.The Cases of Lumbago and Numbness of Legs Successfully Treated with Saikokaryukotsuboreito

The low back pain is often intractable and leads to the drop of the daily living activity. These symptoms are good indications of Kampo medicine. We report 5 cases of low back pain that saikokaryukotsuboreito treatment succeeded regardless of psychogenetic presence. These patients complained of low back pain and lower melagra or numbness of limbs. Four of 5 cases did not have any psychogenetic presence. After administration of saikokaryukotsuboreito, their pain decreased and walking distance was improved in to 4 weeks and analgesics were not required. Generally, saikokaryukotsuboreito is prescribed for patients with psychogenetic symptoms, but these treatments based on abdominal patterns were effective even if they do not have subjective psychogenetic symptoms. Saikokaryukotsuboreito may be the differential prescription for such low back pain cases.
