1.The value of combined determination of serum hepatic fibrosis index for evaluation on the degree of liver fibrosis in patients with chronic hepatitis B

Objective To explore the value of combined determination of serum hepatic fibrosis index for evaluation on the degree of liver fibrosis in patients with chronic hepatitis B(HBV).Methods 275 patients with HBV were selected as chronic hepatitis group,which were divided into the mild chronic hepatitis group(75 cases),chronic moderate hepatitis group (72 cases),chronic severe hepatitis group (65 cases),liver cirrhosis group (63 cases) according to the severity of disease.In addition,50 healthy population were selected as the healthy control group.The four liver fibrosis indicators included serum hyaluronic acid(HA),laminin (LN),collagen type Ⅲ (PC Ⅲ),type IV collagen(IV-C) were determined by chemiluminescence immunoassay and compared among the groups,and the diagnostic results of combined different indicators were compared.Results The indexes of HA,PC Ⅲ,IV-C,LN were significantly higher than those of the healthy control group (t =2.567,2.687,2.811,2.741,all P < 0.01).The hepatic fibrosis indexes were increased by with the severity and duration increased gradually (t =2.665,2.787,2.755,2.811,all P < 0.01).The concentration-time curves area (AUC) of PC Ⅲ,Ⅳ-C,LN,HA were 0.680,0.825,0.716,0.826,respectively.Diagnosis value of three combination indicators in HA + PC Ⅲl + Ⅳ-C was highest.Conclusion The chemiluminescence dynamic detection of four liver fibrosis indicators is a good non-invasive detection method for guiding clinical diagnosis on the degree of liver fibrosis.
2.Three dimensional dynamic contrast enhanced magnetic resonance angiography of the vena cava

Objective To evaluate the usefulness of three dimensional dynamic contrast enhanced magnetic resonance angiography(3D DCE MRA) in the diagnosis of vena caval abnormalities. Methods 3D DCE MRA was performed on fifty two patients with suspected vena caval anomaly, thrombosis, stenosis, occlusion, or compression. Image quality and the presence of venous abnormalities and collaterals were evaluated. Comparison with conventional venacavography( n =15) and surgical findings( n =18) was made. Results 3D DCE MRA images were of diagnostic quality without artifact for all 52 patients. 3D DCE MRA clearly showed vascular anatomy, anomaly and various disorders. The site, extent, cause of venous obstruction, and collateral pathways were well depicted. According to venography or surgery, the sensitivity and specificity of 3D DCE MRA in the diagnosis of abnormalities of vena cava were both 100%. In addition, it had advantages regarding demonstration of extravascular lesions and multiplanar capabilities. Conclusion 3D DCE MRA is simple to perform and accurate in detecting various vena caval abnormalities.
3.Gradient-echo chemical shift imaging in detection of lipid-containing abdominal lesions

Objective To evaluate the value of gradient-echo chemical shift imaging in detection of lipid-containing abdominal lesions. Methods 35 patients with abdominal lesions were divided into 2 groups according to whether or not they contained lipid. Breath-hold in-phase(IP) and opposed-phase(OP) acquisitions were performed. The demonstration of these lesions on IP and OP was compared. The signal intensity of these lesions on both images was measured. The percentage of signal intensity variation on IP and OP was calculated and statistically analyzed with non-parametric method between the 2 groups. Results For lesions containing lipid, the signal intensity dropped significantly from IP to OP, with median value of signal intensity variation attaining 37.1%. On the other hand, for lesions not containing lipid, the signal intensity did not change obviously between IP and OP, whose median value of signal intensity variation was 3.3%. The difference between the 2 groups was statistically significant (u=4.56,P<0.01). Conclusion Gradient-echo chemical shift imaging is an easy and practical way, helpful to detection of lipid component within abdominal lesions.
4.Inhibitive Effects of Antisense Oligonucleotides of c-fos and c-jun on the Proliferation of the CNE-2Z Cells and the Expression of PKC-?

The effects of antisense oligonucleotides of c-fos and c-jun on the proliferation of CNE-2Z cells and on the expression of protein kinase C-? (PKC-?) were investigated. With the interpoising of Lipofectin(LP), the CNE-2Z cells were added with antisens oligonucleotides of c-fos and c-jun. MTT method was used to test the cell proliferation and flow cytometry (FCM) to the PKC-a expression. The results showed that the inhibition of antisense oligonucleotides of c-fos and c-jun to CEN-2Z cells was gradually enhanced with its concentration and prolonging time increasing and that its lowest effective concentration of LP was 1.7 X 10~(-6)?g/ml, the lowest effective concentration was of antisense oligonucleotides of c-fos and c-jun 1.7 x 10~(-5)?g/ml, and the most effective time was 20 hours after treatment. The fluoresence of PKC-a and percentage of positive cells in the groups treated by antisense oligonucleotides of c-fos and c-jun decreased significantly. The results indicated that the autisense oligonucleotides of c-fos and c-jun could inhibit the proliferation of CNE-2Z ceUs the expression of PKC-?.

Objective To evaluate the value of endorectal coil(E-coil) in the diagnosis of prostate diseases.Methods The comparative study was done with E-coil and body coil in 15 patients including 9 cases of prostate carcinoma,4 cases of benign prostatic hyperplasia and 2 normal individuals.The conventional spin-echo sequence(SE T 1WI,FSE T 2WI)were done in all cases.The axial images obtained with two types were compared according to the subjective viewing and scoring.Results Overall imaging quality on E-coil was significantly superior to that on body coil.The average scores were 2.97?0.61 points with body coil vs 3.4?0.60 points with E-coil on T 2WI (?
6.Computed tomographic methods and findings in adenomyomatosis of the gallbladder

Objective To describe the characteristic CT imaging of adenomyomatosis of gallbladder and to improve the diagnostic knowledge of the disease. Methods Eleven (5 males and 6 females, mean age = 43 years) patients with adenomyomatosis of gallbladder (4 diffuse, 2 segmental, and 5 fundal) confirmed by histopathology and performed by CT examination including plain, dynamic or multiphase enhancement and / or oral cholegraphic agent were reviewed. Results Correct diagnosis was made in 7 of 11 patients(3 diffuse, 1 segmental, and 3 fundal). CT findings included segmental or diffuse gallbladder wall thickening in the involved area, associated with well defined and regular borders at both of the inner and outer layers. Significant CT findings of this entity were marked enhancement of wall layers during triphase contrast CT scanning, especially at delayed scans. Rokitansky Aschoff sinuses were clearly shown by CT, presenting as characteristic "aureola " sign, when performed with application of a cholegraphic agent. Conclusion Understanding the CT findings and using the optimal methods, we can use CT scan as a valuable method to help make the diagnosis in most cases of adenomyomatosis.
7.Experience of three cases of heart-lung transplantation

Objective To summarize the successful experience of three cases of heart-lung transplantations performed in our institute.Methods From July 2003 to August 2012,three patients,with diagnosis of end-stage heart-lung diseases,received heart-lung transplantation in our institute.One case was diagnosed as congenital atrial septal defect,Eisenmanger syndrome,NYHA class Ⅳ; one was dilated cardiomyopathy with moderate/severe pulmonary arterial hypertension,NYHA class Ⅲ-Ⅳ,one was diagnosed as double outlet left ventricle (DOLV) with ventricle septal defect and stenosis of pulmonary artery and its left and right branches,NYHA class Ⅲ-Ⅳ.Donor hearts were preserved with UW solution,donor lungs were preserved with Euro-Collin solution in case one and with low potassium dextran containing prostaglandin E1 in the others.Extensive disinfection and strict scrutiny were implemented postoperatively.Immunosuppressive therapy included administration of zenapax or basiliximab preoperatively,methylprednisolone during the operation,and cyclosporine a/tacrolimus + prednisone + mycophenolate postoperatively.Surgical hemostasis is of great importance,as the total pleural effusion reaches 14 640 ml within 31 days postoperatively in case two.Strict postoperative disinfection and isolation were implemented,and management of the respiratory tract was intensified.Therapeutic bronchoscopy was performed frequently for sputum suction.In case two,bronchoscopy was used thirteen times within 40 days after transplantation.Broad-spectrum antibiotics and antifungal antibiotics were used for infection control.Results All three patients were discharged after recovery from operation.Case one died of obstructive bronchitis and lung failure caused by chronic rejection four years and ten months postoperatively.Case two died of sudden cerebrovascular accident 68 days after operation.Case three survives more than one year postoperatively so far and is still alive.Conclusion Proper preservation of the donor heart and lung,perfect surgical hemostasis,strict infection control,frequent application of bronchoscopy and appropriate immunosuppressive management are critical to the success of heart-lung transplantation.
8.Clinical effect of diode laser on treatment of moderate to severe acne

Objective To evaluate the efficacy and safety of 650 nm,810 nm diode laser on the treatment of moderate to severe acne.Methods A total of 87 patients with moderate to severe acne were divided into diode laser group (group A) and medication group (group B).In group A,the patients were irradiated with 650,810 nm diode laser at the fluence of 400-450 mW distance of 2-3 cm.In group B,the patients were treated with roxithromycin combined with vitamin B6 and gel lincomycini.Clinical follow-up was performed every week for 4 months.All the adverse effects were recorded.Results There was no significantly statistical difference in the cure rate between the two groups (group A versus group B at 2 weeks:65.0 % versus 42.6 %,P>0.05; at 4 weeks:92.5 % versus 76.5 %,P>0.05).However,the recurrence rate of group A was much lower than that of group B (30.0 % versus 61.7 %,P<0.01).There were no severe adverse effects in both groups during the period of the follow-up.Conclusions Compared to the medication,650,810 nm diode laser appears to be associated with lower recurrence rate in the treatment of moderate to severe acne.

Objective To integrate existing data and analyze the optogenetic application in the study of the cocaine addiction mechanism.Methods The key words optogenetics,addiction, cocaine and so on were selected.Relevant research information was collected from various authentic databases such as China Journal Full-text Database,Chinese Biomedical Databases,Wanfang Digital Journals,VIP Full-text Database and PubMed,SCIENCEDIRECT,SPRINGERLINK databases by literature study,assess the quality of the studies and the qualitative systematic review was conducted in this study.Results Findings from present data showed that:with great spatial and temporal specificity,the optogenetic application not only proved the important role of traditional theories of mesolimbic dopamine system,but also dissected the neural circuits and molecular mechanisms of dopaminergic and glutamatergic neurotransmission underlying cocaine addiction.Conclusion The use of optogenetic tools has given researchers the potential to dissect the neuronal circuits and molecular changes,leading us to a better understanding of the mechanisms of cocaine addiction and to the development of novel treatment.
