1.Determination of isofraxidin in Xiaoerkaiwei Oral Liquid by HPLC

Objective: To establish a method for determination of isofraxidin in Xiaoerkaiwei Oral Liquid (Radix Acanthopanacis Senticosi, Endothelium Corneum Gigeriae Galli, Semen Alpiniae Katsumadai, Semen Pharbitidis, etc.) by RP HPLC. Methods: The HPLC system consisted of Symmetry C 18 column(3.9m?150mm, 5?m), methanol water (0.1% formic acid) (30∶70) mixture as a mobile phase, detection wavelength at 342nm, 0.8mL?min -1 of flow rate and column temperature at 30℃.Results: The calibration curves of the isofraxidin were linear ( r =0.9993). The precision were perfect ( RSD =1.87). The samples were stable in 6 h. The average recovery of Xiaoerkaiwei Oral Liquid was 102.4% with RSD of 1.76%.Conclusion: This method is simple, quick and specific, suitable for the quality control.
2.RELATIONSHIP OF NOCTURNAL HYPOXEMIA TO VENTRICULAR ECTOPY IN PATIENTS WITH CHRONIC OBSTRUCTIVE PULMONARY DISEASE

Objective:To determine the relationship between nocturnal oxygen desaturation and ventricular ectopy and study the effect of sleep stages on the frequency of ventricular ectopy in patients with chronic obstructive pulmonary disease (COPD).Method:Twenty two clinically stable patients with mild moderate COPD underwent overnight polysomnography,arterial oxygen saturation (SaO2) and ECG were recorded simultaneously.The relationship between the frequency of ventricular ectopy (VE) and SaO2,sleep stages and compared the results with those of twelve normal control subjects.Result:The prevalence of nocturnal VE in patients with COPD was 59.1 percent which was obviously higher than that of control group.The frequency of VE had no correlation with the frequency and the degree of oxygen desaturation,the duration of SaO2 below 90 percent,nocturnal mean SaO2 and nadir SaO2.The frequency of nocturnal VE during wake stage,REM stage and NREM stage had no difference.Conclusion:These results suggest that mild moderate nocturnal oxygen desaturation and sleep stages had no apparently influence on the events of VE in stable patients with COPD.
3.Determination of Quinolones in Spicy Soup Using Packed Fiber Solid Phase Extraction and High Performance Liquid Chromatography-Fluorimetric Detection

An analytical method for simultaneous determination of five quinolones in spicy soup was developed. Spicy soup samples were firstly extracted by EDTA-Mcllvaine buffer at pH 4, then purified and concentrated by a novel Packed fiber solid phase extraction ( PFSPE ) coulumn. The extracted liquid supernatant was loaded onto the column, rinsed with water, and then eluted with 2% ammoniated methanol. The mobile phase was methanol-water-phosphoric acid (25:75:0. 1, V/V, adjusting the pH to 2. 8 with triethylamine) . These analytes were quantified by high performance liquid chromatography-fluorimetric detector( HPLC-FLD) at excitation and emission wavelength of 280 nm and 450 nm respectively. Recoveries of spiked quinolone antibiotics in spicy soup were from 72 . 1% to 110 . 3% with intraday relative standard deviation (RSD) between 1. 6% and 4. 3% and inter-day RSD from 2. 0% to 4. 3%. Limit of detection (LOD) and limit of quantitation(LOQ) were from 1. 2 to 5. 4 μg/L and from 3. 9 to 18 μg/L, respectively. The method could be applied to determine the quinolones in spicy soup.
4.Isolation, cultivation and identification of human skin microvascular endothelial cells

BACKGROUND:Currently, the enzymatic digestion combined with magnetic activated cel sorting for isolating microvascular endothelial cel s are cumbersome and do harm to cel s. Therefore, how to simplify the isolation and culture of human dermal microvascular endothelial cel s to obtain highly purified endothelial cel s in vitro becomes a hotspot.
OBJECTIVE:To explore a simple and effective cultivation method of microvascular endothelial cel s from diabetic patient skins in vitro, and to detect the cel growth.
METHODS:Diabetic patients with chronic foot wounds after amputation were enrol ed to col ect the limb proximal skin and topical skin around the wound superficial dermal tissue. Human dermal microvascular endothelial cel s were obtained using adherent method and trypsin method, fol oewd by purified utilizing trypsin digestion and repeated attachment method when passage culture.
RESULTS AND CONCLUSION:Human dermal microvascular endothelial cells were obtained successfully, Primary cultured endothelial cells completely adhered to the wall at 24 hours, entered the logarithmic phase at the 10th day, and the cell concentration reached 80%at the 12th-13th day. While the passage cells grew more actively than primary cells, and fully covered the bottom in a“cobblestone”arrangement after 5-7 days of culture. Immunohistochemical staining showed that cultured cells were positive for FVIII and CD31-associated antigens with 100%positive rate. MTT assay showed that cell growth curves of 2, 4, and 5 generations of dermal microvascular endothelial presented the invertedSshape. These results suggest that abundant highly purified human dermal microvascular endothelial cells can be obtained through the adherent method and a small amount of short-term trypsin method.
5.Effect of enteral nutrition support on prognosis in patients with severe acute stroke

Objective To investigate the effect of enteral nutrition support on prognosis in patients with severe acute stroke.Methods Ninety-eight cases of severe acute stroke were divided into nutrition supported group(50 cases) and control group(48 cases) randomly.All patients were treated by conventional therapy.Nutrition supported group received high energy nutrition diet and control group received common liquid diet by nasal feeding.The levels of hemoglobin,serum albumin,neurologic impairment score (NIHSS)and the incidence rates of complications were assessed at the 1st,10 th and 21st day after admission.Results Hemoglobin was (122.5 ± 2.4),(106.4 ± 2.8) g/L and serum albumin was (36.5 ± 4.7),(34.2 ± 5.1) g/L at the 21st day after admission in nutrition supported group and control group,and there was significant difference between two groups (P < 0.05).The complication rate in control group was higher than that in nutrition supported group (P< 0.05).NIHSS score was(8.45 ± 3.02) scores in nutrition supported group and (10.24 ± 2.57)scores in control group at the 21st day after admission,and there was significant difference between two groups (P <0.05).Conclusion Early enteral nutrition support can significantly improve nutritional status and decrease the incidence rates of complications in patients with severe acute stroke,and it is helpful for clinical prognosis.
6.Prevalence of fasting blood glucose abnormality and associated factors analysis in health examination population above 18 years old in Beijing

Objective: To study the fasting blood glucose level and associated factors relationship in physical examination population in Beijing. Methods: Totally 38 462 individuals who received physical examination from April 2006 through December 2007 in Peking Union Medical College Hospital physical examination center were enrolled in our study.We divided them into 8 groups according to the age,and the level of fasting blood glucose was tested.The relationship between BMI,age,sex and abnormity of blood glucose were analysed. Results: The incidences of overweight and obesity were 36.4% and 13.5%,respectively.The incidences of impaired fasting glucose and diabetes mellitus were 3.2% and 3.8%,respectively.The rate of abnormal glucose level increased with age and BMI.Compared to male,female persons had lower fasting blood glucose level and lower rate of abnormality(P =0.000).Compared to normal and thin population,overweight and obesity population had higher rate of fasting glucose abnormality(P
7.Study of Fanggan Decoction,s death prevention on mouse and inhibition effects on Influenza A virus

Objective:To study the Fanggan Decoction,s death prevention on mouse and inhibition effects on Influenza A virus in vivo.Methods:After setting up the model of mouse infected with Influenza A virus(H1N1),we observed the death prevention with Fanggan Decoction,done hemagglutination test and detected the dynamic contents of virus with Real-Time Fluorescence Quantitative RT-PCR.Results:Fanggan Decoction can prevent the death of infected mouse and delay the survival time.The death rate was 66.67%,33.33% and 25% respectively in low,middle and high dose of Fanggan Decoction groups and the average survival time was respectively 8.75 days,11.41 days and 12.33 days.Virus contents reached peak on the 5th day,while compared with the model group,virus contents were lower in each Fanggan Decoction groups,especial in the middle and high dose groups.Conclusion:Fanggan Decoction had good effect in inhibiting Influenza A virus,and can prevent the death of infected mouse,delay the survival time,while get better antivirus dose-effect relationship at double dose.
8.Acute interstitial pneumonitis associated pediatric acute respiratory distress syndrome in 8 recipients after liver transplantation

Objective To summarize the clinical course of acute interstitial pneumonitis (AIP) associated pediatric acute respiratory distress syndrome (PARDS) in 8 recipients after liver transplantation,and further discuss the potential risk factors and therapeutic highlights.Methods A total of 476 pediatric patients received liver transplantation in Tianjin First Center Hospital from January 2012 to September 2016.Among them,8 cases of AIP associated PARDS in ICU were recruited in this study.Medical data including clinical presentation,ICU management and outcomes were analyzed retrospectively.Results The onset time-window of AIP associated PARDS was (2.67 ± 0.77) months after liver transplantation,and the time interval between initial symptom and ICU administration was (6.75 ± 5.82) days.Five cases had the history of acute rejection therapy,and 5 cases had CMV and/or EBV viremia history.All 8 cases received mechanical ventilation,2 cases given nasal non-invasive ventilation and the rest 6 cases given invasive ventilation,3 of which were switched to high frequency oscillatory ventilation (HFOV) combined with inhaled nitric oxide.At the stage of hypoxic climax,the fraction of inspired oxygen (FiO2) was up-regulated to 1.0 to maintain the oxygenation index (OI) of (25.24 ± 5.94).Temporary replacement of immunosuppressants with intravenous glucocorticoids was implemented in all 8 cases without acute rejection episode.Of 8 cases,2 cases died from PARDS,1 case died from portal thrombosis associated hepatic failure,and the rest 5 cases survived.Conclusion AIP associated PARDS is a critical complication with high mortality in pediatric patients after liver transplantation.Excessively strong immunosuppression therapy at early post-transplant stage shows a risk factor for AIP.Lung protective ventilation strategy and HFOV are recommended to reduce ventilator induced lung injury in pediatric patients.Temporary intravenous glucocorticoids may reduce acute inflammatory reaction in PARDS patients without increasing the risk of acute rejection.
9.Analysis of 102 cases of delirium factor about psychological health department contact consultation

Objective To study the comprehensive hospital patients with psychological health department liaison consultation characteristics of the factors to cause delirium.Methods General hospital in 102 patients of psychological health department liaison consultation,this paper analyzed the factors which cause delirium.Results 102 cases(63 males and 39 females),the sex ratio was 1 :0.62;Men's and women's average age had no statistical difference(t =1.866,P =0.087 ).Age or age:70 ~ 79years,80 ~ 89 years of age to delirium patients were 39.26%,30.39% respectively.Delirium distribution respectively the main cause of the central nervous system disease,acute vascular disease,infection,disorder of digestion and absorption,anemia,endocrine metabolic disorders, acute metabolic disorders,trauma,surgery,spirit of active substances and non addictive drugs,etc.;Cause delirium in first place of the central nervous system disease,the etiology of acute vascular disease and infection of 29 cases respec-tively(28.43%),11 cases(10.78%),16 cases(15.69%).Additional surgery,spirit of active substances and non addictive drugs were 11 cases(10.78%),13 cases(12.75%),2 cases(1.96%).Many kinds of disease:102 patients with each with 1 ~7 kinds of diseases,the average (4.09 ±1.56)kinds.Share of drug species:102 patients in each oral 0 ~8 kinds of drugs,to average (5.86 ±1.52)kinds.Conclusion Comprehensive hospital patients with age or age group,the main factors to cause delirium etiology,patients with various diseases and share related species of drugs,disease complex,such as strengthening contact consultation for clinical sense.
10.Teicoplanin concentration in cerebrospinal fluid during intravenous infu-sion in patients following neurosurgery operation

Objective To understand teicoplanin concentration in cerebrospinal fluid (CSF)during intravenous in-fusion in patients following neurosurgery operation,and evaluate whether drug concentration can be increased if blood-brain barrier was damaged, and effect of continuous pump of drug on drug concentration in CSF. Methods The post-neurosurgical surgery patients with surgical site/ventricular drainage were enrolled in the study, patients were divided into routine administration group(a dose of teicoplanin of 400 mg/12 h was administered for 30 min)and continuous administration group (a dose of 400 mg teicoplanin was administered for 30 min followed by a continuous infusion of 200 mg/6 h).CSF specimens were collected at respective time points of administration, teicoplanin concentration in specimens was measured.Results For routine administration group,drug concentration in CSF was (0.004 ± 0.0123 )mg/L immediately after teicoplanin was bumped,the peak concentration was (0.712 ± 1.028)mg/L after 1-hour bumping,then concentration decreased gradually,which were (0.254 ±0.222),(0.173 ± 0.152),and (0.355±0.207)mg/L at 12,18,and 24 hours of bumping respectively.For continuous administration group, drug concentration in CSF was(0.017±0.020))mg/L immediately after teicoplanin was bumped,the peak concentration reached (0.587±0.255)mg/L after 4-hour bumping,then concentration were (0.429±0.416),(0.325±0.254),(0.476 ±0.686),and (0.318 ±0.464)mg/L at 6,12,18,and 24 hours of bumping respectively,teicoplanin concentration was relatively stable 6 hours later,which were (0.318±0.464)mg/L-(0.476±0.686)mg/L.The area under the curve during 24 hours (AUC0-24 )in routine administration group and continuous administration group were 5.590 mg/L·h and 9.082 mg/L·h respectively.For two groups of patients,teicoplanin concentration only at the area near peak value a-chieved 50% minimum inhibitory concentration(MIC50 )for coagulase negative staphylococcus (CNS),but the time for a-chieving concentration higher than CNS MIC50 was far less than 50% of total administration time;teicoplanin concentration in CSF of both groups of patients didn’t achieve MIC50 for Staphylococcus aureus .Conclusion After continuous infusion of teicoplanin,drug concentration in CSF can be increased compared with routine administration group,but still can’t achieve the effective MIC;the increase of blood drug concentration is benefit to drug concentration in CSF,it is necessary to in-crease the dose appropriately to achieve clinical effectiveness.
