1.Atherosclerotic reference segments influence assessment of coronary remodeling by intravascular ultrasound in patients with acute coronary syndrome

Objective To investigate the influence of atherosclerotic reference segments on coronary remodeling by intravascular ultrasound(IVUS) in patients with acute coronary syndrome(ACS). Methods One hundred and three consecutively enrolled patients [male 68 cases,female 35 cases,average age (60 ±11) years] with ACS were undergone by coronary artery angiography and IVUS with time selecting operation. The remodeling index(RI) was defined as lesion external elastic membrane cross section aere (EEM CSA) divided by the mean reference EEM CSA at the culprit vessel. Positive remodeling was RI≥ (67 cases), negative remodeling was RI <1(36 cases). The parameters of plaque were analysed between two remodeling groups, as well as compared between distal and proximal references. Results Only one patients do not have plaque at reference sites. In the analysis of 103 lesions and 102 reference sites, there were not significant different in plaque burden,plaque CSA,lumen CSA and lesion EEM CSA between two groups. Distal plaque burden[(39. 76 ±12. 54)% vs (32.38 ± 13.97)%, P <0.001] and plaque CSA [(6.14 ± 3.20)mm2 vs (4. 75 ± 3. 07) mm2, P = 0. 001] were larger than those at proximal reference. EEM CSA at distal reference was smaller than proximal one[(14. 99 ± 4. 12)mm2 vs (28. 37 ± 4. 48)mm2 , P <0. 001]. Plaque burden, plaque CSA and lumen CSA persented positive corelation between distal and proximal reference sites. Conclusions The reference segments lay a severe diffuse atherosclerosis in the group of ACS, which may influence assessment of coronary remodeling. Coronary remodeling as a process, a accurate evaluation could be result from the combination of static and serial approach by IVUS.
2.A Morphologic Study of the Structural Changes of Normal Aging Facial Skin.

In an attempt to elucidate the morphologic changes of normal aging skin, the present study was undertaken in human facial and chest well skin of individuals aged from 4 months to 76 years old. Biopsied skin was studied with light microscope, scanning electron microscope, and transmission electron microscope, using both conventional and tannic acid staining. The morphologic changes in the skin were noted as follow: 1. Structurally, the aged epidermis gradually became somewhat thinner, with flattening of the dermoepidermal interface. The number of melanocytes also decreased. 2. Abnormalities of elastic fibers such as loss of oxytalan fibers were observed from age 30. It was the initial sign of the aging process of elastic fibers, followed by abnormal changes in elaunin and mature elastic fibers. The degree of facial skin abnormality was rather more severe than that of the chest wall skin. 3. In individuals more than 50 years old, the age related changes in mature elastic fibers were more severe. Transmission electron microscopically, the electron density in elastin was irregular. The elastic fibers also showed pores and irregular splitting with fragmentation. Fine granular materials were scattered near the elastin. 4. Scanning electron microscopically, the elastic fibers in young adult skin showd ribbon-like fibers aligned in the same direction. They were either cylindrical or elliptical, having smooth surface. In old skin, the elastic fibers ran in various directions, forming complicated networks. These were larger, more elliptical and more branched than those in young adult skin. In summary, histologic changes of aging are much more prominent in sun-exposed skin(facial skin) than in sun protected skin(chest wall skin). A completely different spectrum of elastic fiber abnormalities was found in individuals more than 30 years old. The result indicates that elastic fiber abnormalities are related to aging skin.

5.Appliation of the combination of antiangiogenic agents and EGFR-TKIs in advanced NSCLC treatment

Epidermal growth factor receptor-tyrosine kinase inhibitors (EGFR-TKIs)and anti-angio-genic drugs have individually demonstrated clinical benefit in the treatment of patients with advanced non-small cell lung cancer (NSCLC).Recent studies demonstrate that the combination of anti-EGFR and anti-angiogene-sis can more significantly enhance clinical benefit,and even can remit EGFR-TKIs resistance in the treatment of advanced NSCLC.According to the different kinds of anti-angiogenesis drugs,recent clinical studies mainly include the combination of anti-vascular endothelial growth factor monoclonal antibody bevacizumab plus EGFR-TKIs and multi-targeted receptor anti-angiogenic tyrosine kinase inhibitor plus EGFR-TKIs,and the for-mer results show a more significant improvement in terms of safety and efficacy in the treatment of advanced NSCLC.Therefore,the combination of bevacizumab plus EGFR-TKIs can be used as a new treatment standard in the treatment of some patients with NSCLC.
6.Study on virus-specific cytotoxic T-lymphocyte responses in Chinese human immunodeficiency virus type 1 infection

Objective To investigate the features of HIV-1-specific Cytotoxic T-Lymphocyte (CTL)responses in Chinese.Methods The HIV-1-specific CTL responses were analyzed in an IFN-?ELISPOT assay by using a matrix system containing 820 overlapping peptides spanning the entire HIV-1 Clade B and C consensus sequence.Results The HIV-1-specific CTL response almost clus- tered in Gag and Nef across either HIV-1 Clade B(HIV-1 B)or HIV-1 Clade C(HIV-1 C),while oth- er proteins could also be recognized at different level.In comparison of the response between HIV-1 Clade B and Clade C,the magnitude and frequency was roughly identical with some difference found at single-peptide level.The most frequently recognized peptides of HIV-1 B were located in Nef,GPKEP- FRDYVDRFYKTLR(5/17,29.4%)and Gag,LWVYHTQGYFPDWQNY(5/17,29.4%),while the most frequently recognized peptide of HIV-1 C was located in Gag GPKEPFRDYVDRFFKTLR(6/17, 35.29%).Conclusions HIV-1-specific CTL responses clustered within HIV-1 Gag and Nef in Chinese. However,there was some difference between HIV-1 B and HIV-1 C at single-peptide level.
7.Efficacy and safety of intravenous cediland and esmolol and daltiazem for controlling rapid atrial arrhythmias.

ObjectiveTo evaluate the efficacy of intravenous cediland(lanatoside C) and esmolol and daltiazem for controlling rapid atrial arrhythmias.MethodsA total of 94 patients with rapid atrial arrhythmias were randomized to receive intravenous cediland(n=29) and esmolol(n=30) and daltiazem(n=35),respectively.ResultThe total efficacy rate were 86%,83% and 85%,with a mean decrease in heart rate by 30.4%,29.3 and 27.6% compared to baseline,and mean response times was 34.3?21.0min、8.1?2.1min and 11.3?3.8min,respectively.ConclusionThree drugs are all effective、rapid and safe in slowing rapid ventricular rate in patients with rapid atrial arrhythmias.

[Objective]To observe the effect of Homoharringtonine on Adjuvant Arthritis Rats.[Methods]Observe the effect of Homoharringtonine on Adjuvant Arthritis Rats’ SP,IL-1? and TNF-?;we take the Adjuvant Arthritis Rats as model and Tripterygium wilfordii polycoside was the positive group.[Results]The Homoharringtonine and Tripterygium wilfordii polycoside group’s level of TNF-?,IL-1? in the serum and synovial membrane decreased instructively,and their dose of SP in the plasma and synovial membrane was suppressed obviously too.[Conclusion]Homoharringtonine has the effect of anti-inflammation,it may treat the Adjuvant Arthritis Rats through reducing the dose of TNF-?,IL-1? in the serum and synovial membrane,inhibiting the secretion and releasing SP in the plasma and synovial membrane.

10.Application of the Differentiation of Body Constitution in Bronchiectasis

[Objective] To investigate the relationship between the body constitution and the syndromes of the bronchiectasis patients and provide the basis for the clinical treatment.[Methods] Col ected 80 out-patients and in-patients of bronchiectasis in the first affiliated hospital of Zhejiang Chinese Medical University from May 2011 to March 2012. Al patients met with the inclusion criteria, adopted the standardization of physical scale for physical identifica-tion and dialectical type and analyzed the relationship between constitution and syndrome.[Results] ①The gentleness type, qi-deficiency type and yin-defi-ciency type were more popular in the bronchiectasis, fol owed by the phlegm-wet type, wetness-heat type, yang-deficiency type and qi-depression type. The special inherited type and blood-stasis type were very rare. ②The syndrome of phlegm heat obstructing lung was the top one in the Chinese tradi-tional medical syndromes(48.75%).The frequency of liver-fire attacking lung syndrome, hyperactivity of fire due to yin deficiency syndrome and qi-yin de-ficiency syndrome were 21.25%, 18.75% and 11.25% respectively. ③From different body constitution distribution of TCM syndromes in patients with bronchiectasis, it could be found:gentleness type, qi-deficiency type, yang-deficiency type, phlegm-wet type and wetness-heat type were most presented as phlegm-heat stasis lung syndrome. Patients with qi-deficiency type were most presented as qi-yin deficiency syndrome .Patients with Yin deficiency type were always performed as hyperactivity of fire due to yin-deficiency syndrome. Patients with lung qi-stasis type were liver-fire invading lung syn-drome.[Conclusion] Phlegm-heat stasis syndrome was the main syndrome in patients with bronchiectasis. It conformed to the pathological characteristics of deficiency essence with virtual reality. Different body constitutions decided the different traditional Chinese medical syndromes after the disease onset.
