1.Induction of DNCB Oral Tolerance in Mice.

Tolerance to contact hypersensitivity was induced by feeding of different DNCB doses in mice. A total of 40 mice were divided into 4 groups(control group, 6 mg feeding group, 10 mg feeding group, 14 mg feeding group) in experiment I, Degree of tolerance to contact hypersensitivity was rneasured by incremert rate of ear swelling after challenge with DNFB. Experiment 2 was performed in the same method of cxperiment: I with addition of 3 mg DNCB feeding group. The increment ratee were significantly decreased in DNCB feeding groups in experirnent 1 and 2(p<0.0l). But there were no differences statisticalIy between increment rates of DNCB feeding groups.

3.A clinical survey of prolonged Q-T syndrome among Korean children with congenital deafness.

The clinical symptom complex characterized by syncopal attacks and sudden death in patients with electrocardiographic anomalies, especially a prolonged Q-T interval is known as the Romano-Ward syndrome. When a similar symptom complex is accompanied by congenital deafness, it is called Jervell and Lange-Nieisen syndrome. This study was undertaken to investigate the investigate the incidence of the latter syndrome in the Seoul area. A total of 1,013 children with congenital deafness who attend schools for the deaf were studied by analyzing their electrocardiograms. A corrected Q-T interval of> or =0.44 seconds was defined as a prolonged Q-T interval. The overall incidence of long Q-T syndrome observed in the deaf in the Seoul area was 0.49% as compared to 0.25% reported from elsewhere in the world. The mean age of patients with long Q-T syndrome was 11.3+/-5.9 years and the first syncopal attacks was 4.6+/-1.1 per patient. A valsalva maneuver and exercise tests in the test group were associated with significant changes in T wave configuration and prolonged Q-T intervals while similar changes were not observed in the control group. A larger survey is needed to derive more statistically significant conclusions.

4.Clinical Survey of Blind Eye in Eye Pattents.

Survey of causes of blindness on 23,249 eye patients who visited to department of Ophthalmology, Han Kang Sacret Heart Hospital, College of Medicine, Hallym University from Jan. 1982 to July 1986 were clinically evaluated. Blindness is defined as visual acuity for distance of 0.1(20/200) or less in the better eye with best correction. Standard Classification of Causes of Blindness recommended from the International Association for the Prevention of Blindness is applied. Estimated number of blindness is 882 persons(1057 blind eyes, 3.80%). For the evaluation of the site of blind eye, 175 persons(19.85%) were binocular and 707 persons(80.15%) were monocular, 375 persons(42.50%) were blind in the right and 332 persons(37.65%) in the left. Distribution of blindness causes by age group is; in male-under 10(3.41%), 11~20(6.06%), 21~30(12.78%), 31~40(10.32%), 41~50(8.42%), 51~60(6.72%), 61~70(5.78%), over 71(4.36%), and in female-under 10(1.80%), 11~20(1.90%), 21~30(4.45%), 31~40(2.65%), 41~50(3.88%), 51~60(5.49%), 61~70(10.13%), over 71(11.92%). Proportion of blindness by causes indicates; Injury and poisoning(31.60%), Senile cataract(28.11%), Infectious disease(9.28%), Others(5.78%), Glaucoma(5.50%), Systemic disease(4.78%), etc. Rate of blindness by site and type of affection is shown as; lens disease(cataract, 37.95%), Corneal disease(10.5%), Glaucoma(5.97%), Phthisis bulbi(5.87%), Anophthalmos(4.93%), Retinal detachment(4.17), etc. Evaluation of result and problems for better reliable survey in future are briefly discussion in comment.

5.Mallory-Weiss Syndrome During Cardiopulmonary Resuscitation by an Untrained Bystander.

There have been many reports regarding complications associated with cardiopulmonary resuscitation(CPR) and it may be true that there are many unrevealed complications. Although it is used to say that 'any CPR is better than no CPR', inadvertent or inadequate CPR may insult in serious organ injury. This report describes a 64-year-old woman who suffered Mallory-Weiss tear resulting from inadvertent cardiopulmonary resuscitation by a bystander who had never been trained. Possible mechanisms of gastric injuries caused by CPR are suggested, and importance of adequate CPR training are emphasized in this report.

6.Usefulness of Colonic Transit Time Measurement in Chronic Constipation.

A retrospective study of 274 patients who presented with constipation was done. The following results were obtained by recording colonic transit time. The male to female ratio was 72 : 197, and most patients were in their third and fifth decade, each age group accounting for 21% of total number. Associated symptoms were anal discomfort(33%), abdominal discomfort (25%), hematochezia (23%), reduced stool caliber (11%), and tenesmus (8%). As a result, 223 patients had normal transit time and 51 patients had abnormal transit time. Eighty-six patients with normal transit time and 51 patients with abnormal transit time underwent barium enema or colonoscopic examination. Abnormal lesions such as polyps and diverticulums were found in 10 patients with normal transit time and 7 patients with abnormal transit time. Therefore barium enema and colonoscopic examination in the patients with abnormal transit time were meaningful (P=0.024). Eighteen out of 25 patients with normal transit time and 8 (67%) of 12 patients with abnormal transit time showed abnormal defecogram results. The rectocele was the most frequent cause of abnormality in defecogram. Abnormal anal manometry results were obtained in 5 (22%) of 23 patients with normal transit time and 3 (50%) of 6 patients with abnormal transit time. Three types were classified in the patients with abnormal transit time. With type II patients, anal manometry findings were normal and 2 patients showed abnormal results in defecogram. With type III patients, 5 (43%) of 12 patients showed abnormal results in defecogram and 3 (50%) of 6 patients had abnormal anal manometry findings. Conclusively, if abnormal transit time is found in the patients with chronic consipation, further evaluations such as barium enema or colonoscopic examination are necessary. And even in the patients without any abnormality in transit time, selected performance of defecogram and anal manometry depending on clinical symptoms are preferable.

7.A Case of Human Infestation of Canine Scabies.

We present herein a case of human infestation of canine scabies occuring in a 7l-year-old female patient. Severe pruritic, erythematous papular lesions and excoriation were found over the abdomen, submammary area, and thigh. The itching sensation was aggravated especially at night. Efforts to demonstrate mites from the lesiona of the patient were unsuccessful. Her pet, a small puppy, showed loss of hair and crusted or scaly erythematous patches on the auricles and trunk 1 month prior to her skin lesions. Sarcaptescabiei var. canis were collected from the puppy by brushing the hair snd crusted or scaly patches on its skin. The patient and puppy were treated successfully with application of 25% benzyl benzoate solution.

8.Total Serum IgE Level in Each Age Group of Patients with Atopic Dermatitis.

We evaluated median value and lower confidence limit of total serum IgE level in each age group of 1,493 pure atopic dermatitis patient without respiratory atopic diseases. The results were as follows . 1. The median value of total serum IgE level of each age group in patients with atopic dermatitis distributed from 56.5 IU/@Ll to 660It.J/C. The distributions of the median value of total serum IgE levels of total patients with atopic dermatitis, total male patients with atopic dermatitis, and total female patients with atopic dermatitis were 185 IU/C, 220 IUj'@C, and 165 IUj'C. There was no statistical significance of median value of total serum IgE level between mole patients and female patients. 2. The 95% and 99% lower confidence limit(one-sided) of total serum IgE level of ea.ch age group in patients with atopic dermatitis distributed from 32.6 IU /n4 to 287IU/v4 and from 31 IU/m0 to 202 IU4. The 95% lower confidence limit (one-sided) of total serum IgE level of total patients with atopic dermatitis, tatal male patients with atopic dermatitis, and. total female patients with atopic dermatitis were 170 II.J/m0, 185 IU,m/l, and 133 IUm/l. The 99% lower confidence limit.(one-sided) of total serum IgE level of total patient with atopic dermatitis, total male patient with atopic dermatitis, and total female patients with atopic dermatitis were 165 IU/ml, 180 IU/ml, and 125 IU/ml.

9.The Evaluation of the Difference between the Calculated Estimated Post-operative Refraction and the Real Post-operative Refraction for Five Types Intraocular Lenses.

PURPOSE: To evaluate and compare the difference of predictive post-operative refraction preoperatively and real post-operative refraction among five types of intraocular lens. METHODS: We reviewed retrospectively 567 cataractous eyes that had undergone phacoemulsification or ECCE with posterior chamber intraocular lens (IOL) implantation by the same surgeon. Applied IOLs were AMO(R) Phacoflex(R) II SI40NB, Acrysof(R) MA60BM, Sensar(TM) AR40e, CeeOn(TM) 811B and CeeOn(TM) 720A. Prediction of post-operative refraction (predictive refraction) was calculated by the SRK/T formula with manufactured A constant. Post-operative manifest refraction (real refraction) was done at least 2 months postoperatively. We compared the difference between the predictive refraction and the real refraction by paired t-test. RESULTS: Total studied eyes were 390 eyes. In all groups, more myopic shift were observed than predicted. In AMO(R) Phacoflex(R) II SI40NB group, statistically significant difference was seen in postoperative manifest refraction over the predictive refraction as much as mean 0.46 diopter myopically (p<0.05, paired t-test), but others were not. In the AR40e implanted group, the error of predictive refraction was the smallest among groups. CONCLUSIONS: Prediction of postoperative refractive state was influenced by various clinical factors. Using a revised A constant, predictive error would be decreased. We recommend that every cataract surgeon had better have one's original A constant over each IOL and A constant may be revised when major surgical or biomedical measurement settings were changed.

10.Measurement of Transendothelial Potential Difference to Evaluate the Chondroitin Sulfate Effect in TC-l99 Cornea Preservation Media.

Chondroitin sulfate (CS) in corneal preservation medium can porolong corneal preservation time. The solutions used in the experimental chambers were made of TC-199 (GIB Co.) either by itself of by adding 1% CS (Sigma) to the TC-199. The evaliation of the viability of corneas can be made by monitoring their physiological parameter. Thus, we monitored the transendothelial electrical potential difference (p.d.) across deepithelialized rabbit corneas. We found that TC-199 containing 1% CS maintained higher p.d.'s than the same solution without CS.
