1.A Clinical Analysis of Collateral Ligament Injuries to Fingers

There are many controversies concerning diagnostic and therapeutic guidlines for injuries to the collateral ligaments of the fingers, which improperly treated, may develop serious late sequale of joint function. Eleven cases of collateral ligament injuries to fingers which occured between June 1976 and June 1986 were reviewed and the following results were obtained. 1. There were 8 males and 3 females, and eight of the 11 cases (73%) were younger than 30 years of age. 2. In 5 cases, the injury was a sports related injury, while slipped down injury caused the injury in 5 other cases. 3. The ratio of acute to chronic injury was 7:4. 4. There were 6 cases of proximal interphalangeal joint of the fifth finger, 3 cases of metacarpophalangeal joint of the first finger and 2 cases of proximal interphalangeal joint of the third finger. 5. The ratio of ulnar collateral ligament to radial collateral ligament was 7:4. 6. Preoperatively, stress X-rays of the both the involved and the contralateral sound joints taken in all cases and analysed. 7. The most common ruptured site of ligament was substance portion, which was found in 7 cases. 8. Primary repair were performed in 8 cases, reconstruction with tendon of palmaris longus muscle in 2 cases and reconstruction with tendon of flexor digitorum superficialis in 1 case. 9. All patients were followed for at least 6 months, and the average was 3 years. 10. Normal range of motion and stable joints were observed in 10 of the 11 cases at the final follow up.

2.Variations of the Straight Sinus in Korean Adults.

The complexily in development of the dural sinus may result in many variations. Studies are being done on the variation of the straight sinus for clinical application of neurological diagnosis and operation. However, no data is available on Koreans, which made this study necessary. The present study was made on 50 Korean adult cadavers (average age of 67). The results were as followlng. 1. A bulbous formation of the rostral aspects of the straight sinus was present in 22 cases(44%). 2. Complete double straight sinus was present in 3 cases(6%). 3. Partial septum was observed in 8 cases (16%) with varied position and length. 4. The straight sinus terminated into both transverse sinuses(52%), the left transverse sinus(24%), the confluence of sinuses (18%), and the right transverse sinus(6%). 5. Average length and width of the straight sinus were 48.5mm and 5.9mm, respectively. 6. Nineteen cases(38%) showed no tentorial sinus drainage into the straight sinus on both sides. 3.Vascularized Bone Graft for Spine and Limb Reconstruction

We analysed the thirty three cases of the free vascularized bone graft performed in the Department of Orthopaedic Surgery, Yonsei University College of Medicine from February 1982 to December 1989 and would like to report the results as follows. 1. There were 27 cases in men and 6 cases in women and the mean age was 27.3 years. 2. We have performed the procedure for the reconstruction of the large bony defects developed after massive trauma in 13 cases, core decompression of the avascular necrosis of the femoral head in 8 cases, tumoral resection in 6 cases, treatment of congenital pseudoarthrosis in 3 cases, and bone infection in 3 cases. 3. The most common recipient site was tibial shaft (18 cases, 54%) and the most common donor site was fibular shaft (23 cases, 69%). 4. The average length of bone defect (or grafted bone) in 23 cases of vascularized fibular graft was 9.8 cm and in 9 cases of vascularized osteocutaneous iliac bone graft, it was 5.8 cm. 5. The follow-up period was ranged from 13 months to 5 years 8 months (mean, 2 years 2 months). 6. All patients were evaluated clinically and roentgenographycally on the basis of fuctional recovery and bony union. Of the 33 cases, 31 cases (93%) showed satisfactory functional recovery with sound bony union. 7. It is highly expected that the vascularized bone graft is a one of the better alternatives for the reconstruction of the large bone defect and prevention of the early state of the avascular necrosis of the femoral head from progession.

4.Foot Tumor

Benign tumors and tumor like lesions in the foot are uncommon and those that are malignant are rare. Metastatic cancer of bone distal to the knee and elbow is unusual. By contrast, non-tumorous lisions that cause swelling, pretuberance, or deformity are common. Fifty two cases of tumors on foot were treated at our department from Jan. 1970 to Dec. 1991, we have treated fifty two cases of foot tumor and analysed their clinical characteristics. Bone tumors were more frequent than soft tissue tumors (2:1). Among soft tissue tumors benign tumors were frequent than malignant(1.4:1), and fibroma was the most frequent tumor(30%). Benign soft tissue tumors occured frequently at Edward and Michael's Zone 3(57%). Benign bone tumors has higher incidence than malignant(20:1). Osteochondroma was the most frequent tumor (48%). Predilection sites for osteochondroma and enchondroma of foot was distal phalanges and mitatarsal bone, respectively.

5.A Clinical Study of Dupuytren's Contracture

Dupuytren's contracture is caused by a proliferative fibroplasia of the subcutaneous palmar tissue. It is most common in those of Scandinavian and Celtic origin, although it has rarely been reported in Orientals. Many types of surgery are advocated, all with apparent success. Then operative result of 23 cases of Dupuytren's contracture including seven bilateral involvment was analyzed partial fasciectomy was performed in all cases by one surgeon. Average follow-up period was 18 months. We analyzed the results of the operation, to Honner s classification. Excellent result was obtained in 6 cases(26%), good in 14(61%) fair in 3(13%), and poor in 0(0%). The complication were 8(34%) case as followshematoma 4(17%), stiffness 3(13%), skin necrnsis 1(4%). All patrents were not performed that skin graft and total fasciectomy. The partial fasciectomy is suggested as one of the successful method for the treatment of Dupuytren's contracture.

6.Comparison of Mycobactericidal Activity of 12 Kinds of Disinfectants for Mycobacterium chelonae.

BACKGROUND: Cleaning and disinfection of fiberoptic bronchoscope requires careful attention, especially to mycobacterium species because the contamination of mycobacteria could raise confusion on diagnosis. Recently, we detected contamination of Wydex(R) solution used in bronchoscope washer with Mycobacterium chelonae. In this study, we evaluated the mycobactericidal effect of 12 kinds of disinfectants for M. chelonae. METHOD: To evaluate the bactericidal effect of Wydex(R) 2%, Cidex(R) 2.25%, Cidex(R) 3%, Bacteriokiller (BK) disinfectant, Perasafe(R), HICLO-S(R), Lamicine(R), ethanol, Instrusept(R), Virkon(R), Betadine(R), and Vipon(R) against M. chelonae, culture was performed after exposure of two M. chelonae strains (ATCC 35749, the type strain and the strain isolated from contaminated Wydex(R) solution) to each disinfectant solution. The growth of organism was examined for up to 8 weeks. RESULTS: Growth of M. chelonae (reference strain of ATCC 35749 and isolated strain) was observed after a week incubation for Wydex(R) 2%, Cidex(R) (2.25%, 3%) and control. For BK disinfectant and Perasafe(R), they grew after 2-3 weeks, and 3-4 weeks, respectively. For HICLO-S(R) and Lamicine(R), only the contaminated strain grew after two and three weeks, respectively. For ethanol, Virkon(R), Betadine(R), Vipon(R), and Instrusept(R) , growth was not observed from either strain. CONCLUSIONS: On the basis of these results, Instrusept(R), virkon(R), ethanol, Betadine(R), and Vipon(R) were effective for the disinfection of M. chelonae. Especially, Instrusept(R) was thought to be useful as a disinfectant for bronchoscopes because it has advantages including non-corrosiveness, chemical stability, and non-irritativeness. And additional washing with ethanol might be effective. The finding that strain isolated from contaminated bronchoscopes was more resistant to disinfectants than reference strain suggested that the more resistant strains are selected throughout the improper disinfection.

7.Outbreak of Nosocomial Infections caused by Acinetobacter baumannii resistant to imipenem and Cefoperazone/Sulbactam.

BACKGROUND: With increase of antibiotics use and invasive procedures, infections caused by multi-resistant Acinetobacter baumannii (MRAB) are increasing. Recently, we experienced the outbreak of- nosocomial infections caused by MRAB resistant to imipenem and cefoperazone/sulbactam in intensive care units (ICU) and general ward. We analyzed the clinical characteristics of the infected patients and antibiotic susceptibility of the organisms. And surveillance cultures and IRS-PCR were performed to find out the transmission route. METHODS: We collected data from physical examination and clinical records. We performed surveillance cultures of environment, patients not infected with MRAB in ICU, and hands of health care workers. RESULTS: Between November 1996 and June 1997, 49 strains of MRAB were isolated from the 26 patients hospitalized in Kangnam St. Mary' s Hospital. The lower respiratory infection (13 cases) was the most common infection and sputum was the most common sources (47.1%). All strains of MRAB showed the same genotype. In disk diffusion test, all strains were resistant to piperacillin, gentamicin, amikacin, ceftazidime, cefoperazone/sulbactam, aztreonam, imipencm, ciprofloxacin. From the surveillance cultures, the genotypically identical strains were isolated from ventilator Y-piece, the floor of ICU, and hands of health care workers. It suggested that this strain was transmitted through ventilatory device or hands of health care workers. We instructed all the health care workers to wash hands, to disinfect hospital environment completely. Since July 1997, no further case has occurred. CONCLUSIONS: Since A. baumannii could be transmitted through ventilatory devices and the hands of health care workers, it is important to wash hands and to disinfect hospital environment completely.

8.Triscaphe Fusion in Kienbock's Disease

There are many controversies concerning therapeutic guidelines for the treatment of Kienbock's disease. We experienced 17 cases of stage II or III Kienbock's disease(Lichtman's classification), which were treated with triscaphe fusion from March 1983 to March 1992. The mean Follow-up peri- od was 25 months. The purpose of this study is to evaluate the clinical and radiological result of triscaphe fusion of 17 cases of Kienbock's disease. 1. The pain was relieved in all cases, but range of motion was not improved after operation. 2. The postoperative results of triscaphe fusion were evaluated by Licthman's method. 9 cases (53%) were rated as satisfactory and 8 cases as unsatisfactory. 75%(3 cases of 4) were rated sat isfactory in IIIA a group and 22%(2 casaes of 9) were rated satisfactory in IIIB group. 3. The psudoarthrosis was noted in 2 cases of 17.

9.The Comparison of the Effect of Enflurane and Propofol on Arterial Oxygenation during One-Lung Ventilation.

BACKGROUND: Controversy exists as to whether or not inhalation anesthetics and intravenous anesthetics impair arterial oxygenation (PaO2) during one lung ventilation (OLV). Accordingly, we examined the effect of enflurane and propofol on PaO2 and pulmonary vascular resistance (PVR) during OLV. METHODS: Forty patients, who had prolonged periods of OLV anesthesia with minimal trauma to the nonventilated lung were studied in a cross over design. Patients were randomized to four groups; Group 1 received 1 MAC of enflurane and oxygen from induction until the first 20 min after complete lung collapse, then were switched to propofol 100 g/kg/min (P100). In group 2, the order of the anesthetics was reversed. Group 3, Group 4 received the same order of the anesthetics as Group 1, Group 2, respectively but received propofol 200 g/kg/min (P200). RESULTS: During OLV, the PaO2 values were lower than those with two lung ventilation (TLV), there were no significant differences among each groups and between propofol and enflurane in PaO2, but in the selected patients (n=10, PaO2<120 mmHg during OLV), PaO2 in propofol group was higher than that of enflurane group (p<0.05). Conversion from TLV to OLV caused a significant increase in PVR, but there were no difference in PVR between propofol and enflurane group. CONCLUSIONS: These results suggest that the usual clinical dose of propofol affords no advantage over 1 MAC of enflurane anesthesia except low PaO2 patients during OLV. Propofol might be of value in risk patients of hypoxemia during thoracic surgery when OLV is planned.
