1.Observation on clinical effects of tuina plus montelukast sodium tablets for children with cough variant asthma

Objective:To observe the clinical effects of tuina plus oral administration of montelukast sodium tablets for children with cough variant asthma (CVA). Methods:A hundred and six children with CVA were randomly divided into a treatment group and a control group, 53 cases in each group. The treatment group was treated with Tuina therapy plus oral administration of montelukast sodium tablets, and the control group was treated only by oral administration of montelukast sodium tablets. Tuina treatment was given three times per week. 5 mg of montelukast sodium tablets was given orally before bedtime every night. The therapeutic effects were assessed after the treatment of four weeks in both groups. Results:The total effective rates were 92.5% in the treatment group and 77.4% in the control group. The difference in the total effective rate between the two groups was statistically significant (P<0.05). Conclusion:The therapeutic effect of tuina plus montelukast sodium tablets for children with CVA is better than that of oral administration of montelukast sodium tablets alone.
2.Neuroprotective Effects of Amiloride

Amiloride is an epithelial Na+ channel(EnaC)blocker.As a potassium-sparing diuretic,it has been used in clinical practice for decades of years.Studies have shown that many ion channels were semitive to amiloride in the central nervous system,such as acid-sensitive ion channel(ASIC)and Na+/H+ exchanger.These channels have important physiological functions,and paticipate in pathological processes such as cerebral ischemia and tissue acidosis.It has demonstrated that amailoride reduces the effects of ischemia-and acid-mediated neuronal injury by blocking these channels,which may become a novel neuroprotective agent for the treatment of cerebral ischemia.
3.Anatomic observation of the atlanto-axial joint in functional positions by CT three-dimensional imaging

BACKGROUND: Clinical diagnosis criteria for rotary subluxation of atlanto-axial joint have not been identified. Many clinical diagnoses are suspicious. Therefore, investigating a new diagnostic method and diagnostic evidence is beneficial to enhancing diagnosis accuracy, and is a study hotspot in the current field.OBJECTIVE: This study aimed to observe the relationship between anatomy of normal human atlanto-axial joint and functional positions, and to provide anatomical basis for the diagnosis of atlanto-axial rotary subluxation.DESIGN: Single sample self-control study.SETTING: Department of Imageology, Zhongshan Hospital Affiliated to Xiamen University.PARTICIPANTS: This study was carried out in the CT Room, Department of Imageology, Zhongshan Hospital Affiliated to Xiamen University between June 2003 and November 2004. Fifty-three volunteers, 27 males and 26 females, with an average age of 31.2 years (range 4.5-75 years), without cervical trauma or history of cervical diseases, were randomly recruited in to this study. Written informed consents were obtained from each subject or their guardians.METHODS:CT helical scanning and three-dimensional reconstruction imaging of atlanto-axial joint in the neutral position were performed in all the volunteers and were supplemented in the right and left rotary position in 21 subjects on a helical CT scanner (somatom plus 4, Siemens, Germany), SUN magic view 1000 workstation and three-dimensional reconstruction software.MAIN OUTCOME MEASURES: ① Lateral atlanto-axial articular facet contraposition. ② Lateral atlanto dental space③ Atlanto dental interval. ④ The largest extent of articular facets dislocation in the right and left rotary positions. ⑤ Head's rotary maximal angle in the right and left rotary positions.RESULTS: ① Three-dimensional images with atlanto-axial joint in the functional position of 53 patients showed that the articular surfaces of lateral atlanto-axial joint had basic contraposition, the width of atlanto-dental interval ranged from 1.0 to 3.3 mm, and the lateral atlanto-dental distance devirated in 24 patients (45.28%).②Three-dimensional images with atlanto-axial joint in the rotary position of 21 patients showed that the width of atlanto dental interval had no change, the articular facets of lateral atlanto-axial joint had rotary displacement with the width ranging from 5.76 to 8.18 mm, and anastomotic area of articular facet was decreased by 1/2 to 2/3. The condyles had deflected to the rotary direction in the patients with symmetrical lateral atlanto dental space, and for 15 patients with asymmetrical lateral atlanto dental space, the condyle deflected slightly or had no changes.CONCLUSION: CT-three dimensional imaging can clearly show the anatomical structure of atlanto-axial joint in functional position and the changing rule of atlanto-axial joint movement, providing theoretical evidence for the clinical diagnosis and the treatment of atlanto-axial subluxation.
4.Study on the relationship between children's functional gastrointestinal disease and food allergy

Objective To study the relationship between the functional gastrointestinal disease of children and food allergies,and recent prognosis.Methods 108 children with functional gastrointestinal disease were selected as the observation group,at the same time 36 healthy cases were selected as control group.ELISA was used to detect IgG antibody concentration and total IgE level in ten kinds of allergy food.The different methods were used to guide the patients' diet,the improved quality were oberseved in the two groups after 3 ~6 months.Results In the observation group 108 cases of children with functional gastrointestinal disease,1 ~ 10 kinds of inequality of food allergen specific IgG children have increased in 90 cases,accounting for 83.3%,whereas the control group had 9 cases,only accounting for 25.0% (x2 =20.87,P < 0.05).In the observation group,the foods with highest sensitivity were milk,egg white/yellow,respectively for 84 patients (77.8%) and 60 cases (55.6%).Serum total IgE level between the observation group and control group had no significant difference (P > 0.05).17 cases (15.7%) within one month,the symptoms were improved,13 cases (12.1%) in three months,the symptoms were improved,the symptoms of 21 cases (19.4%) were not improved after 3 months.The total effective rate was 80.6%.Conclusions Children with functional gastrointestinal decease is easily to occur allergy caused by different food,and IgG antibody level can correctly reflect the relationship between them,reasonable diet can improve the symptoms of functional gastrointestinal disease.

6.Treatment of hyperplastic nonunion of the tibia with interlocking intramedullary nailing,fascia pedicled callus flap and bone grafting

[Objective]To observe the clinical effect of the treatment of hyperplastic nonunion of the tibia with interlocking intramedullary nailing,fascia pedicled callus flap and bone grafting.[Method]Twelve cases of nonunion of the tibia were treated with intramedullary interlocking nailing,fascia pedicled callus flap and bone grafting,and there were 8 males and 4 females.The patients age ranged from 26 to 64 years,with an average of 45 years.[Result]All cases were followed-up for an average of 48 months and achieved a solid union within 4~6 months.[Conclusion]Intramedullary interlocking nailing,fascia pedicled callus flap and bone grafting have the design of the reasonable biomechanics,the function of untispinning and unticurtailing,less partial blood decompose.The fascia pedicled callus flap and iliac bone grafting could result in solid union and it is a recomment method for promoting bone-healing up after grafting and forming.
7.Effects of the temperature of irrigation fluid on body temperature and cardiovascular performance during transurethral surgery

Objective To evaluate the effects of temperature of the irrigation fluid on body temperature and cardiovascular performance during transurethral surgery. Methods 87 cases were randomly divided into two groups according to the temperature of irrigation fluid used: the ambient (21~ 24 ℃) and the isothermic (35~36 ℃). Body temperature, blood glucose, ECG, HR, SaO 2, cardiac output (CO), mean arterial pressure (MAP) and systemic vascular resistance (SVR) were monitored during the transurethral surgery. Results Body temperature was decreased 2.1 ℃, MAP elevated, CO decreased and SVR increased in the ambient group, while body temperature was decreased only 0.3 ℃ and cardiac performance was maintained relatively stable in the isothermic group. Conclusions Hypothermia, resulted from the use of large amount of low temperature irrigation fluid during transurethral surgery has detrimental effects on body temperature and cardiovascular performance. Isothermic irrigation fluid would stablize the cardiovascular performance and so increases the safety of transurethral surgery.
8.Clinical analysis of tsutsugamushi disease misdiagnosed as tonsillitis.

Tsutsugamushi disease is an acute infectious rickettsial disease caused by the intracellular parasite Orientia tsutsugamushi. Due to its variety of clinical signs, this disease is often misdiagnosed. This article examines a total of 4 patients who visited our clinics with fever and sore throat. 3 of them had body temperature of 39.5 Celsius degrees when admitted. The characteristic black eschar occurred on 4 of them. Lymphadenopathy occurred on 2 of them. Cough occurred on 1 of them. Lab tests showed that 3 of them had Leukocytosis, 1 of them had increased bronchovascular markings, and 3 of them had Weil-Felix test positive. After admission, all patients, who were confirmed of diagnosis of tsutsugamushi disease instead of tonsillitis, received the comprehensive treatment and cured afterwards.

9.A Clinical Analysis of Collateral Ligament Injuries to Fingers

There are many controversies concerning diagnostic and therapeutic guidlines for injuries to the collateral ligaments of the fingers, which improperly treated, may develop serious late sequale of joint function. Eleven cases of collateral ligament injuries to fingers which occured between June 1976 and June 1986 were reviewed and the following results were obtained. 1. There were 8 males and 3 females, and eight of the 11 cases (73%) were younger than 30 years of age. 2. In 5 cases, the injury was a sports related injury, while slipped down injury caused the injury in 5 other cases. 3. The ratio of acute to chronic injury was 7:4. 4. There were 6 cases of proximal interphalangeal joint of the fifth finger, 3 cases of metacarpophalangeal joint of the first finger and 2 cases of proximal interphalangeal joint of the third finger. 5. The ratio of ulnar collateral ligament to radial collateral ligament was 7:4. 6. Preoperatively, stress X-rays of the both the involved and the contralateral sound joints taken in all cases and analysed. 7. The most common ruptured site of ligament was substance portion, which was found in 7 cases. 8. Primary repair were performed in 8 cases, reconstruction with tendon of palmaris longus muscle in 2 cases and reconstruction with tendon of flexor digitorum superficialis in 1 case. 9. All patients were followed for at least 6 months, and the average was 3 years. 10. Normal range of motion and stable joints were observed in 10 of the 11 cases at the final follow up.

10.Clinical observation on moxibustion at Baihui (GV20) plus Tuina for children with postnasal drip syndrome

Objective: To observe the clinical efficacy of moxibustion at Baihui (GV20) plus Tuina (Chinese therapeutic massage) for children with postnasal drip syndrome (PNDS).Methods: A total of 60 cases were divided into an observation group and a control group according to the random number table method, with 30 cases in each group. Cases in the control group received mometasone furoate nasal spray and nasal irrigation with 0.9% normal saline. Cases in the observation group received additional moxibustion at Baihui (GV20) plus Tuina at the head and face. All the cases were treated for four consecutive weeks. The clinical efficacy and traditional Chinese medicine (TCM) symptoms score were compared between the two groups. Results: The total effective rate of the observation group was 93.3% while that of the control group was 70.0%. The difference between the two groups was statistically significant (P<0.05). After treatment, the TCM symptom scores in both groups declined significantly, showing a statistical difference from that before treatment (P<0.05), and the score of the observation group was notably lower than that of the control group (P<0.05). Conclusion: The clinical efficacy of moxibustion at Baihui (GV20) plus Tuina at the head and face on the basis of nasal spray with Western medicine is more significant than that of Western medicine alone for PNDS in children.
