1.Crural Ulcer After Accidental Application of 5 - Fluoroutacil Cream.

Topical fluorouracil therapy is an advanced treatment for actinic keratoses, Bowen's disease, multiple basal cell epithelioma and other precancerous skin lesions. Because of extensive and frequent use of 5-fluorouracil(5 F-U) cream, some untoward effects, mostly allergic contact dermatitis and irritation dermatitis, can be anticipated. We have experienced a case of crural ulcer after accidental application of 5 F-U cream for treating the tinea cruris at drug store. The patient was 21 year oid man who had suffered from erythema, erosion and shallow ulceration with oozing, serosanguinous discharge, buming and stinging sensation on the both groins and its adjacent area of the scrotum. The skin lesion was not well responded to topical corticosteroid but controlled by wet dressing and zinc oxide lotion within 10 days of hospitalization. At the 20th hospital day, the skin was healed with remained follicular hyperpigmentation. A few days later, the tinea cruris was confirmed by direct microsopic examination and culture. Generally, topical 5 F-U spares normal cell, while selectively destroying the more rapidly proliferating celI, but there are some possible factors to induce destruction of non-proliferating cell in the highly penetrating area The skin penetration can be different depending upon the anatomical location.

2.Enchondroma of the Hand

Chondroma is a benign tumor composed of mature hyaline cartilage. Most chondroms are centrally located in bone, and such tumors are called "enchondromas", and comprise 90% of the tumors arising in the skeleton of the hand. Twenty nine patients were admitted to the Department of Orthopedic Surgery, Yonsei University College of Medicine with enchondroma of the hand during a period of 10 years from January 1979 to December 1988. The analysis of results of these patients are as follows. l. Among 29 patients 10 were male, 19 patients were female. 2. The mean age was 26.3 years, range 3 to 61 years. 3. Enchondromal occured most frequently in patient between 10 and 30 years of age. 4. The proximal phalanx was the most commonly involved site by the middle phalanx and metacarpal 5. The ulna three fingers were more often afflicted than the radial two. 6. Among 29 patients, 7 were treated by excision, 18 by curettage and autobone graft, 4 pathologic fractures by curettage, autobone graft after open reduction and internal fixation with Kirschner wire. 7. There were no recurrence, except 1 suspicious recurrence for 8 month later.

3.In Vitro Pharmacokinetics of Vancomycin Release from Locally Implantable Materials

Local deposition of antibiotics has become increasing popular in the management of open fractures or osteomyelitis, and several substances have been employed as the vehicle for delivery. Although the elution characteristics of some substances have been documented, a comparative study of the characteristics of the commonly used substances were performed in order to establish the clinical indications for particular vehicles. Carriers were prepared, which were human iliac cancellous bone, bovine cancellous bone matrix(Lubboc?), absorbable gelatin sponge(Gelfoam?), fibrin glue(Beriplast? P) and polymethylmethacrylate(CMW?) for elution characteristics of vancomycin. The each carriers were immersed on the 20 ml of PBS and then obtained the samples for analysis of concentration of vancomycin at first, second, third, fourth, fifth, sixth, fourteenth, twenty-first, twenty-eighth day after immersion. The assay technique was fluorescent polarization immunoassay(Abbott, Dallas, Texas, U.S.A.). Nearly 50% of vancomycin was released from human iliac cancellous bone, Lubboc? and fibrin glue during the first 3 days, and negligible after first week. Gelatin sponge produced high local concentration of vancomycin during the first week. PMMA eluted the only 6% of vancomycin during the first day and trace amount detected as long as 4 weeks. There are significant statistical difference among carriers at second and fourth week(ANOVA test, P < 0.05). The authors considered that human iliac cancellous bone, Lubboc? ,Gelfoam? and fibrin glue may be best employed when brief antibiotic coverage is required, whereas PMMA may be better suited for long-term coverage.

6.Reconstruction of Anterior Cruciate Ligament with Iliotibial Band Transfer

Ligamentous injury of the knee which could be easily occur in sports or traffic accidents resulted in more and severe disability than that of other joints. Rupture of the anterior cruciate ligament which restrain hyperextension, excessive rotation and anterior motion of tibia on femur is the most frequent and significant injury of the knee. Its treatment and clinical significance is one of the most controversial issue and its incidence tends to increase. From 1983 to 1985, modified Scott operation were performed on 10 patients at the department of orthopedic surgery, Chung Ang university hospital. Following results are obtained. 1. No limitation of range of motion of the knee was developed. 2. Intra-articular fixation by use of pull-out wire was done and special consideration was done for the site of fixation on tibia plateau and method of dilatation of posterior capsule. 3. Of the ten patients, nine had negative anterior drawer test, postoperatively.

7.Psychiatric Evaluations of Chronic Urticaria and Alopecia Areata by Cornell Medical Index.

This st;udy was to obtain psychosomatic factors influencing cause of chronic ureicaria and alopecia areata with comparison of dermatophytosis. This survey was carried out by Cornell Medical Index from March 1975 to- September 1976 on total 93 persons which constitute 22 chronic urticaria,38 alopecia areata and 33 dermatophytosis at; the out patient clinic of dermatology department of St. Mary hospital Findings were as follow: 1. The average numbr of complaints of over-all section was 47.36+26.89, the somatic section (31. 73+16. l4) and the psychic (l5. 64+-12. 64) in the chranic urticaria. The average number of complaints of over-all section was R8.50+20.89, the somatic section (22. 63+-13. 10) and psychic section (15. 87+-9. 90) in the alopecia areata, For the control, over-all section was 25. 18+18. 87, the somatic section (16. 30+l2. 38) and the psychic(8. 88+8. 96) in the dermatophytosis. Consquently, there was an increased number of complaints in the group of chronic urticaria and alopecia areata than the control group. 2. There was increased complaint of both the somatic and psychic section in the chronic urticaria. 3. There was increased complaint of psychic section than the somatic section in the alopecia areata.
