1.Acromion Fracture after Hook Plate Fixation for Distal Clavicle Injury: A Report of 2 Cases.

Fractures at the lateral end of the clavicle inevitably require surgical treatment as there is high potential for delayed union or nonunion. Acromioclavicular dislocation also requires stable and solid fixation for healing, and surgical treatment is recommended for the maintenance of joint function. The hook plate maintains the biomechanics of the acromioclavicular joint, enabling early range of motion. Therefore, for the past 10 years, the hook plate has been widely used in distal clavicle fractures and acromioclavicular joint injuries. However, the hook plate is associated with several complications, such as proximal clavicle fractures, widening of the hook hole, rotator cuff tear, subacromial impingement, and often acromial fractures. We report on two unusual cases of acromion fracture after hook plate fixation in patients with distal clavicle fracture and acromioclavicular dislocation alongside a literature review.

6.Presence of E-cadherin in Cultured Amnionic Cells.

OBJECTIVES: Cell adhesion molecules may play a role in integrating amnionic membrane. The objective of this study was to prove E-cadherin mRNA and proteins in cultured human amnionic cells. METHODS: We cultured amnionic cells from 4 women undergoing cesarean section without labor. E-cadherin was determined by immunohistochemistry and Western ligend blotting. To demonstrate E-cadherin mRNA, reverse transcription-polymerase chain reaction was performed. RESULTS: On immunohistochemistry, E-cadherin was abundantly showed on the cytoplasm of the cells. Western ligend blotting showed clear 120 kDa bands on four specimens, and relatively weak band on one specimen. Reverse transcription-polymerase chain reaction showed 432 BP bands. CONCLUSION: We proved E-cadherin and its mRNA by immunohistochemistry, Western ligend blotting and reverse transcription-polymerase chain reaction in cultured human amnionic cells.

9.Thumb Reconstruction with a Free Neurovascular Wrap

On the length, strength, free lateral motion, and perfect mobility of the thumb, depends the power of the human hand. The thumb is called pollex because of its strength and that strength is necessary to the power of the hand being equal to that of all the fingers."So stated Sir Chales Bell in the fourth Bridgewater Treatise, first published in 1833. Morrison and O'Brien advocated reconstruction of the thumb with a free wrap around flap from the big toe to recreate a stable, sensate and functional digit including the nail in 1980. From March, 1982 to July, 1988, Twelve thumb reconstructions were performed using the wrap around procedure at Hanyang University Hospital;12 successful thumb reconstructions were reviewed at an average of 16 months after surgery. There were several complications;these included skin necrosis(4), malunion(1), resorption of the bone graft(7). We considered that cosmesis and function were good;pinch grip averaged 68% of normal, sensibility returned in all patients as assessed by two-point discrimination(average 9.5mm). Over all the results are as follows. 1. The wrap around technic provides a good method of reconstruction. 2. The wrap around technic is one stage operation and can be used in the level of metacarpal amputation. 3. The surgeon should be familiar with microsurgical technic.

10.A FEM analysis for initial stress on the upper canine by orthodontic force of intrusion arch wire activation.

The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA point.
