1.Medial Displacement and Valgus Nailing with Jewett Nail in Unstable Intertrochanteric Fracture

Intertrochanteric fractures frequently occur in elderly patients. Early mobilization after rigid internal fixation of unstable intertrochanteric fractures has recently reduced the mortality and morbidity. Between March, 1976 and February, 1980, eighteen patients over 60 years old with unstable intertrochanteric fractures were treated by Jewett nailing after Dimon & Hughstons reduction at the department of Orthopaedic Surgery, Chung Nam University. Among the eighteen patients, twelve patients could be followed, ranging from 6 months to 2.1 years, with an average follow-up of 11 months. The results were obtained as follows: 1. The main causes of fractures were falling down and slip down. 2. In treatment of unstable intertrochanteric fractures by medial displacement and valgus nailing with Jewett nail, early ambulation and early weight bearing were possible with satisfactory results. 3. The average time for fracture union in twelve cases who were followed up were 17.1 weeks, but two cases with severe comminution of posterior and medial fragment of the trochanter revealed delayed union. 4. The medial displacement of distal fragment and valgus nailing in unstable intertrochanteric fracture shortened the operation time and reduced complications by early ambulation, but had disadvantages such as some limitation of motion of affected hip joints, shortening of affected extremities and delayed union.

2.A photoelastic study on the initial stress distribution of the upper anterior teeth retraction using combination loop archwire and sliding mechanics.

An unfavorable tipping movement can occur during the retraction of anterior teeth because orthodontic force is loaded by brackets positioned far from the center of resistance. To avoid this unfavorable movement, a compensating curved wire or lingual root torque wire is used. The purpose of this study is to investigate, using photoelastic material, the distribution of initial stress associated with the retraction of the incisors according to the degree of the compensating curve, to model changes associated with tooth and alveolar bone structure. The following results were obtained by analysis of the polarizing plate of the effects of initial stress resulting from retraction of the anterior teeth: 1. When the incisors were retracted using combination archwire or sliding mechanics, the maximal polarizing pattern of the apical area decreased as the degree of the compensating curve increased from 0 to 15 to 30. 2. When the incisiors were retracted by the combination archwire or sliding mechanics, the maximal polarizing pattern of t he canine and premolar area increased as the degree of the compensating curve increased from 0to 15to 30. 3. A lower degree of polarizing patterns were associated with the combination archwire technique than the sliding mechanics technique at a given force. The above results indicate that there is no significant difference between the combination loop archwire technique and sliding mechanics, for the retraction of maxillary anterior teeth with decreased lingual tipping tendency by a compensating curve on the arch wire. However, the use of sliding mechanics is more effective for the prevention of lingual inclination of the anterior teeth, because the hook used in sliding mechanics is closer to the center of resistance of the maxillary anterior teeth.

3.Spontaneous Regression of Primary Malignant Lymphoma of the Esophagus.

Gastrointestinal involvement has been reported to occur in about 5 to l0% of patients with lymphoma. The most common gastrointestinal site for a lymphoma is the stomach, followed by the small intestine and ileocecal region. When lymphoma is found in the esap us, generally it is with contiguous involvement of the gastric fundus, the cervical node, secondary to mediastinal lymph node compression, or a late manifestation of advanced desease. Such cases cannot be labeled as primary esophageal lymphoma. A search of the literature reveals that there have been about 20 or more published cases classified as esophageal lymphoma. However, more than ten cases are indistingishable from the case of distal esophageal involvement by gastric lymphoma. In 1935, Chiolero described a case of midesophageal stricture, which at autopsy was consistent with Hodgkin's disease for the first time. Berman reported an authentic case of primary histiocytic lymphoma confined to the lower esophagus. Only six or more cases were reported in literature until the present time. Here, a 30-year-old man with substernal discomfort was shown to have a mass lesion on the esophagus. The mass was proved to be malignant lymphoma on the histologic examination. This may be the first report dealing with spontaneous regression of maligant lymphoma of the esophagus possibly associated with the use of cimetidine.

Xanthogranulomatous pyelonephritis of the kidney usually occurs in association with urinary tract infection. obstruction and calculi. The diagnosis is made when a nephrectomy is performed for a nonfunctioning kidney or a mass indistinguishable from carcinoma. We experienced incidentally a case of xanthogranulomatous pyelonephritis. so the pathologic features and clinical presentation were discussed and the literature was reviewed.

5.A Case of Gangrenous Ischemic Colitis Associated with Rectal Carcinoma.

The ischemic coitis proximal to obstructive colon cancer was firstly reported by MacCallum and Kaisch in 1947. Nowsdays, its clinical importance and the study of pathogenesis is reported frequently. Because of the rare inciedence of this complicated condition this lesion could be overlooked which then could result anastomotic leakage, fecal fistula and peritonitis. Recently, we have examined a 61 year-old Korean male patient who had the complaints of acute abdominal pain and bloody diarrhea. The diagnosis of gangrenous ischemic colitis associated with obstucting rectal cancer was confirmed on the basis of sigmoidoscopic and operative and associated histopathological findings. After prompt surgical intervention, the patient was discharged with good improvement. The literatures are reviewed here with the case presentation.

6.Usefulness of Power Spectral Analysis for Measurement of Local Muscle Fatigue during Keyboard Task.

OBJECTIVES: Work-related upper extremity disorders of the tendons, nerves, and musdes associated with. repetitive motion represent an increasing proportion of occupational illnesses, medical services, and workers' compensation costs. Severe local muscle fatigue may be a precursor of repetition strain injuries. The demand for objective and quantitative assessment of local muscle fatigue is now increasing. The purpose of this study is to determine the usefulness of power spectral analysis of individual muscle as one of the objective spectral parameters in quantification of local muscle fatigue. METHODS: From 10 normal male without neuromuscular dysfuction, motor unit action potentials (MUAPs) of three muscles (upper trapezius, infraspinatus, and anterior deltoid) of left shoulder girdle were obtained. The surface electrodes were used and the changes of power spectrum according to frequency components were calculated per minute during the keyboard task for 30-minute period by power spectral analysis of MUAPs signal. RESULTS: There was a linear relationship between median frequency (MF) and time flow in the upper trapezius muscle, however the same was not observed in other muscles. The MF slope of the upper trapezius muscle showed a decline of the MF over time and reflected the fatigue rate. Other spectral parameters, such as mean frequency and total power were not significantly correlated to time flow during the keyboard task in all three muscles. And no significant correlation was observed between keying rate and slope of median frequency of upper trapezius muscle. CONCLUSIONS: Above results suggested that the power spectral analysis on median frequency in upper trapezius muscle would be useful in quantification of local muscle fatigue during the keyboard task and local muscle fatigue of shoulder was affected by static posture, not by keying rate.

7.A Case of Congenital Trichofolliculoma.

Trichofolliculoma is a benign, highly structured adenoma of the pilosebaceous unit, usually occuring on the face or scalp as 0.5 to 1.0 cm solitary papule in persons past middle age. This benign neoplasm rarely occurs prior to early adult life or neonate. We report a case of congenital trichofolliculoma in a 13-year-old girl who had had a single asymptomatic mass on the posterior aspect of right auricle since birth.

8.A Case of Herpes Esophagitis Confirmed by Electron Microscopic Findings.

Herpetic esophagitis is a rare disease occuring mostly ie immunocompromised and cancer patient. It is also documented as self-limiting esopbagitis in young healthy adults. We report here a 30- year-old immunocompetent male patient with esophageal ulceration. Heryetic esophagitis was diagnosed by characteristic endoscopic and microscopic findings. And the diagnosis was confirmed by electron microscopic appearance. This immunocompetent patient recovered spontaneously with symptomatic treatment only. And repeat endoscopy and histology confirmed absence of herpes infection.

BACKGROUND: We investigated the neuropsychological features of laterality-sex interactions in mesial temporal lobe epilepsy (MTLE), using a broad and comprehensive neuropsychological test battery. METHODS: Eighty-nine consecutive patients (female 46) with MTLE (left 46) were included in the study. They underwent anterior temporal lobectomy and became seizure-free for at least one year postoperatively. All patients had a language dominancy in the left hemisphere. The following neuropsychological tests including : Korean Wechsler Intelligence Scale (KWIS), Wechsler Memory Scale-Revised (WMS-R), Wisconsin Card Sorting Test (WCST), and Color Trails Test (CTT), were performed preoperatively. Each variable of the neuropsychological tests were analyzed with regard to laterality-sex interactions. RESULTS: General memory, verbal memory, and delayed recall of WMS-R and a executive function measured by WCST were significantly decreased in patients with left MTLE compared to those with right MTLE. However, this laterality effect especially on verbal memory and some variables of WCST including the number of category completed was observed only in female patients. Interference index of CTT had a tendency to be increased in the male patients with right MTLE. The intelligence scale of KWIS was not correlated with lesion laterality or sex. CONCLUSIONS: Laterality-sex interactions were evident for verbal memory and an executive function in female patients with MTLE. Further studies need to be done on these neuropsychological features.
