1.Comparative Study on Allocation Levels of Health Resources in Different Provinces of China

Objective:Through comparing per capita allocation level of health resources in 31 provinces of China, the characteristics and the influencing factors of health resource allocation in each province are reveled. Methods: Selecting 7 representative indexes and using cluster analysis methods to divide per capita allocation level of 31 provinces into 5 grades; the influencing factors of per capita allocation level are proposed based on 5 grades and the ordered Probit model is built and estimated. Results: Beijing, Shanghai and Tibet respectively marked the first, the fourth and the fifth grade, 7 provinces ( including Tianjin) belong to the second grade, 21 provinces ( including Hebei ) belong to the third grade; four factors such as per capita GDP , family disposable income and so on all significantly affect per capita allocation level. Conclusion: Economic development and per capita allocation level of health resources are not balanced. The proportion of government health expenditure is negatively correlated with per capita allocation level of health resources.
2.Clinical study of selective salpingography and fallopian tube recanalization by home made coaxial catheter

Objective To evaluate the clinical value of selective salpingography (SSG) and fallopian tube recanalization (FTR) by home made coaxial catheter. Methods 116 cases of tube obstruction were diagnosed by hysterosalpingogram (HSG). SSG was performed with home made coaxial catheter. If proximal tube complete obstruction was still present, then followed by FTR. Results 116 cases of follow up showed 55 intrauterine pregnancies (IUPS) and 6 ectopic pregnancies. IUP rate was 47.4%. 28 of 116 cases were performed by HSG again, 25 of 28 cases were improved, effective rate was 89.3%.Conclusions SSG and FTR were a safe and effective procedure for the diagnosis and treatment of tube obstruction with home made coaxial catheter.
3.Investigation of the application value of arthroscope in unicompartmental knee arthroplasty

[Objective]To explore the value of the application of the arthroscope in unicompartmental knee arthroplasty(UKA)for the patients with medial osteoarthritis of the knee.[Method]A clinical retrospective study has been done on 30 knees in 21 patients.The patients were evaluated in preoperative X-rays,physical examination,the knee society score(KSS)evalution,the presentation of arthroscopical examination as well as the ultimate operation method.[Result]Arthroscopic examination showed that the osteoarthritis were mainly located in medial compartment of the knee.Tibia terrace cartilage and condyle of femur showed outbrige Ⅲ~Ⅳ degrees degeneration.26/30(87%)of the patients showed medial menisci injury in different degree,and 4/30(13.3%)of the patients showed lateral menisci degenerated,and pathological changes of osteoarthritis were located at front medial compartment.And 19/30(63%)of the patients combined Ⅰ~Ⅳ degrees of localized or broad patellar cartilage degeneration,the lateral compartment normal or localized patellar cartilage degeneration less than Ⅱ degrees for all patients.The result of the arthroscopic examination was consistent with the preoperative clinical examination and the imaging examination.All the patients received Oxford Ⅲ prosthesis UKA by microinvasive approach and there was no case that had to change operation method due to arthroscope detection.[Conclusion]1.The main purpose for the application of the arthroscopic in UKA is to deal with other disease inside the knee rather than to confirm the indication of the operation.2.Arthroscopy is an effective way for the patients combined lateral menisci injury and other compartmental mild osteoarthritis.
4.Mechanical performance of proximal human femur after poly-ether-ether-ketone femoral prosthesis implantation

BACKGROUND: Greatly higher elastic modulus of traditional metal prosthesis than bone elastic modulus is mechanical factor for aseptic loosening of artificial joint, resulting in stress shielding, bone resorption and bone atrophy.OBJECTIVE: To measure femoral stress after prosthetic replacement made of carbon fiber reinforced poly-ether-ether-ketone (CF/PEEK), and compare with domestic AML prosthesis, so as to explore the application prospect of compound material in total hip femoral head prosthesis. DESIGN, TIME AND SETTING: Comparative analysis was performed at Orthopedics Laboratory, First Hospital of Soochow University and Institute of Biomechanical Engineering of Shanghai University from September 2002 to March 2004.MATERIALS: CF/PEEK prosthesis was designed by the authors, composed of long CF/PEEK compound material as inner layer, 100 Gpa and short CF/PEEK compound material as outer layer, 20 Gpa; the layer intensity at stress center was strengthened, and the appearance and size of the CF/PEEK prosthesis simulated domestic AML cobalt alloy prosthesis (Beijing Huajiehao Company). Five pairs (10 femurs) of femurs were from fresh adult cadavers. METHODS: Five pairs of human cadaveric femora were divided equally into left and right groups: one group received a cobalt-chrome molybdenum alloy (CoCrMo) implant and the other received CF/PEEK implant. Strain-gauge rosettes were attached to external surface of each implant and proximal femora. The loading condition simulated single-limb stance. MAIN OUTCOME MEASURES: strains distribution on the normal femur and femoral surface after replacement with two kinds of prostheses; shearing strain at femur-prosthesis interface of two groups.RESULTS: Following prosthesis implantation, the strains exhibited a progressive proximal-to-distal increase, which was similar to that in the intact femur, and the strain was maximum near the distal prosthesis. The strains on the medial and lateral surface of the femur were reduced after implantation of both types of implants. The pattern and magnitude of the strains, however, were closer to those in the intact femur after insertion of CF/PEEK prosthesis than in the CoCrMo one. CONCLUSION: Mechanical performance of CF/PEEK compound prosthesis is similar to bone tissue, and can provide similar strain under loading condition. Therefore, it can provide immediate postoperative stability and proximal loading transfer.
5.The initial stability of CF/PEEK femoral stem after total hip replacement

Objective To explore the initial stability of a carbon fiber reinforced polyetheretherketone composite (CF/PEEK) stem in human femurs after total hip replacement. Methods 8 pairs of human cadaveric femurs were used to test the CF/PEEK prosthesis versus a conventional cementless stem of cobalt-chrome molybdemum alloy(CoCrMo). The bone-femoral prosthesis interface motion was measured in vitro using displacement transducers. Combined axial and torsional loads and simulated single-leg stance loading were applied. Subsidence, rotation and interface motion were measured with load cycles up to 1 000 N. Results The extent of the micro-motion in the proximal femoral region was larger than that in the distal femoral region. The CF/PEEK composite prosthesis showed less dynamic micro-motion in the proximal region compared with the CoCrMo alloy prosthesis stem, with the average difference being 42%. 10.Effects of platelet-derived growth factor-BB on the expression of matrix metalioproteinase-2 in trabecular meshwork cells in vitro

Background At present,a new drug,platelet-derived growth factor-BB (PDGF-BB),as a drug knife for the treatment of glaucoma is under study to expect it directly working on the trabecular meshwork without disrupting the normal physiological structure of anterior chamber angle,clearing the trabecular meshwork aqueous outflow channel so as to achieve the purpose of lowering intraocular pressure.Objective This study aimed to investigate the effect of PDGF-BB on matrix metalloproteinase-2 (MMP-2) expression in cultured bovine trabecular meshwork cells.Methods Trabcular tissue was obtained from fresh bovine eyes,and trabcular meshwork cells were cultured and passaged using explant method.Cultured cells were identified by morphological evaluation and neuronspecific enolase (NSE) staining.The third generation of cells were inoculated to 6-well plate,and different concentrations (0,5.0,12.5,25.0 μg/L) of PDGF-BB was added into the medium for 2 hours.Expression levels (A value) of MMP-2 mRNA (MMP-2 mRNA/β-actin) and protein in the cells were assayed by reverse transcription polymerase chain reaction (RT-PCR) and immunochemistry,respectively.Results Trabcular meshwork cells appeared 5-9 days after cultured.The third generation of cells presented with many process and showed the green influence in cytoplasm.MMP-2 mRNA/β-actin value (A) was 0.127 ± 0.026,0.147 ± 0.045,0.178 ± 0.053 and 0.222±0.062 in the O,5.0,12.5,25.0 μg/L PDGF-BB group,respectively,showing a significant difference among them (F =56.71,P<0.05),and the MMP-2 mRNA/β3-actin value in the 5.0,12.5,25.0 μg/L PDGF-BB group was elevated in comparison with that of the 0 μg/L PDGF-BB group (all P<0.05).The expression value (A value) of PDGF-BB protein in the cells was 446.12±13.81,1444.65±54.64,2086.18±73.18,3488.65±25.98 in the 0,5.0,12.5,25.0 μg/L PDGF-BB group,respectively,with a significant difference among the four groups (F=213.12,P<0.01),and the expression value (A value) of PDGF-BB protein was gradually increased with the ascend of concentration of PDGF-BB(all P<0.05).Conclusions PDGF-BB can promote the expression of MMP-2 in bovine trabcular meshwork cells in vitro in concentration-dependent manner.
