2.A Survey of Hepatitis B Surface Antigen and Hepatitis B Surface Antibody Positivity in ROK Air Force Pilots.

OBJECTIVE: The aim of this study was to detect the prevalence of hepatitis B surface antigen (HBsAg) and hepatitis B surface antibody (anti-HBs) in The Republic of Korea Air Force (ROKAF) pilots, and to investigate the factors related to occupational exposure. METHODS: For 720 pilots who visited aeromedical center, ROKAF for general health screening from January, 2001 to August, 2002, HBsAg and anti-HBs were tested using passive hemagglutination and passive hemagglutination, respectively. If HBsAg results had been positive or vague, we performed confirmation test by ASAN Easy Test(R) HBs kit. RESULTS: The overall prevalence of HBsAg and Anti-HBs was 1.8% and 49.4%, respectively. HBsAg positivity was 0.0% in 20-24 years, 0.8% in 25-29 years, 1.1% in 30-34 years, 4.4% in 35-39 years, 1.5% in 40-44 years, 5.4% in 45-49 years, 5.3% in 50-54 years and significantly different below and above thirties (P=0.024). The prevalence of HBsAg in pilots was not different with general adults. Anti-HBs positivity was 100.0% in 20-24 years, 43.9% in 25-29 years, 57.1% in 30-34 years, 50.0% in 35-39 years, 62.1% in 40-44 years, 50.0% in 45-49 years, 73.7% in 50-54 years and significantly increased with age (44.2% in twenties, 53.6% in thirties, 58.9% in more than 40) (P=0.005). The pilots susceptible to HBV infection who were negative in both HBsAg and anti-HBs were found in 49.0% of entire subjects. CONCLUSION: The prevalence of HBsAg and anti-HBs in pilots was lower than in general adults. The pilots tendered to be more susceptible to HBV infection than general adults. Hepatitis B vaccine should be integrated into pilot health programmes.

6.Reliability and Validity of the Korean Version of the Lifetime Stressor Checklist-Revised in Psychiatric Outpatients with Anxiety or Depressive Disorders.

PURPOSE: Traumatic events and adverse stressful experiences are major etiological factors in a wide variety of physical and mental disorders. Developing psychological instruments that can be easily administered and that have good psychometric properties have become an integral part for research and practice. This study investigated the reliability and validity of the Korean version of the Lifetime Stressor Checklist-Revised (LSC-R) in a consecutive sample of psychiatric outpatients. The LSC-R is a 30-item self-reporting questionnaire examining lifetime traumatic and non-traumatic stressors. MATERIALS AND METHODS: A final sample of 258 outpatients with anxiety or depressive disorders was recruited at the psychiatric department of a university-affiliated teaching hospital. Self-reported data included the Life Events Checklist (LEC), the Zung Self-Rating Depression and Anxiety Scales, and the Impact of Events Scale-Revised, in addition to the LSC-R. A convenience sample of 50 college students completed the LSC-R on two occasions separated by a three week-interval for test-retest reliability. RESULTS: Mean kappa for temporal stability was high (κ=0.651) and Cronbach alpha was moderate (α=0.724). Convergent validity was excellent with corresponding items on the LEC. Concurrent validity was good for symptoms of post-traumatic stress disorder, depression, and anxiety. An exploratory factor analysis revealed that 11 factors explained 64.3 % of the total variance. CONCLUSION: This study demonstrated good psychometric properties of the Korean version of the LSC-R, further supporting its use in clinical research and practice with a Korean speaking population.

7.A Case of Kimura's Disease Presenting as Facial Mass and Periorbital Swelling.

Kimura's disease is an uncommon chronic inflammatory disorder of unknown etiology. Clinically, it presents as solitary or multiple subcutaneous nodules, predominantly in the head and neck region, and typically in the preauricular region, forehead, and scalp. Other localizations such as lacrimo-orbital involvement and upper extremities have been reported. The soft tissue localization is often associated with regional lymphadenopathy and, occasionally, with enlargement of the major salivary gland. Orbital, eyelid, and lacrimal involvement by Kimura's disease are rare. The etiology of this disease is still unknown. It is essential to differentiate it with malignant tumor through biopsy. When Kimura's disease is confirmed, the combination of surgical excision and oral steroid appears to be effective therapeutic motality. We report Kimura's disease, along with a review of literature, presenting as facial mass and periorbital swelling in a case of a child.

8.Is Abnormal Electrodiagnostic Finding Related to the Cross-Sectional Area of the Nerve Root in Cervical Radiculopathy?

Objective:
To assess the relevance of electrodiagnosis (EDX) in the cross-sectional area (CSA) of the nerve root of patients with cervical radiculopathy (CR) by using high-resolution ultrasonography (HRUS).
Methods:
The CSAs of the cervical nerve roots at C5, C6, and C7 were measured bilaterally using HRUS in 29 patients with unilateral CR whose clinical symptoms, magnetic resonance imaging (MRI) findings, and EDX
results:
corresponded with each other (CR-A group), and in 26 patients with unilateral CR whose clinical symptoms and MRI findings matched with each other but did not correspond with the EDX findings (CR-B group). Results The CSA of the affected side in each nerve root was significantly larger than that of the unaffected side in both the CR-A and CR-B groups. The side-to-side difference in the bilateral CSAs of the nerve root and the ratio of the CSAs between the unaffected and affected sides were statistically larger in the CR-A group than in the CR-B group.
Conclusion
The increased CSAs in the CR-A group reflect the physiological changes of the cervical nerve root, which is supported by the EDX findings.
9.Comparison of the Outcomes after Primary Total Hip Arthroplasty Using a Short Stem between the Modified Anterolateral Approach and Direct Anterior Approach with a Standard Operation Table

PURPOSE: Total hip arthroplasty was performed using a direct anterior approach (DAA) on an ordinary operation table and a short femoral stem. The clinical radiographic results were evaluated by a comparison with those performed using the modified hardinge (anterolateral approach, ALA) method. MATERIALS AND METHODS: From January 2013 to November 2015, 102 patients who underwent total hip arthroplasty using DAA (DAA group) and the same number of patients using ALA (ALA group), both performed by a single surgeon, were compared and analyzed retrospectively. The operation time and amounts of bleeding were compared, and the improvement in post-operative pain, ambulatory capacity and functional recovery of the hip joint were checked. The location of insertion of the acetabular cup and femoral stem were evaluated radiologically, and the complications that occurred in the two groups were investigated. RESULTS: The amount of bleeding was significantly smaller in the DAA group (p=0.018). Up to 3 weeks postoperatively, recovery of hip muscle strength was significantly higher in the DAA group (flexion/extension strength p=0.023, abduction strength p=0.031). The Harris hip score was significantly better in the DAA group for up to 3 months (p<0.001) and the Koval score showed significantly better results in the DAA group up to 6 weeks (p≤0.001). The visual analogue scale score improvement was significantly higher in the DAA group by day 7 (p=0.035). The inclination angle (p<0.001) and anteversion angle (p<0.001) of the acetabular cup were located in the safe zone of the DAA group more than in the ALA group, and there was no statistically significant difference in the position of the femur stem and leg length difference. During surgery, two cases of greater trochanter fracture occurred in the DAA group (p=0.155). CONCLUSION: The DAA performed in the ordinary operation table using a short femoral stem showed post-operative early functional recovery. Because a simple to use fluoroscope was used during surgery with an anatomical position familiar to the surgeon, it is considered to be useful for the insertion of implants into the desired position and for an approach that is useful for the prevention of leg length differences.
